Sekurytyzacja jest pozabilansową metodą pozyskiwania kapitału na cele inwestycyjne i obrotowe, polegającą na przekształceniu wyodrębnionych aktywów w papiery wartościowe, które mogą stać się przedmiotem obrotu. Z finansowego punktu widzenia dochodzi do zamiany mniej płynnych aktywów na aktywa o większej płynności. Podstawowym warunkiem wdrożenia programu sekurytyzacji jest posiadanie przez określony podmiot odpowiedniego wolumenu aktywów, które można poddać sekurytyzacji, lub też posiadanie przez ten podmiot możliwości generowania tych aktywów w przyszłości. W szczególności aktywami tymi mogą być wierzytelności wynikające z umów (pożyczki, kredytu, leasingu), należności handlowe przedsiębiorstw (wymagalne, przeterminowane), należności eksportowe, a także należności przyszłe. Wszystkie podmioty występujące w transakcjach sekurytyzacyjnych są odrębnymi jednostkami organizacyjnymi. Do najważniejszych zalet sekurytyzacji należy niższy koszt pozyskania kapitału, pozytywny wpływ na wskaźniki kapitałowe oraz obniżenie poziomu ryzyka kredytowego.
Na przykład, jeżeli spółka ma należności o małym ryzyku, to papierom wartościowym wyemitowanym pod ich zastaw przyznany będzie zapewne wysoki rating, nawet jeśli całe przedsiębiorstwo charakteryzowało się wysokim ryzykiem kredytowym. Podstawowym źródłem zaspokojenia roszczeń inwestorów są wpływy związane z sekurytyzowanymi należnościami — emitowane papiery nazywane są papierami wartościowymi zabezpieczonymi aktywami (ABS — Asset Backed Securities).
Sekurytyzacja, jako metoda pozabilansowa, nie zmienia struktury pasywów przedsiębiorstwa. Wynika to z faktu, że przedsiębiorstwo sprzedaje spółce specjalnego przeznaczenia (SPV — Special Purpose Vehicle)19 wyodrębnione aktywa i otrzymuje w zamian środki pieniężne. Zobowiązania nie ulegają zmianom. Dlatego nie wzrasta ryzyko finansowe przedsiębiorstwa, a obniża się koszt pozyskania kapitału. Ponadto sekurytyzacja ułatwia dotarcie do większej grupy inwestorów i pozwala jednocześnie zdywersyfikować formy pozyskania kapitału, co jest szczególnie istotne w przypadku przedsiębiorstw korzystających z funduszy obcych. Takie postępowanie zapobiega wzrostowi kosztów finansowania, związanych z wykorzystaniem jednego instrumentu finansowego.

Securitisation is an off-balance sheet method of raising capital for investment and working purposes, whereby separate assets are transformed into tradeable securities. From a financial point of view, less liquid assets are converted into assets with higher liquidity. The basic prerequisite for the implementation of a securitisation program is the possession of an appropriate pool of assets that can be securitised by a specified entity, or the ability of this entity to generate these assets in the future. Such assets include, in particular, debts arising from contracts (loans, credit, leasing), trade receivables of enterprises (due, overdue), export receivables, as well as future receivables. All the parties that appear in securitisation transactions are separate organisational units. The key benefits of securitisation include lower costs of raising capital, a positive impact on equity ratios, and reduction in the level of credit risk.
For example, if the company holds low-risk receivables, the securities backed by such receivables will probably be granted a high rating, even if the entire company carries a high credit risk. The investors' claims are primarily satisfied by receipts associated with securitised receivables - issued securities are referred to as asset-backed securities.
Securitisation, as an off-balance sheet method, does not alter the structure of liabilities of a company. This is due to the fact that the company sells individual assets to a special purpose vehicle in exchange for cash. Liabilities do not change. Therefore, the financial risk of the company does not increase, and the cost of raising capital is reduced. Furthermore, securitisation makes it easier to reach a wider group of investors, while at the same time allowing for the diversification of forms of raising capital, which is particularly important in the case of companies benefiting from external funds. This prevents the increase in financing costs associated with the use of a single financial instrument.

Scientific guidelines are referred to in the annexes to Directives 2001/82/EC and 2001/83/EC and listed in the annexes to the Notice to Applicants implementing the Common Technical Document (CTD). The quality, non-clinical and clinical standards and protocols in respect of the testing of medicinal products are given in Directive 2001/82/EC and Directive 2001/83/EC as amended. In assembling the dossier for an application for a marketing authorisation, applicants shall take into account the scientific guidelines (including core summary of product characteristics, where applicable) relating to the quality, safety and efficacy of medicinal products as adopted by the scientific committees - the Committee for Medicinal Products for Human Use (CHMP)3 and the Committee for Medicinal Products for Veterinary Use (CVMP)4, the Committee on Herbal Medicinal Products (HMPC), and published by the European Medicines Agency (EMEA).
These scientific guidelines aim to provide a basis for practical harmonisation of the manner in which Member States and EMEA interpret and apply the detailed requirements for the demonstration of quality, safety and efficacy. They are intended to give guidance to applicants and/or sponsors in planning the overall pharmaceutical product development, as well as the non-clinical and clinical tests and studies of a compound intended to be used as human or veterinary medicinal products and to facilitate the preparation of applications for marketing authorisations by the pharmaceutical industry. The Commission in Volumes 3 and 7 of “The rules goveming medicinal products in the European Union” has originally published these guidelines.
Currently, new or updated guidelines are published by the EMEA in its website.

