Незважаючи на значні успіхи фундаментальної та клінічної медицини в усьому світі, однією з провідних причин материнської та перинатальної смертності залишається прееклампсія (ПЕ), яка, за даними ВООЗ, ускладнює 2 – 8 % всіх вагітностей [1].
Етіологія ПЕ є багатофакторною, більш того, хвороба сама по собі є мультисистемним проявом складної патофізіології. Механізми розвитку прееклампсії залежать від багатьох факторів, які є складними та до кінця невивченими. В даний час основною патологічною ланкою в якості пускового механізму розвитку ПЕ більшість авторів сучасності вважають ендотеліоз, що розвивається за допомогою різних процесів, які можуть спостерігатися як окремо, так і в комбінації.
Впродовж останніх кількох років основна увага приділяється теорії порушення плацентації, що призводить до ангіогенного дисбалансу, пов'язаного з ендотеліальною дисфункцією в межах певних кровоносних судин [2], в зв'язку з дисфункціональною перфузією міжворсинчатого простору і подальших окисних та гемодинамічних порушень. Завдяки цьому відбувається вивільнення численних трофобласних факторів плаценти, які сприяють розвитку материнської запальної відповіді, що призводить до гіпертонії та протеінурії [3-4]. Вважається, що плацентарна прееклампсія виникає в процесі плацентації (приблизно з 12 до 16 тижнів вагітності) і може призвести до більш раннього розвитку та більш серйозного захворювання, тоді як материнські чинники ( материнська прееклампсія) призводять до більш пізнього захворювання [5] . Плацентарна дисфункція призводить до плацентарного стресу, що веде до появи ознак і симптомів ПE [6].
Для пояснення плацентарної теорії в патогенезі прееклампсії також були проведенні дослідження, в яких, як фактор ризику, розглянуто вплив розмірів, ваги та розташування плаценти.
L’Abee C та ін. досліджували батьківські фактори, які впливають на плацентарну вагу при збільшені індексу маси тіла (ІМТ), та повідомили, що маса плаценти зросла на 3,6 г для кожного додаткового кг/м2 матері [7]. J.M. Wallace та ін. провели дослідження плацентарної ваги у 55105 вагітних на основі трьох груп, в які ввійшли плаценти з низькою вагою – нижня третина (середня 484 г), середньою вагою – середня третина (середня 622 г) та високою вагою – верхня третина (середня 788 г). Це призвело до висновку, що вага плаценти в нижній третині була фактором ризику прееклампсії (P
Translation - English Placenta and maternal pre-eclampsy markers as a component of early screening
Despite the significant worldwide success of fundamental and clinical medicine pre-eclampsy (PE) remains one of the key causes of both maternal and perinatal mortality; according to WHO data up to 2-8% of pregnancies are associated with pre-eclampsy [1].
Multiple factors contribute to PE etiology. Moreover, PE itself is a manifestation involving various body systems with a complex pathophysiologic background. PE mechanisms depend on various complex factors that require further investigation. At present, most authors consider endotheliosis the main pathologic element that triggers PE. Endotheliosis is a result of multiple processes, both single and acting in combination.
Recent studies were focused on placentation disorders theory resulting in angiogenesis impairments associated with endothelium dysfunction in particular blood vessels [2] – usually due to perfusion dysfunction in chorion villi with resulting in disorders of oxygenation and blood circulation. These processes contribute to release of multiple trophoblastic factors from placenta. These factors facilitate maternal inflammatory response that results in hypertension and proteinuria [3-4]. It is considered that placenta pre-eclampsy develops during the placentation process (gestation weeks 12-16). It can result in earlier and more severe disease, while maternal factors (maternal pre-eclampsy) result in later disease [5]. Placenta dysfunction results in placenta stress followed by manifestation of PE signs and [6].
Placenta-associated theory of pre-eclampsy pathogenesis is supported by study results, where placenta size, weight and location were evaluated as risk factors.
L’Abee C et. Al. studied maternal factors that influence placenta weight increase in association with body weight index (BWI) increase. They report placenta weight increase up to 3.6 g with each extra kg/m2 unit of maternal BWI [7]. J.M. Wallace studied placenta weight in 55105 pregnant patients subdivided into 3 groups according to weight: low placenta weight – lower third (mean weight 484 g), medium weight – medium third (mean weight 622 g) and high weight – upper third (mean weight 788 g).
