Landseer Road Surgery
This letter is to inform you that your health care services at the Landseer Road surgery will be delivered by a different practice from 1 January 2013. You do not need to take any action and there will be no interruption to service.
Management of your services will be provided by the Chesterfield Drive Practice, Ipswich. You will also see a new GP. The staff who are currently working at Landseer Road will remain and this will ensure that you will still receive the best level of care and high quality services.
The Chesterfield Drive Practice is a large surgery in north west Ipswich run by five doctors. The surgery has delivered services from the current site since 1991 although it has many years’ experience of caring for patients in the town. The doctors and staff offer good quality, comprehensive services with excellent access arrangements for patients. The doctors are looking forward to working at Landseer Road and meeting the patients registered at the surgery.
You will be able to continue to access the same services at Landseer Road Surgery in the normal way. However if you do have any questions please contact Patient advice and Liaison Service (PALS) on 0800 389 6819.

To consider whether Mahin, Tahia and the unborn baby are at significant risk of harm due to concerns about Ms Rahman’s mental health and the parents’ marital discord impacting on the care and stimulation of the children.
Natalie Searle reported that there are current concerns about the children’s diet and nutrition. The children have not been observed eating meals. They have often been seen eating crisps and biscuits. The parents say that the children eat meals but this has not been observed despite lengthy visits at different times of the day.
Mr Choudhury explained that he cooks most of the time because Ms Rahman usually does not feel like cooking. The family eats rice, vegetables, chicken and other meat. The children eat meals with the parents at lunch time and in the evening.
FARGANA RAHMAN
• Mental Health
Dr Cheryl Colquhoun, Psychiatrist, reported that Ms Rahman experiences auditory hallucinations. She hears the voice of a priest suggesting that the paternal extended family plans to kill or sell her. Ms Rahman gives a paranoid explanation for everything that Mr Choudhury does, for example, if he goes out he is making plans to kill her.
A diagnosis of Delusional Disorder was made in 2011. This diagnosis was changed to Paranoid Schizophrenia with hallucinations.
Three different medicines have been tried but have not been effective. It was unclear whether medication was not working because Ms Rahman was not complying with taking her medication, or whether she has treatment resistant schizophrenia.
Ms Rahman is meant to take her medication twice a day but she sometimes says that she only takes it once a day. A depot injection could be considered but this would not be indicated during pregnancy.
Ms Rahman’s condition is chronic and it is therefore not advised to try new medications when she is pregnant.
Professionals are concerned about the arguing between the parents and the impact of this on the children.
Helen Belmore expressed the view that Mr Choudhury needs to be the main carer for the children until Ms Rahman is well. He needs to take the children with him if he is going out, unless Mahin is meant to be at pre-school.
It is clear that Ms Rahman’s mental health is impacting on her ability to parent the children. However, what is not clear is how much Mr Choudhury can protect the children and meet their needs whilst they remain living together and while he is her carer also.
A high level of support will be needed to see what changes can be made by both parents. There is a risk of ongoing significant harm.
CONFERENCE DECISION
Child Protection Plan for Mahin Choudhury, Tahia Rahman and unborn baby Rahman, in the category of Neglect.
CHILD PROTECTION PLAN
AIM: Mahim and Tahia need to be protected from significant emotional or physical harm through neglect of their care. They also need to be protected from being exposed to ongoing conflict between their parents and the impact of their mother’s mental health.
The initial focus of the Core Group is to ensure that the children’s basic care needs are met. There after the parents accessing community resources to improve their parenting skills. The Core group will need to prioritise the work.
1. Mr Choudhury is not to leave Tahia or Mahin in the care of Mrs Rahman for longer than one to two hours. This should be only occasional when Mrs Rahman is alert and coping with their care. Core Group to monitor.
Outcome: Tahia and Mahin will be safely supervised and they will not feel anxious.
By Whom and When: Mr Choudhury, ongoing.
2. There is a need for a high level of support and monitoring for the children and the recommendation from the conference is that the children will need to be seen at least twice per week by a child care professional and one visit per week by a health visitor or social worker. Core Group to agree when this can be reduced.
Outcome: The parents’ ability to meet the children’s basic care needs will be clear.
By Whom and When: Core Group, ongoing.
