In permissive scheme permission to trip is sent from local end to remote end(s), i.e., protection at local end have detected a fault on the protected object.
The received signal(s) is combined with an overreaching zone and gives an instantaneous trip if the received signal is pre-sent during the time the chosen zone has detected a fault in forward direction.
The carrier received signal (CR) must be received when the overreaching zone is still activated to achieve an instantaneous trip. In some cases, due to the fault current distribution, the overreaching zone can operate only after the fault has been cleared at the terminal nearest to the fault.
There is a certain risk that in case of a trip from an independent tripping zone, the zone issuing the carrier send signal (CS) resets before the over-reaching zone has operated at the remote terminal.
To assure a sufficient duration of the received signal (CR), the send signal (CS), can be prolonged by a tSendMin reset timer. The recommended setting of tSendMin is 100 ms.

This Independent Contractor Agreement (“Agreement”) is made and entered into by and between XX, acting as a private individual (“CONTRACTOR”), as party of the first part, and YY, acting as a private individual (“PRINCIPAL”), as party of the second part, both parties hereto being hereinafter referred to collectively as the “PARTIES”. The Parties hereby agree as follows:
1. Subject of the Independent Contractor Agreement
1.1. CONTRACTOR shall perform the services (carry out the work) commissioned by PRINCIPAL within the scope and subject to the terms and conditions set forth in this Agreement, whereas PRINCIPAL shall accept and pay for the services (work) rendered.
1.2. CONTRACTOR’S performance shall be regulated by internal workplace policies and procedures.
1.3. Statements of Acceptance of Services Rendered (Work Performed) shall be issued to confirm that the services (work activities) commissioned have been completed by CONTRACTOR.
1.4. Statements of Acceptance of Services Rendered (Work Performed) shall be signed by the PARTIES once the services (work activities) have been delivered and shall form an integral part hereof.
1.5. Should PRINCIPAL justify its refusal to sign the Statements of Acceptance and accept CONTRACTOR’S services by presenting a list of discrepancies committed by CONTRACTOR when delivering the services (performing the work), CONTRACTOR is required to immediately eliminate the discrepancies cited by PRINCIPAL.
7. Force Majeure
7.1. The PARTIES shall not be liable for failure to perform under this Agreement in case of force majeure events.
7.2. The PARTY, which is unable to perform its obligations due to Force Majeure, is required to immediately notify the other PARTY under this Agreement accordingly.
8. Confidentiality Restrictions
8.1. Each of the PARTIES hereunder shall ensure the confidentiality of any technical, financial, commercial, or other information received from the other PARTY, as well as take all possible measures to prevent disclosure of information received in confidence in accordance with the law of Ukraine.
9. Other Provisions of the Agreement
9.1. This Agreement is executed in the English language in two (2) counterparts of equal legal effect, one counterpart for each PARTY.
9.2. All amendments and additions to this Agreement shall be valid only if made in writing and approved by both PARTIES.
9.3. Services (work activities) other than those outlined herein shall be performed on the basis of supplementary agreements.

Профілактика
Збудником гістоплазмозу є диморфний грибок Histoplasma capsulatum. Пацієнтам з вмістом CD4 клітин < 150 клітин/мкл слід уникати діяльності, асоційованої із збільшенням ризику інфікування в ендемічних вогнищах гістоплазмозу.
Вміст CD4 клітин < 150 клітин/мкл, наявність високого ризику зараження внаслідок професійної діяльності, або пацієнти, які мешкають в місцевості з дуже високою захворюваністю на гістоплазмоз.
Профілактика ітраконазолом є доцільною пацієнтам з вмістом CD4 клітин < 150 клітин/мкл, високим ризиком зараження внаслідок професійної діяльності, або пацієнтам, які мешкають в місцевості з дуже високою захворюваністю на гістоплазмоз (> 10 випадків/100 пацієнтів на рік).
Консультування ВІЛ-інфікованих щодо доцільності уникати роботи з ґрунтом, пов’язаної з пилоутворенням; чищення пташників, забруднених послідом; ворушіння гнізд, контамінованих послідом птахів або кажанів; участь в прибиранні, ремонті та знесенні старих будинків.
У пацієнтів з вмістом CD4 клітин < 150 клітин/мкл, з високим ризиком зараження внаслідок професійної діяльності, або пацієнтам, які мешкають в місцевості з дуже високою захворюваністю на гістоплазмоз (> 10 випадків/100 пацієнтів на рік):
• Ітраконазол, по 200 мг, п/о, щоденно.
Рекомендації для відміни первинної профілактики:
• Стабільний вміст CD4 клітин до рівня ≥ 150 клітин/мкл, завдяки АРТ.
Показання для поновлення вторинної профілактики:
• Вміст CD4 клітин < 150 клітин/мкл.
Діагностика
Діагноз гістоплазмозу встановлюється лікарем інфекціоністом закладу, що надає вторинну або третинну медичну допомогу. При підозрі на гістоплазмоз скерувати до лікаря інфекціоніста закладу, що надає вторинну або третинну допомогу.
Лікування
Специфічна терапія призначається інфекціоністом закладу, що надає вторинну або третинну медичну допомогу. Контроль за виконанням рекомендацій інфекціоніста щодо лікування.

