Source text - German Beim Kniegelenkersatz stellt sich regelmässig die Frage nach der erfolgreichsten OP-Methode: Steht im Vordergrund, dass Ober- und Unterschenkelachse im Lot sind? Oder soll den Seitenbändern mehr Beachtung geschenkt werden? Der australische Spezialist für Arthroplastik, David G. Campbell kennt die Vorzüge achs- und weichteilorientierter Verfahren und erläutert im Gespräch, wieso davon nicht nur Patienten profitieren. Herr Dr. Campbell, Sie haben Erfahrung mit achsorienterten und weichteilorientierten Systemen. Könnten Sie Vor- und Nachteile dieser beiden Ansätze kurz beschreiben? Beide Techniken haben eine anatomische Ausrichtung der Beinachse und eine gute Ausbalancierung der Bandstrukturen zum Ziel. Bei der Ausbalancierung des Bandapparates wird besonderer Wert auf die Flexions- und Extensionsbewegungen gelegt. Durch diese Operationstechnik erhalten die Operateure viele wertvolle Informationen, u. a. über die Kollateralbänder und das notwendige Weichteilrelease zur Beurteilung der Einstellung der Femurrotation und der Flexions- und Extensionsgaps. Diese Technik ist im Allgemeinen unkompliziert, und die Femurkomponente wird gut ausbalanciert implantiert. Ein erforderlicher Weichteilrelease wird im Rahmen des routinemässigen Arbeitsablaufs vorgenommen. Die Messungen mit den Instrumenten erfolgen jedoch mit grosser Präzision, und die Operateure ertappen sich dabei, dass sie eventuell notwendige Releases mehr als zuvor berücksichtigen. Die achsorientierte Technik verläuft automatisierter und mechanischer, und die meisten Operateure sind damit vertraut. Zur Ausrichtung der Schnittblöcke und Durchführung der Osteotomien werden ossäre Landmarken verwendet. Die Balancierung der Ligamente erfolgt erst am Ende des Eingriffs. Bei den meisten Patienten werden die natürlichen anatomischen Gegebenheiten und damit gut ausbalancierte Bandstrukturen des Kniegelenks wiederhergestellt, sofern keine grosse Deformität vorliegt. Fehlt diese Balance, sind ein weiteres Weichteilrelease bzw. zusätzliche Resektionen erforderlich, die den Arbeitsfluss stören. Sie haben mit Chirurgen aus ganz Europa einen neuen Ansatz entwickelt, der achs- und weichteilorientierte Instrumente und Abläufe kombiniert: die balanSys Operationstechnik „combination 4in1“. Wie ist dabei der Arbeitsablauf? Die Technik „combination 4in1“ umfasst tatsächlich nur ein einziges Instrumenten-Set für zwei sich ergänzende Operationstechniken. Wir haben versucht, die weichteilorientierte Technik in einen herkömmlichen Arbeitsablauf zu integrieren. Bei beiden Techniken kommen dieselben Instrumente und Arbeitsabläufe für die Tibiaresektion, die distale Femurresektion und die Schrägschnitte zum Einsatz. Sie unterscheiden sich lediglich in der Bestimmung des Flexionsspaltes und der Femurrotation. Bei beiden Verfahren werden zur Aufnahme des Schnittblocks für die Schrägschnitte im distalen Femur zwei Löcher gebohrt. Auf diese Weise besteht nur noch ein minimaler Unterschied zwischen beiden Techniken. Bei der weichteilorientierten Technik verwendet man einen Bänderspanner und bei der achsorientierten Technik eine Rotations- und Grössenlehre. Der Chirurg kann sich im Laufe der Operation für die eine oder andere Technik entscheiden und dabei dieselbe Schnittfolge anwenden. Was würden Sie als das Schlüsselelement der „combination 4in1“ bezeichnen? Meiner Ansicht nach ist das die Möglichkeit, während des Eingriffs von einer weichteilorientierten zu einer achsorientierten Technik oder umgekehrt zu wechseln, ohne sich auf