최근 지속적인 경제발전과 평균수명의 연장으로 개개인의 행복한 삶과 노화방지에 대한 욕구가 높아지고 있다. 아울러 여성의 사회활동 참여 기회가 많아짐에 따라 외모에 대한 관심이 어느 때보다 고조되면서 피부에 대한 중요성이 더욱 주목되고 있다. 특히, 노화에 의한 피부 탄력저하와 주름의 증가는 피부미용에서 중요 관심사중 하나로 피부노화예방 및 개선을 위해 다양한 시술들에 대한 수요가 증가하고 있다. 고주파와 초음파 레이저를 포함한 피부 탄력레이저는 이러한 시술 중 하나로, 피부탄력 개선의 욕구의 증가에 따라, 최근 그 수요가 증가하고 있다. 보다 개선된 탄력 레이저 서비스를 추구하기 위해, 소비자 만족도에 대한 기초 자료가 제시되어야 하나, 현재 병원에서 시술되는 탄력레이저 시술에 대한 만족도를 연구한 선행연구의 부재로, 연령별 니즈를 파악하고, 서비스를 개선하는데 한계가 있다.
따라서 본 연구에서는 연령별로 인식하는 피부상태의 문제점을 파악하고, 연령별 탄력 레이저에 대한 인식, 연령별 탄력 레이저 시술 후 만족도, 탄력 레이저의 부작용에 대해 조사하고 분석하여, 서비스 개선을 위한 기초자료로 활용하고자 하였다.
연구 대상은 2017년 3월 21일부터 4월 17일까지 서울 지역의 피부 탄력 레이저를 시행하고 있는 피부과, 성형외과에서 탄력 레이저를 시술하고 약 한달 뒤 재방문한 20대~50대 여성을 대상으로 331부의 설문지를 배부 회수하였다.
본 연구를 위해 수집된 자료의 통계처리는 SPSS 21.0 프로그램을 이용하여 다음과 같이 분석하였다. 분석기법으로는 연구대상자의 일반적 특성을 파악하기 위해 빈도와 백분율을 구하였다. 또한 탄력 레이저 시술을 받은 고객들의 일반적인 피부상태와 피부 관리 방법 및 탄력 레이저 인식 및 이용실태, 탄력 레이저 병원의 서비스 만족도, 그리고 탄력 레이저 시술 후 만족도와 사후관리를 알아보기 위해 χ2(Chi-square) 검증과 One-way ANOVA(일원변량분석), t-test(검증), 그리고 빈도분석을 실시하였고, 평균과 표준편차를 산출하였다.
연구 결과는 다음과 같다.
첫째, 연령별 인식하는 피부 상태의 문제점으로 20대와 30대, 40대는 모두 색소침착 및 칙칙함이 큰 문제점이라고 인식하였으며, 40대는 탄력 및 주름과 수분부족도 비슷하게 나타났다. 50대는 다른 연령대에 비해 탄력 및 주름이 큰 문제점이라고 인식했다. 연령별 신경 쓰이는 주름 부위로 모든 연령대가 눈가 및 눈밑 주름이라 답했으며, 30대는 다른 연령대보다 눈가 및 눈밑 주름이 더 신경 쓰인다고 인식했다. 현 피부상태에 가장 큰 영향을 미치는 요인으로 20대는 스트레스가 큰 영향을 미치는 요인이라 답했고, 30대와 40대는 생활환경(약물, 식습관, 생활습관 등), 50대는 자연적인 노화(유전, 연령의 증가)가 큰 영향을 미치는 요인이라 답했다. 이러한 연구결과는 연령에 따른 피부상태인식에 대한 차이를 보여준다. 특히 50대 이상의 연령층은 자연적인 노화에 의한 피부의 탄력 감소 및 주름을 큰 문제점으로 인식하는 것으로 나타나, 피부 탄력 레이저 시술의 주요 소비층으로 예상되었다.
