Source text - English Un paciente de sexo masculino de 56 años con antecedentes significativos de osteoartritis desarrolló edema de tobillo bilateral en abril de 2011 y comenzó un tratamiento con aspirina tres meses después de comenzar una terapia intravenosa. La creciente dificultad para soportar peso condujo a exámenes radiológicos y la identificación de fracturas óseas en tibia/peroné izquierdos y segundo metatarsiano derecho el 09Oct2011.
La aspirina, con su excreción de calcio aumentada a través de los riñones, combinada con el estilo de vida aparentemente sedentario del paciente, podría haber contribuido al debilitamiento de la densidad ósea y al desarrollo de las fracturas descriptas. De acuerdo con una revisión de los datos, ningún otro caso de fractura en extremidades superiores o inferiores ha sido relacionado causalmente con el nuevo fármaco.
Actualmente existen consultas pendientes respecto del manejo del seguimiento de este caso, incluyendo resultados si los hay de radiografías y análisis de funciones renales.
Teniendo en cuenta la historia clínica de este paciente, su estilo de vida aparentemente sedentario y el mecanismo de acción conocido del nuevo fármaco, se considera que los eventos de fracturas de tibia/peroné y metatarsiano no están relacionados con la droga en estudio o el procedimiento del ensayo clínico. El presente informe no altera el perfil de riesgo-beneficio del nuevo fármaco.
Translation - English A 56-year-old male patient with a significant history of osteoarthritis developed a bilateral ankle edema in April 2011 and started a treatment with aspirin three months after beginning an intravenous therapy. The growing difficulty to bare weight led to radiologic exams and identification of bone fracture in left tibia and fibula and second right metatarsus on 9 October 2011.
The aspirin, with an increase in its calcium excretion through the kidneys, combined with the apparent sedentary life style of the patient, could have helped to the weakening of the bone density and the development of the mentioned fractures. According to a data review, no other case of upper or lower extremities fracture has been related causally to the new drug.
Nowadays, there are pending consultations about the administration of the monitoring of this case, including results of radiography and analysis of kidney functions.
Considering the clinic history of this patient, his apparently sedentary life stile and the mechanism of action known of the new medicine, it is considered that the events of tibia/fibula and metatarsus fractures are not related to the studied medicine or the procedure of clinic test. The actual article does not alter the profile of risk/benefit of the new medicine.
