What can you expect from our expert service technician during a PMA visit?

Our basic 9-Point inspection includes the following:

1. Check system functions, safety controls, and adjust the operating sequence where appropriate.

2. Inspect electrical components and connections and repair/replace or tighten as required. Faulty electrical connections can cause unsafe operation of your system and reduce the life of major components.

3. Ensure proper airflow and check air filters.

4. Inspect pumps, lubricate and check flow rates where appropriate. Parts that lack lubrication cause friction in motors and increases the amount of electricity you use.

5. Clean and lubricate motors as required.

6. Examine belts, adjust and align as required.

7. Inspect, clean and balance blowers as required.

8. Check thermostat settings to ensure the cooling and heating system keeps you comfortable when you are home and saves energy while you are away.

9. Check and inspect the condensate drain in your central air conditioner, furnace and/or heat pump (when in cooling mode). A plugged drain can cause water damage in the house and affect indoor humidity levels.

She lifts up her narrow body and in a single, swift motion arranges the coat beneath her buttocks and legs. It's a face he recognizes from campus, someone he's crossed paths with in the corridors of buildings as he walks to and from class. He remembers that freshman year she'd had hair dyed an emphatic shade of cranberry red, cut to her jaw. She's grown it to her shoulders now, and allowed it to resume what appears to be its natural shade, light brown with bits of blond here and there. It is parted just off-center, a bit crooked at the base. The hair of her eyebrows is darker, lending her otherwise friendly features a serious expression. She wears a pair of nicely faded jeans, brown leather boots with yellow laces and thick rubber soles. A cabled sweater the same flecked gray of her eyes is too large for her, the sleeves coming partway up her hands. A man's billfold bulges prominently from the front pocket of her jeans.

M: Miss, excuse me, would you go out with me?

W: I don’t go out with strangers.

Really? But I am special.
And you should make an exception.

Is that so? And what is so special about you?

Let’s see. We just met,
and I already brought you flowers!

-Not so sure they are for me.
-You are right. Not for you.

She is introducing me to her parents.
The flowers are for her mom.

-Sounds serious.
-You have no idea!

And your girlfriend doesn’t mind
that you are picking up women?

You have no clue how awesome my girlfriend is!

In that case, I can give you a ride.

Anywhere your heart desires!

Let’s get going. Dad doesn’t like
when people are late.

I have been meaning to tell you

that my parents are not like everyone else.
Really?

You have no idea how much we have in common.

My parents are...
I mean, they are great.

They love me very much.

They’re just a bit out of touch with reality…

No kidding!

I rarely bring friends home.

Every time, we find ourselves in a pickle.

But you… are not just a friend.

I would want very much for them to like you.

Don’t worry. Nutty parents are my specialty!

-What floor are they on?
-Forty-eight.

So, above the clouds… Celestials.

Which of them do you look like?

Everyone thinks that my mom.

Mom! We are here!