둘째, 연령별 탄력 레이저 정보를 습득하는 경로에서 모든 연령층이 대부분 인터넷을 통해 정보를 습득하였다. 50대 이상은 친구 및 가족 등 지인에게서도 정보를 많이 얻는 경향을 보였다. 그러나 병원에서 받고 있는 시술 장비 및 기기에 대한 연령별 인지도에서는 5점 만점 중 전체 평균이 2.55로 사람들은 받고 있는 시술 장비 및 기기에 대해 그다지 잘 알고 있지 않다는 것을 알 수 있다. 특히 50대 이상에서는 다른 연령층에 비해 장비에 대한 인지도가 가장 낮았으나 통계적으로 유의미한 차이를 보이지 않았다.
셋째, 시술 받아본 탄력 레이저에서 연령대에 상관없이 울쎄라 시술을 가장 많이 받았음을 알 수 있다. 연령이 많을수록 탄력 레이저 1회 관리비용이 높은 것으로 나타나, 50대 이상이 다른 연령층에 비해 탄력 레이저에 가장 많은 비용을 지출함을 알 수 있다. 탄력 레이저 시술 후 전반적인 만족도로 5점 만점 중 전체 평균 3.89로 사람들은 탄력 레이저 시술 후 만족도가 높은 것으로 나타났다. 항목별로는 재 시술 의사가 있다가 4.15로 가장 높았고, 시술로 인한 통증이 없었다가 2.87로 가장 낮았다. 연령별 탄력 레이저 시술 후 만족도에서는 연령이 많을수록 만족도가 높았고 50대 이상에서 가장 높은 만족도를 보였다. 탄력 레이저별 만족도와 재 시술 의사비교에서 초음파 레이저인 울쎄라와 슈링크의 경우 다른 시술보다 만족도가 더 높은 것으로 나타났다.
넷째, 탄력 레이저 시술로 인한 부작용 종류, 증상의 정도에서 부작용을 경험한 적이 없다 56.8%로, 경험한 적이 있다 43.2% 보다 더 많은 것으로 나타났다. 부작용 증상으로는 따가움이 41.8%로 가장 많이 경험 했고, 붉어짐 28.9%와 열감 18.5% 순으로 나타났다. 탄력 레이저 종류별 부작용 여부에서는 울쎄라 부작용 비율이 가장 적다.
이상의 연구결과에서 연령층이 높을수록 노화에 의한 피부탄력 저하를 더 심각하게 인지하지 하였고, 따라서 50대 이상의 연령층에서 기기에 대한 인지도가 가장 떨어짐에도 불구하고, 가장 높은 시술비용을 지출하며, 탄력 레이저의 만족도가 더 높은 것을 알 수 있었다. 탄력레이저의 종류별로는 초음파레이저인 울세라의 만족도가 가장 높고, 느끼는 부작용은 가장 적은 것으로 파악되었다.
본 연구에서 도출한 연령별 탄력 레이저에 대한 인식과 만족도를 분석한 결과는, 각 연령에 맞는 니즈(Need)에 적합한, 만족도 높은 피부탄력 레이저의 서비스를 제공하기 위한 기초자료로 활용하는데 그 의의가 있다.
Translation - English Abstract
Recently, with the continuous economic growth and extension of life expectancy, each individual's desire for happy life and prevention of aging is increasing. In addition, as the opportunities for women to participate in social activities have increased, interest in looking young has increased more than ever. In particular, the decrease in skin elasticity and the increase of wrinkles due to aging have led to an increasing demand for various treatments for prevention and improvement of skin aging. Laser skin tightening treatment using high frequency and ultrasound lasers is one of these procedures, and its demand has increased in recent years as the desire for improved skin elasticity increases. To pursue better laser skin tightening services, basic data on customer satisfaction should be presented, but currently in the absence of previous researches on satisfaction with laser skin tightening treatment performed at hospitals, there is a limit to grasp each age group’s needs and to improve related services.
In this study, we investigated the problems of skin conditions recognized by women of each age group, and also investigated the understanding of laser skin tightening, satisfaction by each age group after laser skin tightening treatment, and side effects of laser skin tightening, and used the results as basic data for improvement of the related services.