3. The Lead Social Worker is to liaise with the Community Mental Health Team following Mrs Rahman’s appointment with Dr Roberts on 17.12.12
Outcome: The Core Group (including the parents) will be clear about the outcome of the assessment, the treatment plan and the level of ongoing contact with Mrs Rahman by the mental health services.
By Whom and When: The lead Social Worker by 21.12.12
4. A pre-birth planning meeting must be held by 15.02.12. This may be held at a Core Group meeting or a CPA review of Mrs Rahman. It will need to include the lead Social Worker, Health Visitor, community Midwife and the CP hospital Midwife.
Outcome: The Core Group (including the parents) will be clear about the plan following the baby’s birth, the level of monitoring by whom and the hand over procedures.
By Whom and When: The lead Social Worker by 15.02.13.
5. The Family Safeguarding Service (FSS) are to provide parenting intervention to:
• Improve the routines for meals, daily care including bathing, and other basic needs.
• Improve the parents understanding of the need to play and stimulate the children with activities and suitable materials / toys.
• Identify follow on work for the children’s centre.
Outcome: The parents’ basic care of Mahin and Tahia will improve.
By Whom and When: FSS to begin at first Core Group
6. Mrs Rahman is to attend all her mental health appointments including her sessions with the psychologist.
Outcome: Mrs Rahman will demonstrate that she is taking responsibility to achieve improvements in her mental health so that she can meet the children’s needs.
By Whom and When: Mrs Rahman, ongoing.
7. Mahin is to attend pre-school for all four sessions each week.
Outcome: Mahin will have the opportunity to make as much progress with his learning and development as possible.
By Whom and When: Mr Chouhury and Mrs Rahman, ongoing.
8. When the work of the FSS is completed, Mrs Rahman and Mr Choudhury are to attend the Ark Children's Centre with Tahia once a week when Mahin is at pre-school. The parents are to attend one other session each week with Tahia and Mahin. Mr Choudhury is encouraged to take Tahia out on shopping trips.
Outcome: Mahin and Tahia will have improved levels of stimulation for their development. Mrs Rahman and Mr Choudhury will have the opportunity to socialise with other adults and have parenting support.
By When and By Whom: Mrs Rahman and Mr Choudhury, ongoing until the review conference.
9. The Home-Start volunteer will continue to support the family once a week. The parents are encouraged to attend the weekly Home-Start support group.
Outcome: When the work of the FSS is completed the parents will have ongoing support to access services and put what parenting skills they have learnt into practice.
By Whom and When: Home Start, ongoing until the review conference.
10. The Lead Social Worker will invite Paradigm Housing to a Core Croup or meet with them separately to discuss the current situation for the family, in terms of issues with neighbours, possible eviction, and a planned move to another area.
Outcome: The family will not experience harassment and in the longer term they will be able to move to an area where there is a Bangladesh community and gain more support from the community.
By Whom and When: The lead Social Worker, issues of harassment to be acted on by 21.12.12.
11. The Health Visitor is to visit at least monthly to support and monitor the children’s health and development.
The Health Visitor will liaise with the Paediatrician about the current investigations into Mahin’s developmental delay and ask the Paediatrician to send copies of the report / assessment to the lead Social Worker.
Outcome: The children’s health and development will be monitored and the parents will have an opportunity to demonstrate what they are doing well and what they are learning.
By Whom and When: Health Visitor, monthly.
