秋になると私はさみしかった。しかしそれも、小学生だったころの話だ。私は高校生になったばかりだった。もう秋になっても、特にさみしくはなかった。もしさみしくなったとしても秋のせいじゃなくて、それはいつの季節にでも起こりうる、誰にでも起こりうる、ただのありきたりのさみしさでしかなかった。そしてそんなさみしさでは、私がどういう人間かを証明することはできないのだ。ましてや、じきに夏だった。「新緑の季節じゃなくなったからほっとする」と、カネコフが言った。カネコフというのは、高校に上がって同じクラスになった私たちがつけたあだ名だった。四月の寒い日、たしかにあの日は寒かったけれど、カネコフのさむがりようはふつうじゃなかった。人よりちょっと背丈が低くて、人よりちょっとぽっちゃりした体を自分で抱きしめ、カネコフはくちびるを紫色にして寒い寒いと言っていた。そのうちに、カネコフはロッカーから真新しいあずき色のジャージを出してきて制服のスカートの下に履いた。ジャージの上も着ようとしたけど、ブレザーの上からじゃ袖を通すのも難しくて、肩に引っ掛けておくしかなかった。私たちは笑ったが、カネコフはまだ寒いみたいだった。そこで、女の子たちがばたばたとロッカーへ走って行き、それぞれの真新しいジャージを手にして戻ってきた。ジャージのズボンが、カネコフの縮こまった首にマフラーみたいにぐるっと巻かれた。腹巻きみたいにお腹に巻きつけた子もいたし、単に肩や膝にどんどん重ねて載せてやる子もいた。男の子たちは半笑いになって、遠巻きにそれを見ていた。最後に、誰かのふっくらした手がうしろからやさしげにカネコフの髪を、分け目からそっとなぞって耳にかけ、きっちり眉の上で、ズボンと丁寧に頭に巻いた。特大サイズのヘアクリップが差し出され、それが手から手へと渡って、その即席の帽子がほどけないよう留められた。そのせいで、金子さんだったのがカネコフになった。「ロシア人みたいだから」というのが理由だった。「逆じゃないの？　ロシア人だったら寒さに強いから恰好しなくていいんじゃないの？」カネコフはジャージに埋もれながら抵抗を試みたが、無駄だった。それでも、カネコフはみんなのジャージを振り払おうとはしなかった。むしろたくさんのジャージに埋もれて安心したみたいで、急に顔色が良くなって、その日はそのまま授業を受けた。教室の真ん中で、あずき色のジャージの山が、猫背になってノートを取っていた。先生は、これがいじめでないかどうかをカネコフ本人に何度も確認し、ちがうとわかるといつもよりリラックスした調子で授業を進めた。カネコフは、ときどきゆるんできたヘアクリップを自分で留め直していた。あたたかなくすくす笑いが漏れた。カネコフがうまくクリップを留め直せないでいると、必ず助けの手が伸びた。クリップは銀色で、カネコフが頭を動かすたびにチカチカと救難信号のように光った。カネコフの肩や膝からジャージがずり落ちそうになるのを、私たちがそのままにしておくことはなかった。近くの席の者か、ちょっと離れた席の者でさえ、中腰になってさささっと机と机のあいだをすり抜けカネコフの身なりを手早く整えては去って行った。ヘアクリップを差し出したのは私だったが、昼休みにお弁当を食べるとき、カネコフは私のグループとはちがうグループにいた。でも、この日、たまたまお互い一人きり、帰り道で前後に歩いていて、私が振り返って目が合って、なんとなく横並びになったとき、カネコフは私に自分がどういう人間かを知ってもらおうとしていた。私の通学用のボストンバッグの外側のポケットには、いつものようにあのヘアクリップが挟んであった。肘のうしろに位置していたあれが、カネコフになにかの合図をしたのかもしれない。「なんでほっとするの？」私はカネコフのこめかみをちらっと見ながら尋ねた。「新緑、嫌いだから」とカネコフが答えた。「なんで？」私は、新緑は好きでも嫌いでもなかった。それはそこにあるもので、好きになったり嫌ったりする対象ではなかった。そもそも、カネコフが新緑と口にするその瞬間まで、私は新緑を新緑と認識したこともなかった。「新緑ってすごい色してない？」「わかんない」「蛍光色みたいなすごい色」「そうかも」「そんなすごい葉っぱがさあ、硬い枝を突き破って内側からぶわっと出てきてるんだよ」「あー」と私は言った。わかったのではなくて、ただの相槌だった。「ほんと気持ち悪い」カネコフが吐き捨てた。「それに、痛そう」街路樹が私たち頭上に覆いかぶさるように繁っていた。膝くらいまである生垣が、木々の根元を隠していた。黒々とした、不潔で不吉な生垣だった。私たちはしばらく黙って歩いた。「痛そうって？　何が？」地下鉄の出入り口が迫ってくるのを見て、やっと私は言った。「木。ていうか、枝。新緑が出てくるとき、木はきっと痛いよ」「ええー」と私は言った。私たちは階段を降りた。階段は幅が広く、私たちの前にも後ろにも、私たちと同じ高校の生徒がいた。そうじゃない人たちもいた。一組は親子連れで、端っこで私の腰くらいの背丈もない子どもがうつむいて階段を一歩一歩降りるのを、母親が一段下から見守っていた。子どもが一段飛び降りると、母親も一段降りた。私とカネコフはその二人を追い越した。階段が終わり、踊り場で角を曲がると、振り返っても外は見えなくなった。外の光も入らなくなった。照明があるから、暗くはなかった。目の前には、私たちは歩くべき均一な光で満たされた長い通路があり、また階段があった。壁は白いタイル貼りで、床は灰色のタイル貼りだった。私は何かを思い出そうとしていたが、自分が何かを思い出そうとしていることさえわたっていなかった。強い風が吹き上げていた。私とカネコフは、眉をしかめ、目を細めて、文句も言わず顔面を風に打たれるにまかせていた。地下鉄風だ。あそこはいつもとびきり凶暴なのが吹くのだ。きっと今も吹いてる。「新緑って、なんか厚かましい」カネコフは憎々しげに言った。「油みたいにぎらぎらして、自分たち生きてますって感じで。なんでそうまでしてフレッシュなのを主張したいわけ？」私は笑った。　地下鉄風が口に入らないように、口はできるだけ開けずに笑った。「新緑ってエイリアンみたいじゃない？　内側から木を食い尽くして、食い破って出てくるの、ぶわっと」「ぶわっと」ぶわっとってさっきも言った、という意味を込めて私は繰り返した。だがカネコフは気にしていなかった。「そう、ぶわっと」力を込めてカネコフは何度でも言った。「なんか、すごく、ぶわっと」「でも、新緑が出るから木って生きてんじゃないの？」「ちがうちがう、別の生命体になるんだよ。元の木は死んで、新緑の姿をしたエイリアンに乗っ取られんの」「えー」「そんで、今くらいになると、葉っぱの色も落ち着いてきて、やっとほっとするんだよね。まあすんだことだし、もうしょうがないじゃん？　って」「ふーん」「それなのに次の年になると、また新しいエイリアンが内側からぶわっとやってきて、前のエイリアンを殺す」「へんなの」と私は言った。私は上の空だった。　私はちょっといらいらしていた。何かを思い出そうとして思い出せなくていらいらしていることを、私はわかっていなかった。私が思い出せないことすら思い出せないのは、私がかつて秋にさみしくなる小学生だった、ということだった。私は、私にだって、そういう感受性の強い、傷つきやすい一面があるのだ。私は分かち合うためではなく、対抗するためにその話をしたかった。だから、きっと思い出せなくてよかったのだろう。私たちは改札を抜け、また風にあおられながら階段を軽やかに下り、いくつものドアを開けっ放しにして停車している地下鉄に乗り込んだ。そこが始発駅で、車内のどのロングシートにも必ず一人か二人が座っていたが、まだまだ座るスペースはたくさん残されていた。私たちは二、三両を歩いて通り過ぎてから座った。「私さ、痴漢に遭うんだ、毎日」とカネコフが楽しげにに言った。「えっ？」私は聞き返した。「毎日？」「そう、高校入ってすぐのときから、毎日」カネコフはにやにやしていた。「毎日？」「うそじゃないよ、本当だよ、もう最悪」私はカネコフの眉毛をまじまじと見た。目尻の上あたりはぼさぼさしていて、特に一本、流れを無視してぴょんと跳ね上がっている毛があった。「地下鉄降りたあとJRに乗り換えなんだけど、JRいつも混んでて、そこに痴漢がいるんだよ。朝は絶対いるし、帰りもけっこういる」「えー、お尻触られんの？」「うん」「それってさあ、毎日ってことはさあ、おんなじ人なの？」「わかんない、うしろにいるから顔わかんないし」「なにそれ気持ち悪い。痴漢ですって叫べば？」「うん・・・・」カネコフはもうにやにやしていなかった。無表情になっていた。私はカネコフの膝に目を移した。その隣に、お揃いの制服のスカートからぬっと出た私の膝が並んでいた。カネコフの膝はそれなりにきちんと合わさっていたが、私は浅めに腰掛けていたので、脚がややだらしなく開いていた。私の太ももは白くて肌埋が細かく、内ももにかかる影さえもはかなくすべらかだった。それに、私の太ももの方が、カネコフの座席に押し付けられて横にべちゃっと潰れた太ももよりも明らかに細かった。私は合点がいかなかった。痴漢に遭うということは、その肉体がまぶしく美しいということを意味するはずだ。私はまだ、痴漢に遭ったことがなかった。遭いたいわけではないが、私の太ももよりカネコフの太ももの方が価値が高いとは到底思えなかった。私はそれを言う代わりに、「気持ち悪いっていえばさあ」と話題を変えた。「うちのマンション、ちょっと前に、ホモの人が引っ越してきたんだよ」「うそ、え、本当に？なんでわかるの？」カネコフがぱっと顔を明るくして私を見た。「だっていっつも二人でいるし、くっついて歩いてんの」私はお尻をすべらせてカネコフのブレザーの二の腕に自分のブレザーの二の腕をくっつけた。「こんなふうに」「えーっ、じゃあ本当にそうなんだ」「しかもね、服が変なんだよ。二人ともおじさんなんだけど、いっつもお揃いの変な服着てんの。変な茶色のスーツと、変な茶色の暑苦しい帽子。頭んとこが丸くて、こう、ぐるっとつばがついてるやつ」「へー」カネコフは興味深げにうなった。カネコフより、私の降りる駅が先だった。地下鉄の車内は、乗り込んだ時点よりもずいぶん混んできていた。シートは完全に埋まっていて、私たちの前には人が立っていた。「じゃね」私は立とうとした。「痴漢に気を付けて」「さっき言うの忘れてたんだけど」カネコフは小声でいった。「ホモじゃなくて、ゲイだよ」「なんかちがうの？」「わかんないけど」カネコフは首をかしげた。私は立ち上がり、体を横にして前に立つ人と人のあいだをすり抜けながら、カネコフに小さく手を振った。カネコフも同じように、小さく両手を振っていた。マンションの前で、私はそのゲイの二人に行き合った。彼らは私と入れ違いにマンションから出て、どこかへ行くところだった。「こんにちは」私は軽く目を伏せ、軽く会釈をした。それは、他の住人にやるのとまったく同じ挨拶だった。けれど二人は、まるで私がいないかのような態度で行ってしまった。それは、もうすでに何度か経験していたことだったので、私は今更驚きもしなかったし落胆もしなかった。あの二人とはエントランスやエレベーターを出たところの共用廊下でしばしば行き会うが、私が挨拶をしても絶対に挨拶をし返さないのだ。それどころか、私を見もしない。私はエントランスを歩きながら首をひねって彼らの遠ざかっていく後ろ姿を眺めた。二人とも成人男性にしては背が低くて、たぶん私とそんなに変わらないくらいだった。おまけに小太りで、その太り方までそっくり同じだった。彼らはいつもどおりお揃いの変な茶色のスーツを着ていて、変なフェルトっぽい素材の帽子をかぶり、二人の腕と二人の腕をぴったりくっつけていた。顔はよく見たことがなかったが、体型を見ると双子としか思えなかった。でも、双子は別にくっついて歩いたりはしない。やっぱホモだよなあ、と私は思った　ホモに蔑称の意味合いが含まれると知ったのは、だいぶあとになってからだった。初夏に、ゲイの二人の服装が変わった。変な生成りのぺらっとしたスーツに、変な生成りのぺらっとした帽子になった。母が、あれは麻だと教えてくれた。それから、あの二人が母には挨拶を返すことも明らかになった。濡れた髪を拭きながらエアコンの真下に仁王立ちになり、冷風を全身で受けようとしている晩のことだった。「えーっ」私は大声を上げた。「うっそマジ？なんで？」「え？」母も不審げだった。「ほんとに？ほんとにあんたには挨拶しないの？あんたは？ちゃんとしてる？」「してるよ！」母は、彼らの挨拶のやり方を教えてくれた。「あの二人の挨拶はねえ、こうやって帽子のつばをちょっと摘んで」母がこめかみのあたりに指をやった。「にっこりして会釈」「げーなにそれ。キモい」「キモくありません。イギリス紳士みたいですてきじゃない？」私は母に、彼らの挨拶が動作だけだったのか、何らかの言葉がついていたのかを聞き損ねた。いや、もしかしたらあのとき私は尋ね、答えを聞いたのかもしれない。でも、思い出せない。思い出せないことはもう一つあって、それは、私がいつ、彼らにどうしても挨拶させると決意したかだ。私だけが挨拶を返されないと知ったこのときだったが、はじめて恋人ができて、以前から思っていたとおりやっぱり私の肉体には価値があるのだということをぞんぶんに確認できたときだったか、カネコフに痴漢したときだったか。それとも、彼らに挨拶をさせるために私が行動を起こしたまさにそのときだったのかも。私はもう、これらがどの順番で起こったのかも忘れてしまった。挨拶させようという試みが最後だったのはたしかだ。全部が同じ夏に起こったということも。かつて私だった古いエイリアンを殺して私になりかわった新しいエイリアンだった。カネコフとは、お弁当を食べるグループは相変わらず別だった。いっしょに帰った日以来、少しは互いに親しみを感じるようになってはいたと思うが、私に恋人ができるともうカネコフのことは眼中になかった。私は私と同じように、恋人がいる子たちとつるんでいた。そのころには、カネコフが毎日痴漢に遭うというのはけっこう有名な話だった。カネコフが自分で、誰彼かまわず愚痴を言うようになっていたからだ。「毎日だって。ちょっと大袈裟だよねえ」というのが、おおかたの意見だった。カネコフは同情されてはいたが、同時にやや疎ましがられてもいた。「さあ。本人が毎日だって言うんなら毎日なんじゃないの」と私は私の仲間に言ったことがあったが、カネコフに味方したわけじゃなくて、どうでもよかったからだ。いや、どうでもよくはなかった。私は自分の肉体の持つまぶしさと美しさしか見たくなかった。それだけを堪能したいのに、カネコフの痴漢の話は邪魔だった。私はカネコフを助けようとしたことはなかった。それどころか彼女が助けを求めていたことにも気がついてはいなかった。だから、カネコフに痴漢をしたのは、ただ単に、ふざけただけだった。私の恋人は他校の高校生で、その朝、私はいつもより一時間ほど早く家を出て、登校前に彼の家の近くの駅まで出向き、合って少し話してから、上機嫌で自分の学校へ向かっているところだった。前の日にファストフード店で二人で宿題かなにかをやっていて、私がまちがえて彼のノートを持って帰ってしまったので、それを返しに行ったのだったと思う。たぶん、そのノートはそんなに急いで返さなくてもよかった。私は返しに行くということをやってみたかったのだ。それができて、私はきっと有頂天だった。私は、車内にカネコフの黒光りする頭を見かけるまで、自分の乗っているぎゅうぎゅう詰めのJRが、彼女の行き帰りのルートであることも、彼女が痴漢に遭うことも主張している現場であることもすっかり忘れていた。肺いっぱい吸い込んでいた朝特有の清潔な空気は、いつのまにかのどこに少し名残があるだけになっていた。吊り革に両手で捕まり、爪先立って伸び上がると、すぐそこの乗車口近くの乗客のあいだに、暗い穴があった。それが、私に背を向けて、うなだれているカネコフの姿だった。彼女の肩を隠すように左右に立っている乗客は、どちらも男性だった。私はさらに伸び上がってよく見ようとしたが、彼らがカネコフに痴漢を働いているのかどうかはわからなかった。私は吊り革を離し、横歩きでカネコフに近づいた。くっつきあって壁になっている乗客たちの継ぎ目を肩でこじ開けて進む。舌打ちが聞こえたが、怖くもなんともなかった。他人のスーツのジャケットやブラウス越しの熱い霧が、私の頬に押し寄せてくるようだった。私は平気だった。私は、左肩を先頭にして、乳房やお尻で乗客たちの分厚い体をはねのけ、じりじりとカネコフの背後に迫った。カネコフがうなだれているせいで持ち上がった後頭部が、私のすぐ目の前にあった。本当はカネコフに重なるように真後ろに立ちたかったが、姿勢を変えるのは難しかった。私は横歩きしてそこまでやってきた姿勢のまま、左手をパーのかたちに広げて、当たりをつけてカネコフの制服のお尻を触った。熱くて埃っぽい制服の布地の感触があった。だが、私の手は彼女のお尻に沿わなかった。お尻というのが丸いということを、私はまるで知らないみたいだった。私は指をいっぱいに伸ばしていて、手の形としてはむしろ反り返っていた。だから、尻のあたりの布地に触れているのは、指の付け根のラインくらいのものだった。私は新鮮な驚きに打たれながら、そっと指の力を抜いた。それでやっと指がお尻に落ち着き、私は左手全部で尻を触ることに成功した。私はそのまま、カネコフの耳にこう吹き込むつもりだった。　「あれ？毎日痴漢に遭ってるんじゃなかったっけ？」私は、カネコフが嘘をついていると断罪する気はなかった。カネコフが痴漢に遭っていることを否定する気もなかった。私には悪気なんてなかった。ふざけていただけなのだ。でもそれを言えば、カネコフはそうは受け取らなかっただろう。それにそもそも、私が触る直前まで彼女が別の痴漢の被害を受けていなかったとは言えない。私が口を開く前に、カネコフが動いた。カネコフは強い力で、周りの乗客たちを摩擦しながら振り返った。カネコフは涙ぐんだ目で私を睨み上げ、けれど口元は笑っていた。カネコフが私の名前を親しげに呼び、自分たちや他の乗客の体に押されて見えない位置で、さっき彼女の尻を触った私の左手をぐっと握った。私たちはそのまま、JRを降りるまで手をつないでいた。地下鉄に乗り換える途中で、カネコフは「ありがとう」と言った。「え？なにが？なんで？私、痴漢したのに」私はおどけて答えた。「痴漢？あれが？」カネコフは声を立てて笑い出した。「なに？お尻触ったでしょ、私」「あんなの痴漢じゃないよ。私、すぐわかったもん」カネコフは笑いすぎて苦しそうだった。「痴漢の触り方って、あんなんじゃないんだよ。揉むの」「揉む」「そう、揉むの」とカネコフが片手を胸の高さにあげ、宙に向かって指をざわざわと不気味に動かした。それからすぐに、あの濃い尿のなかをあえぎ泳いでるみたいな夕方がやって来た。マンションに帰り着いた私の前を、例のゲイの人が歩いていた。一人だった。二人いっしょじゃないところを見たのははじめてだった。一人でも、彼は変な生成りのぺらっとしたスーツに、変な生成りのぺらっとした帽子をかぶっていた。彼がちらりとこちらをうかがって足を早めたのがわかった。私も足を早め、彼との距離を縮めた。彼が郵便受けをチェックしなかったので、私もしなかった。彼は傍目にも慌てているとわかる動作でオートロックの暗証番号を押し、自動ドアを開けた。その向こうは照明が消えていて暗かった。夜にならないと点かないようになっているのだ。暗い廊下のまっすぐ先にエレベーターがあった。ひとつしかないそのエレベーターの箱は一階に降りてきていて、やけに黄色い照明を灯してぽつんと私たちを待っているのが見えた。彼はほとんど小走りになっていた。私には彼の考えていることがわかった。彼は、私とエレベーターに乗り合わせるのを恐れているのだ。逃がすものかと思ったし、逃がす距離でもなかった。彼がエレベーターのボタンを忙しげに叩き、ドアが開いた。彼はさっと中に滑り込むと、こちらに向き直って階数ボタンを押した。ゆっくりとドアが閉まっていく。が、そのドアの動きの緩慢さを、私は知り尽くしていた。半分も閉じないうちに、私は右手でドアを受けた。安全装置が働いて、ドアがずるずると開いた。「こんにちは」私は軽く目を伏せ、軽く会釈をした。彼はすでにめいっぱい後ずさり、階数ボタンと対角に位置する角にその子太りの体をぎゅっと押し込んでこちらをうかがっていた。三人も入れば狭苦しく感じる、小さなエレベーターだった。私は悠々と乗り込み、階数ボタンを押した。先に押されていたのが七階で、私は十二階だった。ドアが閉まった。暗い共用廊下の向こうに、四角く外が見えていた。まだ尿のように輝いていた。箱の上昇をはじめ、粗いコンクリートの内壁が輝く外界を押しつぶした。　私は体ごとゆっくりと彼に向き直った。あからさまに、彼は身を強張らせた。私は彼を頭の上から爪先まで見回した。なのに、顔はどうしても思い出せない。思い出せるのは、鼻の頭にびっしりと汗をかいていたことと、彼がどうあっても私と目を合わせようとしなかったことくらいだ。しかしこれは、私も同じだった。私も鼻の頭に汗をかいていたし、それどころじゃなく全身に汗をかいていた。私は高校で、生理がはじまったのかもしれないとトイレに確認しに行ったが、下着を濡らしているのは汗だった。また、私も彼と目を合わせることはしなかった。目を合わせずに、彼の姿をじっくりと見ていた。彼は、どう見ても怯えていた。私は不思議でならなかった。私の肉体は、恋人が喜んでいるように人を喜ばせるか、あるいはカネコフを痴漢する男たちが喜んでいるように人を喜ばせるか、どちらかだけであるはずだった。それに、彼と私だったら、こんなふうに上から下まで見られるのは私のほうだし、怯えるのも私のほうなのがふつうだ。彼がゲイであるということを差し引いても、関心がないというならともかく、怯えられるのは心外だった。一歩前に出ると、もうそれだけで、私の体はエレベーターの箱の中心にあった。腕を前にやれば、角に背中を押し付けてすくみあがっている中年男に触れられる距離だった。「こんにちは」私はもう一度言った。返事はなかった。彼は口元を両手で覆っていた。私は、彼が小刻みに震えているのに気づいた。震えている！この男は、私に近づかれて、声も上げられずに怯えて震えている。そのとき、私の全身にじんわりと広がったのはたしかに喜びだった。それは、それまでに味わったことのない種類の喜びで、尿に経血がひとすじひらめくように怒りが混じっていた。この男は、この私を受け入れないつもりなのだ。こんなつまらない外見をしたつまらない男のくせに。私は歓喜し、激怒していた。私は汗の冷えた太ももを、もう一歩踏み出した。男の顔もわからないくせに、彼が涙目になっていたのはしっかりとおぼえている。「こ、ん、に、ち、は」私は笑みを含んだ声で言った。男が、私のうしろに目をやった。エレベーターががたんと止まり、うしろでドアが開いた。七階だった。男が出られないように、私はさっと足を肩幅に広げた。ドアが閉まった。男が泣き出した。きゅうと閉じられた目元から、涙が噴き出すのを私は見た。男はそれでも声を上げないよう顔をくしゃくしゃにしてこらえ、そのせいで痙攣をはじめていた。エレベーターが上昇していくのとうらはらに、男は崩れ落ちつつあった。私は座り込んでいく男の、そのきちんと揃えられた膝をまたぐように立って、彼の生成りの帽子を見下ろした。制服のスカートが、つばに触れていた。ふつうだと、これがうれしいはずなのに。私は男をさげすんだ。泣くほど怖いんだ。へんなの。十二階に着いたのと、男が濡らした尿が足もとにじりじりと押し寄せてきたのは、ほぼ同時だった。　「うわあ、きたない」私が小声で吐き捨て、身をひるがえしてエレベーターから降りた。小学生だったころ、秋になると、私はさみしかった。上下に弾む赤と黒のランドセル、他校から転校してきた子のナイロン製の黄色いのランドセル、空は灰色で、道もコンクリートで灰色だった。パンツの履き口がずれて、お尻の割れ目に食い込んで、私はスカートの生地ごとそこをつかんでちょうどいい位置に戻そうとした。一度は戻ったかに思えた。だけど、すぐにまたずれた。目に見える空も道も平坦で、奥行きがないのに続いていた。続いていると知っていた。空の上には宇宙があるし、道はいつかどこかで途切れても、その先には何にもなくなるんじゃなくて土地や海がある。とても信じられないことだったが、そうだと知っていた。続いていると思うだけでさみしくてたまらない日が、秋には必ずあった。ほかの季節には、そう思ってもさみしくなんかなかった。それとも、思いもしなかったのかもしれない。銀杏の黄色はくすんで汚れていて、ちっともきれいではなかった。私はがっくりと頭をのけぞらせてスキップをしてみた。そうやって跳ねると、空に向かってまっさかさまに落ちてしまいそうだった。それでもかまわなかった。それなのにそうはならなくて、そのことが私をいっそうさみしくさせた。夜の電車のボックス席の窓際に座り、つめたい車窓にこめかみをつけて、私はあの夏のことと、それよりずっと遠い秋のことを思い出している。私は寝たふりをしている。さっきまでは、本当に眠っていた。隣から、控えめだが荒い息遣いが聞こえるので目をさましたのだ。私はどうやらあの朝、幸運にもカネコフのちょっとした助けになったらしいが、そのあと彼女のために何かをしたおぼえはない。ゲイの二人はあれからすぐに引っ越してしまったらしく、二度とあの男の人も、パートナーの人の姿も見ることはなかった。私のやったことは明るみには出なかった。もっとも、彼が私を告発するなどとは私は夢にも思っていなかった。あのころの私なら、ただ挨拶してもらいたかっただけ、と言ってのけるだろう。ああ、それにしてもあの、体を突き破るような歓喜と激怒！でも、私もかつては秋になるとさみしくてたまらなくなるような罪のない子どもだったのだ。どうか許してほしい、けれど、そんなことで私を許すのは私自身だけだと知っている。私は、今、私の隣に座って息を荒げているのが一人のみすぼらしい老人だと知っている。目を覚ましたとき、私はなにかの予感に打たれて、頭を上げることをしなかった。それと気づかれないように、そっと目だけ動かして盗み見をした。なにもかもが、一目で了解できた。たぶん、この車両には私とこの人だけなのだろう。だからこそ、彼はわざわざ眠っている私の隣にやってきた。服の中に中身が入っているのかどうかもあやしいくらいぺったんこに瘦せた老人だった。毛羽立った袖口から覗く手首には灰色の血管が浮いている。しかしその手が握り、ひたむきにこする性器は、お湯から上がった赤ん坊みたいにきれいなピンク色をしている。私はうつむいて、寝たふりをして、泣きそうになっている。この人もかつては、たとえば秋になるとそれだけでさみしくなるような、あるいは新緑を嫌うような子どもで、そのことを言い訳にわたしに対してすまないと、許してほしいと後で願うことがあるのだろうか、と考えている。　でも実際のところ、わたしを動けなくさせているのは圧倒的に恐怖だ。私はこの老人に勝てるだろうか。おそらくは勝てるだろう。私は重いブーツを履いている。けれど、本当にそうだろうか？もし私の読みが甘かったら、この老人が見た目よりも力があったら？私は顔を上げないまま、とつぜん肩から立ち上がる。通路への道を、老人のへなへなしたズボンとピンク色の性器が阻んでいる。私は緑色をしたシートにブーツのまま乗りあがり、老人を飛び越して通路に降りる。老人の怒り狂った唸り声が聞こえる。悲鳴かもしれない。悲鳴だといい。足がうまく上がらなくて、彼の性器を蹴ったかもしれない。私は振り返らない。思ったとおりこの車両には誰もいない。私も悲鳴を上げたいが、それどころではなくて、私は必死に車両を区切るドアを開ける。老人のうめき声が聞こえている。彼が追いかけてきているのかうずくまっているのかわからない。次の車両には少し人がいるが、みんな眠っているようだ。私は駆け抜ける。私は恐ろしくて恐ろしくて、さみしくてたまらない。車両をいくつか駆け抜けた先はもうどこにも行くところがないと知っていて、それがさみしくてたまらない。私はこんなものはいらなかった。振り払って振り払って、ブーツの底で踏み潰してしまわなければ。　私はこんなものはいらなかった、こんなさみしさは。だってこんなさみしさでは、私がどういう人間かを証明することはできないのだ。	Translation - English In autumn, I was lonely. But that was back in elementary school. I had just entered high school, and I no longer felt particularly lonely in autumn. Or if I did, it wasn’t caused by autumn - it was merely the kind of ordinary loneliness that could occur to anyone in any season. And that kind of loneliness could not prove what kind of person I was. All the less now that it was almost summer. ‘I’m so relieved that the season of new green leaves is over’, Kanekoff said. Kanekoff was a nickname this girl in my class had received a few weeks ago, soon after we started high school together. That day had certainly been chilly in the way April days can be, but her sensitivity to the cold was frankly abnormal. She was hugging her own body, which was slightly shorter and chubbier than average, and grumbling about the cold through purple lips. Eventually, she went to fetch a brand new chestnut-coloured tracksuit from her locker and pulled the trousers on under her uniform skirt. She tried to put the jersey top on as well, but getting it over the sleeves of her school blazer proved too troublesome, so the best she could do was to hang it over her shoulders. Her antics made us laugh, but she still seemed to be feeling cold. Consequently, the other girls rushed off to their respective lockers and returned with their own brand-new tracksuits in hand. A pair of jersey trousers was wound around her hunched neck like a scarf. Some girls wrapped joggers into a bellyband around her stomach, while others simply piled layers upon layers of them on her shoulders and in her lap. The boys watched the scene unfold from afar with bemused half-smiles on their faces. As a finishing touch, someone’s plump fingers gently draped strands of her hair away from its partition and tucked them behind her ears before wrapping another pair of trousers neatly around her head, exactly above the eyebrows. An oversized hair clip was proffered and passed from hand to hand to her head, where it kept the makeshift hat from coming undone. That is how Kaneko became Kanekoff. ‘Because she looks Russian’, went the reasoning. ‘Isn’t it the other way around? I wouldn’t need this many clothes if I were Russian, because they’re cold-resistant’, Kanekoff tried to protest through the mountain of tracksuits, but her resistance was futile. Nevertheless, she made no move to shake off the jerseys. If anything, being buried in them seemed to soothe her, and her complexion swiftly regained its healthy colour. That day, she proceeded to attend classes in this guise. A mountain of chestnut jerseys was hunched over a desk in the middle of the classroom, taking notes. The teachers kept asking Kanekoff if she was being bullied, which she denied. After that had been cleared up, the lessons continued with a more relaxed atmosphere than usual. Every now and then, Kanekoff adjusted the hair clip that kept coming loose, eliciting amicable giggles. Whenever she had trouble fixing the clip, a helping hand would readily reach out to her. Every time she moved her head, the silver clip flashed like a distress signal. We wouldn’t let the jerseys slide off her shoulders and her lap, either. Those sitting close to her, and even those a little further away, would skitter between the desks in a half-crouch, nimbly adjust her attire, and slip away as quickly as they’d come. I was the one who had offered her hair clip for Kanekoff to wear, but she spent her lunch break in a different group. However, later that day, both of us happened to be walking home alone, me in front of her, and our eyes met when I glanced over my shoulder. For whatever reason, we ended up walking side by side, Kanekoff trying to tell me what kind of a person she was. The hair clip was back in its usual place, clasped to the outer pocket of the Boston bag I used for school. Seeing it hanging from my elbow might have given Kanekoff some kind of a signal. ‘Why are you relieved?’ I asked, glancing at her temple. ‘Because I hate fresh green leaves’, Kanekoff responded. ‘Why?’ I neither liked or disliked fresh green leaves. They were simply there, not the target of any strong emotion. Until the moment Kanekoff uttered the word, I’d never even recognised fresh green leaves as such. ‘The colour of fresh green leaves is just dreadful, isn’t it?’ ‘I don’t know.’ ‘It’s a dreadful fluorescent colour.’ ‘I guess.’ ‘Those dreadful leaves penetrate the hard tree branches from the inside and rush out like ‘whoosh’!’ ‘Right’, I said. It’s not that I really got it, I just didn’t know what else to say. ‘It makes me feel sick’, Kanekoff spat out. ‘And it seems painful.’ The verdure of the roadside trees hung heavily over our heads. The roots of the trees were hidden by a knee-height hedge. It was a dark, filthy and sinister hedge. We walked in silence for a while. ‘Painful? What is?’ I finally asked, as I saw the entrance of the subway station getting closer. ‘Trees. Or rather the branches. When the fresh green leaves break out, the trees must be in pain.’ ‘Huh’, I said. We joined the throng of people going down the wide stairs. Many of them were other students from our high school, but not all. Near the end of the staircase, a child barely tall enough to reach my waist was descending the steps one by one, looking down in deep concentration. The mother kept watch from one step below. When the child jumped down one step, the mother did the same. Me and Kanekoff overtook the two. After reaching the end of the stairs and turning the corner at the landing, I could no longer see the world outside even if I looked back. The sun could not reach here. Because of artificial lights, it was not dark. In front of us was a long corridor brimming with uniform light, leading us to another set of stairs. The walls were tiled white, while the floor was tiled grey. I tried to remember something, but I didn’t even realise that I was trying to remember. A strong wind began to blow. Me and Kanekoff knitted our brows and squinted our eyes and resigned to the wind hitting our faces. Subway wind. It always blows the most ferociously down there. I’m sure it’s still blowing. ‘The fresh green leaves, they’re so shameless somehow’, Kanekoff said hatefully. ‘With their oily sheen, they’re making such a show of being alive. Why do they need to flaunt their freshness so garishly?’ I laughed. I tried to make sure my mouth stayed shut while I laughed, to avoid the subway wind getting in. ‘Don’t you think the fresh green leaves are like aliens? They eat the trees up from the inside and tear their way out, like whoosh.’ ‘Whoosh’, I repeated, trying to point out that she had already said ‘whoosh’ before. But Kanekoff didn’t seem to care. ‘Yes, whoosh’, she repeated over and over again, with all her might. ‘Like, incredibly, whoosh’. ‘But the emergence of fresh leaves is what keeps a tree alive, right?’ ‘No, it’s not. It becomes a different life-form. The original tree dies and is taken over by aliens in the shape of fresh leaves.’ ‘Hmm.’ ‘And so, around this time of year, when the colour of leaves has calmed down, I can finally relax. Like it’s over now, so there’s nothing we can do about it, right?’ ‘Hmmmm.’ ‘Even so, next year there will be a new batch of aliens whooshing their way through the tree innards, killing the previous aliens.’ ‘That’s weird’, I said. My mind was wandering. I felt a little annoyed. I didn’t realise I was annoyed because I kept trying to remember something and couldn’t. What I couldn’t even recall failing to remember was that I’d used to be lonely in autumn, back in elementary school. I, too, do have that kind of a sensitive, fragile side to myself. I wanted to talk about it not to share it, but to counter it. So it must have been for the best that I couldn’t remember. We passed the ticket gates and, still fanned by the wind, descended another set of stairs with a light step. We boarded the subway train, which was waiting for us with its doors wide open. Because this was the first station of the line, there was still lots of space left, with only one or two people in each row of seats. We walked through two or three carriages before picking our seats. ‘You know, I get groped on the train every day’, Kanekoff said cheerfully. ‘What?’ I asked. ‘Every day?’ ‘Yeah, ever since I started high school, it’s happened every day’, Kanekoff said with a smirk. ‘Every day?’ ‘I’m not lying, it’s true, I’m so sick of it.’ I glared at Kanekoff’s eyebrows. They were unkempt above the outer corner of her eye, particularly one hair, which sprung up without any sense of flow. ‘I get off the subway and change to a local train, which is always crowded, and there are molesters. Every single morning, and often on my way back too.’ ‘Whoa. Do they touch your bottom?’ ‘Yeah.’ ‘So if this happens every day, do you think it’s always the same person?’ ‘I don’t know. They’re behind me so I can’t see their face.’ ‘What the hell, that’s so disgusting. What if you tried screaming that you’re being groped?’ ‘Yeah…….’ Kanekoff wasn’t smirking anymore. Her face had gone blank. I turned my eyes to Kanekoff’s knees. Beside them, my own knees stuck out from underneath the matching uniform skirt. Kanekoff’s knees were demurely drawn together, but I was sitting closer to the edge of the seat, with my legs open a little carelessly. They’re both old men, but they always wear weird matching outfits. Weird brown suits and weird brown hats that look sweltering. The head part is round, with a brim around it.’ ‘Really’, Kanekoff said, with great interest. My stop came before Kanekoff’s. The subway carriage had become much more crowded since we got on. The seats were all filled, and there were people standing in front of us. ‘See you’, I said as I was about to get up. ‘Watch out for the perverts.’ ‘I forgot to mention earlier’, Kanekoff whispered. ‘They’re not faggots, they’re gays.’ ‘What’s the difference?’ ‘I’m not really sure’, Kanekoff said, tilting her head. I got up and wove through the crowd of people standing in the carriage, giving Kanekoff a little wave as I went. Kanekoff lifted her hand and gave me a little wave in return. In front of our apartment building, I ran into the two gays. They walked out of the building just as I was about to enter. ‘Hello’, I said, lowering my eyes slightly and giving a light nod. It was the exact same greeting I used with all the other residents. But these two behaved as though I didn’t even exist. This had already happened several times before, so it neither surprised nor disappointed me anymore. I often ran into these two in the communal hallway between the entrance and the elevator, and they never once returned my greeting. On the contrary, they refused to even look at me. As I walked through the entrance, I turned my head and watched their backs grow more distant. They were both short for adult men, probably around the same height as me. On top of that, they were on the plump side, and even the way they carried their weight was exactly identical. As always, they wore matching outfits - weird brown suits and weird hats made from a felt-like material - and their arms were tightly clasped together. I had never seen their faces clearly, but based on their body types, I could only think of them as twins. However, twins don’t normally walk around attached to each other. So they really are faggots, I thought. I only learned much later that the word ‘faggot’ is considered derogatory. The attire of the two gays changed from early summer. They began wearing strange suits made of thin undyed cloth and strange hats made of thin undyed cloth. Mum told me the material is called hemp. On top of that, I learned that the gays returned my mum’s greetings. That evening, I was drying my damp hair directly beneath the air conditioner, stretching my spine and holding my head up high to maximise the surface area of my body hit by the blast of cool air. ‘What?’ I said, raising my voice. ‘Are you for real? Why?’ ‘Huh?’ Mum said, equally incredulous. ‘Is it true that they really won’t greet you? And what about you? Do you greet them properly?’ ‘Yes, I do!’ Mum proceeded to educate me about their manner of greeting. ‘Those two, they greet people by slightly pinching the brim of their hats’, she said, placing her finger on her temple. ‘And then give a little nod and a smile.’ ‘Yuck, what even is that. Disgusting.’ ‘It’s not disgusting. They behave like English gentlemen. Isn’t that lovely?’ I failed to ask mum whether their greeting consisted only of these physical gestures or whether it also involved words. Although it’s also possible that I did ask and listen to the answer. Either way, I can’t remember. There is one more thing I cannot remember, that is, when exactly I decided I’d make them greet me no matter what. Was it when I found out I was the only one whose greeting they wouldn’t return, or when I got my first boyfriend and fully validated my earlier belief that my body was valuable, or when I groped Kanekoff on the train. Or it could be that my resolve only solidified in the very moment I took action to make them greet me. I’ve already forgotten the order in which these things happened. I’m only certain that my attempt to force their greeting came last. And that it all happened during the same summer. I was a new alien killing the old alien of my past self to take her place. As always, Kanekoff ate her lunch in a different group from me. I think we’d grown a little closer since the day we went home together, but as soon as I got a boyfriend, I lost all interest in her. I hung out with the other girls who had boyfriends. At the time, the story of Kanekoff getting molested every day had become rather notorious. It was because Kanekoff herself kept complaining about it to anyone and everyone. ‘Every single day, she claims. Surely that’s a little exaggerated’, went the general consensus. While people did feel sympathy towards Kanekoff, they were simultaneously a little repulsed by her. ‘Well. If the victim herself says it’s every day, then surely it must be every day’, I would say to my friends, but it’s not like I was really on her side, because I didn’t care either way. Or actually, I did care. I only wanted to see the radiance and beauty of my own flesh. All I wanted was to take pleasure in it, but Kanekoff’s molestation stories had become a hindrance. I never tried to help her. Far from it, I didn’t even notice that she was asking for help. With that being said, the time I groped Kanekoff was, to put it very simply, just meant as a prank. My boyfriend went to a different school, so I left home an hour earlier than usual that morning and headed to the station near his house before going to class. We met up and chatted for a while, and I proceeded to my own school in good spirits. The night before, we’d done homework together in a fast food joint, and I’d accidentally taken his notes home with me, so the reason for the morning meet-up was to return them. There was probably no need to return those notes so urgently - it was just something I wanted to try. Completing this task made me feel ecstatic. Until the moment the jet-black shine of Kanekoff’s hair caught my eye, I had completely forgotten that the jam-packed local train I’d boarded was the same one she took to school, and that this was the scene of her alleged groping incidents. Before I knew it, nothing but a lingering trace remained of the clear morning air I’d inhaled deeply into my lungs. Holding on to a hanging strap with both hands and standing on my tiptoes, I saw a black hole opening right there, in the middle of the crowd standing by the carriage door. It was Kanekoff, with her head hung and her back turned to me. The passengers standing on either side of her, covering her shoulders, were all men. I tried to stretch myself taller to see better, but I couldn’t tell if they were groping her or not. I let go of the hanging strap and sidled towards Kanekoff. I wrenched open the seam of passengers clung together to form a wall. Someone clicked their tongue at me, but it did nothing to scare me off. The hot mist rising from other people’s suit jackets and blouses seemed to surge through my cheeks. I was fine. I led with my left shoulder, pushing aside the passengers’ thick bodies with my breasts and my buttocks, slowly but steadily approaching Kanekoff. The back of Kanekoff’s hunched head was right there in front of me. What I really wanted to do was to stand close enough to press into her back, but it was difficult to adjust my stance. In the same position I’d walked over in, my left hand wide open as though signalling ‘paper’ to an imaginary opponent, I aimed for the part of Kanekoff’s uniform that covered her buttocks and placed my hand on it. The fabric of her uniform felt hot and dusty. However, my hand wasn’t aligned with her buttocks. It was as though I hadn’t realised that buttocks were supposed to be round. I’d extended my fingers as far as they would go, curving my hand into the shape of a sickle. The part of my hand that actually touched the fabric over Kanekoff’s buttocks was no larger than the line at the base of my thumb. Caught by surprise, I quietly let the tension in my fingers dissipate. They finally settled down on her buttocks, and I succeeded at touching her bottom with the entirety of my left hand. In that position, I planned to whisper into Kanekoff’s ear like this. ‘Huh? Didn’t you say you get groped on the train every single day?’ My intention was neither to condemn Kanekoff for lying nor to deny that she’d been molested. I had no malicious intent whatsoever. It was just meant to be a joke. But I knew that if I said it, Kanekoff might interpret it differently. Besides, I couldn’t rule out the possibility that she had been assaulted by another groper just before I laid a hand on her. Before I could open my mouth, Kanekoff stirred. She turned around with considerable force, brushing against the passengers who stood around her. She glared up at me with tears in her eyes, but there was a smile on her lips. She called out my name in an affectionate tone, and then, under the cover of other passengers’ bodies and our own, she firmly clasped my left hand that had just touched her buttocks. We held hands like that until it was time to get off the local train. While we were changing to the subway, Kanekoff said: ‘Thank you.’ ‘Huh? For what? Why? I just groped you’, I responded playfully. ‘Groped? Is that what it was supposed to be?’ Kanekoff burst out laughing. ‘What? I touched your butt.’ ‘That’s not groping. I could tell the difference right away.’ Kanekoff laughed so hard it seemed painful. ‘Gropers have their own way of touching. They rub.’ ‘Rub?’ ‘Yes, rub’, Kanekoff said, lifting one hand to chest level and drawing her finger through the air in a creepy rustling motion. Soon after, I was enclosed by the dusk that made me feel like I’m drowning in pungent urine. As I was returning home, I saw one of the gays walking into the apartment building in front of me. He was alone. It was the first time I’d seen one of them without the other. Even though he was alone, he still wore the strange suit of thin undyed fabric with the strange hat of thin undyed fabric. I could tell he started walking faster after he glanced at me. I quickened my step too, shortening the distance between us. He didn’t check his mailbox, so I didn’t stop to look at mine either. Visibly flustered, he pressed the PIN code into the automatic lock and opened the door. The corridor was dark, because the lights were programmed to stay off until night-time. The elevator was right at the very end of the dark corridor. The sole elevator was on the ground floor, and we could see it waiting for us, shining its violently yellow signal light. The man in front of me nearly broke into a run. I knew what he was thinking. He was scared of sharing the elevator with me. I wasn’t going to let him get away, and he was too close to make an escape from me. He pressed the elevator button frantically, and the door opened. He slipped inside as quickly as possible, turning to face me as he pushed the floor button. The door started slowly closing. However, I was thoroughly familiar with the sluggishness of that door’s movement. Before it had shut even halfway, I stopped it with my right hand. The safety mechanism did its job, and the door slithered back open. ‘Hello’, I said, casting my eyes lightly downwards and giving a slight bow. He had already backed as far away from me as possible, squeezing his chubby body into the corner opposite the floor buttons and glancing at me. The elevator was so small that three passengers were enough to make it feel cramped. I walked in calmly and pushed the button of our floor. The one he had pressed was the seventh, and mine was the twelfth. The door closed. Beyond the dark communal corridor, a little rectangle of the outside world was visible, still glowing with urine-like light. As the elevator cab began its ascent, the glow was crushed by its rough concrete walls. I slowly turned my entire body towards him. I could clearly see he stiffened in response. I scrutinised him from head to toe. And yet, I can’t recall his face at all. All I remember is that there was sweat pooling at the tip of his nose, and that he wouldn’t look me in the eye no matter what. But I was in the same position. Not just the tip of my nose but my entire body was covered in sweat. At school, I’d thought my period might have started and went to the toilet to check, but it turned out that the only thing dampening my underwear was sweat. I didn’t try to make eye contact with him, either. Without meeting his gaze, I was dragging my eyes slowly and thoroughly over his body. There was no question that he looked terrified. I was totally bewildered. My body was supposed to delight people either in the way it delighted my boyfriend, or the way it delighted the guys who would grope Kanekoff on the train. Besides, in a situation like this, it would have been more normal if I was the one getting ogled from top to bottom and feeling scared. Even accounting for the fact that he was gay, even if he had no interest in me at all, I still found his fear offensive. I took a step towards him, and that small movement was enough to bring me to the centre of the elevator cab. The middle-aged man pushing himself into the corner, cowering in fright, was so close I could have touched him by just extending my arm. ‘Hello’, I repeated. There was no response. He was covering his mouth with both hands. I noticed he was trembling. Trembling! This man, when approached by me, did not even raise his voice but simply trembled in fear. At that moment, what coursed steadily through my whole body was undeniably a sense of pleasure. It was a kind of pleasure I had not experienced before, tinged with anger like a streak of menstrual blood mixing into urine. This man had no intention to accept me. This boring little man in his boring little outfit. I was exhilarated and enraged. I took another step towards him, the sweat cooling on my thighs. Even though I don’t remember his face, I can clearly remember that he had tears in his eyes. ‘Hel-lo’, I enunciated with a smile. The man’s eyes were fixed in the space behind me. The elevator stopped with a clang, and the door opened behind my back. It was the seventh floor. I quickly spread my legs shoulder-width apart to prevent him from getting out. The door closed. The man burst out crying. I watched as tears gushed from his tightly closed eyes. Trying to keep himself from making a sound, he scrunched up his face so hard that it started twitching. As the elevator rose, the man collapsed on the floor. I stood straddling his neatly lined-up knees, looking down on his undyed hat. The skirt of my school uniform was touching its brim. Normally, that should have made him happy. I despised him. Scared to tears. How weird. Around the time we reached the twelfth floor, I noticed he’d wet himself, because a trickle of urine began to flow towards my feet. ‘Ew, that’s dirty’, I spat out in a low voice and turned away to exit the elevator. Back in elementary school, I would be lonely in autumn. Red and black leather backpacks bouncing up and down, the yellow nylon backpack of a new transfer student, the sky as grey as the concrete surface of the road. My panties kept slipping and wedging themselves into my butt crack, so I tried to grab them through the fabric of my uniform skirt and return them to their proper place. I thought I had them, but they slipped right back. Even though the sky and the road looked flat, and my eyes could detect no depth to them, I knew they continued. Above the sky there is space, and even if the roads eventually run out, instead of nothing there is land and ocean beyond them. It was unbelievable, but I knew it was true. In autumn, there were always days where thinking about the way everything continued was enough to make me feel unbearably lonely. In other seasons, I could think about it without feeling lonely. Or it could be that I just didn’t think about it. There was nothing beautiful about the dull and dirty yellow of gingko leaves. With a heaviness in my heart, I tilted my head back and tried to jump. When I leapt like that, it felt like I was about to fall headfirst into the sky. I didn’t really mind either way. But when it didn’t happen, I felt all the more lonely. Sitting in the booth of a night train, pressing my temple against the cold window glass, I’m recalling the events of that summer and of much more distant autumns. I’m pretending to be asleep. Until a moment ago, I was really sleeping. The subdued but heavy breathing coming from the neighbouring seat woke me up. It seemed that I had, by a sheer stroke of luck, managed to provide some emotional support to Kanekoff that one morning, but I can’t remember doing anything for her after that. The two gays must have moved out in a hurry, as I’ve never seen the man or his partner again. What I did never came to light. Then again, I never even dreamed that he would try to prosecute me. Me back then would have asserted that I simply wanted to be greeted back. And yet, oh, that ecstasy and fury bursting through my body! Despite all that, I used to be just an innocent child who felt unbearably lonely in autumn. I wish I could be forgiven, but I know the only one able to forgive me is myself. I know the person sitting next to me now, breathing heavily, is a scruffy old man. The moment I woke up, I was struck by a sense of foreboding that kept me from raising my head. Careful to stay unnoticed, I just moved my eyes to steal a glance at him. A single look was all it took to take stock of the situation. Me and him are probably the only people in this carriage. Which is why he must have deliberately picked the seat right next to me while I was asleep. The old man is so thin I wonder if there is anything inside his clothes. The wrists sticking out of his frayed sleeves are covered in ashy veins. But the penis which he is holding in his hand, rubbing earnestly, is the pretty pink colour of a baby just lifted out of a hot bath. Slumping down and pretending to sleep, I’m on the verge of tears. I wonder if this man, too, used to be a child who felt lonely just because it was autumn or despised fresh green leaves, and if he will later apologise and ask for my forgiveness, using these things as an excuse. But in reality, what keeps me from moving is an overwhelming fear. Could I beat this man? I probably could. I’m wearing heavy boots. But am I really sure? What if I’m underestimating this old man’s strength? Without looking up, I rise up abruptly, leading with my shoulders. The path to the carriage aisle is blocked by the old man’s sagging trousers and pink penis. I step on the green-covered booth seat with my boots and leap over him. I hear the old man snarling furiously. He might be screaming in pain. I hope that’s the case. Maybe I accidentally stomped on his genitals. I don’t turn to look back. As I thought, there’s nobody else in this carriage. I want to scream too, but I focus on frantically opening the door to the next carriage. I can hear the old man groaning. I don’t know if he’s chasing after me or cowering on the floor. There are a few people in the next carriage, but they all seem to be asleep. I run through to the next carriage, feeling scared to death and unbearably lonely. I know that after I’ve run through a few more carriages, there will be nowhere left to go, and that makes me unbearably lonely. This is not what I wanted. I have to shake it off, shake it off and crush it under the soles of my boots. This is not what I wanted, this loneliness. Because this kind of loneliness cannot prove what kind of a person I am.

Source text - English ARCHITECTURE AND TRANSCENDENCE. A COMPARATIVE VIEW OF MINIMALISM IN FINLAND AND JAPAN This paper examines minimalism in architecture in relation to the phenomenon of transcendence in a comparative perspective between Finland and Japan. Both Finnish and Japanese cultures have been labelled cultures of silence, and minimalism is the art of silence. Minimalism as art has also been implicitly linked to transcendence. When I speak of transcendence, I refer to transcendental values or meanings. The philosopher Ludwig Wittgenstein splits meanings in two levels, verbal and value. In accordance with Wittgenstein, values are based on feeling and are transcendental because they can only be experienced in silence. SOURCES OF MINIMALISM When analysing a culture, we need to look at the relationship between man and his living environment. In fact, the formation of the spirit of a culture is a result of the encounter between man and nature, and between man and society. Moreover, architecture and art in general, as man’s creation, may be seen as a function of these relationships. Architect Louis Kahn has said: “Art is the language of the spirit. Art is the making of life. To create is the sense realization of the psyche and obedience to the laws of nature.” Nature and history have had a determining impact on the formation of value patterns of both Finland and Japan: environmental conditions, the attitudes towards nature, and historical peculiarities have generated lasting cultural values in each country. Despite striking differences in their geography and history, one may discover certain similarities between Finnish and Japanese cultures. For instance, simplicity, tranquillity, a close relationship to nature, and the appreciation of silence belong to shared cultural value patterns. Another similarity may be seen in the conception of space. Spatial arrangements in Finnish architecture are often quite close to the traditional Japanese conception of space called ma. Like the Japanese ma, the Finnish word tila has many meanings: apart from measurable geometric space, it can refer to a place, a physical state, or a state of mind. The common trait is that both the Japanese and the Finnish conception of space comprise the psychological experience of space in time. In both cultures, the conception of space is also linked to the relationship with nature; in Japan with the consciousness of place related to the Shintoist view of nature, and in Finland with the experiential space of the forest. Though the philosophical and aesthetic roots of minimalism are mainly historical, its use as a style designation dates only from the 1960’s. It first became known in the music and visual arts as a reductive style, and later in the 1980’s in architecture. However, the foremost slogan of minimalist architecture, “less is more,” was coined much earlier, in 1947, by one of the pioneering Modernists, the German-American architect Ludwig Mies van der Rohe, to explain the design philosophy of his work. In this regard, the influence of traditional Japanese aesthetics and architecture should not be ignored. Roots of Minimalism in Finland Finnish culture has its roots in its Nordic climate and geography as well as in its societal history between Sweden in the West, Russia in the East, and the Baltic countries and Germany in the south. The long past of Finland as a sparsely populated country with an un-sophisticated way of life close to nature, and as a predominantly agrarian society until the mid-20th Century, has instilled Finnish culture with features mostly arising from peasant culture contrasting with its relatively short urban history, mainly developed from the 1960’s onwards. Therefore, Finnish culture has often been characterized with epithets such as simplicity, naturalness, and silence. These are emotionally felt existential values, which have also influenced the cultural foundations of the development of modern Finnish architecture and design. While in Finland the natural conditions of the environment – its rugged nature and cold climate with long and dark winters – are quite rigorous, it is clear, that they have influenced the Finnish temperament and cultural spirit. Courage, tenacity, determination, and persistence, as well as simplicity and straightforwardness are dominant traits by which Finns have traditionally been described. Reservedness, introspection, and stubbornness constitute the other side of the coin and may have negative effects when excessively manifest. But there is something poetic and lyrical in the Finns’ feelings for nature, their meditative relationship to the forest, as well as in their devout sauna habits and joyful enjoyment of summer celebrations during the period of midnight sun. One should also mention the influence of Christianity, especially its Lutheran imprint, on Finnish culture. The Bible includes numerous passages which have generated deeply-felt spiritual values for people of faith – such as to “not collect treasures on earth, but instead in heaven,” by behaving modestly, not decorating oneself, and keeping one’s heart pure. The appreciation of a simple and unadorned way of life thus derives from the religious side as well. In terms of architectural development, all the historical building types in Finland are isolated structures. This was historically the norm in a country with a low population density, and where the main means of subsistence has been farming, with large areas of uninhabited land. Hence there is no Finnish historical equivalent to contemporaneous European architecture of urban conglomerations with built-in monuments. However, this does not mean that Finnish architecture was closed to external influences. During the almost seven hundred years under the reign of the Swedish kingdom, Finland too adopted continental tastes and styles, primarily through Swedish connections with European cultural capitals. Yet influences came slowly and stylistic applications were modest, albeit generally not without artistic refinement. On the contrary, during the Russian period when Finland became an autonomous Grand Duchy of the Tsarist empire with Helsinki as its capital, architectural development was strongly favoured. Though the architecture of the epoch’s Empire Style leant on antique prototypes, it was adapted to the country’s local economic and cultural conditions. While Helsinki may be considered an exception with its monumental Empire city centre, the provincial wooden towns as well as the residential wooden quarters of the capital became typical Finnish small-scale urban environments. It was only at the end of the 19th Century, and at the turn of the 20th Century, that a liberation in architectural