1. Study title
Full title: A phase 2, open-label test-retest study to assess the reproducibility of quantitative measurements of 18F uptake by solid tumours using PET imaging following intravenous administration of AH111585 (18F) Injection.
Shortened title: A study of the reproducibility of tumour imaging using Positron Emission Tomography (PET) following administration of an imaging drug known as [18F]AH111585.
2. Introduction
You are being invited to take part in a research study being conducted at the Manipal Hospital because you have recently been diagnosed or suspected of having a tumour. Your participation in this study is voluntary, which means you can decide whether or not you want to be in the study. If you don’t want to be in the trial, you will not be prevented from receiving any medical care or other benefits that you are entitled to.
Before you agree to be in this study, it is important that you read the following information completely and ask as many questions as you feel is necessary of the study doctor or nurse to be sure you understand what you will be asked to do. This patient information form provides you with detailed information about the study and is part of the consent form that you sign.
Once you have read this information, understand the trial process, have had all your questions answered and still wish to be in the study, you will be asked to sign and date the patient Informed Consent Form at the end of this document. A copy of the signed Informed Consent Form will be provided to you.
3. What is the purpose of the study?
The goal of this clinical research study is to see how well the experimental "imaging" drug [18F]AH111585, can help researchers see your tumour and the cells that may grow around This drug contains an amount of radiation, which is detected by using a special type of camera called a Positron Emission Tomography (PET) scanner. The pictures taken can show where the drug goes to in the body. It is thought that that this drug will specifically show up tumour cells, so this may help the doctor to ‘see’ tumour cells in your body.
You will be given the drug on two different days and will receive a scan on both of these days. The pictures will be compared to see if a similar amount of drug has been taken up by the cells, and that the quality of the two pictures is the same. In addition, the safety of this drug will be studied.
This study is taking place at 2 research study centres; one in the United Kingdom and one in India, and approximately 45 patients will be entered into the study. This is a research study as [18F]AH111585 is not approved for commercial use. At this time it is being used for research purposes only.
4. What will happen to me if I take part?
If you decide to take part in the study, your involvement will only last about 7 weeks, up to 11 weeks maximum in total. You will be asked to read, sign and date the Informed Consent Form.
You will be required to come to the hospital for a visit during which the study will be explained to you and the doctor will decide whether you are eligible to take part. This is called the Screening visit.
You will then be required to visit the hospital for two scans which will be 3 to 8 days apart. Each of these scans will take about half a day to complete. You will also be required to visit the hospital 6 weeks after the second scan. This follow up visit may take up to 2 hours of your time
As part of the safety tests, samples of your blood will be taken. The equivalent of about 10-15 tablespoons of blood will be taken for testing, for a serum pregnancy test (if you are able to become pregnant) and to test if your body has any immune reaction to the drug.
If you take part in the study, more detail about what happens during each part of the study follows.
Screening Visit
• You will be asked to have a physical examination (a check of your general appearance, skin, neck, lungs, heart, abdomen, and a limited neurological examination) and a number of blood tests before you start the study. These will take about 2 hours of your time and will take place at the hospital.
• You will be asked to provide background information about yourself including your medical history and what medicines you are taking.
• You will have your height, weight, blood pressure, heart rate, breathing rate and body temperature measured.
• You will be asked to report if you have any unusual or unexpected feelings or symptoms.
• A small amount of blood will be taken.
• If you are female and are able to get pregnant, you will have a blood test to test whether you are pregnant or not.
Once your doctor has confirmed that you meet all the requirements and you agree to take part in the study he/she will arrange a date with you for the first scan to take place.
Day of First Scan
About an hour before you receive the drug, you will have some medical assessments and the following will happen:
• You will be asked some questions about your health and medicines that you take to see if there have been any changes since your last visit.
• Blood samples will be taken to test your blood.
• If you are female and are able to become pregnant you will be asked to provide a urine sample to confirm you are not pregnant (you must not be pregnant to continue).
• You will also have your heart checked and have blood pressure, heart rate, breathing rate and body temperature measured again and you will have the area where you will be injected examined.
For the injection of the drug, you will have a small plastic tube put in a vein in your arm (similar to that used to take blood), which will be used to give you the drug. It is possible you may have some bruising on your arm where the plastic tube is placed.
The dose of radioactivity from the study would be equivalent to around an additonal 10.4 years of normal background radiation that you would be exposed to in your day to day life
Five to ten minutes after you have received the drug, you will have your blood pressure, heart rate, breathing rate, and body temperature measured and your doctor will look at the area where the injection was made to check for any abnormal reactions.
You will have to wait about 35 minutes after you have received the drug before the scan begins. During this time you will have your blood pressure, heart rate breathing rate, and body temperature measured again and will be prepared for the imaging scans.
You will then be asked to lie on a bed and a scan of your whole body will be taken using a Computed Tomography (CT) scanner to ensure that the position of your body is correct for the PET scan.
Scans of your whole body will then be taken using a PET scanner. You will be asked to lie very still on your back for about 75 minutes.
After the PET imaging is finished, you will be asked how you are feeling, have a short physical exam, the doctor or nurse will examine the site where you were injected and you will have another ECG. The follow up tests may take around 2 hours to complete after which you will be able to go home.