12. The Nursery Nurse will attend the next CDC appointment for Mahin with the parents.
13. If Mrs Rahman’s mental health further deteriorates with aggressive or frightening behaviour, or there is an escalation of domestic violence, the social work are expected to consider the safety of the children in the home and what additional safeguards can be provided by the extended family and seek legal advice if necessary

মিজ রহমানের মানসিক স্বাস্থ্য এবং শিশুদের যত্ন ও প্রণোদনায় বিরূপ প্রভাব বিস্তারকারী পিতামাতার দাম্পত্য কলহ যার কারণে মাহিন, তাহিয়া ও জঠরস্থ শিশু রহমান বিশেষ বিপদের ঝুঁকিতে রয়েছে কিনা তা বিবেচনায় আনতে।
নাতালিয়ে সার্লে রিপোর্টে উল্লেখ করেছেন যে বর্তমান উদ্বেগের বিষয় হচ্ছে শিশুদের খাদ্য তালিকা ও পুষ্টি। এই শিশুদের খেতে (আহার গ্রহণ করতে) দেখা যাচ্ছে না। তাদেরকে প্রায়ই বিস্কুট ও শুকনো ও মচমচে(চিপ জাতীয়) খাবার খেতে দেখা যায়। অভিভাবকগণ বলে থাকেন, শিশুরা খাচ্ছে তবে দিনের বিভিন্ন সময়ে দীর্ঘ পরিদর্শন করেও তা প্রত্যক্ষ করা যাচ্ছে না।
মি. চৌধুরী ব্যাখ্যা দিয়েছেন যে মিজ রহমানের রান্নার ইচ্ছে হয় না বলে তিনিই বেশিরভাগ সময় রাঁধেন। পরিবারটি ভাত, শাকসবজি, মুরগী ও অন্যান্য মাংস খায়। শিশুরা তাদের অভিভাবকদের সাথে দুপুরে ও সন্ধ্যায় খেয়ে থাকে।

• মানসিক স্বাস্থ্য
ড. শেরিল কোলকুহৌন, মনোচিকিৎসক, জানান যে মিজ রহমান শ্রবণজনিত ভ্রমজটিলতায় ভুগছেন। তিনি এক পাদ্রীকে বলতে শোনেন, পৈত্রিক সূত্রে এক আত্মীয় তাঁকে মেরে ফেলতে বা বিক্রি করে দিতে চায়। মিজ রহমান সব কিছুর ভুল ব্যাখ্যা করেন (মিছে সন্দেহবাতিকে ভোগেন) যেমন ধরুন মি. চৌধুরী যখনই বাইরে যান তখনই তাকে(মিজ রহমানকে) হত্যা করার পরিকল্পনা আঁটেন।
2011 সালে মতিভ্রমজনিত মনোবিকলন সনাক্ত হয়।পরে তা দৃষ্টিবিভ্রমসহ সন্দেহজনিত স্কিতসফ্রেনিয়ায় (শিজোফ্রেনিয়া) রুপান্তরিত হয়। বিভিন্ন ওষুধের মাধ্যমে সারানোর চেষ্টা চলে তবে তা ফলদায়ক হয়নি। এটা স্পষ্ট হয়নি যে ওষুধ-পথ্য কাজ করেনি কারণ তিনি ঠিকমত ওষুধ খাননি নতুবা তিনি চিকিৎসারোধী স্কিতসফ্রেনিয়ায় ভুগছেন।
মিজ রহমানকে দিনে দুইবার ওষুধ-পথ্য খেতে বলা হয় কিন্তু তিনি মাঝে মাঝে বলে থাকেন তিনি দিনে একবার ওষুধ সেবন করেন। ডিপো ইনজেকশনের কথা ভাবা যেত কিন্তু গর্ভাবস্থায় এটাও দেয়া যাবেনা।
মিজ রহমানের অবস্থা দীর্ঘস্থায়ী এবং নতুন কোন ওষুধ সেবনের পরামর্শ দেয়া যাচ্ছেনা কেননা তিনি গর্ভবতী।
অভিভাবকদের মাঝে দাম্পত্য কলহ ও সন্তানদের উপর এর বিরূপ প্রভাব নিয়ে চিকিৎসকেরা উদ্বিগ্ন।
