Source text - English Columellar Strut for Nasal Tip Support
Various surgical techniques and procedures that provide long-term support to the tip of the nose and stabilization of the nasal base have been used in the past. One of the widely used techniques is the placement of a columellar strut graft. The graft is usually and typically made of autologous septal or rib cartilage, which is sutured between the medial crus of the lower lateral cartilages. The columellar strut graft can extend to the nasal spine or be placed above the nasal spine. Another surgical method or procedure to provide nasal tip projection and support is the “tongue-in-the–groove” technique, wherein the medial crus of the lower lateral cartilages are sutured to the caudal end of the septum. A septal extension graft may also be used to ensure that the nasal tip projection is maintained postoperatively. Although nonabsorbable implants may be used to support the nasal tip, this method of tip support treatment is not preferred by surgeons because of associated complications such as infection, skin necrosis, and implant extrusion, as well as factors such as patient awareness, appearance.
In order to improve existing surgical procedures and patient outcomes, there is a continuing need in this art for low mass columellar struts with geometric characteristics that enhance the associated implantation procedure and provide for superior patient results. In particular, there is a need in this art for novel implants made from bioabsorbable polymers that are useful in nasal reconstruction surgical procedures.

В прошлом использовались различные хирургические техники для обеспечения долгосрочного укрепления кончика носа и стабилизации основания носа. Одним из широко используемых хирургических методов является размещение в перегородке преддверия носа имплантата. Этот имплантат обычно и как правило является аутотрансплантатом хряща носовой перегородки или фрагмента реберного хряща, который вшивается между медиальными ножками нижнелатерального хряща. Имплантат может достигать передней носовой ости, либо располагаться выше нее. Еще одним хирургическим методом, обеспечивающим поддержку кончика носа, является метод "шип-в-паз", отличающийся тем, что медиальные ножки нижних боковых хрящей ушивают до хвостового конца носовой перегородки. Имплантат также может применяться для реконструкции носовой перегородки с тем, чтобы проекция кончика носа сохранялась после операции. Хотя нерассасывающиеся имплантаты и могут быть использованы для поддержки кончика носа, этот метод лечения не всегда предпочитается хирургами как по причине связанных с ним осложнений, таких как инфекции, некроз кожи и экструзия имплантата, так и предрассудков пациента, внешнего вида.
В целях улучшения существующих хирургических методов и результатов лечения пациентов, возникает настоятельная потребность в искусственном имплантате небольшой массы с соответствующими геометрическими характеристиками, который призван усовершенствовать существующюю методику имплантации, а также обеспечить превосходные результаты у пациентов. В частности, в данной области хирургии имеется необходимость в новых имплантатах, изготовленных из биорассасываемых полимеров, которые могут использоваться в реконструктивной ринопластике.

Source text - English Case 1. A 23-year-old male was found at the bottom of the stairs with initial GCS 3/15 on scene but recovered to GCS 7/15 before being transferred to a local hospital by LAS, where a CT head was performed (Figure 1(a)) showing bifrontal contusions and subarachnoid blood with evidence of ICP. He was then transferred to St. Mary’s Hospital six hours later where he was immediately taken to theatre to undergo an ICP bolt insertion. He was then admitted to our ICU where aggressive medical treatment was commenced despite which the ICP raised up to 35 mmHg. After 2 hours without improvement, he was transferred back to theatre where a bifrontal craniotomy was performed and subdural blood evacuated. Due to refractory ICP and evolving cerebral oedema, he was taken back to theatre 24 hours later to undergo an extensive bifrontal decompressive craniectomy (Figure 1(b)) and the evacuation of a right frontal subdural haematoma, requiring neuroprotection for another 48 hours. The bone flap was left in his abdomen for further reconstruction. An early tracheostomy was performed at day 4 of admission to allow early weaning but complicated by an RLL pneumonia needing deep sedation to control ventilation and ICP. Despite that an EEG did not demonstrate seizures, he was commenced on phenytoin prophylactically.
Translation - Russian Клинический случай1. 23-летний мужчина был найден внизу лестничного марша на месте происшествия в состоянии ШКГ 3/15( Шкала (тяжести )комы Глазго) , но затем ,на фоне незначительного улучшения (ШКГ 7/15) он был доставлен Лондонской службой скорой помощи (LAS, London Ambulance Service) в местный госпиталь, где была выполнена КТ ( компьютерная томография) головы (рисунок 1(а)) , на которой был выявлен бифронтальный ушиб мозга и кровоизлияние в субарахноидальное пространство с повышением внутричерепного давления. Спустя шесть часов пострадавший был переведен в госпиталь Святой Марии и немедленно доставлен в операционную , где ему была произведена операция введения интракраниального катетера (ICP) для мониторинга внутричерепного давления. Затем он поступил в наше отделение интенсивной терапии, где начали проводить активное медикаментозное лечение , несмотря на которое внутричерепное давление поднялось до 35 мм рт.ст. После 2-х часовой безуспешной терапии он снова был доставлен в операционную, где была выполнена бифронтальная краниотомия и удалена субдуральная гематома. Поскольку, несмотря на проводимые мероприятия , внутричерепное давление не снижалось и начали появляться симптомы отека головного мозга, пациент, по прошествии 24 часов вновь был доставлен в операционный зал, где была произведена обширная бифронтальная декомпрессирующая краниэктомия (рисунок 1(b)) и удалена правосторонняя лобная субдуральная гематома , которая потребовала проведения нейропротекторной терапии в последующие 48 часов. Костный лоскут был оставлен в брюшной стенке пациента для последующей реконструкции костного дефекта черепа. Ранняя трахеостомия , проведенная на 4 день после поступления с целью возможно более раннего отключения пациента от аппарата искусственной вентиляции легких , осложнилась правосторонней нижнедолевой пневмонией, что заставило продолжить глубокую седацию для контроля вентиляцией легких и внутричерепного давления. Хотя ЭЭГ( Электо-энцефалограмма)и не продемонстрировала постравматических эпилептоидных изменений, пациенту все же профилактически был назначен фенитоин .