expression took place. In the 20th Century, and especially after the Second World War, there was a vital interaction between Finnish architecture and the building of a new industrial society. Despite the limited resources of the post-war period, there was a lot of optimism in this endeavour which was also connected to Finland’s identity as an independent country since 1917. The greatest contribution was made by Alvar Aalto. He created a new paradigm for contemporary architecture adapted to the country’s resources and environment. Professor Hilding Ekelund has incisively described the epoch’s architectural accomplishments by saying that Finns then were capable of creating an organic connection between function, material and construction, associated with a well-considered simplicity inherited from the past. Characteristic of the architectural works created in the first half of the 20th Century was the preference for pure geometric forms resulting in a clear and simple form language. It manifests an approach of moderation connected with philosophical asceticism. The cultural spirit of this approach has often been described as idyllic. A similar view has been expressed by Professor Aulis Blomstedt in his manifest-like aphorism: “Architecture needs the existence of something old and permanent, like the cycle of the years, the rhythm of the moon, the majesty of the streams, or the old moss-covered rock. I feel that the most important aspect of modern architecture is the attempt to make man discover these values.” Roots of Minimalism in Japan Japanese minimalism, for its part, is based on a sophisticated historical and cultural background and an existential philosophy encompassing almost all art forms. Its roots lie in Taoist, Confucian, and Buddhist philosophies, as well as in the sources of wabi aesthetics as developed since the 12th Century and crystallized in the philosophy of tea culture in the 16th Century. The aesthetic ideals arising from these traditions, and the respect for nature inherent to Shintoism, form the normative value system manifested in Japanese minimalism. While its purest form may be experienced in sabi-wabi style tea houses and Buddhist rock gardens, the scope of its philosophical applications is far wider in view of both time and aesthetic expression. The Taoist tradition of China, originally stemming from Laotse’s poems written in the 4th Century BCE, has had a profound influence on Japanese cultural evolution over the course of the centuries. One of the most essential ideas of Taoism is that of emptiness, called wu in Chinese and mu in Japanese. In Japan, mu has been linked with Buddhist thinking referring to a transcendental nothingness. It is the term for “absolute nonexistence, from which all existence arises and to which mankind ultimately returns.” The philosophy of emptiness, however, has other sources, too. Originally it was based on the Indian Buddhist concept of sûniyatâ. In Japanese, its equivalent is kû, which means that all existing things are interdependent, or dependent on causes and conditions, and nothing exists by itself. Kû is not the opposite of existence, but signifies an emptiness that is beyond all conceptualization and verbal expression. All things are devoid of substantial reality, for the substance of all existence is “emptiness.” Thus, in the Buddhist view emptiness, kû, transcends all distinctions of affirmation and negation, existence, and non-existence. An ideal of emptiness is also reflected in the Japanese spatial conception ma. The original meaning of ma is interval, but it can also denote a silent moment, an empty space, or a “meaningful void.” There is no equivalence to the word ma in Western languages, but it always refers to some sort of void experienced in space and time. Be it in architecture or spatial arrangement, poetry, dance or even nô, this ideal of emptiness reduces the artistic expression to its most essential quality. However, it is undoubtedly the aesthetic and moral principles of wabi which have most strongly influenced the development of Japanese minimalism. Again, there is no possible translation of the term wabi into Western languages. In fact, the concept of wabi comprises several aspects, which together engender a special aesthetic ideal, and even more, an aesthetic consciousness – or a state of mind: a wabi mind (wabi kokoro). On a general level, wabi has been described as an existential approach “advocating the enjoyment of a quiet, leisurely life, free from worldly concern,” and “making poverty and loneliness synonymous with liberation from material and emotional worries and by turning the absence of apparent beauty into a new and higher beauty.” The Japanese historian Haga Kôshirô has defined three categories of beauty which all characterize the aesthetics of wabi: (1) simple, unpretentious beauty, (2) imperfect irregular beauty, and (3) austere stark beauty. There exist other categorizations, too, for instance, the seven aesthetic characteristics listed by Hisamatsu Shin’ichi in one of his articles on Japanese tea culture: irregularity, simplicity, austerity, naturalness, mystery (yûgen), ethereality, and tranquillity. These principles form a holistic aesthetic and moral value system guiding the design process of an art work in the wabi spirit. There is another similar aesthetic concept, sabi, which often is associated with wabi, notably in the context of teahouses and chanoyou. The word sabi is used to describe a special aesthetic atmosphere created by conflicting appreciative values linked with, e.g. the beauty of blossoming flowers on the one hand, and loneliness, timely wearing, resignation, tranquillity, and old age, on the other. Thus, the aesthetics of wabi-sabi value modesty and the imperfect and irregular quality of a house, or an object made of natural materials, as well as the absence of all unnecessary features, while appreciating the impermanent nature of existence and a serene quietness. The ideas of valuing simplicity, naturalness, and authenticity also reflect Buddhist teachings in parallel with the aesthetic ideals of wabi, which evolved in the Japanese Middle-Ages. The moral concepts of Zen philosophy “looking into the nature of truth” have especially influenced the generation of a holistic aesthetic consciousness inherent in wabi and sabi. Buddhism, reminding us about the nature of suffering as a characteristic of human existence, aims at leading us towards understanding the meaning of emptiness (kû) – or the absence of self-nature – and the attainment of transcendental nothingness (mu) evoked by spiritual awakening and the liberation from ignorance. Seen from these different perspectives, the aesthetic and moral values of wabi-sabi are at the core of the Japanese culture. In addition to wabi and sabi, there are still other values that influence the aesthetics of Japanese minimalism. One of them is shibui, the concept of subtle beauty. Shibui is usually associated with “quietly appealing ambience,” appreciating aesthetically underestimated but elegant artistic expression, and avoiding colourful beauty or otherwise dazzling details. The taste for shibui can be felt in all Japanese art forms, but its aesthetic influence is especially visible in architecture and interior design. MINIMALIST ARCHITECTURES Minimalism in Finnish Architecture While the history of Finnish architecture goes back to the beginnings of the Swedish reign in the 13th century, its reductive approach covers the whole range of both vernacular houses, buildings of historical style periods, as well as modern architecture. Continental stylistic tendencies used to reach Finland via Sweden, Russia, and Germany until the 20th Century, when intercontinental influences took over architectural development on a global scale. Since then Japan has been an important source of inspiration not only for Finnish but for Western architects in general all through the 20th century. The early Finnish version of Modernism had primarily an aesthetic and functional character. It lacked the decidedly ideological content that was apparent for instance in the Russian avant-garde and the German Bauhaus movements in the 1920´s and 30’s. Alvar Aalto, with a few other pioneering architects of the period, showed the way to a contemporary architecture, which was intellectually and stylistically international, but aesthetically and technically adapted to the country’s cultural and economic conditions. While it is evident that Aalto’s organic architecture may not usually comply with the stylistic criteria of minimalism, he can be considered the spiritual forerunner of a well-balanced, locally adapted building art. Minimalist expression in Finland can historically be seen as a demonstration of the inherent ethos of Finnish people, accustomed to a simple life style in a quiet, natural environment with modest resources. As described earlier, the appreciation of silence, naturalness, authenticity, and simplicity have become archetypal values of Finnish culture. Besides these values, however, the modern trends of Finnish minimalism manifest more precisely defined stylistic goals, such as an extremely stripped architectural expression and overall visual austerity. Since the 1950’s Japanese architecture has been a conspicuous source of inspiration for Finnish architects. In the 1960’s there was even an explicit trend of Japonism. ¬However, we should not forget that since the end of the 19th Century, when the first books on Japanese architecture were published in the West, Japanese influence on the development of modern architecture on a global scale has been of central importance. Especially after the “discovery” of the 17th Century Katsura Imperial Villa, its sophisticated simplicity and functional beauty, which became widely admired and published in western countries, traditional Japanese architecture has left its mark not only on minimalism but also by and large on the evolution of Modernism. There are numerous buildings representing modern minimalism in Finland. It may not be a coincidence that most of its finest examples are churches. A standard example is the Chapel of Otaniemi (1957) designed by architects Kaija and Heikki Sirén. It is an unpretentious structure of brick and wood, situated in a small forest on the campus of Aalto University. A modest footpath leads to the yard of the building, resembling a traditional farmhouse with its wooden fence. The chapel itself is a simple cubic-like space. The wall behind the altar is of glass, permitting the interior to merge into the forest. Behind the glass wall, a bare white-painted steel cross stands in between the chapel and the forest. The essence of nature fills the sacral space, imbuing a transcendental atmosphere. Yet perhaps the most striking manifestations of Finnish minimalism, as a style, are the church buildings designed by architect Aarno Ruusuvuori. One of Ruusuvuori’s best known works is Hyvinkää Church built in 1961, which may still, more than half a century later, be considered a prominent example of the tendency to abstraction in Finnish Modernism. The church hall is a closed space shaped like a pyramid, with one high window in the apsis opening towards the sky. The pyramidal walls involve giant folded concrete slabs forming furrow-like beams. Ruusuvuori’s design demonstrates clear structural thinking intertwined with a powerful ascetic poetry. Two other sanctuaries by Ruusuvuori, the Huutoniemi Church of 1964 and the Tapiola Church of 1965, are also built of concrete, both characterized by pure geometric forms and an austere architectural expression. The closed, stripped character of the two church halls is emphasized by bald surfaces of the concrete walls and well-calculated placements of openings for light. There is a spiritual power in the architecture of these churches, both of which have been considered prominent exponents of the brutalist tendency of minimalism. Even the Japanese architect Tadao Ando, famous for his minimalist buildings, has revealed to have been influenced by Ruusuvuori’s “silent” church architecture. Minimalism in Japanese Architecture The ideals of simplicity and austerity are central values in Japanese culture as well. This has as much to do with the Confucian and Taoist traditions as well with Buddhism, in which the purity of mind, heart and spirit (kokoro) is emphasized. The combination of these traditions with the aesthetic values of wabi-sabi has led to the exquisite achievements of Japanese minimalism. Japanese minimalist architecture creates an aesthetic atmosphere that most characteristically may be experienced in wabi-sabi style teahouses and the so-called sukiya style buildings inspired by tea-room architecture. The stone gardens of Zen Buddhist temples, on the other hand, epitomize an architecture of transcendental emptiness. A foremost example of architecture embodying transcendental emptiness is the stone garden of Ryôan-ji, built on the grounds of a Buddhist temple in Kyoto at the turn of the 16th Century. It manifests the ideals of simplicity, essentiality, and spirituality, with its vast empty space, formed by white sand and fifteen grey stones bounded by a wall. Ryôan-ji, as well as other stone gardens of the kare sansui type represent the worldview of Zen Buddhism and are intimately linked with kû, the metaphysical emptiness. In monasteries, stone gardens are places of silence and meditation. Meditation aims to calm the mind and attain transcendental emptiness, the holy experience of kû. The Taian tea-room, built in the 16th Century in accordance with the instructions of the tea master Sen no Rikyû, is without doubt the most prominent example of the wabi style teahouse architecture. Said to be the oldest teahouse of Japan, it was originally located within the castle of Hideyoshi in Yamazaki, but has been either moved or reconstructed in the Myokian Temple in Kyoto, where it still exists. The Taian tea-room is a tiny rustic hut, referred to as a sôan, or grass hermitage. Its interior is the size of two tatamis, one mat for the host and a second one for the guest, with simply a hearth for heating the water. In addition, it comprises a tokonoma for flower arrangement, an anteroom, and a small preparation room, or katte. Sparse details including a crawl door and a few small windows with exposed wattle are placed with care. While there is an aesthetic quality to the visual effects, the atmosphere is intentionally modest and dim. As described by Kakuzo Okakura, “(e)ven in the daytime the light in the room is subdued, for the low eves of the slanting roof admit but few of the sun’s rays. Everything is sober in tint from the ceiling to the floor.” Not only do stone gardens and traditional teahouse architecture represent minimalism, but also Japan’s contemporary architecture comprises an authentic minimalist movement. Modern minimalism is usually interpreted in a continuous line with the traditional philosophical and aesthetic values described earlier. Thus, it is the traditional aesthetic categories of beauty which form the basis for Japanese minimalism as a style. As mentioned earlier, the threefold division by Haga Kôshirô into simple, unpretentious beauty, imperfect irregular beauty, and austere stark beauty, is a classical categorization of minimalist aesthetics, which also prevails in contemporary architecture. In addition, the seven aesthetic characteristics listed by Hisamatsu Shin’ichi ¬¬– irregularity, simplicity, austerity, naturalness, mystery, ethereality, and tranquillity – provide a set of stylistic criteria for minimalist design. While contemporary Japanese architecture cannot be interpreted as historicist in any regard, the historically defined wabi-sabi values, however, combine the philosophical and aesthetic