Your preterm, otherwise healthy infant is being asked to participate in a research sub-study that is related to the main clinical study.
This form provides:
Information about the sub-study procedures so you can make an informed decision about allowing your infant to participate in the sub-study.
A description of how your infant’s health information will be used and disclosed in the sub-study.
A request for your permission for that use and disclosure.
WHY IS THIS SUB-STUDY BEING DONE?
You are being asked to provide sample(s) of DNA obtained from a cheek swab for analysis of your infant’s DNA.
The analysis may include reading its content, called its sequence.
DNA is the substance that carries genetic (inherited) information, which makes up your genes.
It serves as the “instruction book” for the cells that make up our bodies and help determine the characteristics of people (such as eye color).
This may include whether or not people are likely to get a certain disease, or how their disease may respond to drug treatment.
