As we age, cholesterol builds up in the arteries of the heart and neck, placing us at risk for heart attacks, strokes, and premature death. Treatment that lowers cholesterol in the blood has therefore become the cornerstone of prevention of future heart attack, stroke, and death, both in persons who definitely have heart disease and in those who are at risk of developing it in the future. Evidence suggests that millions of people who could benefit from cholesterol lowering medicines are currently untreated. According to the American Heart Association, nearly 38 million patients in the United States would benefit from the combination of diet and drug therapy, and an additional 30 million should follow diet and exercise programs to reduce cholesterol levels.1 Given that there are 290 million adults in the Unites States, this issue affects 1 of every 3 to 5 adults. Unfortunately, cholesterol-lowering treatment is widely underutilized because of the lack of awareness, fear of side effects, reluctance of otherwise healthy people to take medication, and cost. We hope this overview of the treatment of elevated cholesterol for patients (and persons who are not “patients”) will help ensure that all affected people will get their elevated cholesterol treated.
What Is Cholesterol?
Cholesterol is a fatty substance that circulates in the blood and is an important structural component of all human cells. The body obtains cholesterol through 2 methods: via the liver, which produces about 75% of cholesterol, and through food, which accounts for 25%. Cholesterol comes in several forms, notably a “good” and a “bad” cholesterol. High levels of low-density lipoprotein (LDL)
cholesterol, the “bad” cholesterol, are associated with an increased risk of heart attacks and strokes. By contrast, increased levels of high-density lipoprotein (HDL) cholesterol, the “good” cholesterol, offer protection against heart attacks and strokes. (…) The total cholesterol value you receive after a blood test is the sum of all types of cholesterol, as discussed in a previous Cardiology Patient Page (http://circ...) Triglycerides are another form of circulating fatty substance that, when elevated, are also associated with heart attacks and strokes.

English to Spanish: The Parable of the Prodigal Son Detailed field: Religion
Source text - English The Lord Jesus has been telling a series of parables. He has just told the parable of the lost coin and the lost sheep. In the context of this chapter, Jesus is answering the Pharisees who have been questioning his practice of eating with sinners. Through these parables, the Lord reminds his listeners of the value he places on the soul of a sinner.
In this parable, Jesus spoke of a man who had two sons. The younger of the two sons asked his father to give him his inheritance. The father agreed and the inheritance is divided between the two sons.
The younger of the two sons decided it was time for him to have his independence. He no longer wanted to remain at home and so he set out for a distant country. Not having the maturity required to be responsible with his inheritance, the younger son lived a wild and careless lifestyle. It was not long before his inheritance was gone and he began to be in need. Added to this was the fact that there was a severe famine in the country. He had made no provision for the future and now feared for his life.
To provide for his needs, the younger son had to hire himself out to a citizen of that country. He got a job feeding pigs. It should be understood here that the pig was an unclean animal in the Jewish mind. The son had to do something that he would never have done in his own country. In many ways, he had reached the bottom. From verse 16 we understand that the job did not pay enough to provide him with his needs. He worked hard but he was still hungry. He would have gladly eaten the pig’s food just to survive but no one offered him any so he went hungry.
This whole incident made him think, “What am I doing here in this place?” He thought about his home. He remembered the servants who worked on his father's property. He said to himself: “How many of my father's hired men have food to spare, and here I am starving to death” (verse 17)! After being through the events of the last few months he began to appreciate what he had at home. His father's servants were better treated than he was treated in this foreign country.
These thoughts brought him to a point of decision in his life. He decided to return to his father and tell him what he had done. He recognized his sinful life and how he had wasted his inheritance. Notice in verse 18 that the son recognized that he had not only sinned against his father but he had also sinned against heaven. He had not honoured his earthly father in the use of his inheritance. His father had worked hard all his life to give his son this inheritance but the son had not appreciated what the father had done for him. He was also guilty before his heavenly father of a number of sins. His life in this foreign country had been a wild life.
Translation - Spanish La Parábola del Hijo Pródigo: El Señor Jesús ha estado diciendo una serie de parábolas. Acaba de decir las de la moneda y de la oveja perdidas. En el contexto de este capítulo, Jesús está respondiendo a los Fariseos que han estado cuestionando Su práctica de comer con pecadores. Por medio de esas parábolas, el Señor les recuerda a Sus oyentes el valor que Él da al alma de un pecador.
En esta parábola, Jesús habló de un hombre que tenía dos hijos. El menor de los dos le pidió a su padre que le diera su parte de la herencia. El padre estuvo de acuerdo y sus bienes fueron divididos entre los dos hijos.
El menor de los dos hijos decidió que ya era tiempo de tener su independencia. Ya no quería estar en la casa y se marchó a un país lejano. Como no tenía la madurez requerida para ser responsable de su herencia, comenzó a llevar un estilo de vida salvaje y descuidado. No pasó mucho tiempo antes de que hubiera desaparecido su herencia y él comenzara a pasar necesidad. Añadido a eso estaba el hecho de que había una gran hambruna en el país. No había hecho provisiones para el futuro y ahora temía por su vida.
Para encontrar una solución a sus necesidades, el joven se puso al servicio de un ciudadano de aquel país. Obtuvo un trabajo alimentando cerdos. Aquí debemos entender que en la mente Judía el cerdo era un animal inmundo. El hijo tuvo que hacer algo que nunca habría hecho en su país. En muchas maneras, había tocado fondo. Al leer el versículo 16 entendemos que el trabajo no le daba dinero suficiente para cubrir sus necesidades. Trabajaba duro pero aún tenía hambre. Gustosamente habría comido del alimento de los cerdos pero nadie le ofrecía de modo que continuó sintiendo hambre.
Todo aquello le hizo pensar, “¿Qué hago aquí en este lugar?”. Pensó en su casa. Recordó los sirvientes que trabajaban en la propiedad de su padre y se dijo, “¡Cuántos jornaleros en casa de mi padre tienen abundancia de pan, y yo aquí perezco de hambre!” (versículo 17). Después de pasar por los sucesos de los últimos meses él comenzó a apreciar lo que tenía en casa. Los siervos de su padre eran mejor tratados que él en aquel país extranjero.
Aquellos pensamientos lo llevaron a un punto crucial en su vida. Decidió regresar a su padre y decirle lo que había hecho. Reconoció su vida de pecado y cómo había malgastado su herencia. Note, en el versículo 18, como aquel hijo reconoció que no sólo había pecado contra su padre, sino que también lo había hecho contra el cielo. No había honrado a su padre terrenal en el uso de su parte de la herencia. Su padre había trabajado duro toda su vida para darle a su hijo aquella parte de su herencia, pero el hijo no había sabido apreciar lo que el padre había hecho por él. También era culpable delante de su padre celestial por un gran número de pecados. En aquel país extranjero había llevado una vida salvaje.