You must tell your doctor or nurse if you have any unusual or unexpected feelings or symptoms at any time before or after your injection.
Day of Second Scan
Your second day of imaging will be between three and eight days later and will follow exactly the same schedule as before and you will have the same set of tests as before
6 week follow up visit
Your doctor will ask you to come in for one last visit about six weeks after you have had the second scan and a final blood sample taken which will be analysed at a central laboratory to test if your body has had an immune reaction to the drug.
5. What do I have to do?
You must tell the study nurse or doctor if you feel unwell or have any symptoms of any type when you are in the study.
If you have private medical insurance you should check with the company before taking part in the study to ensure that taking part will not affect the insurance.
6. What are the possible risks / discomforts?
While taking part in this study, you may suffer side effects. These side effects will vary from person to person, however you may experience no effects at all. There is a slight possibility of infection or bruising from the injection.
The dose of radioactivity from the study would be equivalent to around an additional 10.4 years of normal background radiation that you would be exposed to in your day to day life
The drug [18F]AH111585 is in a solution containing up to 7% ethanol.
The PET scanner may cause you to feel ‘closed in’ while lying in the scanner. However, the scanner is open at both ends and an intercom allows you to talk with doctors and staff. You will need to keep very still under the scanning camera for about 75 minutes.You may find keeping still under the scanner uncomfortable. If you feel ill or anxious during scanning, doctors and/or technicians will be able to give reassurance or the scanning can be stopped if required.
7. Harm to the unborn child
a) Women of childbearing potential
There might be risks to your unborn child that we are not aware of if you are pregnant or if you become pregnant during the trial.
Because of these risks, you must not participate in this trial if you are pregnant, or plan to become pregnant during the research study period, or are breast-feeding a child.
If you are a woman of childbearing potential:
By signing this consent form, you confirm to the best of your knowledge that you are not pregnant now and you do not intend to become pregnant during the trial.
A pregnancy test will be done to confirm that you are not pregnant before your participation in this trial.
You must use at least on one of the following forms of birth control during this trial:
a) Be sexually inactive
b) Are surgically sterile (e.g tubal ligation, hysterectomy)
c) Intrauterine device in place for at least three (3) months
d) Adequate barrier methods with spermicide (e.g diaphragm)
e) Surgical sterilisation of your partner (e.g vasectomy) or
f) Hormonal methods including oral contraceptives (e.g the pill).
This should continue for at least a couple of days after the research study has finished.
If at any time during the study you think you might be pregnant, or later learn that you were pregnant during the study, you must contact the study doctor immediately for further instructions about your participation in this study and what to do next.
b) For Men
You should inform the study doctor immediately if your partner becomes pregnant following administration of the study drug.
8. What are the possible benefits of taking part?
There are no medical benefits to you for your participation in this study. You may feel satisfaction that you have participated in a research study that may help to lead to new information related to the use, and safety of the drug in a condition like yours.
9. What are the alternatives for diagnosis or treatment
This study is designed to test a drug for diagnosis, and is not a cancer treatment. If you do choose to take part in this study, it will not affect the standard of care for cancer that you would receive normally.
10. Compensation
If you should suffer an injury or illness caused by taking part in this study GE Healthcare will compensate you for the injury or illness and also for the cost of any medical treatment you need as a result. Such compensation will be at least as favourable as we are required to make in accordance with the national regulations, for example the Association of the British Pharmaceutical Industry (ABPI).
11. Expenses and payments
It is important that you understand that you are not being paid to be a participant in this study and there will not be any additional cost to you as a result of being in the study. You will be compensated for travel expenses for the required visits made to you at INR 1000/- per visit. The amount will be prorated for visits performed.
12. What will happen if I don’t want to continue with the study?
After signing this form you are still free to withdraw at any time and without giving a reason. A decision to withdraw at any time will not affect the standard of care you receive. If you decide to withdraw, please tell your study doctor and they will discuss with you the best way to stop your participation in this study. Your study doctor or the study sponsor may remove you from the study if they think it is in your best interest, if you do not follow the trial instructions, or for administrative reasons.
If you withdraw from the study we may go on to use the personal information already collected about your health for the purposes mentioned in this Patient Information Sheet and Informed Consent (see section 3: ‘What is the purpose of the study’), however no new information will be collected and added to existing data or database. Any stored blood or tissue samples that can still be identified as yours will be destroyed if you wish.
13. Will my taking part in the study be kept confidential?
By signing this consent form and participating in this study, you agree to allow the study doctor to make your medical records available to GE Healthcare (sponsor of this trial), and to the use and transfer of your confidential subject information associated with this study and any data or results of your participation in the study.
Confidentiality of your medical records will be maintained to the extent permitted by law. However, GE Healthcare expects to disclose and use these as described below. If you join the study, relevant parts of your medical records and the data collected for the study will be looked at by authorised persons from GE Healthcare. They may also be looked at by representatives of regulatory authorities from India and other countries, and by authorised people from the Ethics Committee to check that the trial is being carried out correctly. All of these people will have a duty of confidentiality to you as a study participant and nothing that could reveal your identity will be disclosed outside the hospital, other than as may be required by law.
