[0001] This invention relates to nucleic acid amplification reactions, including amplifications utilizing the polymerase chain reaction, and assays utilizing such reactions in combination with sequencing and hybridization probe detection methods.
BACKGROUND
[0002] Nucleic acid amplification techniques and assays are well known. Some amplification reactions are isothermal, such as nucleic acid sequence based amplification (NASBA). Others employ thermal cycling, such as the polymerase chain reaction (PCR). Preferred amplification assays employing fluorescence detection of amplified product are homo- geneous, that is, they do not require the physical separation of reagents to permit detection (for example, separation of bound probes from unbound probes) and can be performed in a single closed vessel. Such assays may be end-point, wherein product is detected after amplification, or they may be real-time, wherein product is detected as amplification proceeds.
[0003] Nucleic acid amplification and assays employing PCR are described, for example, in U.S. Patents 4,683,202, 4,683,195 and 4,965,188, and, generally, PCR PROTOCOLS, a guide to Methods and Applications, Innis et al. eds., Academic Press (San Diego, CA (USA) 1990). Homogeneous PCR assays, including real-time assays, in which amplified product is detected during some or all of the PCR cycles as the reaction proceeds are described, for example, in U.S. Patents 5,994,056, 5,487,972, 5,925,517 and 6,150,097.
[0004] PCR amplification reactions generally are designed to be symmetric, that is, to make double-stranded amplicons exponentially by utilizing forward primer and reverse primer in equimolar concentrations and equal melting temperatures (Tm’s). A technique that has found limited use for making single-stranded DNA directly in a PCR reaction is "asymmetric PCR." Gyllensten and Erlich, "Generation of Single-Stranded DNA by the Polymerase Chain Reaction and Its Application to Direct Sequencing of the HLA-DQA Locus," Proc. Natl. Acad. Sci. (USA) 85: 7652-7656 (1988); and U.S. Patent 5,066,584. Asymmetric PCR is a non-symmetric PCR amplification method that differs from symmetric PCR in that one of the primers is diluted fivefold to one hundredfold so as to be present in limiting amount of 1-20 percent of the con- centration of the other primer. As a consequence, the amplification consists of an exponential phase in which both primers are extended, generating double-stranded product, or amplicon, followed by a linear amplification in which only one primer remains, generating single-stranded amplicon.
[0005] More recently we have developed a non-symmetric PCR amplification method known as "Linear-After-The- Exponential" PCR or, for short, "LATE-PCR." LATE-PCR is a non-symmetric PCR amplification consisting of an expo- nential phase in which both primers are annealed and extended followed by a linear phase after exhaustion of the Limiting Primer, when only the Excess Primer is annealed and extended. See Sanchez et al. (2004) Proc. Natl. Acad. Sci. (USA) 101: 1933-1938, published international patent application WO 03/054233 (3 July 2003), and Pierce et al. (2005) Proc. Natl. Acad. Sci (USA) 102: 8609-8614.
[0006] A convenient and inexpensive method for monitoring double-stranded amplicon production in a PCR amplifi- cation is to use a dye that fluoresces upon intercalating into or otherwise interacting with double-stranded DNA, such as SYBR Green I or SYBR Gold. See, for example, U.S. Patent 5,994,056. Melting temperature analysis of amplicons performed either in real time during a PCR amplification or performed after amplification is used for product identification. One problem with utilizing such melting temperature analysis is that dye fluorescence is a function of amplicon size. Another problem is that dyes redistribute from amplification products, or amplicons, having low melting temperatures to amplicons having higher melting temperatures during analysis, thereby distorting results. Two approaches to solve these problems have been advanced. One approach, G quenching, imposes severe restrictions on primer design and causes large background fluorescence (Crockett AO, Wittwer CT. "Fluorescein-Labeled Oligonucleotides for Real-Time PCR: Using the Inherent Quenching of Deoxyguanosine Nucleotides" Anal. Biochem. 290:89-97 (2001)). The other, replacing SYBR dyes with LC Green dye, yields very small percentage of signal for sequences not present in abundance and requires highly specialized software and hardware (Wittwer et al. High-Resolution Genotyping by Amplicon Melting Analysis Using LCGreen," Clin. Chem. 49:853-860(2003).