guidelines for minimalist architecture in today’s Japan, too. Architect Tadao Ando’s buildings manifest the spirit of traditional Japanese culture by suggesting a clearly archetypal approach. Ando’s designs stem from his innate cultural nature, reflecting the aesthetic ideals of Shinto, wabi, and ma. His design concepts are simple, he favours basic structural forms and pure geometry, uses concrete and natural wood as building materials, and creates austere but powerful spaces with an interplay between shadow and rays of natural light. The intertwining interior and exterior spaces are skilfully organized in accordance with a feeling for nature and emptiness. An impressive example of Tadao Ando’s recent minimalist designs is the Art Museum dedicated to the works of the Korean-Japanese artist Lee Ufan. The museum, built in 2010 on the Island of Naoshima, forms a unique combination with Ufan’s minimalist sculptures and paintings, which engage in an intimate dialogue with Ando’s architectural approach. The result is a unique total artwork, a spiritual Gesamtkunstwerk. The aesthetics of minimalism become almost palpable in the museum’s spaces, which are intimate yet express universal metaphysical emptiness. Another Japanese minimalist architect, Kazuyo Sejima, is appreciated for her sensitive feminine approach. Her work is different from Ando’s rather masculine architecture. Sejima, too, favours pure geometric forms but her designs are more colourful, and she uses more glass and other contemporary materials. An example of her minimalist works is the ethereal N-Museum (1997) in the mountainous environment of Kumano. Typical of it are spaces created with transparencies of several layers. As a result, her architectural approach refers as much to phenomenological as minimalist concepts, though still conveying values deep in the Japanese culture and mind. ARCHITECTURE AND TRANSCENDENCE Both Finnish and Japanese culture stress the value of silence. In Finland this is an overall cultural attitude which inherently affects Finnish identity. In Japan, where Buddhism has fundamentally influenced the evolution of the nation’s culture, the need for controlling one’s mind is always strongly stressed. There is a saying of Buddha: “It is in the sanctuary of silent places, with mindful attention, that the liberation of the heart (kokoro) can be gained.” Understanding silence on a spiritual level, only once our mind is quiet can we experience a higher reality. We long for a transcendental experience that allows us to free ourselves from the boundaries of our worldly mind, and to get in touch with the realm of Oneness or harmonious unity. Similarly, when every unnecessary feature has been eliminated and stripped from an art work, only the essential remains. Japanese philosopher Kitarô Nishida has described the essential – or the quintessential essence of being – in Buddhist terms. For Nishida, all forms of existence, form and space, are merely marks on the surface of nothingness (mu). The Christian philosophical equivalent corresponding to the Buddhist notion of nothingness would then be – silence. Meister Eckhardt has said: “It is in silence that God tells his eternal word to the soul.” How could we describe silence? The Swiss Philosopher Max Picard wrote a book on silence called Die Welt des Schweigens (The World of Silence, 1948). In Picard’s terms, silence is “an autonomous phenomenon,” and, “(t)hrough this power of autonomous being, silence points to a state when only being is valid: state divine. The mark of the divine in things is preserved by their connection with the world of silence.” True being, true architecture, or anything true, is thus born out of the silence – or in the Buddhist view, from the eternal “nothingness.” Louis Kahn depicted this in the following words: “As an art a space is made a touch of eternity.” A touch of eternity alludes to what Picard says, that true art has the quality of connecting us with the transcendental world of silence. An authentic experience of silence provides us with a transcendental understanding of the self-made limits of our everyday world and, by the same logic, of what is true. This illuminating understanding is what we long for, and it is equally what we long for in architecture ad art. Transcendence and truth are questions which are also dealt with in the philosophy of art. For example, the Korean-born philosopher and artist Lee Ufan includes transcendence in the mere definition of art. For Martin Heidegger, the German philosopher, the essence of art is poetry, and poetry is the implementation or “happening of truth.” The meaning of truth, however, is vast, and for Heidegger it means an “opening up of world.” While worlds in art may come and go, Heidegger’s definition of a world, in return, is truth, or the “truth of beings” or “truth of being of beings.” There are works of architecture which have an experiential quality of truth. The philosophy of wabi is an example which strives for it, and many Japanese teahouses or zen gardens are obvious witnesses of their inherent capacity to open us into a realm of transcendental experience. Here the meaning of wabi is highlighted to transcend material modesty into an experiential spiritual freedom, and the zen gardens invite us to experience emptiness in silence. In minimalist architecture, be it in Finland or in Japan, the design elements of a building are intended to express silence and simplicity. Pure geometric forms, “honest” use of building materials, the absence of all unnecessary features, archetypal spatial arrangements, harmonious communication of the interior spaces with the exterior, as well as a subtle interplay of shade and light embody a state of aesthetic consciousness “beyond the intellect.” Here the transcendental qualities of silence and simplicity are not only aesthetic values, but they also manifest a transcendental understanding of the nature of truth and unfold the inner being of architecture in its essence. The aim of minimalism as a moral and aesthetic awareness is to manifest spiritual freedom, which ultimately carries a higher meaning. References (..)	Translation - Finnish Tämä artikkeli tarkastelee arkkitehtuurisen minimalismin yhteyttä transsendenssin ilmiöön vertaillen Suomea ja Japania. Sekä suomalaista että japanilaista kulttuuria on kutsuttu hiljaisuuden kulttuureiksi, ja minimalismi on hiljaisuuden taidetta. Minimalismi taiteena on myös epäsuorasti liitetty transsendenssiin. Kun puhun transsendenssista, tarkoitan transsendentaalisia arvoja tai merkityksiä. Filosofi Ludwig Wittgenstein jakaa merkitykset kahteen tasoon, verbaalisiin ja arvoihin. Wittgensteinin mukaan arvot perustuvat tunteisiin ja ovat transsendentaalisia, koska ne voidaan kokea ainoastaan hiljaisuudessa. MINIMALISMIN LÄHTEET Kulttuuria analysoitaessa on tärkeää tarkastella ihmisten suhdetta elinympäristöönsä. Kulttuurin hengen muotoutuminen on itse asiassa ihmisen ja luonnon sekä ihmisen ja yhteiskunnan kohtaamisen lopputulos. Lisäksi arkkitehtuuri ja taide yleensäkin voidaan nähdä eräänä näiden suhteiden funktiona, koska ne ovat ihmisen luomuksia. Arkkitehti Louis Kahn on todennut: “Taide on hengen kieli. Taide on elämän tekemistä. Luominen on psyyken ja luonnonlakien edessä nöyrtymisen ruumiillistamista. ” Luonnolla ja historialla on ollut ratkaiseva vaikutus arvomallien muotoutumiseen sekä Suomessa että Japanissa: ympäristöolosuhteet, asenteet luontoa kohtaan sekä historialliset erityispiirteet ovat synnyttäneet pysyviä kulttuurisia arvoja molemmissa maissa. Merkittävistä maantieteellisistä ja historiallisista eroavaisuuksista huolimatta suomalaisessa ja japanilaisessa kulttuurissa voidaan havaita tiettyjä yhtäläisyyksiä. Esimerkiksi yksinkertaisuus, rauhallisuus, läheinen luontosuhde sekä hiljaisuuden arvostus ovat jaettuja kulttuurisia arvomalleja. Eräänä yhtäläisyytenä voidaan nähdä myös tilan käsitys. Suomalaisen arkkitehtuurin spatiaaliset järjestelmät ovat usein melko lähellä ma-nimistä japanilaista tilan käsitystä. Kuten japanin kielen ma, myös suomen tila on monimerkityksellinen sana: mitattavissa olevan geometrisen tilan lisäksi se voi viitata myös paikkaan, fyysiseen olotilaan tai mielentilaan. Japanilaiselle ja suomalaiselle tilan käsitteelle on yhteistä, että ne sisältävät psykologisen kokemuksen tilasta ajassa. Molemmissa kulttuureissa tilan käsite liittyy myös luontosuhteeseen; japanilainen tilatietoisuus on yhteydessä shintolaiseen luontokäsitykseen, suomalainen taas metsän kokemusperäiseen tilaan. Vaikka minimalismin filosofiset ja esteettiset juuret ovat pääosin historiallisia, sen käyttö tietynlaisen tyylin nimityksenä alkoi vasta 1960-luvulla. Se tuli ensin tunnetuksi reduktiivisena tyylinä musiikissa ja kuvataiteissa, ja myöhemmin 1980-luvulla arkkitehtuurissa. Minimalistisen arkkitehtuurin oleellisin iskulause, “vähemmän on enemmän”, keksittiin kuitenkin jo paljon aikaisemmin, vuonna 1947. Sloganin takana oli eräs modernismin pioneereista, saksalais-amerikkalainen arkkitehti Ludwig Mies van der Rohe, joka selitti sillä teostensa muotoilun filosofiaa. Tässä suhteessa perinteisen japanilaisen estetiikan ja arkkitehtuurin vaikutusta ei tule jättää huomiotta. Minimalismin juuret Suomessa Suomalaisen kulttuurin juuret ovat sen pohjoismaisessa ilmastossa ja maantieteessä sekä sen yhteiskunnallisessa historiassa läntisen Ruotsin, itäisen Venäjän sekä eteläisten Baltian maiden ja Saksan välillä. Suomen pitkä menneisyys harvaan asuttuna maana, jossa ihmiset elivät yksinkertaisesti ja lähellä luontoa, sekä aina 1900-luvun puoliväliin asti lähinnä maatalouteen perustuvana yhteiskuntana, on juurruttanut suomalaiseen kulttuuriin talonpoikaiskulttuurin piirteitä, jotka toimivat vastavoimana sen suhteellisen lyhyelle, pääasiassa 1960-luvulta alkaen kehittyneelle urbaanille historialle. Näin ollen suomalaista kulttuuria on usein luonnehdittu epiteeteillä kuten yksinkertaisuus, luonnollisuus sekä hiljaisuus. Nämä ovat tunnetasolla koettuja eksistentiaalisia arvoja, jotka ovat vaikuttaneet myös modernin suomalaisen arkkitehtuurin ja muotoilun kehittymisen kulttuuriseen perustaan. Vaikka Suomen ympäristöolosuhteet - sen jylhä luonto ja kylmä ilmasto pitkine pimeine talvineen - ovat melko ankaria, on selvää, että ne ovat vaikuttaneet suomalaiseen temperamenttiin ja kulttuuriseen henkeen. Rohkeus, sitkeys, määrätietoisuus ja sisukkuus, kuten myös yksinkertaisuus ja suorasukaisuus, ovat vallitsevia piirteitä, joilla suomalaisia on perinteisesti luonnehdittu. Kolikon toisen puolen muodostavat varautuneisuus, itsetutkiskelu ja jääräpäisyys, joilla voi liiallisina ilmetessään olla negatiivisia vaikutuksia. Mutta suomalaisten tuntemuksissa luontoa kohtaan, heidän meditatiivisessa metsäsuhteessaan, hartaissa saunomistavoissaan ja yöttömien öiden riemullisessa kesän juhlistamisessaan, on jotakin runollista ja lyyristä. On myös mainittava kristinuskon, erityisesti sen luterilaisen muodon, vaikutus suomalaiseen kulttuuriin. Raamattu sisältää lukuisia jakeita, joiden pohjalta on uskovaisten keskuudessa kehittynyt syviä hengellisiä arvoja, kuten esimerkiksi: “Älkää kootko itsellenne aarteita maan päälle, vaan taivaaseen”, mitä ihminen voi tavoitella käyttäytymällä vaatimattomasti, pidättäytymällä itsensä koristamisesta ja pitämällä sydämensä puhtaana. Yksinkertaisen ja koristeettoman elämäntavan arvostaminen juontuu siis myös uskonnon puolelta. Mitä tulee arkkitehtuurin kehitykseen, kaikki historialliset rakennustyypit Suomessa ovat erillisiä rakenteita. Tämä oli historiallisesti normi matalan väestötiheyden maassa, jossa pääasiallinen toimeentulokeino on ollut maatalous ja suuria maa-alueita on ollut asumattomina. Näin ollen Suomesta ei löydy historiallista vastinetta saman aikakauden eurooppalaiselle arkkitehtuurille kaupunkikeskittymineen ja sisäänrakennettuine monumentteineen. Tämä ei kuitenkaan tarkoita, että suomalaisen arkkitehtuurin kenttä olisi ollut suljettu ulkopuolisilta vaikutteilta. Ruotsin kuningaskunnan lähes seitsemän vuosisataa jatkuneen vallan alla myös Suomessa omaksuttiin mannermaisia mieltymyksiä ja tyylejä, joiden ensisijaisena tuloväylänä olivat ruotsalaiset suhteet Euroopan kulttuuripääkaupunkeihin. Vaikutteet saapuivat kuitenkin hitaasti, ja tyylillinen soveltaminen oli vaatimatonta, vaikkakaan ei yleensä vailla taiteellista hienostuneisuutta. Sitä vastoin venäläisenä kautena, jolloin Suomesta tuli tsaarinajan keisarikunnan autonominen suuriruhtinaskunta Helsinki pääkaupunkinaan, arkkitehtuurin kehitystä suosittiin voimakkaasti. Vaikka epookin empiretyylinen arkkitehtuuri perustui antiikkisiin prototyyppeihin, se mukautettiin maan paikallisiin taloudellisiin ja kulttuurisiin olosuhteisiin. Helsinkiä voidaan pitää poikkeuksena monumentaalisen Empire-keskustansa ansiosta, mutta tyypillisiä suomalaisia pienen mittakaavan urbaaneja ympäristöjä tulivat edustamaan maakuntien puukaupungit sekä pääkaupungin puiset asuinalueet. Arkkitehtuurisen ilmaisun vapautuminen tapahtui vasta 1800-luvun lopulla ja 1900-luvun vaihteessa. 1900-luvulla, erityisesti toisen maailmansodan jälkeen, vuorovaikutus suomalaisen arkkitehtuurin ja uuden teollisen yhteiskunnan rakennusprosessin välillä oli oleellisen tärkeää. Sodanjälkeisen ajan rajallisista resursseista huolimatta tätä pyrkimystä kohtaan vallitsi optimismi, joka liittyi myös Suomen identiteettiin itsenäisenä valtiona vuodesta 1917 alkaen. Merkittävimmän panoksen tarjosi Alvar Aalto. Hän loi ajan arkkitehtuurille uuden paradigman, joka oli mukautettu maan resursseihin ja ympäristöön. Professori Hilding Ekelund on kuvaillut epookin arkkitehtuurisia saavutuksia terävästi toteamalla, että ajan suomalaiset kykenivät luomaan orgaanisen yhteyden funktion, materiaalin ja rakennuksen välillä; tämän yhteyden taustalla oli menneisyydestä peritty, tarkoin harkittu yksinkertaisuus. 