All information which is collected about you during the course of the study will be kept strictly confidential. Your name and address will be removed from any information about you which leaves the Manipal Hospital, and replaced by a unique number so that you cannot be identified from the information itself. Your study doctor will keep a list with your name and your unique number. Authorised persons described in the previous paragraph that look at your medical records and trial information will have access to this list.
Data collected during the study may be transferred to GE Healthcare businesses and consultants or regulatory authorities outside of the India. Some countries may not have laws which protect your privacy to the same extent as local and international laws. However, GE Healthcare will take all reasonable steps to protect your privacy to the higher standards.
The procedures for handling, processing, storage and destruction of your data are compliant with local and international laws.
14. What will happen to the results of the research study?
The findings of this study will be recorded and analysed for research purposes. All information collected during the study will be kept in a written report, where you will not be mentioned by name, only by your initials, date of birth and a unique study number assigned only to you. If the results are published, your identity will not be revealed.
The sponsor may combine the health information obtained from study subjects’ study records from this and other research studies in a database and use this for further medical research purposes. All subjection information that is collected and combined will be anonymous, which means that the information cannot be linked to a specific study subject.
15. What if relevant new information becomes available?
Your doctor will be able to inform you of any significant new findings discovered during the course of the clinical trial.
16. What happens when the research stops
Once the study has finished you will continue to be referred for the appropriate standard of care throughout the study. There will be no changes to the normal routine care that you receive.
17. What will happen to any samples I give?
Your blood samples (identified by a code only) will be sent to the hospital laboratory for analysis. Blood samples will be analysed for clinical chemistry, haematology, coagulation measurements and possibly prenancy if appropriate, a full description of measurements can be given by the study doctor. Blood samples to test for an immune reaction to the study drug will be sent a company specializing in these tests. In addition a urine pregnancy tests will also be performed if appropriate. These samples will not be retained for longer than necessary to perform the analysis. You have the right to ask for updated information on the data that are recorded about you, to require correction of errors and to know who will be responsible for keeping the data and have access to them. Any such questions should be addressed to the study doctor.
18. Who is organising and funding the research?
This study is sponsored and being paid for by General Electric Company, through its division known as GE Healthcare and its affiliates. Manipal Hospital will be paid for the additional work that is involved in including you in this study.
19. Who has reviewed the study?
This study was given a favourable ethical opinion for conduct at the Manipal Hospital by the Ethics Committee of Manipal Hospital & Manipal Heart Foundation
The study will also be carried out in accordance with the Declaration of Helsinki, a worldwide requirement on the ethical and medical considerations of medical research in humans.

3. अध्य यन का उद्देश्या क्या है?
इस नैदानिक शोध अध्यंयन का उद्देश्य् यह देखना है कि प्रयोगात्मरक [18F]AH111585 औषधि का ‘प्रतिबिंब’ आपके ट्यूमर और इसके आसपास पैदा होने वाली कोशिकाओं को दिखाने में शोधकर्ताओं की कितनी अच्छीं तरह से मदद कर पाता है। इस औषधि में विकिरण की कुछ मात्रा होती है जिसे पॉजीट्रॉन एमीशन टोमोग्राफी (पीईटी) स्कैहनर नामक विशेष प्रकार के कैमरे से देखा जा सकता है। खींची गई तस्वी1रें दिखा सकती हैं कि शरीर के किस भाग में औषधि पहुंचाई गई है। ऐसा माना गया है कि यह औषधि विशेष तौर पर ट्यूमर की कोशिकाओं को दिखाती है जिससे आपके शरीर में ट्यूमर की कोशिकाओं को ‘देखने’ में चिकित्सतक की मदद की जा सके।
आपको यह औषधि दो अलग-अलग दिन दी जाएगी और इन दोनों दिन आपका स्कैयन लिया जाएगा। खींची गई तस्वीऔर की तुलना इसलिए की जाएगी जिससे पता लगाया जा सके कि कोशिकाओं द्वारा औषधि की समान मात्रा ग्रहण की गई है और दोनों तस्वीीरों की गुणवत्ता भी समान है। इसके अतिरिक्त , इस औषधि की सुरक्षा का भी अध्यहयन किया जाएगा।
यह अध्यधयन दो शोध केंद्रों पर किया जा रहा है जिसमें एक यूनाइटेड किंग्डअम में है और दूसरा भारत में है। इस अध्यधयन में लगभग 45 रोगियों को भर्ती किया जाएगा। वास्तडव में यह एक शोध अध्यायन है क्यों कि [18F]AH111585 को वाणिज्यिक उपयोग हेतु स्वी कृत नहीं किया गया है। इस समय इस औषधि का उपयोग केवल शोध प्रयोजनों के लिए ही किया जा रहा है।
4. यदि मैं इस अध्ययन में भाग लेता हूं तब मुझे क्याी होगा?