[0007] Fluorescent-labeled probes are used in homogeneous nucleic acid amplification assays, including PCR assays, to measure the accumulation of desired amplicon, either in real time or by end-point analysis. Several available types of probes are significantly allele-discriminating as compared to linear single-stranded probes. Real-time probes include dual-labeled linear probes that are cleaved by 5’-to-3’ exonuclease activity of DNA polymerase during the extension step of a PCR cycle (see U.S. patents 5,210,015, 5,487,972 and 5,538,848); molecular beacon probes (see U.S. patents 5,925,517, 6,103,476 and 6,365,729); minor groove binding probes (see Afonina et al. "Minor Groove Binder-Conjugated DNA Probes for Quantitative DNA Detection by Hybridization-Triggered Fluorescence," Biotechniques 32: 946-949

(2002)); linear probe pairs that FRET when hybridized adjacently on a target strand; quenched double-stranded linear probes for which a target competes to hybridize to the labeled probe strand (see Li, Q. et al. (2002), Nucl. Acid. Res. 30: e5); and so-called "light-up" probes, which are peptide nucleic acid (PNA) oligomers linked to an asymmetric cyanine dye that fluoresces when the probe binds to target to form a double-stranded region. For LATE-PCR we have utilized low-temperature allele-discriminating probes, such as low temperature molecular beacon probes (See WO 03/045233). Labeled oligonucleotide probes may be attached to primers by linkers such that during amplification the probes are not copied but are free to hybridize to a target sequence resulting from extension of the primer. Examples are Scorpions®, primers to which are attached molecular beacon probes, and Anglers®, primers to which are attached fluorophore- labeled linear probes. Lee, M.A. et al. (2002), Analytica Clinica Acta 457: 61:70; Whitcombe, D. et al. (1999), Nature Biotechnology 17: 804-807. The probe portion of such composite structures, which carries the fluorescent label, hybridizes separately from the primer portion. They are, thus, labeled probes and not labeled primers, as those terms are used herein. Target-specific probes lack the capacity to monitor total production of double-stranded products, however. [0008] Certain probes are mismatch-tolerant. Mismatch-tolerant probes hybridize with and generate detectable signal for more than one target sequence at a detection temperature in an assay, and various hybrids so formed will have different melting points. Linear, or random coil, single-stranded probes are generally mismatch tolerant. Examples of such probes are linear probes with an internal fluorescent moiety whose level of fluorescence increases upon hybridization to one or another target strand; fluorescently labeled linear probes used in combination with SYBR Gold and SYBR Green I dyes, such that fluorescence of the label occurs by FRET from the dye when the probe hybridizes to one or another target (see U.S. patent publication US 2002/0119450, 28 August 2002), so-called "sloppy beacons", and vari- ations of linear probe pairs that FRET (see U.S. patent 6,472,156).
[0009] Utilizing multiple probes that each bind only to one possible target amplicon generated in an amplification reaction creates a problem for analyzing complicated reaction mixtures or for detecting one or a few targets from among numerous possible targets. Available fluorescence detection permits resolution of a limited number of fluorophores, generally no more than eight. Limited multiplexing is possible, for example, by designing a different allele-discriminating molecular beacon probe for each target and labeling each probe differentially. (See, for example, Tyagi et al. (2000) Nature Biotechnology 18: 1191-1196). Mixtures of allele-discriminating probes, each comprising aliquots of multiple colors, extends the number of probe signatures and works well if only one of many (at least up to 56) targets is actually present, but it encounters ambiguous results if more than one target is present.
