5 Procedures for Safety and Efficacy Evaluations
5.1 Efficacy/Pharmacodynamic Evaluations
No efficacy or pharmacodynamic parameters will be evaluated. However, ECOG performance status will be recorded as an overall measure of subject status. Body weight changes will also be captured; however, given the lack of specificity of this measure to distinguish lean muscle mass from body fat, this will not necessarily be considered indicative of therapeutic effects.
5.2 Safety Evaluations
5.2.1 Adverse Events
The investigator will determine during the course of the study whether any AEs have occurred. The subjects will be questioned in a general way and no specific symptoms will be suggested. Section 9 Adverse Events contains additional information with regard to AEs.
5.2.2 Clinical Laboratory Tests
Clinical laboratory tests will be performed by investigator/PRA International (PRA) at an accredited, clinical laboratory facility.

Source text - English 7.5 Dispensing and Administration of Treatment
Subjects enrolled into the study will receive the following (with sufficient overage to cover at least three additional days of dosing) at each clinical visit:
Formoterol: 1 packet for 40 and 80 µg bid dose cohorts and 2 packets for 120 and 160 µg bid dose cohorts.
Roxithromycin: 5, 10-tablet blister packs
Megestrol: 1, 150-mL bottle of Megace ES® (contains 125 mg/mL of megestrol)
Study subjects will take the prescribed doses of study drug just prior to their morning meal and just prior to their evening meal (formoterol and roxithromycin only).
7.6 Compliance with Prescribed Study Drug Dosing Regimen
When the patient returns to the clinic after Visit 3 (baseline and first day of dosing), research clinic staff will conduct an inventory of any returned unused study drug, determine if the patient was compliant with his/her prescribed dosing regimen, and document the results in the drug accountability log and case report form as appropriate. If the patient was less than 80% compliant, that patient should be counseled by the research clinic staff prior to dispensation of any further study drug for continuation of dosing.
7.7 Drug Accountability
PRA will request the investigator or their designee to sign a receipt for the study drugs on the day of receipt. A drug accountability record should be maintained by the person responsible for dispensing the study drug to the subject, which should record which supplies are issued to which subjects and any drugs returned unused. PRA should be notified immediately of the details of any supplies that are inadvertently damaged or lost. PRA will be responsible for drug accountability at the central drug depot storage and distribution site(s).
All drugs will be inventoried by PRA during and at the conclusion of the study. Secure disposal at the end of the study will be arranged.

Indication: Cancer Anorexia and Cachexia Syndrome
Primary Objective: To determine the safety profile, MTD, and dose-limiting toxicities of formoterol in combination with a fixed dose of roxithromycin in patients with advanced NSCLC who are receiving standard-of-care chemotherapy (carboplatin doublet regimen) and a widely used appetite stimulant (megestrol acetate).
Secondary Objective: To select a dose or doses of formoterol to investigate in combination with roxithromycin in future development for the treatment of cancer cachexia.
Study Design:
This is an open-label study of a combination of formoterol (dose-escalated in sequential cohorts) and roxithromycin (fixed dose) as an adjunct to megestrol acetate in patients with advanced non-small cell lung cancer (NSCLC). The table below shows the dose-escalation scheme for sequential patient cohorts. Each cohort will enroll six eligible subjects. An initial safety assessment will be made after all six subjects in a cohort have completed dosing through Day 1 of Week 4. If the dose-escalation stopping rules are met at this initial dose, the study will terminate. If the dose-escalation stopping rules are not met, the six subjects in that cohort will continue through Week 9, and screening will begin for the subsequent cohort. Formoterol dose-escalation will continue by cohort until a dose level of formoterol is given that meets the dose-escalation stopping rules or until the highest planned dose of formoterol (160 µg bid) is given. If dose-escalation stopping rules are met, then six additional subjects will be enrolled (Cohort 5) to confirm the apparent maximum tolerated dose (MTD) at the next lower dose and provide additional safety data at this dose level for a longer duration of treatment (12 weeks).