ein anderes Instrumentarium einstellen zu müssen. Das ist sehr attraktiv für das Spital und die OP-Schwestern, die sich nur mit einem System vertraut machen müssen. Was halten Sie aus der Sicht des Patienten für den wesentlichsten Vorteil einer weichteilorientierten Technik? Für den Patienten ist das Ergebnis einfach fantastisch: ein gut ausbalanciertes Kniegelenk mit einem weichen Übergang von der Extension über die mittlere Flexion bis zur kompletten Beugung ohne jegliche Instabilität. Die Patienten „fühlen“ die optimale Balance ihres Kniegelenks, und die normale Kinematik lässt sich am besten mit einem Knie beschreiben, das sich „normal anfühlt“. Ich denke, dass wir mit einer weichteilorientierten Technik konsistenter ein gut ausbalanciertes Kniegelenk erreichen können. Wie sieht Ihre Grenze für eine Varusfehlstellung aus, die Sie mit einer weichteilorientierten Technik versorgen würden (z. B. >10°, >15°, >20°, >25°)? Mit welchen Problemen müssten Sie rechnen, und wie würden Sie sie lösen? Für mich gibt es kein Limit für eine Varusfehlstellung – sofern keine übermässigen Knochendefekte oder eine umfangreiche Bandinstabilität vorliegen, für die Revisionsprothesen erforderlich wären. Kann die Fehlstellung eines Kniegelenks korrigiert und nach dem entsprechenden Weichteilrelease in einen Normalzustand gebracht werden, mache ich mir über die ursprüngliche Deformität keine sonderlichen Sorgen. Wir müssen mit fixierten Varusfehlstellungen vorsichtig sein, da dafür umfassende mediale Bandreleases erforderlich sind. Doch der grössere Fehler dürfte ein unzureichendes Weichteilrelease und damit ein Kniegelenk mit fortbestehender Varusfehlstellung sein. Denn die würde eine raschere Abnutzung im Bereich des medialen Kompartments begünstigen. Die Weichteilbalancierung ist in solchen Situationen sehr nützlich, in denen der mediale und der laterale Spalt vor und nach dem Release unter physiologischer Bandspannung gemessen werden kann. Welchen Ansatz würden Sie im Falle einer Valgusgonarthrose im Vergleich zu einem Varusfall wählen? Ein Valgusknie kann besonders schwierige anatomische Situationen und Bandprobleme mit sich bringen. Bei einem gewöhnlichen korrigierbaren Valgusknie mit minimaler Fehlstellung gehe ich standardmässig vor, doch dabei achte ich stets auf den hypoplastischen lateralen Femurkondylus. Eine schwerere Deformität kann eine dreidimensionale Herausforderung darstellen, mit fixiertem Valgus, Femurhypoplasie und Rotationsfehler. Als Versorgungsansatz bevorzuge ich bei einem fixierten Valgusknie den lateralen Zugang mit Tuberositasosteotomie. Er bietet einen umfassenden Überblick, schützt so das möglicherweise atrophische mediale Seitenband und stellt bereits einen Teil des lateralen Releases dar. Bei einem straffen Streckapparat müssen wir beim Einsatz einer weichteilorientierten Technik Vorsicht walten lassen.
Translation - English When it comes to knee replacements, one question is regularly asked: What is the most successful method of surgery? Is it most important that the femoral and tibial axes are perpendicular? Or should more attention be given to the collateral ligaments? The Australian arthroplasty specialist, Dr. David G. Campbell, knows the advantages of bone-referencing and ligament-referencing procedures and explained in our conversation how it’s not only the patients who benefit.
Dr. Campbell, you have experience with bone-referencing and ligament-referencing systems. Could you briefly describe the advantages and disadvantages of these two approaches?