হেলেন বেলমোর মত প্রকাশ করেন যে মি. চৌধুরীর শিশুদের প্রধান অভিভাবক হওয়া প্রয়োজন যতক্ষণ না মিজ রহমান সুস্থ হন। তিনি বাইরে গেলে শিশুদের তাঁর সাথে নিয়ে যাওয়া দরকার, যে পর্যন্ত না মাহিন প্রি-স্কুলের উপযোগী হয়।
এটা স্পষ্ট যে মিজ রহমানের মানসিক স্বাস্থ্য তাঁর শিশুর অভিভাবকসুলভ ক্ষমতার উপর বিরূপ প্রভাব ফেলছে।যাহোক, এটাও স্পষ্ট নয়, মি. চৌধুরী তাঁর শিশুদের কতটা সুরক্ষা দিতে পারেন বা প্রয়োজন মেটাতে পারেন যখন তারা একত্রে থাকেন আবার যেখানে তিনি মিজ রহমানেরও সেবাশূশ্রূষাকারী।
মাতাপিতা উভয়ের মধ্যে কতটা পরিবর্তন এসেছে তা নিরূপণে উঁচু পর্যায়ের সহায়ক প্রয়োজন। কেননা এতে বিশেষ বিপদের ঝুঁকি রয়েছে।

অবহেলিত শিশুদের ক্যাটাগরীতে মাহিন চৌধুরী, তাহিয়া রহমান এবং জঠরস্থ শিশু রহমানের জন্য শিশু সুরক্ষা পরিকল্পনা।

লক্ষ্য: মাহিন ও তাহিয়াকে তাদের প্রতি অবহেলার মাধ্যমে বিশেষ আবেগ বা শারীরিক ভীতি প্রদর্শণের থেকে সুরক্ষা দিতে হবে। তাদেরকে অভিভাবকদের মধ্যেকার ঝগড়া-ঝাঁটি থেকেও দূরে রাখতে হবে। এছাড়া তার মায়ের মানসিক স্বাস্থ্যের প্রভাব থেকে সুরক্ষার প্রয়োজন।
শিশুদের মৌলিক সেবাযত্নের চাহিদা মিটছে কিনা তা নিশ্চিত করাই কোর গ্রুপের প্রাথমিক কাজ। এরপর অভিভাবকগণ তাঁদের অভিভাবকসুলভ দক্ষতা বাড়াতে কম্যুনিটি রিসোর্সে প্রবেশাধিকার নেবেন। কোর গ্রুপ প্রয়োজনে অগ্রাধিকারভিত্তিক কাজের শ্রেণীবিন্যাস করবেন।
1. মি. চৌধুরী তাহিয়া বা মাহিনকে এক থেকে দুই ঘণ্টার বেশি মিসেস রহমানের কেয়ারে রেখে যাবেন না। যখন মিসেস রহমান সতর্ক এবং তাদের লালন-পালনে পারঙ্গম হবেন তখনই কেবল এটি ঘটবে। কোর গ্রুপ পর্যবেক্ষণে থাকবে।
ফলাফল: তাহিয়া এবং মাহিনকে নিরাপদে তত্বাবধান করা যাবে এবং তারা উদ্বিগ্ন হবে না।
কাদের দ্বারা এবং কখন: মি. চৌধুরী,চলমান।
2. শিশুদের উচ্চ পর্যায়ের সহায়তা ও পর্যবেক্ষণ জরুরী এবং সম্মেলন থেকে সুপারিশ এসেছে যে সপ্তাহে অন্তত দুই বার পেশাদার শিশু লালন-পালনকারী শিশুদের দেখাশোনা করবে।এবং স্বাস্থ্য পরিদর্শক বা সমাজকর্মী সপ্তাহে অন্তত একবার তাদের পরিদর্শনে যাবেন। কোর গ্রুপ জানাবে এরূপ তত্বাবধানএর মাত্রা কখন কমান যাবে।
ফলাফল: শিশুদের মৌলিক সেবাযত্নের চাহিদা মেটানোর ক্ষমতা সুস্পষ্টভাবে নির্ধারণ করা যাবে।

3. 17.12.12 তারিখে মিসেস রহমানের ডাক্তার রবার্টসকে নিয়োগের পরে মুখ্য সমাজকর্মী কম্যুনিটি মানসিক স্বাস্থ্য দলের সাথে লিয়াঁজো রক্ষা করবেন।
ফলাফল: কোর গ্রুপ (মাতাপিতাসহ) এসেসমেন্ট, ট্রিটমেন্ট পরিকল্পনা ও মানসিক স্বাস্থ্য কর্মিদের সাথে মিসেস রহমানের যোগাযোগের ফলাফল সম্পর্কে পরিষ্কার ধারণা পাবে।
কাদের দ্বারা এবং কখন: মুখ্য সমাজকর্মী 21.12.12 তারিখের মধ্যে।
