Source text - English The main principle of surgery for ascending aortic aneurysms is that of preventing the risk of dissection or rupture by restoring the normal dimension of the ascending aorta. If the aneurysm is proximally limited to the sinotubular junction and distally to the aortic arch, resection of the aneurysm and supra-commissural implantation of a tubular graft is performed under a short period of aortic clamping, with the distal anastomosis just below the aortic arch. External wrapping or reduction ascending aortoplasty (the aorta is not resected but is remodelled externally by a mesh graft) is, in general, not recommended but may be used as an alternative to reduce the aortic diameter when aortic cannulation and cardiopulmonary bypass are either not possible or not desirable. This may be the case in elderly patients with calcified aorta, in high-risk patients, or as an adjunct to other off-pump procedures.
If the aneurysm extends proximally below the sinotubular junction and one or more aortic sinuses are dilated, the surgical repair is guided by the extent of involvement of the aortic annulus and the aortic valve. In the case of a normal tricuspid aortic valve, without aortic regurgitation or central regurgitation due to annular dilation, an aortic valve- preserving technique should be performed. This includes the classic David operation with re-implantation of the aortic valve into a tubular graft or, preferably, into a graft with sinus functionality (Web Figure 9).

Source text - Russian Жалобы на боли, ограничение подвижности в правом тазобедренном суставе. Из анамнеза 30.03.2014 перенес ОНМК по ишемическому типу в бассейне правой СМА. 07.04.14г. падение с высоты своего роста в палате диагностирован перелом шейки правого бедра со смещением. На КТ головного мозга от 01.04.2014: КТ картина не исключает наличие ишемического инсульта в бассейне правой СМА (лобная доля), КТ признаки атрофических изменений головного мозга. Выписан 15.04.2014г после курса стационарного лечения, рекомендовано наблюдение специалистов и курс восстановительной терапии в специализированном отделении. 08.05.2014г.: тотальное эндопротезирование правого тазобедренного сустава (протез Серф, Франция). Послеоперационный период протекал гладко. Получал симптоматическую, антикоагулянтную, антибактериальную терапию, ЛФК. Активизирован, обучен ходьбе на костылях. Поступил в OBJ1 на реабилитацию.

CT scan of the brain from 01.04.2014: CT scan does not exclude ischemic stroke in the right MCA (frontal lobe) basin. CT shows signs of atrophic changes in the brain. The patient was discharged on 15.04.2014 after a course of inpatient treatment with recommendations for specialist supervision and a course of rehabilitation therapy in a specialized department. 08.05.2014: patient underwent total hip replacement of the right hip joint (SERF prosthesis, France). The postoperative period was uneventful. The patient received symptomatic, anticoagulant, antibacterial therapy, exercise therapy, was successfully mobilized, trained to walk on crutches, was admitted to OBJ1 for rehabilitation.
Examination:
The overall condition is satisfactory. Mesomorphic. Skin, visible mucous membranes are of normal color and moisture. Subcutaneous fatty tissue is developed excessively. Peripheral lymph nodes are not enlarged. No edema. Peripheral vascular pulsation is preserved. The superficial veins of the lower extremities are not dilated. The thyroid gland is not enlarged.

Source text - Ukrainian Наведено порівняння показників генетичної стабільності культури клітин кісткового мозку та жирової тканини щурів під час їх культивування in vitro. Зміни у генетичному апараті були виявлені як у культурі клітин кісткового мозку, так і жирової тканини. Аналіз каріотипу культур клітин кісткового мозку та жирової тканини щурів показав, що за використаних нами умов їх культивування, кількість анеуплоїдій та поліплоїдій змінюється з кожним пасажем, але не виходить за межі спонтанного мутагенезу, характерного для ссавців. За результатами цитогенетичної оцінки культури встановлено, що кількість клітин з мікроядрами та двоядерних клітин знаходиться у межах норми з першого до шостого пасажу. Кількість клітин з анеуплоїдією та мікроядрами у культурі клітин жирової тканини був нижчий на всіх пасажах ніж у культурі клітин кісткового мозку, що свідчить про її більшу генетичну стабільність за даними показниками. Найвищий мітотичний індекс визначається на першому пасажі з подальшим зниженням в процесі культивування, що зворотно пропорційно апоптозному індексу.
Translation - English A comparison of the genetic stability indicators of rat bone marrow and adipose tissue cell cultures during their cultivation in vitro is presented. Changes in the genetic apparatus have been found in both bone marrow and adipose tissue cultures. Karyotype analysis of these cultures showed that under the conditions of their cultivation that were set by us, the number of aneuploidies and polyploids changes with each passage but does not exceed the level of spontaneous mutagenesis, typical for mammals. After cytogenetic evaluation of the culture, it was found that the number of binucleated cells and cells with micronuclei stays within normal range from the first to the sixth passage. The number of cells with aneuploidy and micronuclei in adipose cell culture was lower in all passages than in bone marrow cell culture, which indicates its greater genetic stability according to these indicators. The highest mitotic index is determined at the first passage with a subsequent decrease during the cultivation process, which is inversely proportional to the apoptotic index.

A t symbol next to the grade indicates that the candidate was exempt from satisfying the full range of assessment objectives in the examination.