यदि आप इस अध्ययन में भाग लेने का निर्णय लेते हैं, तब आप इसमें केवल 7 सप्ताह तक ही बने रहेंगे जो कुल मिलाकर अधिकतम 11 सप्ताह का हो सकता है। आपसे सूचित सहमति फार्म को पढने और दिनांक सहित हस्ताक्षर करने के लिए कहा जाएगा।
आपको मुलाकात के लिए अस्पजताल आना होगा जिसमें आपको अध्यंयन के बारे में बताया जाएगा और आपका चिकित्सलक ही निर्धारित कर पाएगा कि क्याब आप इसमें भाग लेने के पात्र हैं अथवा नहीं। इसे स्क्री निंग मुलाकात कहा जाता है।
इसके बाद आपसे दो स्कैसन कराने के लिए अस्पकताल में आने के लिए कहा जाएगा और यह दोनों स्कैतन 3 से 8 दिन के बीच किए जाएंगे। एक स्कैिन पूरा करने में कम से आधा दिन लग जाएगा। दूसरे स्कै्न के 6 सप्ताबह बाद आपसे अस्परताल में आने के लिए कहा जाएगा। इस अनुवर्ती मुलाकात में आपको 2 घंटे का समय लग सकता है।
सुरक्षा परीक्षणों के अंग के रूप में आपके रक्ति के नमूना लिया जाएगा। गर्भधारण परीक्षण हेतु आपके रक्ता की 10-15 चम्ममच बूंद निकाली जाएंगी (यदि आप गर्भवती बनने के योग्यह हैं)। रक्तस के नमूना का दूसरा कारण औषधि के प्रति आपके शरीर की प्रतिजनक क्षमता का मूल्यांंकन करना भी है।.
यदि आप इस अध्य्यन में भाग लेते हैं तब अध्यकयन के प्रत्येाक खंड में क्याह होगा, की अधिक जानकारी निम्नयलिखित होगी:-
स्क्रीीनिंग मुलाकात
• आपका शारीरिक परीक्षण किया जाएगा (आपकी सामान्य शारीरिक संरचना, त्व चा, गर्दन, फैफड़ों, हृदय, आमाश्यष और सीमित न्यूसरोलॉजीकल जांच) तथा अध्यखयन आरंभ होने से पूर्व कुछ रक्तफ परीक्षण। इन सबमें आपका लगभग 2 घंटे का समय लगेगा और यह कार्य अस्परताल में ही किया जाएगा।
• आपसे स्व यं के बारे में आपकी पृष्ठाभूमि से संबंधित सूचना उपलब्धत कराने के लिए कहा जाएगा जिसमें आपके चिकित्सीमय इतिहास के अतिरिक्तं आप कौनसी दवाईयों का सेवन कर रहे हैं की जानकारी भी शामिल होगी।
• आपकी लंबाई, वजन, रक्ताचाप, हृदयगति, श्वारसगति तथा शरीर का तापमान मापा जाएगा। .
• यदि आपको किसी प्रकार की परेशानी होती है अथवा आशा के विपरीत महसूस करते हैं अथवा लक्षण दिखाई देते हैं तब आपको सूचना देने के लिए कहा जाएगा।
• रक्तत की थोड़ी सी मात्रा निकाली जाएगी।.
• यदि आप महिला हैं और गर्भवती होने में सक्षम हैं तब आपसे रक्त का नमूना लिया जाएगा जिससे पता लगाया जा सके कि आप गर्भवती हैं अथवा नहीं।
जब आपका चिकित्सेक पुष्टि कर लेता है कि आपने सभी अर्हरताओं को पूरा कर लिया है और आप इस अध्यपयन में भाग लेने पर सहमत हैं तब वह पहला स्कैेन कराने के लिए आपके लिए एक तारीख निश्चित करेंगे।.
पहले स्कैिन का दिन
औषधि लेने से लगभग एक घंटा पहले आपको कुछ चिकित्सीहय मूल्यां कनों से गुजरना होगा जो निम्न प्रकार होगे:-
• आपसे आपके स्वाएस्य् आ और पहले से ली जाने वाली दवाईयों के बारे में कुछ प्रश्नन पूछे जाएंगे जिससे ज्ञात हो सके कि आपकी पिछली मुलाकात के बाद कोई परिवर्तन दृष्टिगोचर तो नहीं हुए हैं।
• आपके रक्तस की जांच के लिए रक्त का नमूना लिया जाएगा।
• यदि आप महिला है और गर्भाधारण करने में सक्षम हैं तब आपसे गर्भवती न होने की पुष्टि करने के लिए पेशाब का नमूना लिया जाएगा (अध्येयन में भागीदारी जारी रखने के लिए आपका गर्भवती न होना अनिवार्य है)
• आपको अपने हृदय की जांच, रक्ते चाप, हृदयगति, श्वारसगति तथा शारीरिक तापमान का मापन कराना होगा और आपके शरीर में जहां इजेक्शंन दिया गया है उसकी जांच की जाएगी।