[0010] There are many situations that involve complex targets or one among many possible targets. Several schemes have been developed or proposed for such situations, but all have serious drawbacks and limitations. Tyagi et al. published international patent application WO 01/31062, have described a technique sometimes referred to as "sloppy beacons," which are molecular beacon probes that have long loop sequences, rendering them mismatch tolerant and able to bind to some extent to multiple targets at the annealing temperature of a PCR amplification reaction. Such probes suffer from poor reaction kinetics against mismatched targets and are likely to remain hybridized to perfectly matched targets at the extension temperature of a PCR amplification and be cleaved by Taq DNA polymerase. Further, only an indirect indication of melting temperatures of probe-target hybrids under the assay conditions is obtained, and that assumes equilibrium has been achieved. Real-time multiplexing in symmetric PCR amplifications with FRET probes has been described. In order not to interfere with amplification, the melting temperatures of all probe-target hybrids are constrained to be in the narrow temperature range between the primer annealing temperature and the primer extension temperature. Also, that scheme is not quantitative. Post-amplification multiprobe assays employing FRET probes of different colors have been disclosed Wittwer et al., "Real-Time Multiplex PCR Assays, Methods" 25:430-442 (2001). The reaction mixture following a symmetric PCR amplification is rapidly chilled, then slowly heated to determine melting curves for the various fluorophores present. This approach is not quantitative. In addition, because of large scatter among replicate symmetric PCR amplifications, end-point assays in general tend to be only qualitative.
[0011] Sequencing reaction products provides an alternative to probing. Traditional dideoxy sequencing may utilize products of amplification reactions, such as symmetric PCR or LATE-PCR, as starting materials for cycle sequencing. Amplified product is purified utilizing ethanol precipitation or an affinity column to remove leftover dNTPs and primers, subjected to a cycle sequencing reaction utilizing one sequencing primer and fluorescently labeled dideoxy nucleotides, and subjected to capillary gel electrophoresis. "Pyrosequencing" is a real-time, isothermal, sequence-by-synthesis meth- od known in the art. If exponential amplification methods, for example PCR, are used in the preparation of starting material for Pyrosequencing, amplified product must be cleaned up by isolation of single-stranded product as well as removal of dNTPs, pyrophosphate and unincorporated primers from the amplification reaction. LATE-PCR simplifies sample preparation, because it generates primarily single-stranded product, but it does not in and of itself eliminate the need to clean-up the product.

Esempi di tali sonde sono quelle lineari con una parte interna fluorescente il cui livello di fluorescenza aumenta in seguito ad ibridazione con uno dei due filamenti target; sonde lineari con marcatura fluorescente usate in combinazione con i cromofori SYBR Gold e SYBR Green I, in modo tale che la fluorescenza della marcatura viene emessa tramite FRET da parte del cromoforo quando la sonda si lega a uno o all’altro target (vedi U.S. patent publication US 2002/0119450, 28 August 2002), definite “sloppy beacons”, e variazioni di coppie di sonde lineari del tipo FRET (vedi U.S. patent 6,472,156).

L’utilizzo di sonde multiple ognuna delle quali si lega soltanto ad un solo amplicone target formatosi nella reazione di amplificazione crea un problema nell’analisi di miscele di reazione composite o nell’identificazione di uno o pochi target tra i vari target possibili. La rilevazione della fluorescenza presente consente l’identificazione di un numero limitato di fluorofori, in genere non più di otto. Un multiplexing limitato è ottenibile, per esempio, disegnando una sonda faro con discriminanza allelica diversa per ciascun target e marcando le varie sonde in maniera diversa (vedi, per esempi, Tyagi et al. (2000) Nature Biotechnology 18: 1191-1196). L’uso di miscele di sonde con discriminanza allelica, ciascuna contenente aliquote di diversi cromofori, consente di estendere il numero delle caratteristiche della sonda e funziona bene solo se è presente uno dei possibili target (sino a 56), ma si hanno risultati ambigui se sono presenti più di un solo target.
