הממצא המרכזי במחקרנו הוא ששיעור מומי הלב המולדים וחומרתם בקרב ילודים של נשים שהרו באמצעות ART אינו שונה באופן מובהק מזה של נשים שהרו באופן ספונטני. (2.3% בהריונות ART לעומת 2.5% בהריונות ספונטניים, p=0.279). במחקר נמצאו הבדלים בשיעורי סיבוכי ההריון והלידה, כאשר בהריונות ART שיעור הסיבוכים היה גבוה יותר.
קיימים מחקרים שמראים על קשר בין טיפולי ART לבין מומים מולדים בעובר. מחקרים מצביעים על 30% יותר מומים מולדים ביילודים להריונות ART לעומת יילודים להריונות ספונטניים.
קיימים מחקרים קודמים אשר בהם נמצא ששיעור מומי הלב במטופלות ART הוא בסביבות פי 2 יותר מאשר בנשים שהרו ספונטנית (11) (15). אחד מהמחקרים הנ"ל הוא מחקר שנעשה בארצות הברית ופורסם בשנת 2009 : assisted reproductive technology and major structural birth defects in the united states (15). מחקר זה ערך הפרדה בין הריונות של עובר יחיד לבן הריונות תאומים. מכיוון שבמחקר שלנו נכללו הריונות יחיד בלבד, נתייחס רק לחלק זה של המחקר האמריקאי. מחקר זה מצא שלעוברים שנולדו בעקבות ART סיכון של פי 2.1 ל Septal heart defect (95% CI 1.1-4.0), ופי 1.4 ל conotruncal heart defect (95% CI 0.6-3.2). בעוד שבמחקר שלנו לא מצאנו קשר בין ART למומי לב מולדים. כדי לנסות להבין מדוע קיים שוני בין המחקרים ננסה לעמוד את השינויים ביניהם. בחירת אוכלוסיית המחקר הינה שונה: במחקר שלנו השוואנו בין נשים שהרו ספונטנית (קבוצת ה case) לבין נשים שהרו באמצעות ART (קבוצת ה control), ובדקנו את שיעור מומי הלב בקבוצות השונות. במחקר האמריקאי השוו בין תינוקות עם מומים מולדים (קבוצת ה case שכללה 13586 תינוקות) לתינוקות ללא מומים מולדים (קבוצת ה control שכללה 5008 תינוקות), ובדקו את שיעור ה ART בקבוצות השונות. המחקר האמריקאי בדק מתוך אוכלוסיית התינוקות, מי מהאמהות עברה ART. בעוד שהמחקר שלנו בודק מתוך אוכלוסיית הנשים מי עברה טיפולי ART, ולאיזה שיעור מתוכן נולד ילוד עם מום לבבי. לדעתנו זו דרך נכונה יותר לבדוק את השפעת טיפולי הפוריות על מומים מולדים ביילוד. בנוסף המחקר שלנו מייצג את כל האוכלוסייה וכולל בתוכו את כל הנשים שילדו בזמן תקופת המחקר, לעומת המחקר האמריקאי שלא כלל את כל התינוקות שנולדו בזמן תקופת המחקר. לדעתנו בחירת קבוצת ה control במחקר האמריקאי לא מייצגת בצורה נכונה את האוכלוסייה ממנה נבחרו ויכול להיות שנעשתה כאן הטייה בבחירה (selection bias). שוני נוסף קיים במספר המקרים של ART: במחקר שלנו 6725 הריונות ART, שמתוכם 3622 בקבוצת טיפול תרופתי, ו- 3103 בקבוצת IVF (שכולל בתוכו IVF ו-IVF ICSI). במחקר האמריקאי ART הוגדר כIVF בלבד (שכולל בתוכו IVF ו- IVF ICSI) וכלל רק 23 מקרים בקבוצת ה control, ו176 מקרים בקבוצת הcase. כלומר מספר הנשים שעברו ART במחקר האמריקאי קטן משמעותית מזה שבמחקרנו, ויכול להיות שאינו מייצג את כל אוכלוסיית הנשים שעברו ART. יכול להיות שנעשתה כאן טעות מסוג type 1 error, שבה יש קבלה שגוייה של השערת המחקר, מסוג false positive. המחקר האמריקאי כלל ילודים עם 30 סוגים שונים של מומים מולדים, בעוד במחקרנו נכללו ילודים עם מומי לב בלבד, ילודים עם מומים אחרים הוצאו מהמחקר.
