Source text - English ADDITIONAL INFORMATION IN THE ANNUAL REPORT -WHO UNDERSTANDS A MULTI-OBJECTIVE REPORT?
Long before people were talking about the balanced-scorecard concept, many realized that service companies, at least, needed to present a fuller picture of their business. Social audits and environmental accounting were tried in the 1970s, human-resources accounting in the 1980s. Around 1990, some companies in Sweden began to provide more information in their financial statements on matters such as employee experience and environ-mental impact. Sveiby (1997, p. 185) furnishes some examples, noting that "even the world's largest computer companies, such as computer consultants EDS and Cap Gemini Sogeti, provide no more than a few hints that they employ human beings", so that what we are discussing is still not commonly encountered.
While we favour this trend on the whole, we do have reser-vations. The measures presented by some companies may be considered variants of the scorecard, if one takes a charitable view. However, these measures have their shortcomings and have been used only to a limited extent over the years. The principal objection is that they fail to meet the need noted above for management to justify investments in various kinds of intellectual capital in relation to the possible futures facing the company. Perhaps we would need new so-called rating institutes to provide a reasonably impartial opinion of the validity of such reasoning. But who would give such an institute its mandate? Probably the company's own management, who would have an interest in per-suading outside parties, just as management pays the company's auditors (although these are formally appointed by the annual shareholders' meeting). Perhaps what is needed is a more thorough familiarity with the company's plans and systems than any outside party could realistically be expected to possess.

Source text - English Reform of the personal income tax is on the current policy agenda in Ukraine. Several reform proposals are under consideration and all of them contain significant changes in both the definition of the tax base and the rate structure. One of them, however, would convert the tax from its present origin-base into a residence-based levy. As suggested by Teriokhun (2002), taxpayers would no longer pay personal income taxes to the local government jurisdiction in which they work but would instead make that payment to the local government where they live. The rationale for this switch in tax destination is that most of the local public services consumed by taxpayers occurs in the area where they maintain a residence rather than in the different area where they may work.
Western countries that impose a local personal income tax almost always do so on a residence basis. In Ukraine, however, the personal income tax is not a local tax but rather a national one whose proceeds are assigned to local governments to assist them with their financing needs. The possibility of moving to a residence basis for assigning these proceeds exposes several important issues that are explored in this note. First, how would application of the residence principle affect the distribution of personal income tax receipts among local governments? The answer, somewhat surprisingly, is that it would have no, or almost no, effect on the distribution of revenues. Secondly, is this answer still valid if local governments were permitted to impose personal income tax surcharges, as the draft Tax Code would allow? Third, what other issues arise besides that of revenue distribution? Finally, what changes in tax administration are needed to successfully apply the residence principle? Before addressing these matters in turn, this note raises the importance for local government efficiency and accountability of having a significant marginal revenue source under local government control.