0010. Ci sono diverse situazioni costituite da più target complessi o uno solo tra vari target possibili. Sono stati sviluppati o proposti diversi schemi per situazioni simili, ma tutti comportano delle limitazioni e inconvenienti. Nella richiesta internazionale di brevetto WO 01/31062pubblicata da Tyagi et al. è stata descritta una tecnica spesso associata agli “sloppy beacons”, che sono sonde faro molecolari contenenti lunghe sequenze anello che le rende tolleranti verso i mismatch e capaci di legare parzialmente target multipli alla temperatura di appaiamento in una reazione di amplificazione di PCR. Queste sonde sono caratterizzate da cinetiche di reazione molto lente per sequenze target con appaiamenti errati ed è plausibile che rimangano ibridate a target perfettamente appaiati alla temperatura di estensione in un’amplificazione di PCR e possono essere tagliate dalla Taq DNA polimerasi. Inoltre, si ottiene solo un’indicazione indiretta delle temperature di melting relative agli ibridi sonda-target nelle condizioni del saggio, è ciò suggerisce il raggiungimento dell’equilibrio. La PCR LATE rende più facile la preparazione dei campioni, poiché produce prodotti essenzialmente a singolo filamento, ma di per sé non consente di bypassare la fase di purificazione del prodotto.

Fifteen years ago, the generation of new neurons in adulthood was documented in the human hip- pocampus, but lingering questions have remained about the extent of this process. In this issue of Cell, Spalding et al. provide elegant evidence for continued neurogenesis into adulthood at rates that suggest it may play a significant role in human behavior.
For most of the 20th century, neuroscien- tists thought that the adult mammalian brain did not generate new neurons. Evidence that this wasn’t the case was reported (Altman and Das, 1965), but only in the 1990s did sufficient data accu- mulate for the field to accept that adult neurogenesis did occur in mammals. Now, it is well established that, in rodents, the olfactory bulb and the dentate gyrus (DG) region of the hippocampus incorpo- rate new neurons in adulthood. However, in nonhuman primates, lower levels of adult neurogenesis were observed. These data, despite evidence that neurogenesis does occur in the adult human DG (Eriks- son et al., 1998), caused considerable anxiety in the field. When it comes to the relevance of adult neurogenesis in humans, two questions have remained: to what extent does this process occur in our species and can a small number of cells generated in the adult hippo- campus impact human behavior? In this issue of Cell, Spalding et al. (2013) have answered the first question by providing powerful evidence for extensive neuro- genesis in humans throughout adulthood with numbers comparable to those seen in rodents.
In 2005, Spalding and Frisen took the concepts underlying radiocarbon-dating techniques used in archaeology and developed an ingenious strategy for birth-dating cells from postmortem tissue (Spalding et al., 2005). The technique is based on the pronounced spike in global levels of 14C that resulted from extensive above-ground nuclear weapon testing during the Cold War and the fact that there has been a steady decline in atmo- spheric 14C since such testing was banned. Because plants absorb 14C via CO2 during photosynthesis, animals that eat them also take in radioactive carbon; therefore, the 14C level in our bodies reflects that of the atmosphere. Conse- quently, when a cell divides, newly syn- thesized DNA integrates a trace amount of 14C that is proportional to the environ- mental level at the time of mitosis; hence, the radioactivity of a cell nucleus can be used as a time stamp of the cell’s genesis.
Using this technique, this group had previously found that the human olfactory bulb did not contain neurons born in adulthood (Bergmann et al., 2012), a result that is strikingly different than the one observed in rodents, where olfactory bulb neurogenesis continues throughout life. But, in the hippocampus, they found that neurogenesis occurs at significant levels through adulthood and until old age.
The first surprise from this study was that, unlike the stark age-related decline seen in rodents, the human hippocampus appears to generate new neurons at a fairly steady rate well into old age (Figure1). Their computational modeling studies indicated that their data was best fit by a scenario in which there was one population of hippocampal neurons that did not turn over, whereas 35% of the neurons did. Given that DG neurons correspond to roughly 35% of the total hippocampal population, this suggests that a majority of DG cells are subject to exchange. This is in dramatic contrast to rodents, where it has been estimated that neurons generated in adulthood correspond to only 10% of DG granule cells (Imayoshi et al., 2008). About 700 neurons are added daily in the adult human DG, corresponding to an annual turnover rate of 1.75%, which is similar to the levels found in middle- aged rodents.
