Source text - English Fabry disease is an inborn error of metabolism that is caused by the deficiency of a-galactosidase A. The loss of a-galactosidase A leads to the progressive accumulation of neutral glycosphingolipids and a-galactosyl breakdown product in affected patients. The process leads to selective damage of renal glomerular and tubular epithelial cells, myocardial cells and valvular fibrocytes, neurons of the dorsal root ganglia and autonomic nervous system, and endothelial, perithelial and smooth muscle cells of blood vessels. The predominant accumulated substance is globotriaosylceramide (Gb3; also known as ceramidetrihexoside). Accumulation of Gb3 may lead to a range of symptoms, characterized initially by pain and angiokeratomas, which usually appear during childhood or adolescence. As the patient ages, various organs become affected by the progressive deposition of Gb3,
and death occurs at an average age of 41 years from renal, cardiac or cerebrovascular complications.
Affected individuals may suffer from chronic debilitating pain, fatigue, heat intolerance and a variety of gastrointestinal symptoms. The accumulating lipid in the kidney eventually leads to end stage kidney failure requiring dialysis and transplantation. Lipid accumulation in the
heart muscle results in an enlarged heart with left ventricular hypertrophy leading to significant morbidity and mortality. Similarly, cerebrovascular complications may develop during the third and subsequent decades, and strokes at a young are not uncommon in this
disease.
Translation - Russian Болезнь Фабри представляет собой врожденное нарушение метаболизма, вызванное дефицитом α-галактозидазы А. Отсутствие α-галактозидазы А приводит к прогрессирующему накоплению у больных нейтральных гликосфинголипидов и продуктов распада а-галактозы. Процесс ведет к селективному поражению эпителиальных клеток почечных клубочков и канальцев, клеток миокарда и фиброцитов клапанов сердца, нейронов спинномозговых узлов и вегетативной нервной системы, а также эндотелиальных клеток, перицитов и гладкомышечных клеток кровеносных сосудов. Преимущественно накапливающимся веществом является глоботриаозилцерамид (Gb3; также известный, как церамтидтригексоксид). Накопление Gb3 может вызвать ряд проявлений, изначально характеризующихся болью и ангиокератомами, которые обычно возникают в детском или подростковом возрасте.
С увеличением возраста при прогрессирующем накоплении Gb3 поражаются различные органы. Летальный исход наступает в среднем в возрасте 41 года в результате развития осложнений со стороны почек, сердца и развития цереброваскулярных осложнений.
Больные могут страдать от хронических изнуряющих болей, слабости, непереносимости повышенной температуры и спектра проявлений со стороны желудочно-кишечного тракта. Накопление липидов в почках, в конечном итоге, приводит к терминальной почечной недостаточности, требующей проведения диализа и трансплантации. Накопление липидов в миокарде приводит к увеличению сердца с гипертрофией левого желудочка, что ведет к значительному уровню заболеваемости и смертности. Аналогично этому, цереброваскулярные осложнения могут развиваться на протяжении третьего и последующих десятилетий жизни. При данном заболевании не являются редкостью инсульты в молодом возрасте.