औषधि के इंजेक्शृन के लिए आपकी बाजू की नस में एक छोटी सी प्लाखस्टिक ट्यूब लगाई जाएगी (ठीक उसी तरह की ट्यूब जिसका उपयोग रक्त निकालने के लिए करते हैं) और इस ट्यूब का उपयोग आपको औषधि देने के लिए किया जाएगा। जिस स्थान पर प्लाएस्टिक ट्यूब लगाई गई है, उससे हो सकता है आपकी बाजू में निशान पड़ जाए।
अध्यनयन में रेडियोधर्मिता की खुराक सामान्या पृष्ठ भूमि वाले विकिरण की उस मात्रा के बराबर होगी जो आप अपने दैनिक जीवन में का लगभग 10.4 वर्ष तक ग्रहण करते हैं।
औषधि लेने के पांच से दस मिनट बाद आपका रक्त चाप, हृदयगति, श्वांसगति तथा शरीर का तापमान लिया जाएगा और आपका चिकित्सदक उस स्थारन की जांच करेगा जिस स्थािन पर किसी असाधारण प्रतिक्रिया की जांच करने के लिए इंजेक्शन लगाया गया था।
स्कै न प्रक्रिया आरंभ होने से पूर्व औषधि लेने के बाद आपको लगभग 35 मिनट तक प्रतीक्षा करनी होगी। इस दौरान आपका रक्तरचाप, हृदयगति, श्वागसगति तथा शरीर का तापमान दोबारा लिया जाएगा और स्कैजन प्रतिबिंब के लिए तैयार किया जाएगा।
इसके बाद आपको बिस्तार पर लेट जाने के लिए कहा जाएगा और कम्यू टिड टोमोग्राफी (सीटी) स्कैकनर के उपयोग द्वारा आपके संपूर्ण शरीर का स्कैेन लिया जाएगा ताकि सुनिश्चित किया जा सके कि आपके शरीर की स्थिति पीईटी स्कै न के लिए सही है।
इसके बाद पीईटी स्कैंनर के उपयोग द्वारा आपके संपूर्ण शरीर का स्कैयन लिया जाएगा। लगभग 75 मिनट तक आपको पीठ के बल बिस्तर पर लेटे रहने के लिए कहा जाएगा।
पीईटी प्रतिबिंब पूरा होने के बाद आपसे पूछा जाएगा कि आप कैसा महसूस कर रहे हैं और उसके बाद आपकी छोटी सी शारीरिक जांच की जाएगी। चिकित्सगक अथवा नर्स उस स्था न की जांच करेगें जिस स्थाीन पर आपको इंजेक्श न दिया गया था और उसके बाद ईसीजी की एक और खुराक दी जाएगी। अनुवर्ती परीक्षणों को पूरा करने में लगभग 2 घंटे का समय लग सकता है जिसके बाद आपको घर जाने की अनुमति दे दी जाएगी।
यदि आपको इंजेक्शदन से पूर्व अथवा बाद में किसी भी समय परेशानी अथवा अटपटा महसूस होता है अथवा कोई असामान्यइ लक्षण प्रतीत होते हैं तब उसके बारे में आपको अपने चिकित्सनक अथवा नर्स को अवश्यै बताना चाहिए।
दूसरे स्कैएन का दिन
आपके दूसरे दिन का प्रतिबिंब पहले प्रतिबिंब से तीसरे और आठवें दिन के बीच लिया जाएगा और इसके लिए पहले जैसी प्रक्रिया ही अपनाई जाएगी और उसके जैसे ही परीक्षण किए जाएंगे।
6 सप्ता ह बाद अनुवर्ती मुलाकात
दूसरा स्कै्न होने के लगभग छ: सप्ताीह बाद आपका चिकित्साक आपको आखिरी बार अस्पसताल आने के लिए कहेंगे और अंतिम बार नमूने के तौर पर आपका रक्त लिया जाएगा जिसे जांच हेतु किसी केंद्रीय प्रयोगशाला में यह जांच करने के लिए भेजा जाएगा कि क्याक आपके शरीर में औषधि की प्रतिजनक क्षमता है अथवा नहीं।
4. मुझे क्याक करना होगा?
अध्य.यन के दौरान यदि आप अच्छा महसूस नहीं करते हैं अथवा आपको किसी तरह का कोई लक्षण दिखाई देता है तब आपको अपने शोध चिकित्सअक अथवा नर्स को अवश्ये बताना चाहिए। यदि आपने प्राइवेट चिकित्सात बीमा करवा रखा है तब अध्यअयन में भाग लेने से पूर्व आपको उस कंपनी से पूछ लेना चाहिए कि इसमें भाग लेने से आपके बीमा पर कोई प्रभाव तो नहीं पड़ेगा।
5. इसके कौन-कौन से संभावित जोखिम/बैचेनी हैं?