4. 15.02.12 তারিখের পূর্বেই জন্ম-পূর্ব পরিকল্পনা বৈঠক অনুষ্ঠিত হবে।মিসেস রহমানের CPA পর্যালোচনা বা কোর গ্রুপের সভায় এই বৈঠক হতে পারে। বৈঠকে মুখ্য সমাজকর্মী, স্বাস্থ্য পরিদর্শক, কম্যুনিটি ধাত্রী এবং সিপি হাসপাতালের ধাত্রীদের ডাকা যেতে পারে।
ফলাফল: কোর গ্রুপ (মাতাপিতাসহ) শিশুর জন্মের পরবর্তী পরিকল্পনা, কাদের দ্বারা পর্যবেক্ষণ হবে আর তারমাত্রা ও কার্যধারার কৌশল(উপকরণ) সম্পর্কে পরিষ্কার ধারণা পাবে।
কাদের দ্বারা এবং কখন: মুখ্য সমাজকর্মী,15.02.13 তারিখের মধ্যে।
5. ফ্যামিলি সেফগার্ডিং সার্ভিস (FSS) অভিভাবকসুলভ হস্তক্ষেপ করবে:
• খাবার, দৈনন্দিন সেবাযত্নসহ অন্যান্য মৌলিক চাহিদা পূরণের রুটিন উন্নত করতে।
• যুতসই জিনিস বা খেলনা দিয়ে শিশুদের প্রণোদনাদান ও খেলার প্রয়োজনীয়তাবোধ অভিভাবকদের মধ্যে প্রবল করা।
• শিশুদের সেন্টারে পরবর্তী কার্যধারা চিহ্নিত করবে।
ফলাফল: মাহিন ও তাহিয়ার জন্য অভিভাবকের মৌলিক সেবাযত্নের মানোন্নয়ন ঘটবে।
কাদের দ্বারা এবং কখন: প্রথম কোর গ্রুপ হিসেবে FSS শুরু করবে।
6. মিসেস রহমান মনোবিজ্ঞানীর সাথে তাঁর বৈঠকসহ অন্যান্য সব মানসিক স্বাস্থ্য বিষয়ক সাক্ষাতকারে হাজির থাকতে হবে।
ফলাফল: মিসেস রহমান তাঁর মানসিক স্বাস্থ্য উন্নয়নে দায়িত্ব সচেতন হচ্ছেন তা প্রদর্শন করা যাতে বোঝা যায় তিনি শিশুদের চাহিদা পূরণ করতে সক্ষম।
কাদের দ্বারা এবং কখন: মিসেস রহমান, চলমান।
7. প্রি-স্কুল পর্যায়েই প্রতি সপ্তাহের চারটি অধিবেশনের সবগুলোতেই মাহিনকে পর্যবেক্ষণে রাখতে হবে।
ফলাফল: মাহিন এতে করে শিক্ষণ ও বিকাশে যথাসম্ভব বেশি সুযোগ পাবে।
কাদের দ্বারা এবং কখন: মি. চৌধুরী ও মিসেস রহমান, চলমান।
8. যখন FSSএর কাজ সমাপ্ত হবে, মিসেস রহমান ও মি. চৌধুরী তখন সপ্তাহে একবার তাহিয়াকে নিয়ে আর্ক চিল্ড্রেন’স সেন্টারে যাবেন, এসময় মাহিন প্রি-স্কুলে থাকবে। অভিভাবকদের তাহিয়া ও মাহিনকে প্রতি সপ্তাহে আরো একটি সেশনে সঙ্গ দিতে হবে।মি. চৌধুরী তাহিয়াকে নিয়ে বাজারে কেনাকাটায় যাবেন।
ফলাফল: মাহিন এবং তাহিয়া তাদের বিকাশে উন্নততর প্রণোদনা পাবে। মিসেস রহমান ও মি. চৌধুরীও অন্যান্য পরিণত ছেলেমেয়ের সাথে মেলামেশার ও অভিভাবকদের সহায়তার সুযোগ পাবে।
কাদের দ্বারা এবং কখন: মি. চৌধুরী ও মিসেস রহমান, পর্যালোচনা সম্মেলন পর্যন্ত চলমান।
9. হোম-স্টার্টের স্বেচ্ছাসেবী পরিবারটিকে প্রতি সপ্তাহে একবার সহায়তা দিতে শুরু করে। অভিভাবকদের সাপ্তাহিক হোম-স্টার্ট সহায়তা গ্রুপে হাজিরা দিতে অনুপ্রেরণা দেয়া হয়।
ফলাফল: যখন FSSএর কাজ সমাপ্ত হবে অভিভাবকগণ সার্ভিসে প্রবেশাধিকার পেতে চলমান সহায়তা লাভ করে এবং অর্জিত অভিভাবকসুলভ দক্ষতা কার্যে পরিণত করে।
কাদের দ্বারা এবং কখন: হোম স্টার্ট, পর্যালোচনা সম্মেলন পর্যন্ত চলমান।
