What are temporomandibular disorders?
Temporomandibular (TM) disorders is the name given to several problems with jaw movement and pain in and around the jaw joints .
You may also hear TM disorders called TMJ, TMD, or TM problems.
The jaw joints, or temporomandibular (TM) joints, connect the lower jawbone (mandible) to the skull. These flexible joints are used more than any other joint in the body. They allow the jaw to open and close for talking, chewing, swallowing, yawning, and other movements.
Many people have problems with jaw movement and pain in and around the jaw joints at some time during their lives. These joint and muscle problems are complex. So finding the right diagnosis and treatment of TM disorders may take some time.
What are the symptoms?
TM disorders can affect the jaw and jaw joint as well as muscles in the face, shoulder, head, and neck. Common symptoms include joint pain, muscle pain, headaches, joint sounds, trouble with fully opening the mouth, and jaw locking.
In most cases, symptoms of TM disorders are mild. They tend to come and go without getting worse and usually go away without a doctor’s care. About 65% to 95% of people who see a doctor when they first have symptoms will get better no matter what type of treatment they get.1
Some people who have TM disorders develop long-lasting (chronic) symptoms. Any chronic pain or difficulty moving the jaw may affect talking, eating, and swallowing. This may affect a person's overall sense of well-being.

What causes TM disorders?
The most common cause of TM disorder symptoms is muscle tension, often triggered by stress. When you are under stress, you may be in the habit of clenching or grinding your teeth. These habits can tire the jaw muscles and lead to a cycle of muscle spasm, tissue damage, pain, sore muscles, and more spasm.
TM disorders can start when there is a problem with the joint itself, such as:
• An injury to the joint or the tissues around it.
• Problems with how the joint is shaped.
• Joint diseases, such as osteoarthritis or rheumatoid arthritis.
• The articular disc that cushions the joint shifts out of place.
How is a TM disorder diagnosed?
Although there is no one way to identify a TM disorder, your doctor can most likely check your condition with a physical exam and by asking questions about your past health. In some cases, an X-ray, CT scan, or MRI is also used to check for bone or soft tissue problems related to symptoms of TM disorder.
How is it treated?
TM disorder symptoms usually go away without treatment. Simple home treatment can often relieve mild jaw pain. There are things you can do at first to reduce pain.
• Rest your jaw joint.
• Use medicines for a short time, to reduce swelling or relax muscles.
• Put either an ice pack or a warm, moist cloth on your jaw for 15 minutes several times a day if it makes your jaw feel better. Or you can switch back and forth between moist heat and cold. Gently open and close your mouth while you use the ice pack or heat. But don't use heat if your jaw is swollen. Use only ice until the swelling is gone.

• Eat soft foods, and avoid chewy foods and chewing gum.
Getting physical therapy and learning ways to reduce stress may also help to reduce pain and TM joint problems. Continue to use some of these methods over time to prevent and manage symptoms that might come back. If your pain is chronic or severe or is caused by problems with how the joint is shaped, your doctor may recommend other treatments.
Splints, also called biteplates, are a common dental treatment for TM disorders. Splints are usually clear pieces of plastic that fit between the upper and lower teeth. They help reduce grinding and clenching. Splints are used for a short time so that they do not cause permanent changes in the teeth or jaw.
Before you try treatments such as surgery or reshaping or shaving down the teeth, think it over. These treatments cannot be reversed and can even damage the TM joint.
For most people, surgery is not used to treat TM disorders. Surgery has few benefits, and there is the chance of causing more serious problems.
You and your doctor can carefully weigh a decision to have surgery. Talking with another doctor to get a second opinion can also help you make your decision.
Chronic pain can lead to depression, anxiety, and other problems. If you have chronic pain, talk to your doctor about medicine and mental exercises to manage the pain. Give special attention to treating any related anxiety or depression.