This study is of profound importance, given that it will further invigorate the field for the study of the contribution of adult hippocampal neurogenesis to human behavior and mental health. Previous concerns about low levels of neurogene- sis in humans and its potentially limited importance in aged individuals can now be tempered. For instance, these findings support the importance of investigating the therapeutic potential of harnessing adult neurogenesis for the treatment of age-related cognitive disorders. In rodents, adult neurogenesis is involved in modulating pattern separation, a cog- nitive process that declines with age (Sahay et al., 2011). In addition, neuro- genesis has been implicated in the behav- ioral effects of antidepressants (Santarelli et al., 2003) and memory generalization in anxiety disorders (Kheirbek et al., 2012), links that can now be investigated in humans with renewed confidence. Interestingly, the individuals used in Spalding et al. (2013) showed significant variability in levels of incorporated 14C, which could serve as an opportunity to retrospectively compare levels of hippo- campal neurogenesis with each individ- ual’s medical history in order to probe for a relationship between psychiatric conditions and rates of cell turnover.
Yet, this remains only the beginning, because much of our knowledge about the contribution of hippocampal neuro- genesis to behavior is derived from rodent studies, and the significant differences between rodents and humans in the neurogenic process remains to be fleshed out. For example, a recent report indicated that the maturation process in nonhuman primates extended up to 6 months, significantly longer than the month or so it takes in rodents (Kohler et al., 2011). In addition, in humans, it has been reported that there exists regional differences in the neurogenic effects of antidepressants (Boldrini et al., 2009). Specifically, the ability of antidepressants to increase neurogenesis in humans appears to be most pronounced in the anterior pole of the hippocampus. Future studies comparing the temporal and regional rates of neurogenesis be- tween humans and rodents will provide essential clues about the contribution of adult-generated granule cells to DG physiology and behavior in humans.
The study from Spalding et al. (2013) also underscores the necessity for developing methods for imaging human hippocampal neurogenesis in vivo. Such methods are still in their infancy but are a necessary step for understanding how neurogenesis may change under disease and treatment conditions.
This landmark study by Spalding et al. (2013) legitimizes the explosion of research into the process of adult neuro- genesis in recent years. The discovery that the human hippocampus can generate new cells at a significant rate until old age puts to rest concerns about whether or not this process occurs at significant rates in people. Moreover, it will energize efforts to answer the second question of how this small number of cells impact human behavior and will fuel efforts to target this process for the development of new therapies for the treatment of disorders of cognition and mood.

Quindici anni fa, la generazione di nuovi neuroni in età matura era stata documentata nell’ippocampo umano, ma sono rimaste aperte diverse questioni in merito all’entità di questo fenomeno. In questo articolo di Cell, Spalding et al forniscono una prova elegante dell’esistenza di neurogenesi continua in età matura ad una velocità tale da suggerire che potrebbe svolgere un ruolo importante nel comportamento umano.
Per gran parte del 20° secolo, i neuroscienziati hanno pensato che il cervello adulto dei mammiferi non fosse in grado di generare nuovi neuroni. La prima prova che ciò non fosse vero fu riportata da Altman e Das nel 1965, ma solo negli anni ’90 ci fu un accumulo consistente di dati che dimostravano la neurogenesi nei mammiferi adulti. Attualmente è ben noto che, nei roditori, che le regioni del bulbo olfattivo e il giro dentato (DG) nell’ippocampo accumulano nuovi neuroni in età matura. Tuttavia, nei primati adulti non umani sono stati osservati livelli minori di neurogenesi. Questi dati, nonostante la prova della presenza di neurogenesi nel DG umano adulto (Eriks- son et al., 1998), hanno creato una certa ansia nell’ambiente. Quando si discute di neurogenesi nell’uomo adulto, rimangono due questioni irrisolte: qual è l’entità del fenomeno nella nostra specie e può un piccolo numero di cellule generatesi nell’ippocampo influenzare il comportamento umano? Tuttavia, hanno scoperto che nell’ippocampo si verifica neurogenesi a livelli significativi durante l’età matura e sino alla vecchiaia.