Діагностика
Виявлення антигенів до Histoplasma в крові або сечі є чутливим методом швидкої діагностики дисемінованого гістоплазмозу та гострого легеневого гістоплазмозу, але нечутливим при хронічних формах легеневої інфекції. Застосування новіших методів аналізу для кількісного визначення дозволяє виявити антигени в сечі у 100%, а в сироватці крові - у 92% пацієнтів, хворих на СНІД та дисемінований гістоплазмоз. Виявлення антигенів в змивній рідині бронхів є корисним методом для діагностики легеневого гістоплазмозу. Гістологічне дослідження біоптатів уражених тканин дозволяє виявити характерні дріжджі в стадії брунькування, що забезпечує швидкий діагноз.
H. capsulatum висівають із зразків крові, кісткового мозку, респіраторного секрету та зразків інших уражених тканин у > 85% пацієнтів, хворих на СНІД та дисемінований гістоплазмоз, але для росту мікроорганізмів потрібно кілька тижнів. Серологічні тести пацієнтам, хворим на СНІД та дисемінований гістоплазмоз, є менш ефективні, ніж аналіз для виявлення антигенів, але є корисними у випадку відносно неушкодженої імунної системи та легеневого захворювання.
• Рентгенографія органів грудної порожнини.
• Бронхоскопія з біопсією.
• Посів зразків крові, кісткового мозку, респіраторного секрету та зразків інших уражених тканин.
• Комп'ютерна томографія легень.
• Визначення антигенів Histoplasma в сечі, сироватці крові, змивній рідині бронхів, лікворі.
Translation - English PROTOCOL GUIDELINES FOR THE TREATMENT OF HISTOPLASMOSIS IN HIV-INFECTED PATIENTS
FOR HEALTHCARE FACILITIES INVOLVED IN DELIVERING PRIMARY CARE
Prevention
Histoplasmosis is caused by the dimorphic fungus Histoplasma capsulatum. Patients with CD4 cell counts below 150 cells/µL should avoid activities associated with increased risk of infection in endemic histoplasmosis areas.
Key risk factors include CD4 cell counts below 150 cells/µL, high risk of occupational exposure or living in areas in which histoplasmosis is highly endemic.
Prophylaxis with itraconazole can be considered for patients with CD4+ counts below 150 cells/µL who are at high risk because of occupational exposure or who live in a community with a hyperendemic rate of histoplasmosis (>10 cases/100 patient-years).
Advising HIV-infected patients to avoid exposure creating dust when working with surface soil; cleaning chicken coops that are contaminated with droppings; disturbing areas contaminated with bird or bat droppings; cleaning, remodeling, or demolishing old buildings.
For patients with CD4+ counts below 150 cells/µL who are at high risk because of occupational exposure or who live in a community with a hyperendemic rate of histoplasmosis (>10 cases/100 patient-years):
• Itraconazole at 200 mg PO daily.
Recommendations for discontinuation of primary prophylaxis:
• Stable CD4 cell counts up to ≥ 150 cells/µL due to АRТ.
The diagnosis of histoplasmosis is made by the infectious disease physician of the healthcare facility delivering secondary or tertiary care. If a case of histoplasmosis is suspected, the patient is referred to the infectious disease physician of the healthcare facility delivering secondary or tertiary care.

Diagnosis
The detection of Histoplasma antigens in blood or urine samples is a sensitive rapid diagnostic technique for disseminated or acute pulmonary histoplasmosis. However, this method is not sensitive for chronic pulmonary infections. The latest diagnostic techniques provide quantification data and detect Histoplasma antigens in urine samples for 100% patients, and in blood serum samples – for 92% patients infected with HIV and disseminated histoplasmosis. Antigen detection tests using bronchial washings are helpful in the diagnosis of pulmonary histoplasmosis. Histopathologic examination of biopsy material from involved tissues demonstrates the characteristic budding yeast and can provide a rapid diagnosis.
H. capsulatum can be cultured from blood, bone marrow, respiratory secretions, or other involved sites in >85% of patients with AIDS and disseminated histoplasmosis, but organisms might require several weeks to grow. Serologic tests are less useful than antigen assays in AIDS patients with disseminated histoplasmosis, but might be helpful in patients with pulmonary disease who have relatively intact immune responses.

X показаны графики максимальных главных деформаций в осевой зоне заготовки в зависимости от номера прохода и степени деформации на каждом проходе при ковке двумя бойками, на РКМ, а также на FDFD с вытеснением и без вытеснения металла между бойками. Наибольшие значения главные деформации в осевой зоне заготовки достигают при ковке на FDFD с вытеснением металла между бойками, а минимальные значения – при ковке двумя бойками. При ковке на РКМ максимальные главные деформации в осевой зоне заготовки также превышают максимальные главные деформации при ковке двумя бойками, но не превышают значения главных деформаций при ковке на FDFD. Таким образом, процесс ковки на FDFD позволяет создать лучшие условия для деформационной проработки металла, по сравнению с ковкой двумя бойками. Fig. X shows graphs for maximum principal strains across the core zone of the workpiece as a function of the number of the pass and the degree of deformation per pass in a two-die forging process, during forging on an RFM, as well as in the case of applications involving the use of FDFDs (four-die forging devices) with and without displacing the metal into the area between the dies. The principal strains across the core zone of the workpiece are found to be highest for processes where FDFDs are used and the metal is displaced into the area between the dies), while such strains are lowest in two-die forging operations. In forging on RFMs, the maximum principal strains across the core zone of the workpiece are also higher than the maximum principal strains recorded during a two-die forging process, but they do not exceed the values of the principal strains in the case of forging applications using FDFDs. Thus, the use of FDFDs ensures more favorable conditions for deformation processing of metal, as compared to two-die forging. Large deformations across the core zone of the workpiece concurrent with additional shear strains in forging applications using FDFDs facilitate reduction in porosity, as well as the intensive crushing of non-metallic impurities, coarse carbides, and clusters thereof in the cast metal.