Wytyczne naukowe zostały opisane w załącznikach do dyrektyw 2001/82/ WE i 2001/83/WE i wymienione w załącznikach do Uwag dla Wnioskodawców realizujących Wspólny Dokument Techniczny (CTD). Jakościowe, niekliniczne i kliniczne normy i protokoły w odniesieniu do badania produktów leczniczych określają dyrektywy 2001/82/WE i 2001/83/WE z późniejszymi zmianami. Gromadząc dokumentację do wniosku o pozwolenie na dopuszczenie do obrotu, wnioskodawcy muszą wziąć pod uwagę wytyczne naukowe (w tym główna charakterystyka produktu leczniczego, o ile dotyczy) odnoszące się do jakości, bezpieczeństwa i skuteczności produktów leczniczych, przyjęte przez komitety naukowe - Komitet ds. English to Polish: Ferrous Sulfate Supplementation Causes Significant Gastrointestinal Side-Effects in Adults: A Systematic Review and Meta-Analysis. General field: Medical Detailed field: Medical (general)

In trying to capture the true incidence of GI side-effects with oral iron therapy, discrepancies between studies and limitations of study design have made overall conclusions difficult to reach [11,14,15]. A recent review has tried to capture data for adverse events from studies with oral iron supplementation in patients without gastrointestinal disease and reported overall adverse event incidences of 32.3% for ferrous sulfate, 47% for ferrous fumarate and 30.9% for ferrous gluconate [11]. However, the meta-analysis reported herein has refined the study eligibility criteria in relation to the prior review [11]. Notably, we have only included randomized controlled studies with a ferrous sulfate intervention arm; we restricted adverse events to the common focus of concern with therapeutic oral iron, namely GI side-effects, and we have only included studies with a common comparator arm, which was either placebo or intravenous iron.
Since ferrous sulfate is the gold standard (most commonly prescribed) oral iron therapy in the UK and many other countries [10,31], our aim was to quantify the odds ratio for oral ferrous sulfate- associated gastrointestinal side-effects versus placebo or IV iron. To this end, we have carried out a systematic review and meta-analysis of all published randomized controlled trials (RCTs) reporting gastrointestinal-specific side-effects that have included ferrous sulfate against placebo or IV iron. We have performed sub-group analysis for pregnant women and patients with IBD since these are two population groups at higher risk of iron deficiency anemia and for whom sufficient robust data were likely to be available [14,32,33]. Finally, we explored whether the iron dose is associated with the odds of gastrointestinal side-effects using meta-regression analysis.

W próbie uchwycenia rzeczywistego występowania skutków ubocznych w przewodzie pokarmowym w doustnej terapii żelaza, rozbieżności między badaniami i ograniczenia metodyki badań sprawiły, że trudno wyciągnąć ogólne wnioski [11,14,15]. Niedawne sprawozdanie miało na celu zebranie danych dotyczących zdarzeń niepożądanych z badań nad doustną suplementacją żelaza u pacjentów bez chorób układu pokarmowego i wykazało ogólną częstotliwość zdarzeń niepożądanych na poziomie 32,3% dla siarczanu żelazawego, 47% dla fumaranu żelazawego i 30,9% dla glukonianu żelazawego [11]. Jednak wspomniana tutaj metaanaliza udoskonaliła kryteria uczestnictwa w badaniach w stosunku do poprzedniego sprawozdania [11]. Mianowicie, użyliśmy tylko randomizowane, kontrolowane badania z grupą interwencyjną siarczanu żelazawego; za pomocą doustnego żelaza terapeutycznego ograniczyliśmy niepożądane zdarzenia do wspólnego obszaru zainteresowania, mianowicie do skutków ubocznych w przewodzie pokarmowym, i uwzględniliśmy tylko badania ze wspólną grupą porównawczą, która składała się z placebo lub podawanego dożylnie żelaza.
Ponieważ siarczan żelazawy jest tzw. złotym standardem (najczęściej przepisywanym środkiem) w doustnej terapii żelazem w Wielkiej Brytanii i wielu innych krajach [10,31], naszym celem było określenie ilorazu prawdopodobieństwa wystąpienia skutków ubocznych w przewodzie pokarmowym powiązanych z doustnym siarczanem żelazawym w porównaniu z placebo lub żelazem podawanym dożylnie. W tym celu przeprowadziliśmy systematyczny przegląd i metaanalizę wszystkich opublikowanych randomizowanych, kontrolowanych badań (RCT) odnotowujących skutki uboczne w przewodzie pokarmowym, w których występował siarczan żelazawy w porównaniu z placebo lub żelazem podawanym dożylnie. Przeprowadziliśmy analizę podgrup kobiet ciężarnych i pacjentów z nieswoistym zapaleniem jelit, ponieważ są to dwie grupy ludności o podwyższonym ryzyku anemii spowodowanej niedoborem żelaza i występowało spore prawdopodobieństwo otrzymania solidnych danych dotyczących tych grup [14,32,33]. Wreszcie, zbadaliśmy czy dawka żelaza jest powiązana z prawdopodobieństwem wystąpienia skutków ubocznych w przewodzie pokarmowym za pomocą analizy metaregresyjnej.