10. মুখ্য সমাজকর্মী কোর গ্রুপকে প্যারাডাইম হাউজিংএ আমন্ত্রণ জানায় বা আলাদাভাবে তাদের প্রত্যেকের সাথে পরিবারের বর্তমান অবস্থা, পড়শিদের সাথে সমস্যা, সম্ভাব্য উচ্ছেদ, এবং অন্যত্র পরিকল্পিত অপসারণ নিয়ে আলোচনা করে।
ফলাফল: পরিবারটি হয়রানির শিকার হবে না এবং তারা বাংলাদেশ কম্যুনিটি যেখানে রয়েছে সেখানে সরে যেতে সমর্থ হবে এবং ঐ কম্যুনিটির আরো সম্ররথন লাভ করবে।
কাদের দ্বারা এবং কখন: মুখ্য সমাজকর্মী, হয়রানির বিষয়টি 21.12.12 তারিখের মধ্যে অনুষ্ঠিত হবে।
11. স্বাস্থ্য পরিদর্শক শিশুদের স্বাস্থ্য ও বিকাশে সহায়তা ও পর্যবেক্ষণ করতে মাসে অন্তত একবার পরিদর্শন করবেন।
স্বাস্থ্য পরিদর্শক মাহিনের বিকাশে বিলম্বের কারণ তদন্তে নিয়োজিত শিশুরোগ বিশেষজ্ঞের সঙ্গে লিয়াঁজো রাখবেন এবং শিশুরোগ বিশেষজ্ঞকে তাঁর রিপোর্ট বা এসেসমেন্টের কপি মুখ্য সমাজকর্মীর কাছে পাঠাতে বলবেন।
ফলাফল: শিশুদের স্বাস্থ্য ও বিকাশ পর্যবেক্ষণ করা হবে এবং তারা কতটা ভাল করছে এবং কি শিখছে অভিভাবকগণ তা প্রদর্শণ করার সুযোগ পাবেন।
কাদের দ্বারা এবং কখন: স্বাস্থ্য পরিদর্শক, মাসওয়ারী।
12. নার্সারী নার্স পরবর্তী CDC সাক্ষাৎকারে অভিভাবকসহ মাহিনের সাথে কথা বলবেন।
13. যদি মিসেস রহমানের মানসিক স্বাস্থ্য আগ্রাসী বা ভীতিকর আচরণের মাধ্যমে আরো অবনতি ঘটে, গৃহ নির্যাতন আরো বেড়ে যায়, তখন সমাজকর্মীরা বাড়িতে শিশুদের নিরাপত্তা বিবেচনা করে এবং সম্প্রসারিত পরিবারের মাধ্যমে আরো বাড়তি নিরাপত্তা যোগান দেয়ার কথাও চিন্তা করে। এছাড়া প্রয়োজনে আইনগত পরামর্শ চায়।

This compact is an agreement between you, your Offender Manager and the Cambridgeshire and Peterborough Probation Trust (CPPT). This compact sets out your rights and responsibilities during your time on Licence or serving a Community Order or Suspended Sentence Order.
What you can expect from us
• To see you on time for an appointment, or explain any delay to you
• To treat you in a fair and respectful manner
• To work with you to identify your reasons for getting into trouble and ways to help you stay out of trouble
• To respond to your needs and work with you to help you reduce any risks you pose, whatever your background
• To explain and share information with you about your progress
• To treat information we have about you with confidentiality, where appropriate. However, if you share information about your offending or risk to others, which causes us concern, we have a responsibility to share that information with other agencies; including the Police and Social Services.