0:00:02 0:00:05 Alright, video three is about patients
0:00:05 0:00:08 in the outpatient setting.
0:00:11 0:00:15 Patient one is a 78-year-old white male,
0:00:16 0:00:18 whom I saw earlier today.
0:00:19 0:00:21 Past medical history of hypertension, diabetes,
0:00:21 0:00:25 hyperlipidemia, prior cholecystectomy.
0:00:30 0:00:33 Referred for follow-up by his PCP internist,
0:00:36 0:00:40 because those saw this patient in the hospital.
0:00:40 0:00:43 Six weeks ago, after he had a stroke.
0:00:45 0:00:49 I didn't really consult any other healthcare providers
0:00:49 0:00:51 in the outpatient setting.
0:00:54 0:00:57 I already even consulted with physical occupational
0:00:57 0:01:00 speech therapist in the inpatient setting.
0:01:00 0:01:02 And that was continued outpatient,
0:01:02 0:01:06 but not by me, necessarily.
0:01:07 0:01:08 My role is follow-up
0:01:08 0:01:09 secondary stroke prevention,
0:01:09 0:01:11 any complications from the stroke.
0:01:19 0:01:23 I started interacting with the patient at the onset
0:01:23 0:01:26 of their stroke when they came in to ER
0:01:26 0:01:29 in hospital, and now this is the first time
0:01:29 0:01:31 I've seen them outpatient.
0:01:32 0:01:34 Patient's cardiologist was notified
0:01:35 0:01:39 in the hospital that he had a stroke, as well as the PCP
0:01:40 0:01:43 was informed when they were hospitalized.
0:01:43 0:01:45 Again, this is a follow-up outpatient visit
0:01:45 0:01:47 from that setting.
0:01:48 0:01:50 What current anticoagulant is this patient taking?
0:01:50 0:01:52 He's taking Xarelto.
0:01:55 0:01:57 Specific treatment characteristics.
0:01:57 0:02:01 Basically, cause he had AFib,
0:02:01 0:02:03 the hospital pharmacist thought
0:02:04 0:02:07 that was most the best, well-covered
0:02:07 0:02:09 on his insurance formulary.
0:02:15 0:02:18 This medication will not changed cause it was
0:02:18 0:02:20 started in the hospital.
0:02:20 0:02:21 I make that decision.
0:02:22 0:02:25 If I think he has had a recurrent ischemia on the Xarelto,
0:02:25 0:02:29 then I would potentially recommend a change.
0:02:29 0:02:32 And I would strongly influence anybody else
0:02:32 0:02:33 involved in their care about that,
0:02:33 0:02:35 but in this case that's not applicable.
0:02:35 0:02:37 Expectations for the patient.
0:02:37 0:02:41 He might have some residual arm spasticity,
0:02:41 0:02:46 and the next step, if that does develop, would be botox,
0:02:46 0:02:50 but at this point I'm giving him baclofen for that.
0:02:51 0:02:55 Patient number two yesterday I saw is
0:02:57 0:02:59 82-year-old white female.
0:03:02 0:03:05 Past medical history of hypertension, hyperlipidemia,
0:03:05 0:03:10 breast cancer, stems post-mastectomy,
0:03:14 0:03:16 osteoarthritis.
0:03:21 0:03:24 Internist is involved, although this was a follow-up
0:03:24 0:03:26 from her hospitalization.
0:03:31 0:03:35 Patient was taking Warfarin before this stroke, but it was
0:03:35 0:03:40 (mumbles) slightly subtherapeutic.
0:03:40 0:03:43 Consulted with her internists in the management
0:03:43 0:03:44 of this patient.
0:03:45 0:03:48 We have joined roles in deciding her anticoagulation,
0:03:48 0:03:51 which we opted to consider continue the Warfarin
0:03:51 0:03:53 with optimized the dosing.
0:03:56 0:04:00 We opted to do that instead of using a new anticoagulant
0:04:00 0:04:03 because of cost issues and also compliance, in that
0:04:03 0:04:08 we would be able to monitor compliance better with the labs.
0:04:09 0:04:13 Cardiologist was not really contacted; this was chronic
0:04:13 0:04:17 AFib, so no change there.
0:04:20 0:04:22 Cardiologist was informed by discharge summary that
0:04:22 0:04:24 patient had a stroke, as was the PCP
0:04:24 0:04:26 during the initial hospitalization,
0:04:26 0:04:29 which was about a month ago.
0:04:30 0:04:33 And current anticoagulant is Warfarin.
0:04:35 0:04:38 Again, treated because of the compliance and laboratory
0:04:38 0:04:40 monitoring issues, as well as cost.
0:04:41 0:04:45 We did not change. I did not opt to change the medication.
0:04:45 0:04:49 I would be mainly responsible for making the decision.
0:04:50 0:04:54 I would change it if I thought it was, basically,
0:04:55 0:04:58 not effective at therapeutic levels.
0:05:03 0:05:06 Informed the internist of that if I felt
0:05:06 0:05:07 we needed a change.
0:05:08 0:05:09 Expectations for the future.
0:05:09 0:05:11 Mild stroke. I don't think she'll have spasticity.
0:05:11 0:05:14 She is really done with her therapy,
0:05:14 0:05:16 and is able to return home.
0:05:16 0:05:19 I am returning this patient back to her internist to manage.
0:05:19 0:05:23 And I will just be available for follow-up if needed.
0:05:25 0:05:26 Third patient.
0:05:29 0:05:31 70-year-old white male.
0:05:33 0:05:38 Past medical history of hypertension, diabetes,
0:05:44 0:05:48 hyperlipidemia, prior CABG,
0:05:49 0:05:52 carotid endarterectomy, that was well over five
0:05:52 0:05:53 or six years ago.
0:05:55 0:05:57 I did not see this patient in the hospital.
0:05:57 0:06:00 He was hospitalized elsewhere for a left middle
0:06:00 0:06:02 cerebral artery infarct.
0:06:08 0:06:11 Did improve, went to rehab and home.
0:06:12 0:06:14 Been home about two weeks.
0:06:16 0:06:18 I talked to the internist briefly
0:06:18 0:06:21 about longer-term monitoring.
0:06:32 0:06:36 Patient was on Aspirin before and was discharged
0:06:36 0:06:39 on Eliquis, presumably for AFib.
0:06:44 0:06:47 At what point during the patient's stroke did I start?
0:06:47 0:06:50 This is a new patient, so only today.
0:06:51 0:06:52 Or did I say yesterday?
0:06:52 0:06:54 Whenever I saw him.
0:06:54 0:06:57 Did not inform a cardiologist that the patient
0:06:57 0:06:59 had a stroke, cause
0:07:02 0:07:05 at this point, again,
0:07:05 0:07:09 in the hospital, that was a different hospital.
0:07:09 0:07:11 Patient's PCP was already informed that he had a stroke
0:07:11 0:07:13 from that hospital.
0:07:13 0:07:15 Current anticoagulation.
0:07:15 0:07:17 Again, he's on Eliquis.
0:07:17 0:07:22 Presumably, the treatment was picked there due to the
0:07:22 0:07:26 AFib being documented or highly suspected.
0:07:28 0:07:30 I'm not sure what characteristics led them to that decision,
0:07:30 0:07:32 but I opted to continue it.
0:07:32 0:07:36 I have no reason to switch it at this time.
0:07:37 0:07:39 He's not had any intraischemic symptoms,
0:07:39 0:07:41 so, again,
0:07:41 0:07:43 I make that decision.
0:07:45 0:07:47 Expectations for the patient in the future.
0:07:47 0:07:50 I would like to, for sure, document AFib,
0:07:50 0:07:54 so I am going to ask their cardiology department to put
0:07:54 0:07:57 a long-term 30-day heart monitor on him.
0:07:59 0:08:02 And if he indeed does have (mumbles),
0:08:02 0:08:04 we'll likely continue the Eliquis.
0:08:04 0:08:07 If we don't, then we might need
0:08:07 0:08:10 to switch to antiplatelet therapy.
0:08:12 0:08:13 Alright.
0:08:15 0:08:18 For the last part, if you currently see sales
0:08:18 0:08:23 representatives. I don't see them on drop-in type thing,
0:08:23 0:08:25 only by invitation.
0:08:26 0:08:29 And I've not invited any some time.