Розглядається ціла низка пропозицій, і всі вони істотно різняться між собою як щодо визначення бази оподаткування, так і щодо структури ставок. Однак серед них є одна пропозиція, яка передбачає перетворення цього податку з податку, що сплачується до місцевого бюджету за місцем походження доходу (тобто за місцем роботи платника податку), на податок, що сплачуватиметься до місцевого бюджету за місцем проживання платника. Йдеться про пропозицію народного депутата С. Терьохіна (2002), згідно з якою платники податків мають сплачувати прибутковий податок з громадян не за місцем роботи, а за місцем проживання. Необхідність цього переходу аргументується тим, що споживання більшості місцевих суспільних послуг платниками податку відбувається там, де вони мешкають, а не там, де працюють.
У тих західних країнах, де застосовується прибутковий податок з громадян у вигляді місцевого податку, він стягується саме за місцем проживання. В Україні, однак, прибутковий податок з громадян є не місцевим, а загальнодержавним податком, закріпленим за місцевими бюджетами, тобто всі надходження від нього спрямовуються до місцевих бюджетів і застосовуються для фінансування їхніх видатків. Можливість переходу до податку за місцем проживання і відповідно зарахування надходжень від нього до місцевого бюджету адміністративно-територіальної одиниці, в якій мешкає платник, порушує низку важливих питань, вивченню яких і присвячена ця стаття. По-перше, як застосування принципу стягнення за місцем проживання вплине на розподіл надходжень від прибуткового податку з громадян між місцевими бюджетами? Відповідь на це запитання є дещо несподіваною: перехід до нової системи не матиме ніякого або майже ніякого впливу на розподіл надходжень. По-друге, чи залишиться ця відповідь незмінною, якщо буде дозволено запровадження додаткових прибуткових податків з громадян, як передбачається в проекті Податкового кодексу? По-третє, на що ще вплине перехід до стягнення за місцем проживання, крім розподілу надходжень? Насамкінець, які зміни слід внести до системи адміністрування податку, аби можна було успішно застосовувати принцип місця проживання? Резюме. Узагальнено результати органозберігаючого лікування 86 хворих на рак молочної залози (РМЗ) І-ІІ А, Б стадій. Наведено дані про 5-річну виживаність пацієнток, частоту рецидивів, а також про частоту розвитку ускладнень лікування. Не виявлено статистично достовірної різниці між показниками виживаності в залежності від виду операції, проведення передопераційної або післяопераційної променевої терапії. Рецидиви розвинулись у 12,8% хворих, процеси, які клінічно симулювали рецидив, – у 18,6%, дифузні рубцеві зміни, що клінічно і рентгенологічно симулювали інфільтративний рак, – у 10,4%, лімфостази – у 30,2%. Окрім рецидивів пухлини, які при РМЗ І стадії діагностувались значно рідше ніж при ІІ А-Б стадіях, частота ускладнень органозберігаючого лікування практично не залежала від стадії захворювання.

Translation - English EXPERIENCE OF APPLICATION OF ORGAN-PRESERVING SURGERY IN TREATING BREAST CANCER

Summary. Results of organ-preserving treatment of 86 patients with breast cancer (BC) at stages ²-²² A, B are summarized. Data are reported describing 5-year survival of patients, recurrence rates, as well as frequency of complications. There was no statistically significant difference in survival depending on the type of surgery, and pre- or post-surgery radiation therapy. Recurrences developed in 12.8% patients, processes that clinically simulate recurrences – in 18.6%, diffuse scarry alterations, which simulate infiltrative cancer when examined clinically and by X rays – in 10.4%, and lymphostases – in 30.2%. Unlike recurrences, which in BC at stage ² were diagnosed much more rarely compared to stage ²² A-B, the frequency of complications of organ-preserving treatment did not depend effectively on the stage of disease.

Автор роману «Люборацькі», писав також короткі прозові твори – оповідання, у ліку його поезій – «Україна мати наша». Напрацював цікаві наукові розвідки з подільського святкового побуту та елементів давньої міфології у світогляді українців
Коли на Галичині в журналі „Зоря” (1886) вперше з’явився в світ роман „Люберацькі” (пізніше відомий як „Люборацькі”), читацька публіка зацікавлено розпочала, на жаль, запізнілий пошук ширших відомостей про автора — Анатолія Патрикійовича Свидницького.
Свидницький народився в Маньківці на Поділлі в священницькій родині. Наполегливо заводжувані духовними консисторіями «новизни» до священничого побуту спонукали сільське духовенство віддавати дітей на навчання до бурси, щоби згодом, по її завершенні, випускники могли вступати до духовних семінарій. Звісно, що традиція хатнього навчання, яка побутувала у священничих сім’ях ще здавна, не перепинялася і в ХІХ ст. Певно, що початки науки малий Анатолій здобув ще від батька. У 1843 р. десятилітнього хлопця відіслано на навчання до Крутянського духовного училища в Балтському повіті. Згодом Свидницький вступив до духовної семінарії у Кам’янці-Подільському. Та семінарського навчання не закінчив. Далі життєвий шлях привів А. Свидницького до Київського університету, де першопочатково його предметом вивчення стає медицина, але невдовзі, через переведення за власним проханням, — предметний курс історико-філологічного факультету.
Та й навчання в університеті Свидницький не завершив, — на заваді стала несплата за навчання. Через те 1860 р. він склав іспити на звання вчителя російської словесності й одержав призначення на вчителювання у Миргороді. У своїй миргородській викладацькій практиці неодноразово долучав до уроків українську мову, коли говорив про українську літературу і культуру. Миргород А. Свидницький полишив 1862 р., аби не потрапити до списку «неблагонадійних». Потім було влаштування в акцизному відомстві в м. Козельці на Чернігівщині. Згодом йому довелося шукати роботу в Одесі та Києві.
Та незважаючи на життєву круговерть, художньо-естетична вдача таки рухала єством Свидницького, його рука частіше бралася до пера. Він писав вірші, які ширилися і як пісні, був і вмілою музикальною особою.
Та особливого піднесення приніс письменнику роман «Люборацькі. Сімейна хроніка». У творі є риси, «перелиті» з власного біографічного «довідника» автора. У романі виписана ціла трагічна подільська епопея втрат — людської, традиційної, ширше — культурної. Значне місце серед робіт А. Свидницького належить і науковим розвідкам, що ґрунтувалися на особисто зібраному фольклорному та етнологічному матеріалі.