1900-luvun alkupuoliskon arkkitehtuurin erityispiirre oli puhtaiden geometristen muotojen suosiminen, joka johti selkeään ja yksinkertaiseen muotokieleen. Se ilmentää maltillista lähestymistapaa, joka liittyy filosofiseen asketismiin. Tämän lähestymistavan kulttuurista henkeä on usein kuvailtu idylliseksi. Professori Aulis Blomstedt on ilmaissut samankaltaisen näkemyksen manifestimaisessa aforismissaan: “Arkkitehtuuri tarvitsee jonkin vanhan ja jatkuvan olemassaoloa, kuten vuosien kiertoa, kuun rytmiä, virtojen majesteetillisuutta, tai vanhoja sammaleen peittämiä kiviä. Koen, että modernin arkkitehtuurin tärkein merkitys on sen pyrkimyksessä saada ihminen löytämään nämä arvot.” Minimalismin juuret Japanissa Japanilainen minimalismi puolestaan perustuu hienostuneeseen historialliseen ja kulttuuriseen taustaan sekä eksistentiaaliseen filosofiaan, joka käsittää lähes kaikki taidemuodot. Sen juuret ovat taolaisissa, kungfutselaisissa ja buddhalaisissa filosofioissa sekä wabi-estetiikan lähteissä, joita on kehitetty 1100-luvulta asti ja jotka ovat kiteytyneet teekulttuurin filosofiaan 1500-luvulla. Näistä perinteistä nousevat esteettiset ihanteet sekä shintolaisuudelle oleellinen kunnioitus luontoa kohtaan muodostavat arvojärjestelmän, jota japanilainen minimalismi ilmentää. Vaikka sen puhtaimpana muotona voidaan pitää wabi-sabi-tyylin teehuoneita ja buddhalaisia kivipuutarhoja, sen filosofisten sovellusten skaala on paljon laajempi sekä ajan että esteettisen ilmaisun saralla. Kiinan taolainen perinne, alkujaan peräisin Laotsen 400-luvulla EAA kirjoitetuista runoista, on vaikuttanut oleellisesti japanilaisen kulttuurin kehitykseen vuosisatojen aikana. Eräs taolaisuuden keskeisimmistä käsitteistä on tyhjyys , kiinaksi wu ja japaniksi mu. Japanissa mu on liitetty buddhalaiseen ajatteluun, jossa se viittaa transsendentaaliseen olemattomuuteen. Sen merkitys on “absoluuttinen olemattomuus, josta kaikki olemassaolo nousee ja johon ihmiskunta lopuksi palaa.” Tyhjyyden filosofialla on kuitenkin muitakin lähteitä. Se perustui alkujaan Intian buddhalaisuuden sûniyatâ-käsitteeseen. Käsitteen japanilainen vastine on kû, joka tarkoittaa, että kaikki olemassa olevat asiat ovat riippuvaisia toisistaan tai syistä ja olosuhteista, eikä mitään ole olemassa itsessään. Kû ei ole olemassaolon vastakohta, vaan merkitsee tyhjyyttä, joka on käsitteellistämisen ja verbaalisen ilmaisun tuolla puolen. Kaikki olevat asiat ovat vailla aineellista todellisuutta, koska kaiken olemassaolon aine on “tyhjyys”. Näin ollen buddhalainen käsitys tyhjyydestä, kû, ylittää kaikki rajaviivat affirmaation ja negaation, olemassaolon ja olemattomuuden välillä. Myös japanilainen tilan käsite, ma, heijastaa tyhjyyden ihannetta. Ma-sanan alkuperäinen merkitys on väli, mutta se voi tarkoittaa myös hiljaista hetkeä, tyhjää tilaa, tai “merkityksellistä tyhjyyttä”. Ma-sanalla ei ole suoraa vastinetta länsimaisissa kielissä, mutta se viittaa aina jonkinlaiseen tilassa ja ajassa koettuun tyhjyyteen. Sovellettiin sitä sitten arkkitehtuuriin tai spatiaalisiin järjestelmiin, runouteen, tanssiin tai jopa nô-teatteriin, tämä tyhjyyden ihanne pelkistää taiteellisen ilmaisun syvimpään olemukseensa. Voimakkaimmin japanilaisen minimalismin kehitykseen ovat kuitenkin vaikuttaneet wabin esteettiset ja moraaliset periaatteet. Wabi-sanaa on mahdotonta kääntää länsimaisille kielille. Wabin käsite koostuu itse asiassa useista eri osa-alueista, jotka yhdessä muodostavat erityisen esteettisen ihanteen ja jopa esteettisen tietoisuuden - tai mielentilan: wabi-mieli (wabi kokoro). Yleisellä tasolla wabia on kuvailtu eksistentiaaliseksi lähestysmistavaksi, joka “puolustaa hiljaisesta, kiireettömästä, maallisten murheiden tahraamattomasta elämästä nauttimista” sekä “köyhyyden ja yksinolon muuttamista synonyymiseksi maallisista ja tunteellisista huolista vapautumiselle sekä ilmiselvän kauneuden puutteen muuttamista uudeksi, korkeammaksi kauneudeksi.” Japanilainen historioitsija Haga Kôshirô on määritellyt kolme kauneuden kategoriaa, jotka ovat luonteenomaisia wabi-estetiikalle: (1) yksinkertainen, teeskentelemätön kauneus, (2) epätäydellinen, virheellinen kauneus, ja (3) koruton, karu kauneus. On olemassa myös muita luokitelmia, kuten esimerkiksi Hisamatsu Shin’ichin erääseen japanilaista teekulttuuria käsittelevistä artikkeleistaan sisällyttämä seitsemän esteettisen luonteenpiirteen luettelo: virheellisyys, yksinkertaisuus, koruttomuus, luonnollisuus, mystisyys (yûgen), katoavaisuus sekä levollisuus. Nämä periaatteet muodostavat kokonaisvaltaisen esteettisen ja moraalisen arvojärjestelmän, joka ohjaa wabi-henkisen taideteoksen muotoiluprosessia. On olemassa myös samankaltainen esteettinen käsite, sabi, joka usein liitetään wabiin, eritoten teehuoneiden ja chanoyu-teetaiteen yhteydessä. Sanaa sabi käytetään kuvailemaan erityistä esteettistä tunnelmaa, joka syntyy ristiriitaisten arvojen yhdistymisestä; esimerkiksi kukkaloiston kauneudesta vastapainona yksinäisyydelle, ajan kulumalle, luovuttamiselle, levollisuudelle ja vanhuudelle. Näin ollen wabi-sabi-estetiikassa arvostetaan vaatimattomia ja laadultaan epätäydellisiä, virheellisiä taloja; luonnonmateriaaleista valmistettuja esineitä; sekä kaikkien tarpeettomien piirteiden poissaoloa, ja vaalitaan olemassaolon katoavaisuutta sekä seesteistä hiljaisuutta. Ajatukset yksinkertaisuuden, luonnollisuuden ja autenttisuuden arvostamisesta heijastelevat paitsi Japanin keskiajalla kehittyneitä wabi-estetiikan ihanteita, myös buddhalaisia opetuksia. Zen-filosofian moraaliset käsitteet, kuten “todellisuuden olemuksen tutkiminen”, ovat vaikuttaneet erityisen voimakkaasti wabille ja sabille oleellisen kokonaisvaltaisen esteettisen tietoisuuden syntyyn. Muistuttaessaan meitä siitä, että kärsimys on eräs inhimillisen olemassaolon ominaispiirteistä, buddhalaisuus pyrkii opastamaan meitä kohti tyhjyyden (kû) - tai yksilöllisen minän olemattomuuden - merkityksen ymmärtämistä sekä hengellistä herätystä ja tietämättömyydestä vapautumista, jotka johtavat transsendentaalisen olemattomuuden (mu) saavuttamiseen. Näistä eri näkökulmista tarkasteltuna wabi-sabin esteettiset ja moraaliset arvot ovat japanilaisen kulttuurin ytimessä. Wabin ja sabin lisäksi on olemassa muitakin arvoja, jotka vaikuttavat japanilaisen minimalismin estetiikkaan. Yksi niistä on shibui, hienovaraisen kauneuden käsite. Shibui liitetään yleensä “hiljaa vetoavaan tunnelmaan”, esteettisesti vähätellyn mutta elegantin taiteellisen ilmaisun arvostamiseen, sekä värikkään kauneuden tai muiden häikäisevien yksityiskohtien välttämiseen. Mieltymys shibuiin voidaan nähdä kaikissa japanilaisissa taidemuodoissa, mutta sen esteettinen vaikutus on erityisen ilmeistä arkkitehtuurissa ja sisustussuunnittelussa. MINIMALISTISIA ARKKITEHTUUREJA Minimalismi suomalaisessa arkkitehtuurissa Vaikka suomalaisen arkkitehtuurin historia juontaa juurensa Ruotsin vallan alkuajoille 1200-luvulla, sen reduktiivinen lähestymistapa käsittää koko skaalan sekä vernakulaarisia taloja, historiallisten taidesuuntauksien mukaisia rakennuksia että modernia arkkitehtuuria. Mannermaiset tyylisuuntaukset tulivat Suomeen Ruotsin, Venäjän ja Saksan kautta aina 1900-luvulle asti, jolloin mannertenväliset vaikutteet alkoivat hallita arkkitehtuurista kehitystä globaalissa mittakaavassa. Tämä jälkeen Japani on ollut merkittävä inspiraation lähde, ei ainoastaan suomalaisille vaan myös ylipäänsä länsimaalaisille arkkitehdeille, läpi 1900-luvun. Modernismin varhainen suomalainen versio oli laadultaan ensisijaisesti esteettistä ja funktionaalista. Siitä puuttui sellainen selkeän ideologinen sisältö, jota ilmeni esimerkiksi venäläisessä avant-gardessa ja saksalaisessa Bauhausissa 1920- ja 1930-luvuilla. Alvar Aalto, yhdessä muutamien muiden aikakauden pioneerihenkisten arkkitehtien kanssa, osoitti polun kohti modernia arkkitehtuuria, joka oli intellektuaalisesti ja tyylillisesti kansainvälistä, mutta esteettisesti ja teknisesti mukautettu maan kulttuurisiin ja taloudellisiin olosuhteisiin. Vaikka on selvää, ettei Aallon orgaaninen arkkitehtuuri yleensä täytä minimalismin tyylillisiä kriteerejä, häntä voidaan pitää tasapainoisen ja paikallisiin oloihin sovelletun rakennustaiteen henkisenä edelläkävijänä. Suomalainen minimalistinen ilmaisu voidaan historiallisesti nähdä Suomen kansan synnynnäisenä eetoksena, joka juontuu tottumuksesta yksinkertaiseen elämäntapaan hiljaisessa, luonnollisessa ympäristössä rajallisin resurssein. Kuten aiemmin todettiin, hiljaisuuden, luonnollisuuden, autenttisuuden ja yksinkertaisuuden arvostuksesta on tullut suomalaisen kulttuurin arkkityyppinen arvojärjestelmä. Näiden arvojen lisäksi suomalaisen minimalismin modernit trendit ilmentävät kuitenkin myös täsmällisemmin määriteltyjä tyylillisiä tavoitteita, kuten äärimmäisen riisuttua arkkitehtuurista ilmaisua ja kokonaisvaltaista visuaalista koruttomuutta. Japanilainen arkkitehtuuri on ollut huomattava inspiraation lähde suomalaisille arkkitehdeille 1950-luvulta asti. 1960-luvulla japonismi nousi jopa selkeäksi trendiksi. Ei kuitenkaan sovi unohtaa, että alkaen 1800-luvun lopulta, jolloin ensimmäiset japanilaista arkkitehtuuria käsittelevät kirjat julkaistiin lännessä, Japanilla on ollut keskeinen vaikutus modernin arkkitehtuurin kehittymiseen globaalissa mittakaavassa. Erityisesti sen jälkeen, kun 1600-luvun Katsuran keisarillinen huvila “löydettiin”, sen hienostunutta yksinkertaisuutta ja funktionaalista kauneutta ihailtiin ja niistä kirjoitettiin laajasti länsimaissa, perinteinen japanilainen arkkitehtuuri on jättänyt jälkensä minimalismin lisäksi myös laajemmin modernismin kehitykseen. Suomessa on lukuisia rakennuksia, jotka edustavat modernia minimalismia. Ei välttämättä ole sattumaa, että useimmat sen hienoimmista esimerkeistä ovat kirkkoja. Tyypillinen esimerkki on Otaniemen kappeli (1957), jonka suunnittelivat arkkitehdit Kaija ja Heikki Sirén. Se on teeskentelemätön puu- ja punatiilirakenne, joka sijaitsee metsikössä Aalto-yliopiston kampuksella. Vaatimaton polku johtaa puuaitoineen perinteistä maalaistaloa muistuttavan rakennuksen pihaan. Kappeli itsessään on yksinkertainen kuutiomainen tila. Seinä alttarin takana on valmistettu lasista, jotta sisätila sulautuu metsämaisemaan. Lasiseinän takana paljas, valkoiseksi maalattu teräsristi seisoo kappelin ja metsikön välissä. Luonnon olemus täyttää pyhitetyn tilan ja valaa siihen transsendentaalista tunnelmaa. Suomalaisen minimalismin tyylillisesti silmiinpistävimpiä ilmentymiä lienevät kuitenkin arkkitehti Aarno Ruusuvuoren suunnittelemat kirkkorakennukset. Eräs Ruusuvuoren tunnetuimmista töistä on vuonna 1961 rakennettu Hyvinkään kirkko, jota voidaan yhä, yli puoli vuosisataa myöhemmin, pitää merkittävänä esimerkkinä suomalaisen modernismin taipumuksesta abstraktioon. Sen kirkkosali on pyramidin muotoinen suljettu tila, jonka kuorissa yksittäinen korkea ikkuna aukeaa taivasta kohti. Pyramidin muotoiset seinät sisältävät taitettuja betonilaattoja, jotka muodostavat kyntövakomaisia palkkeja. Ruusuvuoren muotoilu osoittaa selkeää rakenteellista ajattelua nivoutuneena voimalliseen askeettiseen runouteen. Myös kaksi muuta Ruusuvuoren suunnittelemaa pyhäkköä, Huutoniemen kirkko vuodelta 1964 sekä Tapiolan kirkko vuodelta 1965, on rakennettu betonista, ja molemmille ovat ominaisia geometristen muotojen puhtaus sekä arkkitehtuurisen ilmaisun koruttomuus. Näiden kahden kirkkosalin suljettua, riisuttua luonnetta korostavat betoniseinien paljaat pinnat sekä harkitusti sijoitetut valoaukot. Kirkkojen arkkitehtuurissa on hengellistä voimaa; molempia on pidetty merkittävinä brutalistisen minimalismin edustajina. Jopa minimalistisista rakennuksistaan kuuluisa japanilaisarkkitehti Tadao Ando on paljastanut, että Ruusuvuoren “hiljaisella” kirkkoarkkitehtuurilla on ollut häneen suuri vaikutus. Minimalismi japanilaisessa arkkitehtuurissa Yksinkertaisuuden ja koruttomuuden ihanteet ovat keskeisiä arvoja myös japanilaisessa kulttuurissa. Tämä kytkeytyy yhtä paljon kungfutselaisiin ja taolaisiin perinteisiin kuin buddhalaisuuteen, joka painottaa mielen, sydämen ja hengen (kokoro) puhtautta. Näiden perinteiden kokonaisuus yhdessä wabi-sabin esteettisten arvojen kanssa on johtanut japanilaisen minimalismin erinomaisiin saavutuksiin. Minimalistinen japanilainen arkkitehtuuri luo esteettisen ilmapiirin, joka voidaan tunnusomaisimmillaan kokea wabi-sabi-tyylisissä teehuoneissa sekä niin kutsutuissa sukiya-tyylin rakennuksissa, joiden inspiraationa on toiminut teehuonearkkitehtuuri. Zenbuddhalaisten temppelien kivipuutarhat puolestaan ilmentävät transsendentaalisen tyhjyyden arkkitehtuuria. Johtava esimerkki transsendentaalista tyhjyyttä ilmentävästä arkkitehtuurista on Ryôan-jin kivipuutarha, joka on rakennettu kiotolaisen buddhalaistemppelin pihamaalle. Sen avara tyhjä tila, joka koostuu seinämillä rajatusta valkeasta hiekasta ja viidestätoista harmaasta kivestä, ilmaisee yksinkertaisuuden, olennaisuuden ja hengellisyyden ihanteita. Kuten muutkin kare sansui-tyypin kivipuutarhat, Ryôan-ji edustaa zenbuddhalaisuuden maailmankuvaa ja kytkeytyy läheisesti metafyysisen tyhjyyden (kû) käsitteeseen. Luostareissa kivipuutarhat ovat hiljaisuudelle ja meditaatiolle omistettuja paikkoja. Meditaation tavoitteena on mielen tyynnyttäminen ja transsendentaalisen tyhjyyden tavoittaminen eli pyhä kûn kokemus. Ilman muuta merkittävin esimerkki wabi-tyylin teehuonearkkitehtuurista on Taian-teehuone, joka rakennettiin 1500-luvulla teemestari Sen no Rikyûn ohjeiden mukaisesti. Teehuone, jonka sanotaan olevan Japanin vanhin, sijaitsi alun perin Hideyoshin linnassa Yamazakissa, mutta se on sittemmin joko siirretty tai uudelleenrakennettu Myokian-temppeliin Kiotoon, missä se yhä seisoo. Taian-teehuone on pikkuruinen, rustiikkinen vaja, jota kutsutaan nimellä sôan eli “ruohoinen suojapaikka”. Sen sisätila on vain kahden tatamin suuruinen, yksi tatami isännälle ja toinen vieraalle, ja sisältää yksinkertaisen tulisijan veden lämmittämiseen. Lisäksi se sisältää tokonoman kukkien asettelua varten, odotushuoneen sekä katten eli pienen valmisteluhuoneen. Harvat yksityiskohdat, kuten matala ovi ja muutama pieni ikkuna, joiden punottuja päreitä ei ole verhottu, on sijoiteltu harkitusti. Vaikka nämä visuaaliset tehosteet sisältävät esteettisiä piirteitä, ilmapiiri on tarkoituksellisen vaatimaton ja himmeä. Kuten Kakuzo Okakura on kuvaillut, “[j]opa päiväsaikaan huoneen valaistus on vaimeaa, koska viiston katon matalat räystäät päästävät sisään vain harvoja auringonsäteitä. Kaikki katosta lattiaan on sävyltään hillittyä.” Sen lisäksi, että kivipuutarhat ja perinteinen teehuonearkkitehtuuri edustavat minimalismia, myös nykyajan japanilainen arkkitehtuuri sisältää autenttisen minimalistisen suuntauksen. Moderni minimalismi käsitetään yleensä osana jatkumoa aiemmin kuvailtujen perinteisten filosofisten ja esteettisten arvojen kanssa. Näin ollen japanilaisen minimalismin tyylillisen perustan muodostavat perinteiset kauneuden esteettiset kategoriat. Kuten aiemmin mainittiin, Haga Kôshirôn kolminkertainen jaottelu yksinkertaiseen, teeskentelemättömään kauneuteen, epätäydelliseen, virheelliseen kauneuteen sekä koruttomaan, karuun kauneuteen on minimalistisen estetiikan klassinen kategorisaatio, joka vallitsee myös nykyaikaisessa arkkitehtuurissa. Lisäksi Hisamatsu Shin’ichin luettelemat seitsemän esteettistä piirrettä - virheellisyys, yksinkertaisuus, koruttomuus, luonnollisuus, mystisyys, katoavaisuus ja levollisuus - tarjoavat sarjan tyylillisiä kriteerejä minimalistiselle muotoilulle. Vaikka nykyaikaista japanilaista arkkitehtuuria ei voida millään muotoa pitää historisistisena, historiallisesti määritellyt wabi-sabi-arvot muodostavat kuitenkin minimalistisen arkkitehtuurin filosofiset ja esteettiset suuntaviivat myös tämän päivän Japanissa. Arkkitehti Tadao Andon rakennukset ilmentävät perinteisen japanilaisen kulttuurin henkeä viittaamalla selkeän arkkityyppiseen lähestymistapaan. Andon muotoilu juontuu hänen synnynnäisestä kulttuurisesta luonteestaan ja kuvastaa shintolaisuuden, wabin ja man esteettisiä ihanteita. Hänen muotoilusuunnitelmansa ovat yksinkertaisia, hän suosii perustavanlaatuisia rakenteita ja puhdasta geometriaa, käyttää rakennusmateriaaleinaan betonia ja luonnonpuuta, sekä luo koruttomia mutta voimakkaita tiloja hyödyntämällä varjojen ja luonnonvalon vuorovaikutusta. Yhteenkietoutuneet sisä- ja ulkotilat on taitavasti järjestetty luonnon ja tyhjyyden tuntemisen mukaisesti. Vaikuttava esimerkki Tadao Andon viimeaikaisesta minimalistisesta muotoilusta on korealais-japanilaisen taiteilija Lee Ufanin töille omistettu taidemuseo. Naoshiman saarelle vuonna 2010 rakennettu museo muodostaa ainutlaatuisen kokonaisuuden, josssa Ufanin minimalistiset veistokset ja maalaukset käyvät intiimiä vuoropuhelua Andon arkkitehtuurisen lähestymistavan kanssa. Lopputulos on ainutlaatuinen totaalinen taideteos, hengellinen Gesamtkunstwerk. Minimalismin estetiikka tulee lähes käsin kosketeltavaksi museon tiloissa, jotka intiimiydestään huolimatta ilmaisevat universaalia metafyysistä tyhjyyttä. Erästä toista japanilaista minimalistiarkkitehtiä, Kazuyo Sejimaa, puolestaan arvostetaan hänen hienotunteisen feminiinisen lähestymistapansa ansiosta. Hänen teoksensa poikkeavat Andon jokseenkin maskuliinisesta arkkitehtuurista. Myös Sejima suosii puhtaita geometrisiä muotoja, mutta hänen suunnitelmansa ovat värikkäämpiä, ja hän käyttää enemmän lasia ja muita nykyaikaisia materiaaleja. Esimerkki hänen minimalistisesta arkkitehtuuristaan on Kumanon vuoristoisessa ympäristössä sijaitseva eteerinen N-Museo (1997). Sille tyypillisiä ovat lukuisista läpinäkyvistä kerroksista muodostetut tilat. Näin ollen Sejiman arkkitehtuurinen lähestymistapa viittaa yhtä lailla fenomenologisiin kuin minimalistisiin käsitteisiin ilmaisten silti samalla syviä japanilaisen mielen ja kulttuurin arvoja. ARKKITEHTUURI