Pregnancy in most women with heart disease has a favourable maternal and fetal outcome. With the exception of patients with Eisenmenger syndrome, pulmonary vascular obstructive disease, and Marfan syndrome with aortopathy, maternal death during pregnancy in women with heart disease is rare.1–4 However, pregnant women with heart disease do remain at risk for other complications including heart failure, arrhythmia, and stroke. Women with congenital heart disease now comprise the majority of pregnant women with heart disease seen at referral centres. The next largest group includes women with rheumatic heart disease. Peripartum cardiomyopathy, though infrequent, will be discussed in view of its unique relation to pregnancy. Two groups of conditions not discussed further are coronary artery disease, infrequently encountered, and isolated mitral valve prolapse, which generally has an excellent outcome.
Cardiovascular physiology and pregnancy
Hormonally mediated increases in blood volume, red cell mass, and heart rate result in a major increase in cardiac output during pregnancy; cardiac output peaks during the second trimester, and remains constant until term. Gestational hormones, circulating prostaglandins, and the low resistance vascular bed in the placenta result in concomitant decreases in peripheral vascular resistance and blood pressure. During labour and delivery, pain and uterine contractions result in additional increases in cardiac output and blood pressure. Immediately following delivery, relief of caval compression and autotransfusion from the emptied and contracted uterus produce a further increase in cardiac output. Most haemodynamic changes of pregnancy resolve by two weeks postpartum.5
Pulmonary stenosis
Mild pulmonic stenosis, or pulmonic stenosis that has been alleviated by valvuloplasty or surgery, is well tolerated during pregnancy and fetal outcome is favourable. Though a woman with severe pulmonic stenosis may be asymptomatic, the increased haemodynamic load of pregnancy may precipitate right heart failure or atrial arrhythmias; such a patient should be considered for correction before pregnancy. Even during pregnancy, balloon valvuloplasty may be feasible if symptoms of pulmonary stenosis progress.

En la mayoría de las mujeres con enfermedades cardíacas, el embarazo tiene un resultado favorable tanto para la madre como para el feto. A excepción de pacientes con Síndrome de Eisenmenger, enfermedad pulmonar vascular obstructiva y Síndrome de Marfan con aortopatía, la muerte materna durante el embarazo en mujeres con afecciones cardíacas es rara. Sin embargo, las embarazadas con afecciones cardíacas si tienen riesgo de presentar otras complicaciones incluyendo insuficiencia cardíaca, arritmias, y paro.