מחקר אחר, בדק את הקשר בין ART לארבעה מומי לב מולדים ספציפיים, שפורסם בשנת 2013 ונקרא: The risk for four specific congenital heart defects associated with ART (11). מחקר זה מצא שיילודים שנולדו לאחר ART היו בסיכון של פי 2.4 למום לבבי מסוג Tetralogy of Fallot (TOF). במחקר זה השוו בין ילודים עם ארבעה מומי לב ספציפים (קבוצת ה case, שכללה 1583 ילודים) לבין ילודים עם מומים מולדים אחרים ספציפיים, שאינם מומי לב (קבוצת ה control, שכללה 4101 ילודים). קבוצת ה case כללה את ארבעת המומים הבאים: Hypoplastic left heart syndrome, Transposition of great arteries, Tetralogy of Fallot, ו- Coarctation of the aorta. קבוצת ה control כללה מספר מומים ספציפיים, שנבחרו ע"י עורכי המחקר. המחקר הנ"ל לא בדק את כל האוכלוסייה, כמו שנעשה במחקרנו, אלא בחר ילודים עם מומים מאוד ספציפים, הן בקבוצת ה case והן בקבוצת ה control, דבר שלדעתנו קרוב לוודאי גרם להטייה בבחירה. גם כאן קיים שוני באופן בחירת אוכלוסיית המחקר, אנחנו בדקנו את הנשים שילדו בתקופת המחקר, בעוד שהמחקר השני בדק רק אוכלוסיית מומים. בנוסף, תוצאות המחקר הראו קשר בין ART למום מסוג TOF בלבד, ולא נמצא קשר בין ART לשאר מומי הלב שנבדקו. חשוב לציין שהמחקר לא ביצע הפרדה בין הריונות יחיד להריונות מרובי עוברים. ידוע שהריונות מרובי עוברים מהווים גורם סיכון למומים מולדים בעובר (22). לכן יכול להיות שהריון מרובה עוברים הוא הגורם ליותר מקרי TOF בילודים, ולא טיפולי הפוריות שעברו הנשים.
המחקר שלנו מייצג את האוכלוסייה בשלמותה ולא חשוף להטייה בבחירה. בחרנו כאוכלוסיית המחקר בכל הנשים שילדו בתקופת זמן שהגדרנו, ולא ביצענו בחירת תינוקות עם מומים מסויימים, כמו שנעשה במחקרים האחרים. מבחינה סטטיסטית גודל המדגם שלנו גדול (238537 נשים), וכמו כן גם מספר הנשים שעברו ART גדול (6725 נשים). מסיבות אלה אנחנו מאמינים שהמחקר שלנו מייצג נכונה יותר ובצורה מהימנה יותר את הקשר בין טיפולי פוריות והיארעות מומי לב מולדים.
אוכלוסיית הנגב, וכמו כן גם אוכלוסיית מחקרנו, כוללת בתוכה נשים ממוצא יהודי וממוצא בדואי. אוכלוסיית הנשים הבדואיות לא התפלגה באופן שווה בין קבוצות המחקר שלנו, ולכן עלתה אצלנו השערה שאולי מוצא האישה השפיע על התוצאות. באוכלוסייה הבדואית בישראל 60% מהנישואים הם נישואי קרובים. ידוע שנישואי קרובים מהווים גורם סיכון למחלות גנטיות ולמומים מולדים בעובר (17). בנוסף שיעור הילודה באוכלוסייה זו גבוה מאוד והסטטוס הסוציו אקונומי הינו נמוך, אשר מוסיפים לשיעור הגבוה של המומים המולדים בקרב הבדואים (18). לכן חשבנו שאולי מיעוט הנשים ממוצא בדואי בקבוצת ה ART (15% בIVF, 23.4% בטיפול תרופתי) ומספרם הגבוה בקבוצת ההריון הספונטני (53.7%) הסיט את תוצאות. מסיבות אלה ראינו לנכון לבדוק את שיעור מומי הלב בקרב אוכלוסיית הנשים היהודיות בלבד. גם בבדיקה זו מצאנו שאין הבדל בין הקבוצות, כלומר גם כשמבודדים את האוכלוסייה היהודית מהאוכלוסייה הבדואית, נמצא כי אין קשר בין טיפולי פוריות למומי לב מולדים.
סוכרת טרום הריונית מהווה גורם סיכון לסיבוכים אמהיים ועובריים, ביניהם יותר מקרי אי פריון ויותר מומים מולדים בעובר, ובפרט מומים לבביים. בעקבות ידיעה זו ערכנו השוואה נוספת שבדקה מומי לב ביילודים לאמהות בריאות לעומת יילודים לאמהות חולות בסוכרת טרום הריונית, בקרב מטופלות שעברו IVF. גם בהשוואה זו לא מצאנו הבדלים מובהקים בין הקבוצות, כלומר ההתערבות הרפואית של ART לא העלתה, גם בקרב אוכלוסיית הנשים חולות סוכרת רום הריונית, את שיעור מומי הלב.
