Source text - English The Chlamydiae are a group of obligate intracellular parasites with viral and bacterial characteristics. The order consists of three species, Chlamydia trachomatis, Chlamydia pneumoniae and Chlamydia psittaci.
C. trachomatis has been linked to urogenital infections (e.g. urethritis, epididymitis, cervicitis, PID), trachoma, infant pneumonia, and lymphogranuloma venereum. C. pneumoniae and C. psittaci are linked to pneumonia and respiratory diseases.
Identification of Chlamydia directly in specimen material by culture, monoclonal DIFA, ELISA and nucleic acid amplification techniques are the most often used methods for the aid in diagnosis of Chlamydia infections. Serology may be helpful as additional information, providing indirect evidence of exposure, but may not correlate with infected status. Chlamydia serologies are not recommended for diagnosis of active disease except in suspected cases of LGV, psittacosis, and infants with pneumonia (IgM only).
While direct antigen tests are ideal to detect infections of the lower genital tract, they fail in the case of silent chronic infections of the upper genital tract, which represent a frequent cause of female infertility. This is, by excellence, the field of investigation for serology. Due to sampling problems, they are of interest to investigate respiratory infection to one of the three Chlamydia species.
Translation - Ukrainian Хламідії – це група облігатних внутрішньоклітинних паразитів з вірусними та бактеріальними характеристиками. Підклас утворений трьома видами: хламідія трахоматіс (Chlamydia trachomatis), хламідія пневмонія (Chlamydia pneumoniae) та хламідія псіттачі (Chlamydia psittaci).
Хламідія трахоматіс може викликати урогенітальні інфекції (наприклад, уретрит, епідидиміт, цервіцит, запальне тазове захворювання), трахому, запалення легень у дітей та венеричну лімфогранулому. Хламідія пневмонія та хламідія псіттачі спричиняють запалення легень та респіраторні захворювання.
Виявлення хламідії безпосередньо у матеріалі зразка за допомогою культивування, моноклонального прямого імунофлуоресцентного аналізу, ІФА та технологій ампліфікації нуклеїнової кислоти є найбільш поширеними методами, що застосовуються при діагностиці інфекцій хламідії. Серологічні дослідження можуть бути корисними як джерело додаткової інформації, подаючи непряме свідчення впливу паразитів, однак можуть не співвідноситись зі станом інфікування. Серологічні методи виявлення хламідії не рекомендуються для діагностики активних захворювань, за винятком випадків підозри на венеричну лімфогранулому, псіттакоз та запалення легень у дітей (лише імуноглобулін М).
Якщо безпосередні антигенні тести ідеально підходять для виявлення інфекцій нижніх статевих шляхів, вони не дають результату у випадку прихованих хронічних інфекцій верхніх статевих шляхів, які часто є причиною жіночого безпліддя. Source text - English 3-D Doppler analysis of placental vascularisation in pregnant women after oocyte donation
Introduction: Our aim was to describe changes in vascularisation and perfusion pattern of placentas in pregnancies after oocyte donation compared to spontaneous pregnancies.
Materials and methods: 33 pregnant patients treated with oocyte donation were studied and compared to 42 spontaneous pregnant subjects. Inclusion criteria were: 1) only single pregnancies, 2) no smokers, 3) no exposure to other toxins or use of any medication, (v) absence of any previous or current maternal pathology, 4) absence of evident placental pathology, 5) normal foetal biometrics and morphology, 6) normal amniotic fluid and umbilical artery flow. Then a 3-D Power Doppler was used to calculate three separate volumes of placental tissue between the 18th and 22nd week of pregnancy in each case. Then three portions of each obtained volume were calculated off-line using virtual organ computer-aided analysis (VOCALTM) software to asses a representative spherical issue probe that qualifies as a virtual “placental biopsy”. The vascularisation indexes analysed were the Grey index (IG), the Vascularisation Index (IV), the Flow Index (IF) and the Vascularisation-Flow Index (IVF). The average values of these nine measurements were calculated and cross compared. After pregnancy follow up the perinatal results were also evaluated.
Results: Vascular alterations could be shown by the statistically significant higher Grey Indexes (IG: p=0,001) and lower Vascularisation Indexes (IV: p=0,039) in the group of oocyte donation pregnancies, independently of age. A statistical significant (p=0,046) lower Vascularisation-Flow Indexes (IVF), characteristic of a deficient placental perfusion profile could also be observed in this group. Regarding perinatal results a lower mean birth weight, a shorter duration of pregnancy and a higher cesarean section rate could be found.
Conclusions: Placentary perfusion is deficient in pregnancies obtained following oocyte donation in comparison to spontaneous pregnancies. This observation may help to explain the inferior perinatal results observed in this group of patients, regardless of maternal age and the presence of more than one foetus. 3-D Power Doppler is more accurate than other previously reported methodology, and may be a new prognostic parameter to assist the ultrasound diagnosis of such high risk pregnancies.
Translation - Ukrainian Трьохвимірний енергетичний допплерівський аналіз плацентарної васкуляризації у вагітних жінок після донації ооцитів
Вступ: Наша мета – опис змін у васкуляризації та характері перфузії плаценти при вагітностях після донації ооцитів у порівнянні з спонтанними вагітностями.
Матеріали та методи: Досліджено випадки 33 вагітних пацієнток, яким було проведено донацію ооцитів, та проведено порівняння з 42 випадками спонтанних вагітностей. Критерії включення: 1) лише одноплідні вагітності, 2) жінки, які не палять, 3) відсутність впливу інших токсинів чи застосування будь-яких ліків, (v) відсутність будь-якої попередньої чи існуючої материнської патології, 4) відсутність явної патології плаценти, 5) нормальна біометрія та морфологія плоду, 6) нормальні навколоплідні води та кровотік пупкової артерії. Тоді було застосовано трьохвимірний енергетичний допплер для обчислення трьох окремих об’ємів плацентарної тканини між 18-м та 22-м тижнем вагітності у кожному випадку. Тоді три частини кожного отриманого обсягу були автономно обчислені за допомогою програмного забезпечення з віртуального комп’ютеризованого аналізу органів (VOCALTM) для оцінки репрезентативного зразка сферичної тканини, що визначається як віртуальна “плацентарна біопсія”. Були проаналізовані такі показники васкуляризації: сірий показник (IG), показник васкуляризації (IV), показник кровотоку (IF) та показник васкуляризації-кровотоку (IVF). Було обчислено та порівняно перехресно середні значення цих дев’яти замірів. Після вагітності було також проаналізовано перинатальні результати.
Результати: Про судинні зміни можуть свідчити статистично значимі вищі сірі показники (IG: p=0,001) та нижчі показники васкуляризації (IV: p=0,039) у групі вагітностей після донації ооцитів, незалежно від віку. У цій групі також спостерігались статистично значимі (p=0,046) нижчі показники васкуляризації-кровотоку (IVF), характерні для недостатньої плацентарної перфузії. Що стосується перинатальних результатів, були виявлені нижча середня вага при народженні, коротша тривалість вагітності та вищий відсоток кесаревого розтину.
Висновки: Недостатність плацентарної перфузії спостерігається у вагітностях, що настали в результаті донації ооцитів, у порівнянні з спонтанними вагітностями. Це спостереження може допомогти пояснити гірші перинатальні результати, що спостерігаються у цієї групи пацієнток, незалежно від материнського віку та присутності більше, ніж одного плоду. Трьохвимірний енергетичний допплерівський аналіз є більш точним, ніж інша попередньо заявлена методологія, і може стати новим прогнозним параметром для поліпшення ультразвукової діагностики вагітностей з таким високим ризиком.
