Source text - English JX-594 is a replication-competent, transgene-expressing therapeutic vaccinia
virus derived from the commonly used Wyeth vaccine strain (Dryvax®, Wyeth
Laboratories). Three genetic modifications are included in JX-594:
1. thymidine kinase (TK) gene deactivation,
2. granulocyte macrophage colony stimulating factor (GM-CSF) gene insertion
under control of the synthetic early-late promoter, and
3. lac-Z gene insertion under control of the p7.5 promoter.
JX-594 is shipped and handled as a Biosafety Level 2 (BSL-2) infectious substance.
Phase 1 Primary Objectives:
Determine the maximally-tolerated dose (MTD) or maximum feasible dose (MFD) of JX-594 administered by 5 IV infusions
Determine the MTD or MFD of JX-594 administered by 5 IV infusions in combination with irinotecan
Determine the safety of JX-594 administered by 5 IV infusions followed by up to 3 IT JX-594 boosts alone and in combination with irinotecan
Phase 1 Secondary Objectives:
Determine radiographic response rate (response evaluation criteria in solid tumors [RECIST] and Choi response criteria)
Determine disease control rate (DCR) at 8 weeks according to standard RECIST criteria
Determine progression-free survival (PFS) according to standard RECIST
Determine quality of life (QoL)
Determine overall survival time
Translation - Korean JX-594는 복제 가능, 이식 유전자 발현의 치유용 우두 바이러스로서 일반적으로 사용되는 Wyeth 백신균주에서 추출된다(Dryvax®, Wyeth Laboratories).

Source text - Korean 세 번째는 malignant transformation을 예방하기 위해서 initial wound를 chronic unstable scar 상태로 변하지 않게 치료하는 것이다. burn wound 에서 deep 2nd degree 이상의 wound는 spontaneous healing 보다는 깨끗하게 excision 후 skin graft 를 시행하여야 한다.
일단 진단이 되면 철저한 preoperative evaluation은 필수적이다.
주변 lymph node 의 physical exam 뿐만 아니라 sonography를 통해서 lymph node metastasis 여부를 꼭 확인하여야 한다. lymph node 전이가 의심되는 경우 primary tumor의 wide excision 과 함께 radical lymph node dissection 및 palliative lymph node dissection등을 시행하여야 한다.
Lymph node 에 대한 영상의학적 검사 외에도 PET-CT를 이용한 distant metastasis 역시 필수적인다. 실제로 distant metastasis 가 1명 (5%) 에서 발견되었다. 전이부위는 abdominal cavity로 physical exam 만으로는 진단하기 어려우며, 전이의 유무는 환자의 사망률, 수술적 치료로서 완치 가능여부에 영향을 준다. 때문에 majolin's ulcer환자에게 PET-CT 촬영은 꼭 행해져야 하는 evaluation 이다
MRI 는 Marjolin's ulcer의 넓이와 깊이를 보여주며, 주위 조직 bone vessel muscle 등의 invasion 여부를 확인할 수 있다. 특히 scalp 의 경우 skull의 invasion 유무는 operation plan을 결정하는데 중요한 역할을 한다.
Evaluation이 완료되면 tumor 의 radical excision 방법을 결정하여야 한다. cancer의 재발 및 전이 가능성을 고려하여 wide excision 및 amputation 등을 결정 하여야 한다. 본원의 경우 1명의 case에서 재발 후 pevis level에서 lower extremity amputation을 계획하였으나 환자의 동의를 얻지 못하여 치료가 지연되고 있다. 성형외과의사로서 amputation을 결정하기란 어려운 일이며 또 예후 등을 예상하여 amputation 시행 하여야 하는 기준을 만드는 것은 어려운 일이다. 하지만 다음과 같은 요소를 고려하여 볼 수 있다. ulceration nature가 discharge를 동반하며 exophytic and resemble grannulation tissue 를 가지는 ulcer, size가 100㎠ 이상인 경우, 주변 multiple soft tissue invasion 존재하는 경우, lyphatics 및 vascular invasion이 있는 경우, lymph node metastasis 및 distantmetastasis가 존재하는 경우, ulcer 발생 후 빠르게 size가 증가하는 경우, ulcer 주변부위로 contracture를 동반하는 scar tissue가 존재하는 경우 등이 있다. size와 깊이 nature에 따른 재발률 및 전이율 그리고 그에 따른 적절한 수술적 방법등에 대해서 추후 많은 환자군을 대상으로 하여 연구가 더 진행되어야 할 것이다.
Translation - English Third, to prevent malignant transformation, the initial wound needs to be treated to prevent its transformation into chronic unstable scar. For a deep 2nd degree wound and more serious ones, rather than spontaneous healing, it is better to perform skin graft after clean excision.
After diagnosis, thorough preoperative evaluation is essential.
Not only physical exam for surrounding lymph node but also sonography is a must in order to find out whether lymph node metastasis occurred.
If lymph node metastasis is suspected, radical lymph node dissection or palliative lymph node dissection need to be performed along with wide excision of primary tumor.
Apart from radiological test for lymph node, PET-CT to check distant metastasis is also essential. In fact, distant metastasis was found in 1 patient (5%).
Metastasized area was abdominal cavity which is difficult to diagnose by using only physical exam. Occurrence of metastasis affects the death rate of patients and possibility of complete recovery after the surgery. Due to such issues, PET-CT test is a necessary evaluation for Majolin's ulcer patients.
MRI shows width and depth of Marjolin's ulcer and provides data to confirm the invasion into surrounding tissue, bone, vessel, and muscle.
Especially for scalp, the invasion into the skull plays a key role to determine an operation plan.
After evaluation, radical excision method for tumor needs to be determined.
Wide excision or amputation need to be determined while considering the possibility for recurrence and metastasis of cancer.
In our case, our hospital had planned to perform lower extremity amputation at pelvis level for 1 patient after the recurrence, but the procedure is on hold due to the lack of consent of the patient.
Decision of amputation is difficult for plastic surgeons. Also, it is difficult to make a standard for amputation while predicting prognosis.
But we can consider below factors:
When ulceration nature is accompanying discharge and ulcer has exophytic and resemble granulation tissue, size is bigger than 100㎠, existence of multiple soft tissue invasion in surrounding area, existence of lymphatics and vascular invasion, existence of lymph node and distant metastasis, rapid size increase after ulceration occurrence, and existence of scar tissue accompanied by the contracture surrounding the area of ulcer.
Further research with more patients is required for recurrence and metastasis rate based on its size, depth, and nature, and consequently, a proper operational method needs to be determined.
