Once developed, a scaphoid non-union is thought to progress inexorably through stages of carpal collapse (scaphoid non-union advanced collapse, “SNAC”). As the distal part of the scaphoid collapses into flexion this puts abnormal forces through the radioscaphoid joint and the first signs of osteoarthritis/degenerate change appear at the tip of the radial styloid which starts to “beak”. As the process continues the entirely scaphoid fossa of the distal radius becomes denuded of cartilage (degenerate change); with ongoing time the scaphocapitate joint becomes arthritic and finally with collapse of the carpus the entire midcarpal and radiocarpal joints can become arthritic. This process is thought to take many years and itself is under the influence of a variety of factors