Что такое височно-челюстное расстройство?
Височно-челюстное расстройство (Temporomandibular (TM) disorders) включает ряд нарушений работы челюстного сустава, что сопровождается болезненными ощущениями. Существуют и другие названия этого синдрома, например, ВНЧС. Челюстные суставы или височно-челюстные суставы соединяют нижнюю челюсть (mandible) и основание черепа. Эти гибкие суставы используются человеком чаще всего, поскольку они раскрывают и закрывают рот в процессе общения, жевания, глотания, зевания и других движений. Многие люди испытывают болезненные ощущения при движении этих суставов в определенные периоды своей жизни. Природа дисфункции этих суставов и мышц сложная, поэтому установление диагноза и курса лечения требует определенного времени.
Симптомы синдрома дисфункции височно-челюстного сустава
Заболевание поражает челюсть и челюстные суставы, а также мышцы лица, плеча, головы и шеи. Как правило, больные испытывают боль в суставах и мышцах, головную боль, звуки при движении суставов, трудности открытия рта и смыкании челюсти.
В большинстве случаев заболевание протекает мягко, при этом симптомы то появляются, то сами исчезают. От 65% до 95% больных, которые обратились за помощью к врачу при появлении этих симптомов, испытывают облегчение, не смотря на вид назначенного лечения. Иногда развивается хроническая форма заболевания, что влияет на процесс общения, прием пищи и глотание. К тому же, это отображается в целом на состоянии здоровья человека.
Что провоцирует это расстройство?
Как правило, синдром дисфункции височно-челюстного сустава вызывается мышечным напряжением, которое провоцируется стрессом. В стрессовых ситуациях очень часто мы стискиваем зубы, что напрягает челюсть и приводит к мышечному спазму, повреждению ткани, боли в мышцах и повторению спазма. Но иногда синдром развивается при нарушении функции самого сустава, что наблюдается в результате:
• травмы сустава или мышечной ткани;
• изменения формы сустава;
• заболевания суставов, например, остеоартрита или ревматоидного артрита;
• смещения или деформации суставного хряща.
Методы диагностики синдрома дисфункции височно-челюстного сустава
Не существует одного способа диагностики этого заболевания, но лечащий врач сделает физический осмотр и задаст вопросы об истории заболевания. Иногда назначается рентген, срез КТ или МРТ для того, чтобы определить состояние костной и мышечной ткани.
Методы лечения синдрома дисфункции височно-челюстного сустава
В большинстве случаев симптомы заболевания исчезают сами по себе. В домашних условиях можно облегчить умеренную боль следующими способами:
• дайте отдых челюстному суставу;
• некоторое время принимайте медицинские препараты для снятия припухлости и напряжения мышц;
• прикладывайте лед или теплую влажную салфетку на 15 минут несколько раз в день; можно также чередовать лед и влажную салфетку; осторожно открывайте и закрывайте рот во время этой процедуры; ни в коем случае не прикладывайте горячее к припухлой щеке; лед можно использовать только после исчезновения припухлости;
• кушайте мягкую пищу и избегайте продуктов, которые нужно тщательно пережевывать.
Физиотерапия и контроль стрессовых ситуаций способствуют устранению боли и самого заболевания. Продолжайте использовать эти методы некоторое время в профилактических целях. В случае хронической формы заболевания (при изменении формы сустава) врач назначит другой вид терапии. Иногда используются шины или ортодонтические пластинки. Шины – прозрачная конструкция из пластмассы, которая помещается между верхними и нижними зубами, что в свою очередь понижает силу стискивания зубов. Их используют непродолжительное время, поскольку они могут вызвать изменения зубов или сустава.
Перед тем, как решиться на операцию или выравнивание зубов, хорошо подумайте, поскольку результаты этих вмешательств необратимы и могут нанести вред височно-челюстному суставу. В большинстве случаев хирургическое вмешательство не проводится, потому что может спровоцировать проблемы. Вместе с врачом следует взвесить все за и против, а иногда даже проконсультироваться с еще одним специалистом. Хроническая боль приводит к депрессии, беспокойству и другим проблемам. При постоянной боли поговорите с врачом о возможном применении медицинских препаратов и методах снятия боли. Особенно внимательно отнеситесь к лечению сопутствующих заболеваний – депрессии и беспокойства.

