Source text - English Prognosis of patients with HER-2 positive metastatic breast cancer has been revolutionized with the development of dual antibodies targeting HER-2 and subsequent use of antibody-drug conjugate, T-DM1. Nevertheless, resistance to anti-HER2 antibodies can develop and some tumors show de novo resistance. Although the benefit of trastuzumab beyond progression remains one of the standard of care even after failure from dual HER-2 blockades, there is no guideline to refer based on individual tumor biology in clinical practice. Moreover, the prognosis of patients who failed in a few standards HER-2 directed treatment is dismal.
Proposed mechanisms of resistance to trastuzumab include: (i) overexpression of other ERBB receptors and/or to other EGFR family ligands, (ii) altered activation of downstream pathways through the loss or inactivation of the PTEN tumour suppressor gene , activating PI3K mutations, or dependence on signalling through alternative growth factor receptors leading to hyper-activation of the PI3K/Akt/mTOR signalling pathway, (iii) alterations in antibody binding sites or masking of binding epitopes, (iv) increased circulating HER2 extracellular domain, (v) altered immune mechanisms affecting ADCC-response and p27kip1 downregulation, and elevated CDK2 activity. Of the multiple suggested mechanisms, loss of phosphatase and tensin homolog (PTEN) and activation of mutations of PI3KCA appears to be associated with primary or de novo trastuzumab resistance. The PI3K pathway plays a central role in the transduction of signals from RTKs regulating protein synthesis, cell growth, survival, motility, and angiogenesis. In addition, multiple PIK3CA regulators, including EGFR, Met, Kit, PDGFR, HER2, Akt, PTEN, p110α are known to be genetically altered by the mechanisms of activating mutations, deletion, and amplification.
For these reasons, continuation of anti-HER2 therapy in combination with targeted agents based on precise mechanistic approach should be considered.
Translation - Korean HER-2 양성 전이성 유방암 환자의 예후는 HER-2를 표적으로 하는 이중 항체의 개발과 항체-약물 결합체 T-DM1의 사용으로 혁명을 가져왔다. 그럼에도 불구하고 항-HER-2항체에 내성이 생기고 또 일부 종양은 새로운 내성을 보인다. 트라스츄주맙(trastuzumab)의 효과는 이중 HER-2차단의 실패 후에도 치료의 기준으로 남아있지만 임상 현장에서 개별 종양 생물학에 따라 언급하는 지침이 없다. 또한 일부 표준 HER-2 지향성 치료에 실패한 환자의 예후는 좋지 않다.
트라스츄주맙에 대한 내성 기작은 (i) 다른 ERBB 수용체 및/또는 다른 EGFR과 리간드의 과잉 발현, (ii) PTEN 종양 억제 유전자의 소실 또는 불활성화에 의한 하류 경로의 활성화의 변화, PI3K 돌연변이의 활성화, PI3K/Akt/mTOR 신호전달 경로의 과잉 활성화를 가져올 대안적인 성장인자 수용체에 의한 신호 전달에 의존, (iii) 항체 결합 부위의 변화 또는 결합 항원 결정인자 차단, (iv) 순환 HER2 세포 외 도메인의 증가, (v) 변화된 ADCC-반응 및 p27kip1 다운 조절에 영향을 미치는 면역 기작 및 CDK2 활성의 증가를 보인다. 제안된 여러 기작 중 포스파타아제 및 단신 상동체(PTEN) 의 소실과 PI3KCA 돌연변이의 활성화는 주된 또는 새로운 트라스츄주맙 내성과 관련이 있는 것으로 보인다. PI3K 경로는 단백질 합성을 조절하는 RTKs, 세포 증식, 생존, 운동 및 혈관 신생에 관여하는 신호 전달에 있어서 중추적인 역할을 한다. 또한 EGFR, Met, Kit, PDGFR, HER2, Akt, PTEN, p110α를 포함하는 여러 PIK3CA 조절자는 돌연변이, 결실 및 증폭을 활성화하는 기작에 의해 유전적으로 변화하는 것으로 알려져 있다.
이 때문에 정확한 기작적 접근에 따라서 표적 약물과 함께 항-HER2 치료의 지속이 반드시 고려되어야 한다.