Author of novel “The Liuboratskies”, he also wrote short narratives, such as novellas, and poems, “Ukraine, Our Mother,” among others. Anatoly Svydnytsky fulfilled interesting research dealing with Podillia’s festive customs and elements of an ancient mythology in Ukrainians’ view of life
When the novel “The Liuberatskies” (later known as “The Liuboratskies”) was first published in the Zoria magazine, Galicia (1886), the reading public began an enthusiastic but unfortunately belated search for wider information about the author, Anatoly Patrykiyovych Svydnytsky.
Svydnytsky was born in Mankivtsi, Podillia, in a priest’s family. The “novelties” that were insistently introduced into priests’ way of life by clerical authorities prompted the rural priesthood to send their children to bursas in order for them to enter, after graduation, theological seminaries. Assuredly, the tradition of home learning, which had been existing since olden times in priests’ families continued in the 19th century. It is obvious that the first teacher of young Anatoly was his father. In 1843 the boy of ten was sent to Kruty, to a theological school in the Balta County. Later on, Svydnytsky entered the theological seminary in Kamianets-Podilsky. But he did not finish his course at the seminary. Further on, Svydnytsky’s walk of life brought him to the Kyiv University, where he originally studied medicine, but shortly, at his own will, he switched to the curriculum of the historical-and-philological department.
Yet, he did not complete the university course as he failed to pay the tuition fee. Because of this, in 1860, he passed exams to receive the degree of teacher of Russian language and literature and was appointed to a position of teacher in Myrhorod. In his teaching practice in Myrhorod, he repeatedly took on the Ukrainian language in his lessons when speaking about Ukrainian literature and culture. A. Svydnytsky left Myrhorod in 1862 in order to avoid being included in the list of “political suspects”. Later, he occupied a position in the excise department in Kozelets, Chernihiv Province. Afterwards, he had to search for a job in Odesa and Kyiv.
Despite the whirligig of life, Svydnytsky’s artistic and aesthetic disposition drove him to write, and his hand ever more frequently turned to pen. He wrote poems, many of which became known as songs, since he was a skilled musician as well.
But his greatest celebrity as a writer came with his novel “The Luboratskies. Saga.” This work contains some features derived from the author’s own biographic “reference book”. The novel describes quite a tragic epic canvas of losses, including human, traditional, and, in wider terms, cultural losses. A considerable place among A. Svydnytsky’s works is occupied by his research based on folklore and ethnologic materials collected by himself.