במחקרנו מצאנו שטיפולי פוריות מהווים גורם סיכון לסיבוכים רבים הן לאישה והן ליילוד, דבר אשר ידוע בספרות (23). ישנם הבדלים מובהקים בין הקבוצות מבחינת סיבוכי הריון- יותר מחלות היפרטנסיביות של ההריון, יותר פתולוגיות בשלייה, יותר מקרים של ירידת מים מוקדמת ולידות מוקדמות, בנשים שעברו ART. גם מבחינת סיבוכי לידה נמצאו שיעורי סיבוכים גבוהים יותר בנשים שעברו ART: יותר מנחים ומצגים פתולוגים, יותר ניתוחים קיסריים. מגמה דומה גם בנוגע לסיבוכי משכב לידה: משך אשפוז ארוך יותר, יותר מקרים של חום בלידה ושל אנדומטריטיס במטופלות ART. בנוסף גילינו שילודים שנולדו בעקבות ART נתונים ליותר סיכונים: משקל הלידה שלהם נמוך יותר, ציוני האפגר נמוכים יותר, יותר מוות עוברי APD ,PPD. ניראה שיש מקום ליידע זוגות המתכננים לעבור טיפולי פוריות בסיכונים הנ"ל.
לסיכום, במחקרנו שהינו מחקר קוהורט המייצג אוכלוסייה, לא נמצא קשר בין צורת הכניסה להריון, בין אם באמצעות טיפולי פוריות ובין אם ספונטני, בהריונות יחיד, לבין מומי לב מולדים ביילוד.
Translation - English Discussion
The main finding in our study is that there was no statistically significant difference in the rate of congenital heart defects and their severity among neonates of women who conceived via ART than the rate and severity of those born to women who conceived spontaneously (2.3% in ART pregnancies compared to 2.5% in spontaneous pregnancies, p=0.279). The study found differences in the rate of pregnancy and delivery complications, where the rate of complications was higher among the ART pregnancies.
There are studies that demonstrate the link between ART treatments and fetal congenital defects. Studies indicate 30% more congenital defects in neonates conceived due to ART compared to neonates conceived spontaneously (8). The main defects that appear in the literature are:
Esophageal atresia, anorectal atresia, cleft lit with or without cleft palate, septal heart defects, and hypospadias (15).
There are previous studies that found that the rate of heart defects among women treated via ART is about twice as high as it is among women who conceived spontaneously (11) (15). One of the aforementioned studies is a study done in the United States and published in 2009: Assisted Reproductive Technology and Major Structural Birth Defects in the United States (15). This study distinguished between singleton pregnancies and pregnancies with twins. Since our study includes only singleton pregnancies, we will only take into consideration that part of the American study. This study found that fetuses born due to ART are 2.1 times more at risk of having septal heart defect (95% CI 1.1-4.0), and 1.4 times more at risk of having conotruncal heart defect (95% CI 0.6-3.2), while our study did not find correlation between ART and congenital heart defects. In order to try and understand why there is disparity between the studies, we will try to assess the differences between them. The population selection for the studies was different. In our study we compared between women who conceived spontaneously (the case group), and women who conceived via ART (the control group), and we examined the rate of heart defects in the different groups. In the American study they compared babies with congenital defects (the case group, which included 13,586 babies) with babies without congenital defects (the control group, which included 5,008 babies), and they examined the rate of ART in the different groups. The American study looked into the babies’ population, and examined which of their mothers had undergone ART, while our study looks into the population of mothers who have undergone ART treatments, and examines within it the rate of neonates born with a heart defect. In our opinion this is a more accurate method of examining the effects of fertility treatments on neonate congenital heart defects. Additionally, our study represents the entire population, and includes within it all women who have given birth during the study period, compared to the American study, which did not include all babies born during the study period. In our opinion, the selection of the control group in the American study does not accurately represent the population from which it was chosen, and there could have been a selection bias. Another disparity is evident in the number of ART cases: our study includes 6,725 ART pregnancies, out of which 3,622 are in the medicinal treatment group, and 3,103 are in the IVF group (which includes within it IVF and IVF ICSI). The American study defined ART as IVF only (which includes within it IVF and IVF ICSI), and included only 23 cases in the control group, and 176 cases in the case group. Meaning, the number of women who have undergone ART treatments in the American study is significantly smaller than that in our study, and it is possible that it does not represent the entire population of women who have undergone ART. It is possible that a type 1 error was made here, in which there is a wrongful acceptance of the study hypothesis – a false positive. The American study included neonates with 30 different types of congenital defects, while our study included neonates with heart defects only. Neonates with other defects were excluded from the study.