Diversity
CPPT is committed to valuing and respecting differences between individuals.
If you have concerns or questions about race, religion, culture, gender, sexual orientation, child care or other issues, please discuss this as soon as possible with your Offender Manager.
Complaints
If you have any complaints about CPPT please discuss with your Offender Manager in the first instance. They will be able to give you a complaints leaflet that tells you about our complaints procedure.
What we expect from you
• Turn up for appointments and be on time
• Arrive with a clear head, not under the influence of alcohol or drugs
• Actively take part
• Provide good reason with proof, for any failure to attend
• Treat Probation staff and other people you meet during your sentence with respect
• Let Probation staff visit your home as required
• Agree to information about you being given to other agencies to help you address your crime-related needs
Tell your offender manager at once ;
- if you can’t keep an appointment
- if you plan to change your address or phone number
- if you are having a problem with any part of your sentence
What happens if you don’t comply?
If you break the rules of your order or licence;
• You may be returned to court and/or sent to prison
• You may have to comply with more requirements
• Your requirements may be made harder
• You may have to pay the cost of any breach hearing at court

• যেখানেই সম্ভব আপনার ব্যাপারে গোপনীয়তা রক্ষা করতে। তবে, পুলিশ ও সোশ্যাল সার্ভিস সহ অন্যান্য এজেন্সীর সাথে দায়িত্ব ভাগাভাগি করবো যদি আপনি অন্য কারো সাথে তথ্য আদান-প্রদান করে আইন বা হুমকির সৃষ্টি করছেন বলে আমদের মনে হয়।
দায়বদ্ধতা
CPPT অন্যের মতামতকে শ্রদ্ধা ও মূল্য দিতে দায়বদ্ধ।
বর্ণ, ধর্ম, সংস্কৃতি, লিঙ্গ, সেক্সুয়াল রীতি, শিশুর যত্ন বা অন্য যে কোনও বিষয়ে আপনার উদ্বেগ বা প্রশ্ন থাকলে, অনুগ্রহ করে আপনার অফেন্ডার ম্যানেজারের সাথে আলোচনা করুন।
অভিযোগ
CPPTর ব্যাপারে আপনার কোনো অভিযোগ থাকলে প্রথমেই আপনার অফেন্ডার ম্যানেজারের সাথে আলোচনা করুন। তারা আপনাকে অভিযোগ পত্র সরবরাহ করবে যাতে অভিযোগ করার নিয়মাবলী রয়েছে।
আমরা আপনার নিকট কি প্রত্যাশা করি
• অ্যাপয়েন্টমেন্ট নিন ও সঠিক সময়ে হাজির হউন
• মদে বা ড্রাগে আচ্ছন্ন না হয়ে পরিচ্ছন্নভাবে আসুন
• সক্রিয় ভাবে অংশ নিন
• হাজিরা দিতে ব্যররথ হলে প্রমাণ সহকারে যথার্থ কারণ সরবরাহ করুন
• প্রোবেশন স্টাফ ও অন্যান্যের সাথে সন্মান বজায় রেখে কথা বলুন
• প্রোবেশন স্টাফকে প্রয়োজন অনুযায়ী আপনার বারি পরিদর্শন করতে দিন
• অপরাধ সংক্রান্ত প্রয়োজনে সহায়তা করতে অন্যান্য এজেন্সীকে তথ্য দিতে সম্মত থাকুন।
অফেন্ডার ম্যানেজারকে সঙ্গে সঙ্গে বলুন :
- অ্যাপয়েন্টমেন্ট রক্ষা করতে না পারলে
- আপনার ঠিকানা বা ফোন নাম্বার পরিবর্তন হলে
- শাস্তির কোনো অংশ নিয়ে আপনি সমস্যায় পড়লে
মেনে না চলতে পারলে কী হবে?
আদেশ বা লাইসেন্সের কোনো বিধি ভাংলে;
• আপনাকে কোর্টে বা জেলে ফেরত পাঠানো হতে পারে
• আরো বেশি শর্তা মানতে হতে পারে
• শর্তগুলো কঠোর করা হতে পারে
• আদালতের শুনানিকালে শর্ত উপেক্ষা করলে অধিক জরিমানা গুণতে হতে পারে