The subjects of this study were 331 women in their 20s through 50s who had revisited the dermatology or plastic surgery clinics in Seoul about a month after they had laser skin tightening procedures; the investigation was conducted from March 21 to April 17, 2017 by means of questionnaires distributed and recollected.
Statistical analysis of data collected for this study was conducted using the SPSS 21.0 program as follows. As an analytical technique, frequency and percentage were calculated to identify the general characteristics of the subjects. In addition, the χ2 (Chi-square) test, One-way ANOVA, t-test, and frequency analysis were performed to examine the general skin conditions, skin management methods, perception of laser skin tightening and its utilization status, satisfaction with hospital service related to laser skin tightening, and satisfaction with laser skin tightening treatment and follow-up management, and in accordance with these results, the mean and standard deviation were calculated.
The results of the study are as follows:
First, pigmentation and dullness were recognized as major problems in their 20s, 30s, and 40s, and for women in their 40s, elasticity, wrinkles and dry skin were considered as seriously. Women in their 50s perceived elasticity and wrinkles as more serious problems than in other age groups. Women in all ages perceived wrinkles around the eyes and under the eyes as major problems. In their 30s, women were more worried about the wrinkles around the eyes and under the eyes. Women in their twenties replied that stress was the most influential factor in their current skin condition, women in their 30s and 40s replied that the living environment (drugs, dietary habits, lifestyles, etc.) was the most influential factor, and women in their 50s replied that natural aging (heredity and age increase) was the most influential factor. These results show women's perception of their skins is different depending on their ages. In particular, the women in their 50s recognized skin elasticity reduction and wrinkling due to natural aging as a serious problem, and they were expected to be a major consumer group of laser skin tightening treatment.
Second, most of the age groups acquired the information on the laser skin tightening treatment through the Internet. Women in their 50s showed a tendency to acquire a lot of information through acquaintances such as friends or family members. However, in the age-based awareness test of the surgical instruments and devices at the hospitals, the overall average was 2.55 out of the total 5, indicating that people do not know much about the equipment and devices with which they are treated. Particularly women in the 50s showed the lowest awareness of the treatment equipment, but there was no statistically significant difference between the age groups.
Third, we can see that most of the women received Ultherapy procedures regardless of age. The higher the age, the higher the cost of receiving a single elastic laser treatment, showing that those over 50 years of age spend the most money on laser skin tightening compared to other age groups. The overall satisfaction with the laser skin tightening treatment was rated 3.89 out of total 5, which indicates that people were highly satisfied with such treatments. By item, ‘willingness to receive reoperation’ was the highest at 4.15, and ‘no pain from the procedure’ was the lowest at 2.87. The higher the age, the more the satisfaction from the laser treatment, resulting in the highest satisfaction rate from women in their 50s. Ultrasonic lasers, such as Ultherapy and Shrink Wrap Liposuction, were found to be more satisfactory than other procedures when each laser skin tightening procedure was compared in terms of satisfaction and willingness to receive re-procedures.
Fourth, 56.8% of the patients did not experience any side effects from the laser skin tightening, which was higher than 43.2% of the patients who said they experienced such side effects. Among the symptoms of the side effects, stinging was 41.8%, followed by facial redness 28.9% and burning feeling 18.5%. Among the several types of laser skin tightening, Ultherapy was the lowest in side effect rates.
The above results showed that the higher the age group, the more sensitive they feel about the loss of skin elasticity due to aging; therefore, with the least understanding of the procedure-related equipment, the women in their 50s spent the most money in laser skin tightening, and showed more satisfaction with the procedure than any other age groups. Ultherapy, the ultrasonic laser, received the highest satisfaction rate while it caused the least side effects.
The results of analyzing the understanding and satisfaction of laser skin tightening according to each age group derived from this study are meaningful in that they can be used as basic data to provide highly satisfactory service of laser skin tightening suitable for the needs of each age group.
