MULTIPLE SCLEROSIS
DEFINITION
Multiple sclerosis (MS) is a slowly progressive disorder of the central nervous system (CNS) in which disseminated patches of the brain and spinal cord become demyelinated, causing nervous system signal transmission to be distorted, delayed, or blocked. Although lesions are widely disseminated they have a tendency to occur more frequently in the periventricular areas of the cerebrum, cerebellar peduncles, brain stem, and dorsal spinal cord.The disease is characterized by remissions and exacerbations.

ETIOLOGY
The etiology of MS is unknown. An immunological abnormality is suspected. Women are more affected by the disease than are men. The disease is more prevalent in temperate climates than in tropical climates. The disease is linked to the location where the patient spent the first 15 years of life.

DISEASE COURSE
The onset of the disease usually is between the age of 20 and 40 years; the average age at onset is 31 years old. The course of the disease is highly varied and unpredictable. The disease may have a very acute onset or it may be sub-acute, involving slowly progressive lower extremity weakness. Remissions and exacerbations are characteristic of the disease, but there is no way to predict the frequency or length of these episodes. Symptoms reflect the location of lesions, and thus each patient presents a very different clinical picture. Symptoms may include incoordination(dysmetria, dyssynergia,ataxia, intention tremor) due to loss of cerebellar control; muscle weakness and spasticity; cerebella hypotonia; sensory involvement including anesthesia, paresthesia, and loss of proprioception and stereognosis; diplopia, nystagmus, and decreased visual acuity or blurring; dysarthria; dysphagia; mild dementia, anxiety, depression, euphoria, and irritability;bladder incontinence and sexual impotence.

MEDICAL INTERVENTION
MS is difficult to diagnose because no specific laboratory finding is conclusive. Physicians observe for clinical evidence and review the patient’s history and laboratory findings.205(p461) Physicians generally look for evidence of multiple CNS lesions, which may become apparent through neuroradiological studies such as MRI. In addition, studies of the CSF can provide significant information; i.e., 25% of patients with active disease show an increase in white blood cells, 30% show an increase in the total CSF protein, and 70% to 90% show an increase in the gamma globulin fraction. Evoked response (electrical manifestation of the brain’s reception of and response to an external stimulus) evaluations, which measure a patient’s impulse conduction in CNS white-matter tracts, are also commonly abnormal in patients with MS.

There is presently no method of reversing or preventing the demyelination that occurs in MS. Medical management of these patients focuses on easing symptoms, maximizing health, and minimizing associated complications.208 (p536)

The use of the minimal record of disability (MRD) for MS patients developed in 1985 by the International Federation of MS Societies, is encouraged by the National MS Society. The MRD offers standardized scales for reporting the results of a patient’s neurological assessment and status in ADL.208(p540)

The use of information is helpful for research purposes and communicating results between medical facilities. The MRD can be obtained from the National MS Society.209