JA TRANSSENDENSSI Sekä suomalainen että japanilainen kulttuuri painottavat hiljaisuuden arvoa. Suomessa tämä on kokonaisvaltainen kulttuurinen asenne, joka vaikuttaa synnynnäisesti suomalaiseen identiteettiin. Japanissa, jonka kulttuurin kehitykseen buddhalaisuus on vaikuttanut perustavanlaatuisesti, mielen hallinnan tärkeyttä on aina korostettu voimakkaasti. On olemassa Buddhan sananparsi: “Hiljaisten paikkojen pyhyydessä, tietoisen tarkkaavaisuuden avulla, voi saavuttaa sydämen (kokoro) vapautumisen.” Kun hiljaisuus ymmärretään hengellisellä tasolla, korkeampi todellisuus voidaan kokea ainoastaan vaientamalla mieli. Janoamme transsendentaalista kokemusta, joka vapauttaa meidän maallisen mielemme rajoituksista ja antaa meidän koskettaa Ykseyden tai harmonisen yhtenäisyyden maailmaa. Samalla tavalla jokaisen tarpeettoman piirteen poistaminen ja riisuminen taideteoksesta jättää jäljelle vain oleellisen. Japanilainen filosofi Kitarô Nishida on kuvaillut oleellisuutta - tai olemisen syvintä olemusta - buddhalaisten käsitteiden avulla. Nishidalle kaikki olemassaolon muodot, muoto ja tila, ovat vain tahroja tyhjyyden (mu) pinnalla. Kristillisessä filosofiassa buddhalaista tyhjyyden käsitettä vastaisi - hiljaisuus. Meister Eckhardt on todennut: “Hiljaisuudessa Jumala antaa ikuisen sanansa laskeutua sieluun.” Miten voisimme kuvailla hiljaisuutta? Sveitsiläinen filosofi Max Picard on kirjoittanut hiljaisuutta käsittelevän kirjan nimeltä Die Welt des Schweigens (Hiljaisuuden maailma, 1948). Picardin sanoin hiljaisuus on “autonominen ilmiö”, ja “(t)ämän autonomisen olemisen kautta hiljaisuus osoittaa tilaan, jossa mikään muu ei ole voimassaolevaa kuin oleminen itsessään: jumalaiseen tilaan. Yhteys hiljaisuuden maailmaan varjelee jumalaisuuden merkkiä kaikessa olevassa.” Todellinen olemus, todellinen arkkitehtuuri, mikä tahansa todellinen, syntyy siis hiljaisuudesta - tai, buddhalaisen näkökulman mukaan, ikuisesta “olemattomuudesta”. Louis Kahn on kuvaillut tätä seuraavasti: “Taiteena tilasta tehdään aavistus ikuisuutta.” Aavistus ikuisuutta viittaa Picardin näkemykseen siitä, että todellisen taiteen ominaisuus on sen kyky yhdistää meidän transsendentaaliseen hiljaisuuden maailmaan. Autenttinen hiljaisuuden kokemus tarjoaa meille transsendentaalisen ymmärryksen arkisen maailmamme itsepiirretyistä rajoista ja, samaa logiikkaa seuraten, siitä mikä on todellista. Juuri tätä valaisevaa ymmärrystä me janoamme, yhtä lailla arkkitehtuurissa ja taiteessa. Transsendenssi ja totuus ovat kysymyksiä, joita myös taiteen filosofia käsittelee. Esimerkiksi korealaissyntyinen filosofi ja taiteilija Lee Ufan sisällyttää transsendenssin jo taiteen määritelmään itsessään. Saksalaisfilosofi Martin Heideggerin mukaan taiteen syvin olemus on runous, ja runous on “totuuden tapahtuminen” tai toimeenpano . Totuuden merkitys on kuitenkin laaja, ja Heideggerille se tarkoittaa “maailman avautumista”. Vaikka maailmat taiteessa tulevat ja menevät, Heideggerin määritelmä maailmalle on vastineeksi totuus, tai “olevaisten totuus” tai “olevaisten olemisen totuus”. On olemassa arkkitehtuurisia teoksia, joilla on kokemusperäisen todellisuuden ominaisuus. Wabi-filosofia on esimerkki tämän tavoittelusta, ja monet japanilaiset teehuoneet tai zen-puutarhat todistavat synnynnäistä kykyään avata meille transsendentaalisen kokemuksen valtapiiri. Tällöin wabin merkitys kirkastuu ylittäen aineellisen vaatimattomuuden ja muuttuen kokemusperäiseksi hengelliseksi vapaudeksi, ja zen-puutarhat kutsuvat meitä kokemaan tyhjyyden hiljaisuudessa. Minimalistisessa arkkitehtuurissa, niin Suomessa kuin Japanissakin, rakennuksen muotoiluelementit pyrkivät ilmaisemaan hiljaisuutta ja yksinkertaisuutta. Puhtaat geometriset muodot, rakennusmateriaalien “rehellinen” hyödyntäminen, kaikkien tarpeettomien piirteiden poissaolo, arkkityyppiset spatiaaliset järjestelmät, sisä- ja ulkopuolen harmoninen vuorovaikutus sekä varjon ja valon hienovarainen leikki ilmentävät esteettisen tietoisuuden tilaa, joka on “älyn tuolla puolen”. Tällöin hiljaisuuden ja yksinkertaisuuden transsendentaaliset ominaispiirteet eivät ole vain esteettisiä arvoja vaan myös ilmentävät transsendentaalista todellisuuden luonteen ymmärtämistä ja paljastavat arkkitehtuurin sisäisen olemuksen puhtaimmillaan. Minimalismin tavoite moraalisena ja esteettisenä tietoisuutena on ilmentää hengellistä vapautta, jolla on lopulta korkeampi merkitys. Lähdeluettelo (...)

Source text - Finnish 1 JOHDANTO Silmänpohjan ikärappeuma eli makuladegeneraatio on Suomessa ja länsimaissa yleisin ikääntyneiden näkövammaisuuden aiheuttaja. Silmänpohjan ikärappeumaa esiintyy pää sääntöisesti yli 65 vuotta täyttäneiden keskuudessa. Suomessa arviolta noin 100 000 hen kilön epäillään sairastavan silmänpohjan ikärappeumaa. Noin tuhannen ihmisen näkö vammautuu vuosittain ikärappeuman takia (Kinnunen & Kaamiranta 2014, 2265). Suurimmalla osalla ikärappeumapotilaista on sairauden niin sanottu kuiva muoto, johon ei toistaiseksi ole tehokasta hoitoa. Sen sijaan silmänpohjan kostean ikärappeuman ete nemistä voidaan nykyisin tehokkaasti hidastaa silmän sisään pistettävillä silmäpistoksilla. Silmäpistoksessa silmän sisään ruiskutetaan ohuen neulan avulla lääkeainetta, eli VEGF estäjiä (VEGF = vascular endothelial growth factor). Nämä pistokset eivät kuitenkaan paranna sairautta, vaan pistoksia tulee pistää hoidon alettua säännöllisesti, jotta sairauden eteneminen saadaan hidastettua. Tarvittavien pistosten määrä ja pistosväli vaihtelevat yk silöllisesti, mutta kosteaa ikärappeumaa sairastava potilas saa vuosittain keskimäärin 5- 8 pistosta (Kinnunen & Kaamiranta 2014, 2265). Silmänpohjan kostean ikärappeuman pistoshoitojen määrä on moninkertaistunut viimei sen vuosikymmenen aikana. Jatkuvasti kasvavan hoitotarpeen syynä on se, että uusia po tilaita ilmaantuu vuosittain lisää, mutta vain osa potilaista poistuu hoitojen parista. Vuo sien 2010-2014 aikana silmänpohjan kostean ikärappeuman vuoksi hoidettujen potilai den määrä kasvoi huomattavasti, ja vuosien 2015-2025 aikana potilaiden määrä voi jopa kaksinkertaistua, kun niin sanotut suuret ikäluokat saavuttavat 75 vuoden iän (Kuoppala & Härkönen 2015, 4). Käytännössä silmänpohjan kosteaan ikärappeumaan pistoshoitoa tarvitsevien määrän kasvu tarkoittaa sitä, että silmänpohjan ikärappeuman hoidon pitää muuttua tehokkaam maksi silmäklinikoiden lisääntyvän kuormituksen vuoksi. Silmäklinikoiden pitää jo nyt toiminnassaan ottaa huomioon kasvavat potilasmäärät, mutta samalla taata potilaiden saa man hoidon laatu sekä sen turvallisuus. Kaikki tämä pitää toteuttaa rajallisin resurssein sekä tehokkaasti (Kinnunen & Kaamiranta 2014, 2233). 6 Tässä opinnäytetyössä tarkastellaan silmäpistoshoitoon liittyviä käytäntöjä Suomessa. On tiedossa, että silmäpistoksiin liittyvät käytännöt oletettavasti vaihtelevat silmäklinikoit tain ja jokaisessa silmäklinikassa käytettävät menetelmät, työvaiheet sekä silmäpistosvä lineet ovat erilaiset, eikä yhteistä toimintamallia silmäpistoshoitoon ole luotu. Yllä mai nitut seikat ovat olleet syinä tämän opinnäytetyön tekemiseen ja syntymiseen, jotta ky seessä olevat asiat voidaan todentaa myös tutkimuksen avulla. 7 2 TARKOITUS, ONGELMAT JA TAVOITE Tämän opinnäytetyön tarkoituksena on kartoittaa kostean silmänpohjan ikärappeuman pistoshoitokäytäntöjä Suomessa. Opinnäytetyön ongelmat ovat: 1. Miten silmäpistoshoitoa toteutetaan eri silmäklinikoilla Suomessa? 2. Mitkä ovat silmäpistohoidon vaiheet ja menetelmät? 3. Millä välineillä silmäpistoshoitoa toteutetaan? Tämän opinnäytetyön tavoitteena on tuottaa tietoa silmäpistoshoitoa toteuttaville tervey denhuollon ammattilaisille siitä, mitä hoitomenetelmiä, työvaiheita ja työvälineitä suo malaiset silmäklinikat käyttävät suorittaessaan silmäpistoshoitoa. Kartoituksen perus teella opinnäytetyön toimeksiantaja, logen Oy, kehittää tuottamiaan palveluja sekä silmä pistoksiin liittyviä työvälineitä vastaamaan paremmin suomalaisten silmäklinikoiden tar vetta. 8 3 SILMÄNPOHJAN IKÄRAPPEUMA Silmänpohjan ikärappeuma tarkoittaa ikääntymiseen liittyvää silmänpohjan tarkan näke misen alueen rappeumaa. Silmänpohjan ikärappeumaa on olemassa kahta erilaista muo toa; kuivaa sekä kosteaa. Hitaasti etenevään kuivaan rappeumaan ei ole olemassa teho kasta hoitoa, mutta kosteaan rappeumaan puolestaan on olemassa sairauden etenemistä hidastavia hoitomuotoja. Kostean rappeuman hoidossa on ensisijaisen tärkeää taudin riit tävän varhainen toteaminenja hoitojen aloittaminen riittävän ajoissa (Summanen & Seit sonen 2016). Kostea ikärappeuma on kuivaa ikärappeumaa huomattavasti harvinaisempi, sillä arviolta 90 % kaikista ikärappeumista ovat kuivia ikärappeumia. Kostea ikärappeuma vastaa kui tenkin noin 90 % ikärappeumien aiheuttamista näkövammaisuuksista (Saari 2011, 252). Näkövammainen on sellainen ihminen, jonka näkökyky on heikentynyt niin paljon, että siitä aiheutuu huomattavaa haittaa kyseiselle henkilölle tämän jokapäiväisessä elämässä. Näkövammaiset jaetaan vamman vaikeusasteen mukaisesti heikkonäköisiin ja sokeisiin (Hietanen, Hiltunen & Him 2005, 142). Silmänpohjan ikärappeuma on kehittyessään invalidisoiva sairaus, joka johtaa etenkin hoitamattomana näkökyvyn heikkenemiseen ja näkövammaisuuteen. lkärappeumaa voi daankin elämänlaadullisesti mitattuna verrata muihin kroonisiin sairauksiin, kuten esi merkiksi nivel- ja tulehdussairauksiin tai sydän- ja verisuonitauteihin. Ikärappeumasta aiheutuvien vuosikustannusten arvioidaan olevan globaalisti noin 350 miljardia Yhdys valtain dollaria (Kivinen, Koskela, Kauppinen, & Kaamiranta 2017, 641). 3.1 Yleisyys Silmänpohjan ikärappeuma on yleisin näkövammaisuuteen johtava silmäsairaus Suo messa sekä muissa länsimaissa. Silmänpohjan ikärappeumaa esiintyy 65-74-vuotiaiden ikäluokassa noin 10-15 prosentilla, 75-84-vuotiaista noin 25 prosentilla, ja 85 vuotta täyttäneistä noin 30 prosentilla koko väestöstä. Suomessa arviolta noin 100 000 henkilön 9 epäillään sairastavan silmänpohjan ikärappeumaa. Noin tuhannen suomalaisen näkö vam mautuu vuosittain ikärappeuman takia (Kinnunen & Kaamiranta 2014, 2265). Elinajan pitenemisen ja ikääntyneiden lukumäärän lisääntymisen vuoksi silmänpohjan ikärappeuman esiintyvyys tulee moninkertaistumaan tulevien vuosien aikana. Tämä puo lestaan aiheuttaa terveydenhuollolle huomattavia lisäkuluja ja johtaa silmäklinikoiden työkuorman lisääntymiseen (Kivinen, Koskela, Kauppinen, & Kaamiranta 2017, 641). 3.2 Oireet Tyypillisiä silmänpohjan ikärappeuman oireita ovat viivojen vääristyminen ja näön tark kuuden aleneminen. lkärappeuman oireisiin saattaa kuulua myös selkeät mustat puutos alueet näkökentässä tai reiän omaiset valkoiset läiskät näkökentässä. Usein myös tekstin lukeminen vaikeutuu, koska osa kirjaimista katoaa näköalueen puutosten vuoksi. Sairau den oireet ovat samat sekä kuivassa että kosteassa ikärappeumassa (Seppänen 2013b). Varsinkin ikärappeuman kosteassa muodossa oireet etenevät ja ilmaantuvat kuitenkin yleensä erittäin nopeasti. Näöntarkkuus heikkenee tällöin yleensä jopa muutamissa päi vissä ja vaikuttaa sekä kauko- että lähinäköön, jonka lisäksi myös värinäössä saattaa esiintyä muutoksia (Kostea silmänpohjan ikärappeuma (AMD): Käypä hoito -suositus 2016). Kosteasta ikärappeumasta kärsivällä potilaalla on heikentynyt tarkka keskeinen näkö, joka voi ilmetä esimerkiksi lukemisen ja tuttujen henkilöiden tunnistamisen vaikeutena. Sairaus voi edetessään kehittyä molempiin silmiin, mutta se voi ilmaantua toisessa sil mässä vasta vuosia myöhemmin (Saari 2011, 253). 3.3 Syyt ja riskitekijät Ikärappeuman syntymisen ylivoimaisesti merkittävin aiheuttaja on ikääntyminen. Tämän lisäksi tupakointi suurentaa riskiä sairastua silmänpohjan ikärappeumaan. Tupakoinnin lopettaminen mitä ilmeisemmin pienentää riskiä sairastua, mutta riski sairastumiseen h 10 saattaa säilyä korkeana jopa 20 vuotta tupakoinnin lopettamisen jälkeenkin. Myös perin tötekijöillä on vaikutusta tautiin sairastumiseen varsinkin, mikäli lähisuvussa on aiemmin esiintynyt ikärappeumaa. Perimän osuus taudin ilmaantuvuudesta on jopa 60% (Kostea silmänpohjan ikärappeuma (AMD): Käypä hoito -suositus 2016). Aiemmin mainittujen syiden lisäksi sairastumisen riskiä lisäävät elintapoihin liittyvät te kijät, kuten liikalihavuus, runsas tyydyttyneiden rasvojen saanti ruokavaliosta, vähäinen hedelmien ja vihannesten syöminen sekä useat eri verisuonitaudit (Seppänen 2013a). Myös valkoisen rodun sekä naissukupuolen on arvioitu vaikuttavan sairauden syntyyn (Iivanainen, Jauhiainen & Syväoja 2010, 142). 3.4 Hoito Suurimmalla osalla eli noin 80-90 % ikärappeumapotilaista on sairauden kuiva muoto, johon ei toistaiseksi ole tehokasta hoitoa. Sen sijaan kostean ikärappeuman etenemistä voidaan tehokkaasti estää pistoshoidoilla. Pistoshoidossa silmän sisälle lasiaiseen piste tään verisuonikasvutekijöiden estäjiä, eli VEGF-estäjiä (VEGF = vascular endothelial growth factor). Kosteassa ikärappeumassa suonikalvostosta muodostuu uusia verisuonia, jotka tunkeutuvat verkkokalvolle ja vuotavat helposti terävän näön alueelle. Tämä vuoto aiheuttaa silmänpohjaan turvotusta, joka johtaa näkökyvyn heikentymiseen. Verisuoni kasvutekijöiden estäjät nimensä mukaisesti estävät uusien verisuonien kasvua ja näin ol len pyrkivät hidastamaan taudin etenemistä ja näkökyvyn heikentymistä (Kinnunen & Kaamiranta 2014, 2265). VEGF -estäjähoito tulee aloittaa välittömästi tai viimeistään lähipäivien aikana taudin diagnosoinnin jälkeen, jotta taudin eteneminen saadaan nopeasti hidastettua tai kokonaan estettyä. Injektiohoito toteutetaan kaikille potilaille yleensä kolmen pistoksen sarjoina. Pistokset suoritetaan noin 4-6 viikon välein. Kolmen pistoksen jälkeen suoritetaan seu rantatutkimus, jossa tarkastellaan pistoshoitojen vaikuttavuutta tutkimalla muun muassa silmässä esiintyvän turvotuksen määrää, subretinaalisen (verkkokalvon alapuolisen) nes teen kertymistä sekä näkökyvyn kehittymistä. Mikäli sairaus ei ole kehittynyt huonom paan suuntaan, voidaan hoidossa kokeilla pistostaukoa (Kostea silmänpohjan ikärap peuma (AMD): Käypä hoito -suositus 2016). b 11 Käytännössä varsinainen hoito tapahtuu niin, että ensin potilaan kosteasta ikärappeumasta kärsivän silmän pinta puudutetaan. Tämänjälkeen kyseinen silmä sekä sen ympärys puh distetaan huolellisesti. Seuraavaksi silmän lasiaiseen pistetään ohuen neulan avulla veri suonikasvutekijöiden estäjää. Pistäjänä voi toimia silmätautien erikoislääkäri, silmäsai rauksien hoitotyöhön perehtynyt sairaanhoitaja tai silmätauteihin erikoistuva lääkäri. Varsinainen toimenpide on nopea ja kivuton. Kipulääkitystä ei toimenpiteen jälkeen tar vita (Kostea silmänpohjan ikärappeuma: Käypä hoito -suositus 2017). Näin ollen tarvittavien injektioiden määrä ja pistosvälit vaihtelevat potilaskohtaisesti huo mattavan paljonja kosteaa ikärappeumaa sairastava potilas saakin vuosittain keskimäärin 5-8 injektiota. Hoidon jatkuvuuden sekä sairauden etenemisen seurannan vuoksi kosteaa ikärappeumaa sairastavat henkilöt ovat pysyvässä hoitokontaktissa silmäklinikkaan (Kin nunen & Kaamiranta 2014, 2265). Lääkealan turvallisuus- ja kehittämiskeskus Fimean vuonna 2014 tekemän tutkimuksen mukaan sairaala-apteekit toimittivat yhteensä lähes 54 000 kerta-annosta lasiaisinjekti oita silmäklinikoille. Lasiaisinjektiota käytetään silmänpohjan ikärappeuman hoidon li säksi myös diabeettisen retinopatian hoitoon (Kuoppala & Härkönen 2015, 23). Silmänpohjan kostean ikärappeuman pistoshoitojen määrä on moninkertaistunut viimei sen vuosikymmenen aikana. Jatkuvasti kasvavan hoitotarpeen syynä on se, että uusia po tilaita ilmaantuu vuosittain lisää, mutta vain osa potilaista poistuu hoitojen parista. Vuo sien 2010-2014 aikana kostean silmänpohjan kostean ikärappeuman vuoksi hoidettujen potilaiden määrä on kasvanut huomattavasti, ja seuraavan kymmenen vuoden aikana po tilaiden määrä voi jopa kaksinkertaistua, kun niin sanotut suuret ikäluokat saavuttavat 75 vuoden iän (Kuoppala & Härkönen 2015, 4). Muita, huomattavasti harvinaisempia, hoitomuotoja ovat fotodynaaminen hoito sekä suora laserhoito ja näiden kahden hoidon yhdistelmät. Fotodynaamisessa hoidossa laski moon ruiskutetaan ensin valolle herkistävää ainetta, joka kiinnittyy silmänpohjan uudis verisuoniin. Tämän jälkeen silmänpohjaan kohdistetaan laservaloa, joka tuhoaa verisuo net ja estää siten rappeuman pahentumista. Hoito tulee kuitenkin toistaa muutaman kuu kauden välein parin vuoden aikana (Hannuksela 2012; Salmi 2016, 8). 