अध्य.यन में भाग लेते समय आपको दुष्प्रनभाव हो सकते हैं। ये दुष्प्र भाव एक व्यूक्ति से दूसरे व्यनक्ति में अलग-अलग हो सकते हैं, तथापि, ऐसा भी हो सकता है कि आपको दुष्प्रषभाव बिल्कु ल न हों। इंजेक्श न लगाने के कारण आपको संक्रमण अथवा बैचेनी की थोड़ी संभावना हो सकती है।

[18F]AH111585 नामक औषधि एक घोल है जिसमें 7 प्रतिशत ईथानोल का मिश्रण है।
स्कै1नर पर लेटते समय पीईटी स्कै नर से आपको लगे कि आपकी आंखे बंद हो रही है। तथापि, स्कै नर दोनों सिरों पर खुला रखा जाता है और इंटरकॉम की मदद से आप चिकित्स क और स्टॉीफ से बात कर सकते हैं। स्कैंनिंग कैमरा के नीचे आपको लगभग 75 मिनट तक एकदम शांत लेटे रहना होगा। स्कैतनर के नीचे लेटे रहना संभवत: आपको आरामदायक नहीं लगे। स्कै निंग के दौरान यदि आप स्वरयं को बीमार अथवा परेशान महसूस करते हैं तब चिकित्सपक और/अथवा तकनीशियन आपकी हौसला अफ्जाई करेंगे या जरूरत पड़ने पर हो सकता है कि स्कैचनिंग रोक दी जाए।
6. अजन्में् बच्चेग को नुकसान
क) गर्भवती महिलाएं
यदि आप गर्भवती हैं अथवा यदि आप अध्यदयन के दौरान गर्भवती हो जाती हैं तब आपके अजन्में बच्चेअ को जोखिम हो सकता है। यदि आप गर्भवती हैं अथवा शोध अध्य यन की अवधि के दौरान गर्भवती होना चाहती हैं अथवा स्त नपान कराने वाली माता हैं, तब आपको इस अध्यनयन में भाग नहीं लेना चाहिए।

यदि आप ऐसी महिला हैं जिसमें गर्भधारण करने की क्षमता है:
इस सहमति फार्म पर हस्तासक्षर करने के बाद आप अपने विवेकानुसार पुष्टि करती हैं कि आप इस समय गर्भवती नहीं हैं और अध्ययन के दौरान आप गर्भवती नहीं होना चाहती हैं।
इस अध्य यन में भाग लेने से पूर्व गर्भवती होने की पुष्टि करने के लिए आपका गर्भाधारण परीक्षण किया जाएगा। अध्यंयन के दौरान निम्नरलिखित में से कम से कम परिवार नियोजन का कोई एक साधन आपको अवश्य। अपनाना चाहिए:-
क) संसर्ग अर्थात यौन संपर्क में निष्क्रिय रहें। ख) सर्जीकल रूप से सुरक्षित हैं (अर्थात ट्यूबल लिगेशन, हिस्टेारेक्टोसमी) ग) कम से कम तीन (3) महीनों तक इंटरायूटरीन डिवाइस का उपयोग घ) स्पसर्मीसाइड (अर्थात डॉयफ्राम) ड) आपके साथी की सर्जीकल स्टे रीलाईजेशन (नसबंदी) अथवा च) ओरल गर्भ निरोधक गोलियों सहित हारमोनल तरीके (अर्थात गोलियां)
शोध अध्ययन पूरा होने के बाद भी उपरोक्तम का उपयोग कम से कम कुछ दिन अवश्यो जारी रहना चाहिए। यदि अध्ययन के दौरान किसी भी समय आपको लगता है कि आप गर्भवती हो गई हैं तब इस अध्यययन में आपकी भागीदारी के बारे में तथा आपको आगे क्याा करना होगा, की जानकारी के लिए अपने अध्यएयन चिकित्सगक से तुरंत संपर्क करना चाहिए। .
ख) पुरूषों के लिए
यदि आपकी महिला साथी अध्यियन औषधि का सेवन करने के परिणामस्वगरूप गर्भवती हो जाती है तब आपको अपने अध्यमयन चिकित्सथक को तुरंत सूचित कर देना चाहिए।
7. इस अध्यसयन में भाग लेने के क्यास संभावित लाभ हैं?
इस अध्य्यन में आपकी भागीदारी का कोई चिकित्सीिय लाभ नहीं हैं। आप यह सोच कर संतुष्टय हो सकते हैं कि आपने एक ऐसे शोध अध्य यन में भाग लिया है जिससे संभवत: आप जैसी दशा वाले किसी व्यकक्ति को औषधि के सेवन और सुरक्षा से संबंधित नई सूचना प्राप्त करने में मदद मिल सके।
9. रोग की पहचान अथवा उपचार के क्याू विकल्पि हैं?
इस अध्य यन को रोग की पहचान हेतु औषधि की जांच करने के लिए डिजाइन किया गया है और यह कैंसर का उपचार नहीं है। यदि आप इस अध्यऔयन में भाग लेना नहीं चाहते हैं तब आपके इस कार्य से कैंसर की सामान्य रूप से मिलने वाली देखभाल का स्तमर प्रभावित नहीं होगा।
10. क्षतिपूर्ति
इस अध्ययन में भाग लेने के कारण यदि आपको कोई चोट लग जाती है तब जीई हेल्थकेयर उस चोट अथवा बीमारी तथा इसके परिणामस्वरूप चिकित्सीय उपचार पर खर्च होनी वाली समस्त राशि की क्षतिपूर्ति करेगी। इस प्रकार की क्षतिपूर्ति देश के नियमों के अनुसार ही देय होगी, उदाहरण के तौर पर ब्रिटिश फार्मासियुटिकल इंडस्ट्री (एबीपीआई)
11. खर्च व भुगतान
आपके लिए यह समझना अनिवार्य है कि आपको इस अध्यरयन में एक प्रतिभागी के रूप में भुगतान नहीं किया जा रहा है और इस अध्यययन में भाग लेने के परिणामस्वकरूप आपको किसी प्रकार का कोई अतिरिक्तो खर्च नहीं उठाना पड़ेगा। आपको 1000रू0 प्रति मुलाकात के हिसाब से उन सभी मुलाकातों के लिए भुगतान किया जाएगा जिनके लिए आप अध्य यन से संबंधित जांच व परीक्षण के लिए अस्पबताल आएंगे। यह राशि आपकी मुलाकातों के लिए दी जाएगी।
12. यदि मैं इस अध्यीयन में अपनी भागीदारी जारी नहीं रख पाता हूं तब क्याा होगा?