The inactivity of Ukrainian officials at interrogations in the Lazarenko case raises the danger of his acquittal in the US courts
Almost unnoticed by outsiders and the public - slow and steady wins the race - American investigators involved in fact-finding in the USA vs. Pavlo Lazarenko case to be tried in September are getting on with their work in Kiev. Details are scarce. It is unknown, for instance, whom the parties (the US Prosecutor’s Office and Lazarenko’s lawyers) have already interrogated. The number of people who have visited the office rented by the Americans also remains secret. The interrogations (or, rather, interviews) are being held in premises provided by the Ukrainian General Prosecutor’s Office. The only information disclosed by Maryna Dolhopola, the ex-premier’s Ukrainian attorney, is that the prosecution and defense teams conduct questioning every working day.
The Ukrainian on-line news agency Ukrainska Pravda has learned from sources unrelated to the case some key details about the approval of the defense list. It is now known that it was based on the lawyers’ petition that a US judge issued a subpoena to Leonid Kuchma, which he has chosen to ignore. Also at the request of Lazarenko’s lawyers, members of Kuchma’s entourage have been summoned to testify, including Marchuk, Pustovojtenko, Volkov, and Bakay.
Altogether there were almost 80 names — well-known and less well-known — on the list filed to Judge Martin Jenkins by the lawyers. The judge rejected some potential witnesses. Those “saved from torture” include the First Lady, Liudmila Kuchma, and the President’s daughter, Olena Franchuk. The judge put his decision down to an unwillingness to allow a surfeit of witnesses. This is despite the fact, Ukrainska Pravda reports, that the document filed by lawyers to the Northern District Court in California reads, “Ms Kuchma (this refers to Liudmila — Ed.) can provide testimony on nearly 75 conversations between 1995 and 97, in which she took part together with her husband, daughter, and ex-son-in-law and which concerned Lazarenko’s business activities. She can also testify that Lazarenko did not try to conceal his wealth or pretend that he earned only $5000 a year and that he in fact gave expensive gifts to her, her husband (Leonid Kuchma — Ed.), and daughter.” In a similar vein is information in the petition for “verbal cooperation” from Olena Kuchma: “She was present during a number of meetings when Lazarenko lavished valuable gifts on her parents, including a $42,000 Franck Muller watch given on August 19, 1997.” It is impossible not to notice, Ukrainska Pravda comments, that Lazarenko had already been relieved of the Prime Minister’s post by that time.

Orthostatic Hypotension
NAFADOL may induce orthostatic hypotension associated with dizziness, tachycardia, and in some patients, syncope, especially during the initial dose-titration period, probably reflecting its alpha-adrenergic antagonistic properties. Syncope was reported in 0.2% (6/2607) of NAFADOL-treated patients in Phase 2-3 studies. The risk of orthostatic hypotension and syncope may be minimized by limiting the initial dose to 2 mg total (either QD or 1 mg BID) in normal adults and 0.5 mg BID in the elderly and patients with renal or hepatic impairment (see DOSAGE AND ADMINISTRATION). Monitoring of orthostatic vital signs should be considered in patients for whom this is of concern. A dose reduction should be considered if hypotension occurs. NAFADOL should be used with particular caution in patients with known cardiovascular disease (history of myocardial infarction or ischemia, heart failure, or conduction abnormalities), cerebrovascular disease, and conditions which would predispose patients to hypotension, e.g., dehydration and hypovolemia. Clinically significant hypotension has been observed with concomitant use of NAFADOL and antihypertensive medication.
DOSAGE AND ADMINISTRATION
Usual Initial Dose
NAFADOL can be administered on either a BID or a QD schedule. In early clinical trials, NAFADOL was generally administered at 1 mg BID initially, with increases in increments of 1 mg BID on the second and third day, as tolerated, to a target dose of 3 mg BID by the third day. Subsequent controlled trials have indicated that total daily nafadone doses of up to 8 mg on a QD regimen are also safe and effective. However, regardless of which regimen is employed, in some patients a slower titration may be medically appropriate. Further dosage adjustments, if indicated, should generally occur at intervals of not less than 1 week, since steady state for the active metabolite would not be achieved for approximately 1 week in the typical patient. When dosage adjustments are necessary, small dose increments/decrements of 1-2 mg are recommended.

ДОЗИРОВКИ И СХЕМЫ ВВЕДЕНИЯ

NAFADOL можно применять по схеме одноразового приема суточной дозы или приема суточной дозы за два раза. Во время первых клинических испытаний NAFADOL применяли, как правило, вначале в дозе 1 мг два раза в день с увеличением по 1 мг два раза в день на второй и третий день, и в случае хорошей переносимости доводили до окончательной дозы в 3 мг два раза в день ко третьему дню. Последующие контролируемые испытания показали, что суммарные дневные дозы до 8 мг в режиме один раз в день также безопасны и эффективны. Однако, вне зависимости от применяемой схемы, у некоторых пациентов может быть целесообразно с медицинской точки зрения проводить титрацию более медленными темпами. Дальнейшие корректировки дозы, если таковые показаны, следует, как правило, проводить с интервалами не менее 1 недели, поскольку у типичного пациента устойчивое состояние активного метаболита достигается не ранее, чем примерно за 1 неделю. В случае необходимости корректировки дозы рекомендуется увеличивать/уменьшать дозу небольшими количествами — по 1-2 мг.
