Source text - English The patient on the operating table here at Sydney's Prince of Wales Hospital is undergoing an extremely complex procedure.
Drage Josevski is being treated for a rare form of cancer which occurs in the bone of the skull and the spine.
Doctors here are carrying out a 15-hour operation to take out the cancerous bone and replace it with a precisely measured 3D printed vertebrae.
"The surgery that we're doing today is a particularly complicated and long and difficult surgery. It involves exposure at the top of the neck, where the neck and the head meets and it's essentially disattaching the patient's head from his neck and taking the tumour out and reattaching his head back onto his neck," says neurosurgeon Doctor Ralph Mobbs.
The surgeons are using the patient's mouth as an entry to the site of the surgery.
The surgeons access the top of the patient's spine through the mouth, extract the tumour and the top two vertebrae, replacing them with a titanium 3D printed body part.
The three dimensional body part was designed by a Victoria-based company called Anatomics. The company is creating a range of products that are aimed at widening the scope of surgeons to perform procedures that were not possible in the past.
Mobbs explains how the prosthetic body part will fit: "This is the front of the neck where the tumour is being resection from and this is the space that we will have at the completion of the tumour resection. As you can see its a large gap but we've pre-planned the prosthesis to fit perfectly in place. Like a glove, and that will form the stability for where the head and the neck meet so his head won't fall off."
Mobbs has spent many hours making calculations and setting up computer models to plan the process before beginning.
"Placing the implant at the end of the operation, that is the easy part. The hard part is the one or two months of work beforehand. The hard part is sitting down at the computer console and working out how you're going to do the operation, what angles that you're going to take, how you're going to achieve the resection of the tumour and then planning the implant around that. So the tough bit has already been done. This is the good fun bit," he says.
Despite the complexities of the operation, the patient makes good progress.
A few weeks after the operation, he and his family are meeting Mobbs in his office to discuss his recovery.
Josevski can move his neck with relative ease and the tumour has gone.
He still cannot eat or speak normally, but Mobbs assures the family that this will improve after several months healing followed by rehabilitation therapy.
"The complications that you've got are all related to the fact that we've goneA through your mouth and we've stretched your mouth open for many many hours and the complications are related to the fact that there's been a prolonged exposure through your mouth," he tells Josevski.
The patient's daughter Tanya Josevska says she can see her father improving.
"Every day he gets better and better, I think his speech is improving, I know that there's a long way to go with that."
It's been a traumatic time for the whole family. The diagnosis was made by a doctor during a family holiday back in their native Macedonia.
Mobbs and his team hope the success of operations like this one will encourage more surgical procedures using 3D printed body parts in the future.

Bagian tubuh tiga dimensi ini dirancang oleh sebuah perusahaan yang berpangkalan di Victoria bernama Anatomics. Perusahaan ini menciptakan serangkaian produk yang ditujukan untuk memperluas jangkauan para ahli bedah untuk melakukan prosedur prosedur yang tak mungkin dilakukan di masa lulu.

Bagian sulitnya adalah duduk di depan konsol komputer dan merencanakan bagaimana akan melakukan operasinya, sudut mana yang akan diambil, bagaimana mencapai pemotongan tumor dan kemudian merencanakan implant di sekitarnya. Jadi bagian sulitnya telah dilakukan. Yang ini adalah bagian yang menyenangkan., “ katanya.
Kendati operasinya rumit, sang pasien mengalami kemajuan yang back.
Beberapa pekan usai operasi, ia dan keluarganya bertemu dengan Mobbs di kantornya untuk mendiskusikan kesembuhannya.
Josevski bisa menggerakan lehernya dengan cukup mudah dan tumornya telah hilang.
Ia masih belum bisa berbicara atau makan dengan normal, namun Mobbs meyakinkan keluarganya bahwa ini akan membaik setelah beberapa bulan penyembuhan diikuti dengan terapi rehabilitasi.
“Komplikasi yang kau alami semuanya terkait dengan fakta bahwa kami telah memasuki mulutmu dan kami telah membuka mulutmu selama berjam jam dan semua komplikasinya terkait dengan fakta bahwa telah terjadi eksposur yang lama melalui mulutmu,” katanya pada Josevski.
Putri si pasien Tanya josevska mengatakan ia bisa melihat ayahnya membaik.
“Setiap hari ia makin membaik dan membaik, menurut saya bicaranya makin membaik, saya tahu masih butuh banyak waktu untuk itu. “
Ini adalah saat yang traumatis bagi seluruh keluarga.
Diagnosisnya dilakukan oleh seorang dokter saat liburan keluarga di tanah kelahiran mereka Makedonia.