A different study, which examined the correlation between ART and four specific congenital heart defects, was published in 2013 under the title: The Risk for Four Specific Congenital Heart Defects Associated with ART (11). This study found that neonates born due to ART were 2.4 times more at risk of having the Tetralogy of Fallot heart defect (TOF). This study compared neonates with four specific heart defects (the case group, which included 1,583 neonates) with neonates with other specific congenital defects, that are not heart defects (the control group, which included 4,101 neonates). The case group included the following four defects: Hypoplastic left heart syndrome, Transposition of great arteries, Tetralogy of Fallot, and Coarctation of the aorta. The control group included a number of specific defects, which have been chosen by the study’s editors. The aforementioned study did not examine the entire population, as was done in our study, but selected neonates with very specific defects, in the case group as well as in the control group, which in our opinion most likely lead to a selectin bias. Here, too, there is a difference in the method of selecting the study population. We examined women who gave birth during the study period, while the other study examined only population with defects. Additionally, the results of the study show a correlation between ART and the TOF type defect alone, and found no correlation between ART and the other heart defects that were examined. It is important to note that the study did not distinguish between singleton pregnancies and pregnancies with multiple fetuses. It is known that pregnancies with multiple fetuses are at a higher risk of developing fetal congenital defects (22). Therefore, it is possible that a pregnancy with multiple fetuses is the factor that is responsible for having more TOF cases in neonates, and not the fertility treatments that the women have undergone.
Our study represents the entire population and is not susceptible to selection bias. We have selected, as the study population, all women who have given birth during the period we have defined, and we did not select babies with specific defects, as was done in other studies. Statistically speaking, our sample size is large (238,537 women), as well is the number of women who have undergone ART (6,725 women). For these reasons we believe that our study represents more accurately and more credibly the correlation between fertility treatments and congenital heart defects.
The Negev population, as well as our study population, includes within it women of Jewish descent, and of Bedouin descent. The Bedouin women population was not evenly distributed among our study’s groups, and for that reason we hypothesis that perhaps the descent of the woman affected the results. In the Bedouin population in Israel, 60% of marriages are between relatives. It is known that inbreeding is a risk factor for genetic diseases and fetal congenital defects (17). Additionally, the rate of birth in this population is very high, and the socioeconomic status is low, which add to the high rate of congenital defects among the Bedouins (18). Due to that, we thought that maybe the Bedouin minority in the ART group (15% in IVF, 23.4% in medicinal treatment) and their high number in the spontaneous pregnancy group (53.7%) has diverted the results. For these reasons we have found it fitting to examine the rate of heart defects among the Jewish women alone. In this examination as well we have found no differences between the groups, meaning that when you isolate the Jewish population from the Bedouin population, then, too, you find no correlation between fertility treatments and congenital heart defects.
Pre-gestational diabetes is a risk factor for complications for the mother and the fetus, among which are more cases of infertility and more fetal congenital defects, in particular heart defects. Due to this knowledge we have made another comparison, which examined heart defects in neonates born to healthy mothers, and heart defects in neonates born to mothers who have pre-gestational diabetes, among patients who have undergone IVF. In this comparison as well we found no statistically significant differences between the groups, meaning the medical interference of ART did not increase, among women with pre-gestational diabetes as well, the rate of heart defects.
In our study we found that fertility treatments are a risk factor for many complications for the woman as well as for the neonate, a thing which is known in the literature (23). There are statistically significant differences between the groups in regards to pregnancy complications – more hypertensive diseases of pregnancy, more placental pathologies, more incidences of early water breaking and premature births among women who have undergone ART. In regards to delivery complications we also found higher rates of complications among women who have undergone ART. More pathological presentations and lies, more Caesarian surgeries. A similar trend in regards to postnatal complications: longer hospitalization period, more cases of fever during delivery and endometriosis in ART patients. Additionally, we found that neonates born due to ART are susceptible to more risks: their birth weight is lower, lower Apgar scores, more PPD, APD fetal death cases. It appears that it is proper to inform couples who are planning to have fertility treatments of the aforementioned risks.
In conclusion, our study, which is a population representative cohort study, found no correlation between the method of conception, whether it is via fertility treatment or if it is spontaneous, in singleton pregnancies, and neonate congenital heart defects.