इस फार्म पर हस्तासक्षर करने के बाद आप कारण बताए बिना किसी भी समय इस अध्यबयन से अपना नाम वापस ले सकते हैं। किसी भी समय अपना नाम वापस लेने के आपके निर्णय से आपको दी जा रही चिकित्सीसय देखभाल पर कोई प्रभाव नहीं पड़ेगा। यदि आप अध्य यन से अपना नाम वापस लेने का निर्णय लेते हैं तब आप अपने अध्यायन चिकित्सीक को इसकी सूचना दें और वह इस अध्य यन में आपकी भागीदारी को बंद करने के सबसे अच्छेन तरीके के बारे में चर्चा करेंगे। यदि आपका अध्ययन चिकित्सगक अथवा अध्य्यन का प्रायोजक समझते हैं कि इस अध्यकयन में आपका बने न रहना ही आपके लिए हितकर होगा और आप अध्य्यन के अनुदेशों का पालन नहीं करते हं अथवा प्रशासनिक कारणों की वजह से उन्हेंो ऐसा लगता है तब वे आपको इस अध्यययन में भाग लेने से रोक सकते हैं।
यदि आप अध्यययन से अपना नाम वापस लेते हैं तब रोगी सूचना पत्रक और सूचित सहमति (खंड 3 देखें ‘अध्यधयन का उद्देश्य क्याह है) में उल्लिखित आपके स्वा स्य्ड के बारे में पहले से एकत्रित की गई व्यीक्तिगत सूचना का उपयोग कर सकते हैं। तथापि, न तो किसी नई सूचना को हम अपने वर्तमान डाटाबेस में एकत्रित करेंगे और न ही उसे उसमें शामिल करेंगे।
13. क्याऔ इस अध्यशयन में मेरी भागीदारी को गोपनीय रखा जाएगा?
इस सहमति फार्म पर हस्ताइक्षर करने तथा इस अध्यगयन में भाग लेते हुए आप अपने अध्य यन चिकित्सवक को इस बात की अनुमति देने पर अपनी सहमति देते हैं कि वह आपके चिकित्सीमय अभिलेखों को जीई हेल्थिकेयर (अध्यीयन के प्रायोजक) को प्रस्तकत कर सकते हैं और इस अध्यभयन से संबंधित आपकी गोपनीय सूचना तथा अध्यअयन में भाग लेने के परिणामस्वहरूप किसी भी आंकड़े अथवा परिणामों का उपयोग अथवा हस्तांंतरण कर सकते हैं।
आपके चिकित्सीयय अभिलेखों की गोपनीयता को कानून के अंतर्गत ही रखा जाएगा। तथापि, जीई हेल्थाकेयर निम्नचलिखित के अनुसार इनका खुलासा और उपयोग कर सकती है। यदि आप अध्यथयन में शामिल होते हैं तब अध्यकयन के लिए एकत्रित किए गए आपके चिकित्सीउय अभिलेखों और आंकड़ों के संबंधित खंड को जीई हेल्थ्केयर द्वारा प्राधिकृत व्यकक्तियों द्वारा देखा जा सकेगा। इन्हेंब भारत तथा अन्यं देशों की नियामक प्राधिकरणों के प्रतिनिधियों तथा नीति समिति के प्राधिकृत व्याक्तियों द्वारा अध्ययन के ठीक प्रकार से संपन्न होने के लिए भी देखा जा सकेगा। इन सभी व्यिक्तियों का यह कर्तव्य होगा कि वे अध्यतयन प्रतिभागी के रूप में आपके प्रति गोपनीयता का निर्वाह करेंगे और अस्पिताल के बाहर आपकी पहचान तब तक सार्वजनिक नहीं करेंगे जब तक कानून द्वारा ऐसा करना अपेक्षित नहीं हो।
अध्यनयन के दौरान आपके बारे में एकत्रित की गई समस्तत सूचना को गोपनीय रखा जाएगा। आपके नाम और पते को ऐसी किसी भी सूचना से हटा दिया जाएगा जिसे आपने मणीपाल अस्पसताल में रख छोड़ा हो अथवा उसे किसी विशेष संख्याच से बदल दिया जाएगा ताकि अमुक सूचना से आपकी पहचान नहीं की जा सके। आपका अध्यवयन चिकित्स क आपके नाम और आपकी विशेष संख्याआ की सूची अपने पास रखेगा। इस सूची में आपके चिकित्सीषय अभिलेखों और अध्यपयन सूचना को देखने का अधिकार उपरोक्त वर्णित पैराग्राफ में उल्लिखित प्राधिकृत व्ययक्तियों को ही दिया जाएगा।
अध्यियन के दौरान एकत्रित किए गए आंकड़ों को जीई हेल्थ केयर के बिजनेस और परामर्शदाताओं अथवा भारत के बाहर नियामक प्राधिकरणों को हस्तां तरण किया जा सकता है। कुछ देशों के पास ऐसे कानून नहीं हो सकते हैं जो स्थानीय एवं अंतरराष्ट्रीएय कानून के समान आपकी गोपनीयता की रक्षा कर सकें। तथापि, जीई हेल्थरकेयर सर्वोच्च् सीमा तक आपकी गोपनीयता की रक्षा करने के लिए सभी पर्याप्तप कारणोचित कदम उठाएगी।

14. शोध अध्ययन के परिणामों का क्याा होगा?
