You are suffering from anemia (a condition in which the red blood cell count or the hemoglobin is lower than normal) as a consequence of chronic renal insufficiency. Erythropoietin, a hormone produced mainly in the kidneys in a healthy person, acts on the erythrocyte precursor cells (red blood cells) in the bone marrow and thus promotes the formation of mature red blood cells. Hemoglobin (the substance that gives blood its red colour) is the primary constituent of red blood cells. It binds oxygen in the blood and transports it to the individual organs and cells, where hemoglobin absorbs carbon dioxide in exchange. The purpose of this epoetin treatment is to raise the hemoglobin value and thus to avoid blood transfusions.
2. What is the purpose of the clinical trial?
Hexal Biotech has developed a new genetically engineered epoetin (EPO HEXAL). It is largely comparable to ERYPO®, which has already been granted a marketing authorisation and is on the market in the EU since 1994. The purpose of this clinical trial is to investigate the immunogenicity (the ability of the EPO HEXAL to cause an immune response in the body) and safety of this newly developed epoetin in the treatment of predialyis patients suffering from anemia compared with treatment with ERYPO® (Janssen-Cilag, Germany).
3. What other treatment options are there?
To treat your anemic condition you may also be given a different epoetin or a blood transfusion.
4. What is the procedure for the clinical trial?
This clinical trial will be conducted in several locations in Germany, Austria, Romania, Czech Republic, Slovakia, Poland or other EU and non-EU countries. In total approximately 300 patients will be involved.
Half of the patients will receive EPO HEXAL during the study and half will receive ERYPO®. All patients will be assigned to one of the two treatment groups according to a predefined code which has been generated according to a random procedure (= “Randomized”). This code will not be known to your or your physician. We therefore say that the study will be conducted under “double-blind” conditions. Randomization and double-blinding help us to ensure that physicians and patients judge the efficacy and safety of both medicinal products impartially. In an emergency, however, your physician can find out which group you belong to at any time.
Your participation in this clinical trial is planned to last 52 weeks. You will visit the doctor responsible for your treatment once each week or once every second week during the first 7 weeks of treatment, once every two weeks from week 8 until week 24 of study treatment, and finally every four weeks from week 24 to the end of the study treatment in week 52. Additional control visits may be necessary for a control of blood.
Before being included in this clinical trial, the previous history of your illness will be recorded, and you will undergo a thorough medical examination that will also include an ECG (electrocardiogram to determine your heart’s electrical activity) and laboratory tests. In women with childbearing potential, a pregnancy test will also be performed. In the two-week run-in phase, a review will be undertaken to check whether you meet all the preconditions for participation in this clinical trial.
After two weeks for screening and confirmation of your hemoglobin level, you will be randomized to one of the two treatment groups and receive the allocated medication (EPO HEXAL or ERYPO®) until the end of the trial. At the beginning, the medication will be injected 3 times weekly subcutaneously (directly under the skin of your stomach or of your upper thigh). Repeated subcutaneous doses are possible, depending on which dose you are given. It is possible that the frequency of injections will be reduced to once or twice per week during the course of the study. The medication will be administered during your visit by the doctor treating you. If applicable, further appropriate doses of the medication will be given to you to take home to inject yourself. The study medication is temperature sensitive. Therefore, it has to be stored in your refrigerator at home. You will be provided with special cooling bags for the transport home and also with special instructions on how to handle the medication. In the beginning, all patients will receive the same dose.
In order to establish your current state of health and to test the response to treatment, a blood sample will be taken from you at each visit (a total of about 180 ml blood for all 25 visits, which corresponds to about half of a normal blood donation). Your vital functions will be measured (blood pressure, pulse and weight) and any health changes will be recorded. Further blood samples may have to be taken as a result of your anemia or therapy and the amount of blood taken could therefore increase. However, it is also possible that no additional blood samples will be necessary. The results of the blood analysis may result in an adjustment of the epoetin dosage during the course of the study.
At the beginning of week 24 and 52 of the clinical trial, the thorough examination performed at the start of the study will be repeated and you will be asked to assess the tolerability of the study treatment. If you withdraw from the study prematurely, the final examination will still be carried out.
If you decide to take part in this study, you will have to follow all the instructions given by the investigating doctor/s or his /her assistants assisting in this study.