La prima sorpresa da questo studio fu che, a differenza del forte declino correlato con l’età osservato nei roditori, l’ippocampo umano sembra poter generare nuovi neuroni ad una velocità pressoché costante anche in età avanzata (Figura 1). I loro modelli di studio computazionali suggerivano che i loro dati erano molto compatibili con uno scenario in cui c’era una sola popolazione di neuroni ippocampali che non venivano sostituiti, mentre il 35% andava incontro a turnover. Poiché i neuroni del DG corrispondono a circa il 35% della popolazione ippocampale complessiva, ciò suggerisce che la maggior parte delle cellule del DG è soggetta a turnover. Questo dato è in forte contrasto con i roditori, in cui si stima che i neuroni generati in età matura corrispondano solo al 10% delle cellule granulari del DG Imayoshi et al., 2008). Circa 700 nuovi neuroni vengono aggiunti ogni giorno nel DG umano adulto, corrispondente ad un tasso di turnover annuale dell’1,75%, simile ai livelli osservati nei roditori di mezza età.
Questo studio ricopre un’enorme importanza, poiché supporta ulteriori studi nel tentativo di analizzare il contributo della neurogenesi ippocampale matura nel comportamento umano e nella salute mentale. I primi dubbi in merito ai bassi livelli di neurogenesi nell’uomo e la sua importanza potenzialmente limitata negli individui di età avanzata possono quindi essere ridimensionati. Ad esempio, questi dati supportano la necessità di analizzare il potenziale terapeutico di sfruttare la neurogenesi matura per il trattamento dei disturbi cognitivi correlati con l’età. Nei roditori, la neurogenesi matura è coinvolta nella modulazione del “pattern separation”, un processo cognitivo che diminuisce con l’età (Sahay et al., 2011). Inoltre, la neurogenesi è implicata negli effetti comportamentali degli antidepressivi (Santarelli et al., 2003) e nella generalizzazione della memoria nei disturbi legati all’ansia (Kheirbek et al., 2012), e tali connessioni possono essere ora studiate nell’uomo con una consapevolezza rinnovata. In particolare, gli individui usati da Spalding et al (2013) mostravano una notevole variabilità nei livelli di 14C incorporato, e ciò potrebbe costituire un’opportunità per paragonare in modo retrospettivo i livelli di neurogenesi ippocampale con l’anamnesi di ogni soggetto per analizzare le relazioni esistenti tra la condizione psichiatrica e il turnover cellulare.
Tutto ciò rappresenta solo l’inizio, poiché molta della nostra conoscenza sul contributo della neurogenesi ippocampale al comportamento deriva da studi sui roditori, e le differenze significative in merito ai processi neurogenetici tra roditori e uomo devono essere ancora approfondite. Ad esempio, uno studio recente suggerisce che il processo maturativo nei primati non umani si protrae sino a 6 mesi, un tempo molto più lungo rispetto al mese necessario ai roditori (Kohler et al., 2011). Inoltre, nell’uomo è stato dimostrato che esistono differenze regionali negli effetti neurogenetici degli antidepressivi (Boldrini et al., 2009). In particolare, la capacità degli antidepressivi di indurre la neurogenesi nell’uomo sembra essere molto più spiccata nel polo anteriore dell’ippocampo. Ulteriori studi che mettano a confronto le velocità di neurogenesi a livello diverse nel tempo e a livello regionale tra l’uomo e i roditori forniranno indizi fondamentali in merito all’influenza che le cellule granulari formatesi in età matura hanno sulla fisiologia del DG e sul comportamento umano.
Lo studio di Spalding et al (2013) sottolinea anche la necessità di sviluppare nuovi metodi per rappresentare la neurogenesi ippocampale nell’uomo in vivo. Tali metodi sono ancora in fase embrionale ma rappresentano uno step necessario per capire come la neurogenesi possa modificarsi durante la malattia ed in fase di trattamento.