इस अध्यायन के परिणामों को शोध प्रयोजनों के लिए अभिलेखित एवं विश्लेोषित किया जाएगा। अध्यईयन के दौरान एकत्रित की गई समस्त सूचना को एक लिखित रिपोर्ट का रूप दिया जाएगा जिसमें आपके नाम का उल्ले ख नहीं किया जाएगा, उसमें केवल आपके आद्यक्षर, जन्मि तिथि तथा आपको दी गई विशेष अध्ययन संख्याए द्वारा ही पहचाना जा सकेगा। यदि परिणामों को प्रकाशित किया जाता है तब भी आपकी पहचान विदित नहीं हो पाएगी।
प्रायोजक अध्य।यन में भाग लेने वाले रोगी से प्राप्त स्वाास्य् भ संबंधी सूचना, अध्यअयन के रिकार्ड तथा अन्ये प्रकार के शोध अध्यायनों को एक डाटाबेस में सुरक्षित रखा जाएगा और उनका उपयोग भावी चिकित्सीचय शोध प्रयोजनों के लिए किया जाएगा। एकत्रित एवं समावेशित समस्त सूचना अज्ञात होगी अर्थात उसे किसी विशेष अध्योयन के रोगी से जोड़ा नहीं जाएगा।
15. यदि कोई नई सूचना मिल जाए तब क्याए होगा?
आपका चिकित्स क नैदानिक ट्रॉयल के कोर्स के दौरान मिलने वाली किसी महत्वरपूर्ण उपलब्धि के बारे में बताएगा।.
16. जब शोध पूरा हो जाएगा तब क्या् होगा
अध्यगयन पूरा होने पर आपको अध्यॉयन की संपूर्ण अवधि के दौरान उपयुक्तम मापदंडों के लिए भेजा जाता रहेगा। आपको मिलने वाली नियमित देखभल में किसी प्रकार का कोई परिवर्तन नहीं किया जाएगा।
17. मैं जो नमूना दूंगा उनका क्याभ होगा?
कोड द्वारा पहचाने जाने वाले आपके रक्तल के नमूने को अस्पपताल की प्रयोगशाला में जांच के लिए भेजा जाएगा। रक्तप के नमूनों को नैदानिक जांच, हेमोटोलॉजी, कोगुलेशन मापन तथा उपयुक्त हुआ तो गर्भधारण के लिए भी परीक्षण किया जाएगा। मापन का संपूर्ण ब्यौोरा अध्यकयन चिकित्सजक द्वारा ही दिया जा सकता है। अध्यभयन औषधि की प्रतिजनक प्रतिक्रिया की जांच के लिए रक्तच के नमूनों के परीक्षण हेतु किसी विशेषज्ञ कंपनी के पास भेजा जाएगा। इसके अतिरिक्तक यदि उपयुक्तक हुआ तो गर्भाधारण की जांच के लिए पेशाब का नमूना भी लिया जाएगा। इन नमूनों को जांच के लिए नियत अवधि से अधिक समय तक सुरक्षित नहीं रखा जाएगा। आपके बारे में जितने आंकड़े लिए गए हैं उनके बारे में आपको प्रश्न पूछने, अशुद्धियों को ठीक करने तथा यह जानने का भी अधिकार है कि इन आंकड़ों को रखने के लिए कौन जिम्मे्दार होगा और इन्हें देखने का किसे अधिकार होगा। इस प्रकार के किसी भी प्रश्नन को अध्य्यन चिकित्सोक के पास भेजा जाएगा।
18. इस शोध का आयोजन वित्तपोषण कौन कर रहा है?
इस अध्य यन का प्रायोजन और वित्तपोषण जनरल इलेक्ट्रिक कंपनी की इकाई जीई हेल्थ केयर तथा इसकी संबद्ध इकाईयों के माध्ययम से किया जा रहा है। मणीपाल अस्पकताल को उसके अतिरिक्तक कार्य के लिए भुगतान किया जाएगा और उसके इस अतिरिक्तम कार्य में इस अध्यहयन में आपका शामिल होना भी है।
