1. Purpose of the study
The purpose of this study pilot study is the assessment of safety, tolerability and efficacy of two escalating doses of THR-18 (0.25 and 0.5 mg/kg) to placebo when administered to patients suffering acute ischemic stroke that are treated with tissue plasminogen activator (tPA). In addition the pharmacokinetic blood levels of a THR-18 after a single intravenous injection will be investigated.
The test substance THR-18 is being developed for the treatment of acute ischemic stroke and is intended for use with thrombolitic treatment (tPA). In the course of this study you will receive either the test substance THR-18 or a placebo without active ingredients (“sham medication”).
A total of up to 22 healthy subjects may take part in this study.
If you receive the active test material (THR-18), based on data derived from animals, you may benefit as the test material was shown to improve the outcome and reduce the side effects (such as bleeding) of the thrombolitic treatment using tPA. If you receive the placebo treatment your participation in this study has no benefit for you, but will help to determine the safe dosage of this new treatment method that may benefit future patients. The test material was tested in healthy volunteers and this study is the first time it is tested in patients suffering from ischemic stroke.
2. Properties of the medication
THR-18 is a peptide composed of 18 amino acids (the building blocks of peptides) that has the ability to bind at the docking site of tissue plasminogen activator (tPA) and not to its catalytic site that is responsible for dissolving the clots. THR-18 is developed as an adjunct to tPA thrombolytic therapy in the treatment of acute ischemic stroke.
THR-18 is easily dissolved in isotonic mannitol and will be injected as I.V injection. THR-018 will be infused as a 10% bolus over 2 minutes (15 ml) followed by a 60 min infusion of the remaining dose (135 ml) in conjunction with tPA. THR-18 aims to address some of the tPA treatment limiting properties; short therapeutic window, limited response rate, relatively high re-occlusion rate and limited safety profile.