The goal of both techniques is to achieve an anatomical alignment of the mechanical leg axis and good balancing of the ligamentous structures. In balancing the ligamentous apparatus, flexion and extension movements are particularly important. This surgical technique gives surgeons a lot of valuable information, for example about the collateral ligaments and the soft-tissue release required to evaluate the adjustment of the femoral rotation and the flexion and extension gap. This technique is generally straightforward, and the femoral components can be implanted with good balancing. If a soft-tissue release is required, this is performed as part of the routine workflow. However, the instrument measurements are performed with a high level of precision, and surgeons find themselves considering the possible need for releases more than they used to.
The bone-referencing technique is more automated and mechanical, and most surgeons are familiar with it. To align the cutting blocks and perform the osteotomies, osseous landmarks are used. The ligament balancing is only performed at the end of the procedure. In most patients, the natural anatomical conditions and thus well-balanced ligamentous structures of the knee joint are reconstructed, provided that there is no gross deformity. If this balance is absent, an additional soft-tissue release or further resections are required, which interrupt the workflow.
In collaboration with surgeons from all over Europe, you’ve developed a new approach which combines bone-referencing and ligament-referencing instruments and workflows: the balanSys surgical technique “combination 4in1”. Can you describe the workflow of this technique?
The “combination 4in1” technique actually involves only one set of instruments for two complementary surgical techniques. Our aim was to integrate the ligament-referencing technique into a conventional workflow. Both techniques use the same instruments and workflow for the tibial resection, the distal femoral resection and the oblique sections.
They differ only in the mechanism of determining the flexion gap and the femoral rotation. Both techniques involve the drilling of two holes to support the cutting block for the oblique sections in the distal femur. So there’s only a minimal difference between these two techniques. The ligament-referencing technique uses a ligament tensioner, while the bone-referencing technique uses a rotation and sizing gauge. During the operation, the surgeon can decide to use either technique and in doing so can follow the same sectioning sequence.
What would you say is the key feature of “combination 4in1”?
In my opinion, it’s the ability to switch from a ligament-referencing to a bone-referencing technique, or vice versa, during the procedure, without needing to adjust to a different set of instruments. That’s very attractive to hospitals and theatre nurses, who only need to familiarise themselves with one system.
What do you consider to be the biggest advantage of the ligament-referencing technique, from the patient’s point of view?
The outcome for the patient is simply fantastic: a well-balanced knee joint with a soft transition from extension through mid-flexion up to complete flexion, without any instability. The patients “feel” the optimal balance of their knee joint, and the normal kinematics can best be described in a knee which “feels normal”. I think we can achieve a well-balanced knee joint more consistently using a ligament-referencing technique.
What is your threshold for treating a varus malposition with a ligament-referencing technique (e.g. >10°, >15°, >20°, >25°)? What problems would you face, and how would you solve them?
For me, there’s no limit for a varus malposition – as long as there are no excessive bone defects or extensive ligament instability which would require revision prostheses. If the malposition of a knee joint can be corrected and brought into a normal state after the appropriate soft-tissue release, I’m not especially concerned about the nature of the original deformity. With fixed varus malpositions, we must be careful, because they require extensive medial ligament releases, but it would be a bigger mistake to release the soft tissues insufficiently, resulting in a knee joint which still has a varus malposition. That would promote more rapid wear in the area of the medial compartment. In such situations, soft-tissue balancing is very useful; here, the medial and lateral gaps before and after the release can be measured with physiological ligament tension.
What approach would you select in the case of valgus gonarthrosis, as opposed to a varus case?
A valgus knee can be accompanied by particularly difficult anatomical situations and ligament problems. For a valgus knee with minimal malpositioning which can be corrected in the usual way, I use the standard approach, but I always pay attention to the hypoplastic lateral femoral condyle. A severe deformity may represent a three-dimensional challenge, with a fixed valgus, femoral hypoplasia and a rotation error. For a fixed valgus knee, I prefer to use a treatment approach involving lateral access with a tubercle osteotomy. This provides a broad overview, which protects the medial collateral ligament which may be atrophic, and already forms part of the lateral release. In the case of a tight extensor apparatus, caution is required when using a ligament-referencing technique.