Most ectopic pregnancies can be detected using a pelvic exam, ultrasound, and blood tests. If you have symptoms of a possible ectopic pregnancy, you will have:
A pelvic exam, which can detect tenderness in the uterus or fallopian tubes, less enlargement of the uterus than expected for a pregnancy, or a mass in the pelvic area.
A pelvic ultrasound (transvaginal or abdominal), which uses sound waves to produce a picture of the organs and structures in the lower abdomen. A transvaginal ultrasound is the most dependable way to show where a pregnancy is. A pregnancy in the uterus is visible 6 weeks after the last menstrual period. An ectopic pregnancy is likely if there are no signs of an embryo or fetus in the uterus but hCG levels are elevated or rising.
Two or more blood tests of pregnancy hormone (human chorionic gonadotropin, or hCG) levels, taken 48 hours apart. During the early weeks of a normal pregnancy, hCG levels double every 2 days. Low or slowly increasing levels of hCG in the blood suggest an early abnormal pregnancy, such as an ectopic pregnancy or a miscarriage. If hCG levels are abnormally low, further testing is done to find the cause.
Sometimes a surgical procedure using laparoscopy is used to look for an ectopic pregnancy. An ectopic pregnancy after 5 weeks can usually be diagnosed and treated with a laparoscope. But laparoscopy is not often used to diagnose a very early ectopic pregnancy, because ultrasound and blood pregnancy tests are very accurate.
Follow-up testing after treatment
During the week after treatment for an ectopic pregnancy, your hCG (human chorionic gonadotropin) blood levels are tested several times. Your doctor will look for a drop in hCG levels, which is a sign that the pregnancy is ending (hCG levels sometimes rise during the first few days of treatment, then drop). In some cases, hCG testing continues for weeks to months until hCG levels drop to a low level.
What to think about
If you become pregnant and are at high risk for an ectopic pregnancy , you will be closely monitored. Doctors do not always agree about which risk factors are serious enough to watch closely. But research suggests that risk is serious enough if you have had a tubal surgery or an ectopic pregnancy before, had DES exposure before birth, have known fallopian tube problems, or have a pregnancy with an intrauterine device (IUD) in place.1
A urine pregnancy test-including a home pregnancy test-can accurately diagnose a pregnancy but cannot detect whether it is an ectopic pregnancy. If a urine pregnancy test confirms pregnancy and an ectopic pregnancy is suspected, further blood testing or ultrasound is needed to diagnose an ectopic pregnancy.

В большинстве случаев внематочную беременность можно определить в процессе вагинального осмотра, УЗИ и анализа крови. При симптомах внематочной беременности:
• нужно пройти вагинальный осмотр, в процессе которого врач определит болезненность в области матки или фаллопиевых трубах, увеличение размеров матки больше обычного;
• сделать УЗИ (трансвагинально или абноминально), которое предоставляет четкое изображение внутренних органов и их структуры в нижней части брюшной полости. Трансвагинальное исследование (УЗИ) - более надежный способ диагностирования беременности, которую можно определить уже на 6 неделе после последнего менструального цикла. В случае внематочной беременности врач не увидит признаков эмбриона или плода в матке, но анализ крови будет свидетельствовать о повышенном уровне гормонов;
• сдать анализ крови (два или больше раза) для определения уровня гормонов (человеческий хорионический гонадотропин) с промежутком в 48 часов. В первые недели нормальной беременности уровень этого гормона удваивается каждые два дня. Низкий или слабо возрастающий его уровень свидетельствует о внематочной беременности или выкидыше. Если уровень этого гормона слишком низкий, нужно сделать дополнительные анализы для выявления причины.
Иногда проводят лапароскопию для определения внематочной беременности, которую можно увидеть и прервать на 5 неделе. Но этот метод используют не часто, поскольку УЗИ и анализ крови дают точные результаты.
Последующая диагностика после лечения
Через неделю после лечения внематочной беременности нужно несколько раз проверять уровень гормона беременности (человеческого хорионического гонадотропина). Если его уровень падает, то внематочная беременность прервана (иногда в первые дни после лечения уровень гормона может повышаться, но потом, как правило, падает). В некоторых случаях анализы повторяются более длительное время (от недель до месяцев), пока врач не убедится, что уровень гормона упал до минимума.
О чем следует подумать
Если Вы беременны и находитесь в группе риска, следует тщательно обследоваться. Врачи не всегда соглашаются по поводу факторов риска внематочной беременности, но очевидно одно – риск возрастает после истории внематочной беременности, операции на фаллопиевых трубах или беременности с одновременной внутриматочной спиралью.
Тест на определение беременности, который продается в аптеках и предполагает анализ мочи, всегда точно укажет на состояние беременности, но не может выявить ее патологию, а именно, внематочную беременность. Поэтому после того, как Вы получили позитивный результат в домашних условиях, и подозреваете внематочную беременность, нужно обратиться к врачу, который назначит анализ крови и УЗИ в случае необходимости.