Lo studio epocale di Spalding et al (2013) legittima l’enorme interesse nello studio dei processi della neurogenesi matura negli ultimi anni. La scoperta che l’ippocampo umano è in grado di generare nuove cellule ad una velocità sostenuta sino all’età avanzata mette a tacere quei dubbi in merito alla veridicità che tale fenomeno si verifichi o meno nell’uomo. Inoltre, promuoverà altri studi per poter rispondere al secondo quesito su come un piccolo numero di cellule possa influenzare il comportamento umano e alimenterà altri sforzi per indirizzare tale processo verso lo sviluppo di nuove terapie per il trattamento dei disturbi cognitivi e dell’umore.

Typically, a low GI diet produces greater satiety, directly provoking greater in¬creases in cholecystokinin and post-meal fullness/satiation. In contrast, the rapid absorption of glucose from a high GI meal may increase voluntary food intake by rapid increases in glycaemia and insulinaemia in the post¬prandial period, leading to a reactive hypoglycaemia that stimulates appetite and increases the daily caloric intake.
Our data seem to suggest that XXX® may help reduce insulinemic peaks, enhancing 3-cell function more effectively than the LGI diet alone does, and particularly the ERD, which seems to be unable to restore the insulin secretory reserve in patients with IGT or T2DM. However, taking into account that IGT is un¬stable, particularly in adolescents, long-term and more large-scale trials are required before more definitive statements can be made. Anyway, it is interesting to note that our data at baseline on the total of our patients and on 3 Arms after randomization are very similar to those reported by Brufani et al. with a cross-sectional study of 510 overweight and obese subjects aged 3–18 yr.

In linea di massima, una dieta a basso IG (Indice Glicemico) induce uno stato di maggiore sazietà, facendo aumentare direttamente i livelli di colecistochinina e il senso di pienezza/sazietà post-prandiale. Al contrario, il rapido assorbimento di glucosio dovuto ad un pasto ad alto IG potrebbe far aumentare volontariamente l’assunzione di cibo a causa dei repentini incrementi della glicemia e insulinemia nella fase post-prandiale, determinando una condizione di ipoglicemia reattiva che stimola l’appetito e fa aumentare l’apporto calorico giornaliero.
I nostri dati sembrerebbero suggerire che il XXX contribuirebbe a ridurre i picchi insulinemici, migliorando la funzionalità delle cellule beta pancreatiche in maniera più efficace rispetto ad una sola dieta a basso IG, e soprattutto l’ERD (metabolismo di base), che potrebbe non essere in grado di ripristinare la riserva secretoria di insulina in pazienti con IGT (ridotta tolleranza al glucosio) o T2DM (diabete mellito di tipo 2). Tuttavia, considerando che l’IGT è instabile, soprattutto negli adolescenti, sono necessari ulteriori studi a lungo termine e su più larga scala prima di poter giungere a conclusioni definitive. E’ comunque interessante notare che i nostri dati analizzati in condizioni basali sul totale dei nostri pazienti e attraverso uno studio randomizzato a 3 bracci sono molto simili a quelli riportati da Brufani et al. relativi a uno studio trasversale condotto su 510 soggetti in sovrappeso e obesi di età compresa tra 3 e 18 anni.

To shore up its retail credentials, Europe’s biggest aviation hub is expanding personal shopping services and adding specialized retail lounges and a data-collection system aimed at understanding passengers’ purchasing trends. Next June, the airport is scheduled to open its revamped Terminal 2, which will have 135,000 square feet devoted to shops. Heathrow passengers generated 1.7 billion pounds ($2.75billion) in retail revenue in 2012, almost double what was spent on Bond Street, London’s luxury hub. Sales of luxury items at the airport have grown 17 percent on average every year since 2008, and average revenue per square meter of retail has increased by almost half since then, to 30,052 pounds in 2012. “Heathrow Terminal 2 offers us access to one of the most concentrated, valuable and influential markets in the world,”said Sean Allam, commercial operations director for British retailer John Lewis, which will open its first airport store in the new facility.

this is Giuseppe, graduated in Biological Sciences in 2006 and having also achieved 2 PhDs (Biotechnology and Molecular Medicine) at University La Sapienza in Rome, with an International English Language Certificate (TOEFL, score:93/120), as well as being listed among authors of scientific papers published on www.pubmed.com.
