Source text - English The Office of Head Start National Center on Cultural and Linguistic Responsiveness has the responsibility to provide training and technical assistance to Head Start programs throughout the nation. The Center is operated by Bank Street College in collaboration with the Education Development Center (EDC). The Center is funded by the U.S. Department of Health and Human Services, Administration for Children and Families, Office of Head Start. These organizations are referred to collectively in this document as “The Center”.
Photographs, video, and audio recordings that we take with your permission will be used for educational purposes. The Center may incorporate photos, videos, or audio recordings into training handouts, PowerPoint presentations, videos, audio recordings, online courses, workshops, websites, webinars, and similar materials. The Center may make these promotional and educational materials available to Head Start staff, partners, universities, colleges, and early childhood educators, in hard copy, electronic or broadcast formats and will be used to teach others about early childhood education. The Center is asking for your permission to photograph/videotape/audiotape you and/or your child and to use the photographs/videotapes/audio recordings for educational purposes as described above.
Please read the following and if you agree, please sign where indicated below.
•I give consent to be photographed/videotaped/audiotaped and for the images to be used for educational purposes as described above.
•I give permission to the National Center on Cultural and Linguistic Responsiveness at EDC to use any images of myself and/or the child named above for educational purposes as described above.
•I consent to any images being published, distributed, presented online, etc. for educational purposes.
•I understand that no payment is offered or implied, and that I and the child named above do not own or retain any rights in any such photographic image.
•This release shall remain in effect unless it is revoked by me or another parent or legal guardian in writing.