12 Suora laserhoito oli aiemmin ainoa silmänpohjan kosteaan ikärappeumaan saatavilla ollut hoitomuoto. Suoraa laserhoitoa voidaan antaa vain osalle potilaista, joilla silmien uudis verisuonitus sijaitsee riittävän kaukana tarkan silmän näköalueen keskeisimmästä osasta. Suoran laserhoidon teho ei yllä samalle tasolle kuin muiden silmänpohjan kostean ikä rappeumaan käytettyjen hoitojen. Joissain tapauksissa myös niin sanottu yhdistelmähoito voi tulla kyseeseen. Tällöin silmänpohjan kostean ikärappeuman hoidossa voidaan yhdis tää esimerkiksi fotodynaamista hoitoa sekä kortisoni-injektiohoitoa tai vaihtoehtoisesti voidaan yhdistää fotodynaaminen hoito sekä VEGF:n -estäjähoito erillisinä hoitokertoina (Seppänen 2013b). 3.5 Silmän anatomia ja pistoskohta Silmäpistos, joka sisältää verisuonikasvutekijän estäjää (VEGF-estäjää), injisoidaan la siaiseen silmän sivusta intravitreaalisesti (Saari 2011, 255). (Kuva 1) sllmätlpassa oleva lääkealne etukammio lntravitreaallnen suonikalvo mykiön ripustin säikeet KUVA 1. Silmän anatomia sekä silmälääkkeiden annostelukohdat (Mäenpää 2017) 13 Intravitreaalisesti lasiaiseen annosteltu lääkeaine jakaantuu nopeasti lasiaistilaan ja siir tyy siitä vähitellen verkkokalvon sisempiin kerroksiin. Osa lasiaiseen ruiskutettavasta lää keaineesta pääsee verkkokalvolle vaikutuskohtaansa, kun taas osa lääkeaineesta poistuu silmästä suonikalvoston oman verenkierron kautta (Mäenpää 2017). Silmäpistoksien suorittamiseen on olemassa monenlaisia, tekniikaltaan erilaisia apuväli neitä. Esimerkiksi InVitria -injektiotyökalu helpottaa intravitreaalisten injektioiden anta mista. Se mahdollistaa kiinteän ja oikeaoppisen asennon injektiokulmalle sekä pistoksen syvyydelle. InVitria vähentää muun muassa potilaan tai pistäjän liikkeistä johtuvia pis toksen aikaisia riskejä. InVitriaa käytettäessä toimenpiteessä ei pääsääntöisesti tarvita muita työkaluja, kuten luomenlevittimiä, mittoja tai silmän paikallaan pitäjänä, eli va kauttajana (InVitria -Intravitreal Injection Assistant n.d.). Toinen käytössä oleva apuväline silmäpistoksien suorittamiseen on SP-Eye -injektiotyö kalu, joka muun muassa kontrolloi silmäpistoksen syvyyttä, injektiokulmaa ja varmistaa lääkkeen toimittamisen turvallisesti haluttuun osaan silmää (SP.eye IVT device n.d.). 3.6 Lääkehoito Silmänpohjan ikärappeuman etenemisen estämiseen pistoksin annosteltavia lääkeaineita on Suomessa käytössä tällä hetkellä kolmea erilaista; bevasitsumabi (kauppanimeltään Avastin), ranibitsumabi (kauppanimeltään Lucentis) sekä aflibersepti (kauppanimeltään Eylea). Bevasitsumabi, ranibitsumabi sekä aflibersepti ovat tämän hetkisen tiedon mu kaan yhtä tehokkaita ylläpitämään näöntarkkuutta kostean silmänpohjan rappeumassa. Lääkeaineiden osalta ei ole myöskään huomattu kliinisesti merkittäviä eroja turvallisuu den osalta (Kostea silmänpohjan ikärappeuma: Käypä hoito -suositus 2017). Lääkkeiden käytöstä ja hintaeroista on keskusteltu vuosien ajan. Silmänpohjan ikärap peumaan käytettyjen lääkeaineiden hinnoissa on huomattavia eroja. Bevasitsumabi (Avastin) on yleisin, ja sitä on käytetty noin kymmenen vuoden ajan eli pidempään kuin muita lääkkeitä ja se on hinnaltaan myös muita lääkevalmisteita huomattavasti edulli sempi vaihtoehto. Bevasitsumabi on alun perin paksusuoli- ja peräsuolisyövän hoitoon tarkoitettu lääke. Alkuperäisestä käyttötarkoituksestaan huolimatta se kuuluu Suomen 14 julkisesti rahoitetun terveydenhuollon palveluvalikoimaan. Terveydenhuollon palveluva likoimaneuvoston vuonna 2015 antaman suosituksen mukaan Bevasitsumabin käyttö sil mänpohjan kostean ikärappeuman hoidossa on hyväksyttävää ja taloudellisesti järkevää (Soininen 2015; Tuulonen 2014, 2291). Kuopion yliopistollisessa sairaalassa vuonna 2016 bevasitsumabin hinta oli 70 €/injektio, ranibitsumabin hinta 909 €/injektio ja afliberseptin hinta 718 €/injektio. Mainituissa hin taesimerkeissä on siis pelkän lääkeaineinjektion hinta, jotka eivät sisällä silmäpistos hoidosta aiheutuneita muita epäsuoria kustannuksia, kuten sairauden etenemistä seuraa via kuvantamistutkimuksia tai näkövammaisuudesta aiheutuvia kustannuksia. Näistä syistä johtuen on perusteltua käyttää halvinta lääkevalmistetta, jon.ka teho ja turvallisuus eivät poikkea muista kyseiseen sairauteen käytetyistä lääkeaineista (Vottonen 2016, 19- 21). 3.7 Silmäpistoshoidon riskit Palosaaren ym. (2008, 2229-2236) mukaan silmäpistokseen liittyy myös riskejä, kuten kaikkiin kajoaviin toimenpiteisiin. Mahdollisina silmäpistoksen seurauksina voi ilmetä esimerkiksi silmänsisäinen tulehdus (endoftalmiitti) sekä suurentunut riski verkkokalvon irtaumalle. Suurentuneen tulehdusriskin vuoksi ennen toimenpidettä molempiin silmiin tiputetaan yleensä antibioottitippoja silmälääkärin ohjeiden mukaisesti. Toimenpiteenjäl keen antibioottitippoja usein laitetaan siihen silmään, johon lasiaisinjektio on annettu. Silmään kohdistuvat, injektioon liittyvät, vakavat haittavaikutukset ovat harvinaisia, mut ta voivat aiheuttaa pahimmillaanjopa näkökyvyn täydellisen romahtamisen. Silmäpistok sista johtuvien haittavaikutusten vähentämiseksi huolellinen aseptiikka pistostapahtu massa on ensiarvoisen tärkeää (Palosaari ym. 2008, 2233). 3.8 Endoftalmiitti Endoftalmiitti on bakteerin tai sienen aiheuttama silmän sisäosien tulehdus, joka voi ai heutua esimerkiksi aiemmin tehdyn silmänsisäisen toimenpiteen yhteydessä. Endoftal miitin oireisiin kuuluu muun muassa voimakas luomiturvotus, silmän verestys, silmän 15 kipu sekä näön varsin nopea heikkeneminen. Tämän lisäksi silmä yleensä kipuilee voi makkaasti ja näkökyky heikkenee erittäin nopeasti. Endoftalmiitti johtaa hoitamattomana sokeutumiseen (Summanen & Setälä 2016). Jotta endoftalmiitista kärsivän potilaan näkökyky pystytään pelastamaan, tulee hänet lä hettää välittömästi silmätauteja hoitavaan yksikköön, jotta silmän hoito voidaan aloittaa. Endoftalmiittia hoidetaan sairaalassa paikallisesti vaikuttavien mikrobilääkkeiden avulla (Summanen & Setälä 2016). Yhdysvalloissa tehdyn meta-analyysin mukaan yksittäiseen silmänpohjan ikärappeuman pistokseen liittyvä endoftalmiitin riski on 0,056 %. Näin ollen 10 000 pistoskertaan liittyy noin 5 endoftalmiittia (Fileta, Scott & Flynn 2014). 3.9 Potilaan ohjaus ja omaseuranta Ikärappeumapotilaan näöntarkkuuden säännöllinen tutkiminen on tärkeää, sillä kosteassa rappeumassa myös toinen, aiemmin oireeton silmä, voi sairastua viiden vuoden kuluessa. Potilaiden kuntoutuksessa apuvälineet, kuten esimerkiksi suurennuslasin käyttö sekä ko din riittävästä valaistuksesta huolehtiminen on tärkeää. Usein henkilösuhteissa tapahtuva kommunikaation vaikeutuminen aiheuttaa myös kuormitusta potilaalle. Heikentyneen näön vuoksi myös mahdollinen kotiavun tarve tulee tarvittaessa terveydenhuollon toi mesta selvittääjajärjestää (Mustajoki ym. 2013, 753). Omaseurannalla on tärkeä osuus taudin etenemisen tarkkailussa. Mikäli potilaalla on to dettu kuivaa rappeumaa, seurataan tilannetta aktiivisesti, jotta mahdollisten kosteaan rap peumaan viittaavat muutokset näössä voidaan todeta. Tällöin potilaita ohjataan käyttä mään niin sanottua Amslerin karttaa, joka on pieni ruudukko, jossa on piste keskellä (Kuva 2). Mikäli ruudukkoa katsoessa viivat vääristyvät tai näkökenttään tulee valkoisia tai mustia aukkoja, se voi viitata kosteaan ikärappeumaan (Salmi 2016, 8). F 16 KUVA 2. Amslerin kartta, jossa vasemmalla normaali löydös ja oikealla ikärappeuma potilaan löydös (Näkövammaisten liitto ry, muokattu). 3.10 Silmäpistoshoito ja terveydenhuoltojärjestelmä Silmänpohjan kostean ikärappeuman pistoshoito on mullistanut hoidettavien potilaiden elämää, mutta samalla myös terveydenhuoltojärjestelmän on seurattava mukana. Vaikka itse pistoshoito on nopea toimenpide, pelkästään hoitoarvioiden määrän vauhdikas lisään tyminen sekä itse hoito edellyttävät kontrollikäyntejä silmäklinikoilla ja uusintahoitoja noin 1-2 kuukauden välein. Nämä tekijät kuormittavat terveydenhuoltojärjestelmää ja nostavat kustannuksia huomattavasti. Silmäterveydenhuollon onkin ratkaistava haaste, miten hoitoprosessit järjestetään sekä vaikuttaviksi että kustannustehokkaiksi (Tuulonen 2008). Kostean ikärappeuman silmäpistoshoidon huomattavasta lisääntymisestä kertoo esimer kiksi Tampereen yliopistollisen sairaalan Silmäkeskuksessa ikärappeuman silmäpistosten määrän 9-kertaistuminen vuosien 2008-2013 aikana. Tästä huolimatta potilaille on kyetty antamaan heidän tarvitsemansa hoito kehittämällä uusia toimintatapoja ja käyttämällä edullista lääkevaihtoehtoa (Tuulonen 2014, 2290-2295). Tuulonen (2014, 2293-2294) toteaakin artikkelissaan, että silmäsairauksien hoito sekä niihin käytettävät voimavarat ovatkin tarkoituksenmukaista sovittaa yhteen sellaisilla me nettelytavoilla, jotka luovat tasapuolisen hoidon kaikille hoidosta hyötyville potilaille. Ikärappeuman hoidon kehittymisen suuntaa hoidollisesti tai taloudellisesti tulevaisuu dessa on kuitenkin vaikeaa ennustaa. 17 4 MENETELMÄLLINEN LÄHTÖKOHTA 4.1 Tutkimusmenetelmä Tämä opinnäytetyö on laadultaan kvantitatiivinen eli määrällinen. Määrällistä tutkimusta voidaan nimittää myös tilastolliseksi tutkimukseksi. Tämän laatuisissa tutkimuksissa py ritään selvittämään prosenttiosuuksiinja lukumääriin liittyviä kysymyksiä. Kvantitatiivi sessa tutkimuksessa aineiston keruu tapahtuu yleensä käyttäen standardoitua tutkimuslo maketta, jossa on valmiit vastausvaihtoehdot eri kysymyksiin. Tuloksia havainnoidaan yleensä erilaisten kuvioiden ja taulukoiden avulla. Kvantitatiivisen tutkimuksen avulla saadaan yleensä hyvin kartoitettua vallitseva tilanne tutkittavasta aiheesta, mutta ei pys tytä selvittämään syitä tilanteen taustalla (Heikkilä 2010, 16). Kvantitatiivinen tutkimus sopii käytettäväksi sellaisessa tilanteessa, jolloin tavoitteena on saada tutkittavasta ilmiöstä perusteltua, yleistettävää ja luotettavaa tietoa. Kvantitatiivi nen tutkimus pyrkii aina yleistämään. Yksinkertaisimmillaan tämä tarkoittaa sitä, että melko pieneltä joukolta havaintoyksiköitä kerätään tutkimustietoa. Tällöin kvantitatiivi sena mittauksen menetelmänä toimii erilaiset kysymykset, jotka tässä tapauksessa muo dostavat tiedonkeruumenetelmän (Kananen 2011, 17-19). Kyselylomake on yleisin tutkimusaineiston tiedonkeruutapa kvantitatiivisessa tutkimuk sessa, mutta tiedonkeruussaon mahdollista käyttää myös esimerkiksi puhelinhaastatteluja tai havainnointia. Aineiston keräämisen jälkeen se analysoidaan erilaisia tilastointiohjel mistoja käyttäen ja tuloksiksi saadaan usein esimerkiksi prosenttilukuja, jotka voidaan havainnollistaa kuvioinja taulukoin (Vilkka 2005, 74-77). Tämä opinnäytetyö on laadul taan kvantitatiivinen, koska kohdejoukko on melko laaja sekä hajallaan ja tarkoituksena on saada mahdollisimman kattava kartoitus suomalaisista silmäpistoshoitokäytännöistä. Silmäpistoshoidosta on olemassa paljon tietoa entuudestaan, mutta tieto on pääasiassa kertynyt otantaryhmälle, eikä sitä juuri ole kirjallisessa muodossa. Tutkimusaineisto kerättiin kyselylomakkeiden avulla. Kyselylomaketutkimuksessa ky selyyn vastaaja lukee itse kirjallisesti esitetyn kysymyksen ja vastaa siihen itsenäisesti. Kyselylomaketutkimus sopii parhaiten suurelle ja hajallaan olevalle joukolle ihmisiä. 18 Kyselylomakkeen etu on, että vastaaja jää halutessaan nimettömäksi ja näin ollen vas taukset voivat olla anonyymejä (Vilkka 2005, 74). Tämän opinnäytetyön tutkimuksessa käytetyssä kyselylomakk:eessa esitetyt kysymykset olivat strukturoituja. Tämä tarkoittaa sitä, että kysymyslomakk:eessa esitetyille kysy myksille oli annettu valmiit vastausvaihtoehdot. Strukturoitujen vastausten käsittely sekä analysointi on lähtökohtaisesti helpompaa kuin avointen kysymysten käsittely (Ka nanen 2011, 30-31). Kyselylomakk:eessa voi olla joko sekamuotoisia kysymyksiä, monivalintakysymyksiä tai avoimia kysymyksiä. Sekamuotoisissa kysymyksissä osa vastausvaihtoehdoista on val miiksi annettu, mutta mukana voi olla myös avoimia kysymyksiä. Sekamuotoinen kysy mys on toimiva, kun epäillään, että kaikk:ia vastausvaihtoehtoja ei varmuudella tiedetä. (Vilkk:a 2005, 85-86). Tässä opinnäytetyössä käytetyssä kyselylomakk:eessa käytettiin se kamuotoisia kysymyksiä. Kyselylomaketutkimuksen haittana on riski vastausprosentin jääminen alhaiseksi. Vas taajia voidaan muistuttaa kyselytutkimukseen osallistumisesta, mutta tämä aiheuttaa vii veitä tutkimuksen aikatauluun (Vilkk:a 2005, 74). Tässä opinnäytetyössä käytetyn kyse lyn vastausprosentti oli 55 %. Kyselylomake tulee aina testata ennen kuin sitä käytetään tutkimuksen välineenä. Testaa misessa keskitytään muun muassa kysymysten selkeyteenja yksiselitteisyyteen, vastaus vaihtoehtojen toimivuuteen sekä kyselylomakk:een pituudenja vastaamisen ajallisen kes ton kohtuullisuuteen. Testauksen yhteydessä arvioidaan myös, onko kyselylomakk:eessa turhia ja epäolennaisia kysymyksiä tai puuttuuko kyselylomakk:eesta jokin olennainen kysymys (Vilkk:a 2005, 88-89). Kyselylomake, jolla tämän opinnäytetyön kartoitus tehtiin, oli internetpohjainen lo make, jossa oli yhteensä 17 strukturoitua kysymystä silmäpistoshoitoon liittyen. Tämän lisäksi lomakk:eessa oli kaksi avointa kohtaa, joista toiseen vastaaja sai halutessaan kir joittaa huomautuksiaan kyselyyn liittyenja toiseen puolestaan lisätä omat yhteystietonsa (ks. Liite 1). 19 Kysely lähetettiin suomalaisten silmäklinikoiden silmäpistoksista vastaaville työnteki jöille sähköpostitse. Yhteensä kysely lähetettiin kahdellekymmenelle silmäklinikalle. Vastauksia kyselyyn saatiin yhteensä 11 kappaletta, jolloin vastausprosentiksi muodostui 55 %. 4.2 Aineiston kerääminen Aineiston keruumenetelmänä tässä opinnäytetyössä toimi Typeform-palvelun tarjoama kyselylomake. Typeform on tiedonkeruuseen ja sen jakamiseen kehitetty intemetpohjai nen palvelu, joka soveltuu hvyin esimerkiksi kyselyiden tekemiseen niiden perusteella saatujen tulosten analysoimiseen. Typeform-palvelua voidaan käyttää yleisimmillä se laimilla sekä älypuhelimilla (Typeform, n.d.). Kyselylomake muotoiltiin kevään 2017 aikana, jolloin lomake myös testattiin kahteen otteeseen logen Oy:n henkilökunnalla. Testauksen tarkoituksena oli selvittää muun mu assa kysymysten oikeinkirjoitus sekä niiden selkeys ja ymmärrettävyys. Myös kyselylo makkeen tekniseen toimivuuteen ja siihen, kuinka paljon kyselyyn vastaamiseen kuluu aikaa, kiinnitettiin huomiota. Testauksella saadun tiedon mukaan aikaa lomakkeen täyt tämiseen kului enintään 10 minuuttia. Testauksen jälkeen kyselylomake (liite 1.) lähetettiin sähköpostitse saatekirjeen (liite 2.) kanssa oikealle kohdejoukolle. Saatekirjeen tarkoituksena on lähestyä vastaajaa ja moti voida vastaamaan kyselyyn. Saatekirjeen tehtävänä on myös informoida vastaajaa kyse lyn tärkeydestäja luottamuksellisuudesta (Kananen 2011, 46). Kysely lähetettiin kohdejoukolle toistamiseen 8 vuorokautta ensimmäisen lähetyskerran jälkeen. Tällöin saatekirjeenä oli eri tavalla muotoiltu viesti (liite 3.), mutta itse kysely säilyi samana. Typeform-kyselypalvelulla saadaan luotua automaattisesti intemetkysely, joka voidaan lähettää vastaajille sähköpostilla. Vastaaja voi vastata kyselyyn suoraan sähköpostissaan olevan linkin kautta, jolloin vastaukset tallentuvat automaattisesti Typeform-kyselypal velun tietokantaan. Raportointivaiheessa Typeform-kyselypalvelun analysointiohjelman 20 avulla voidaan tallennetut vastaukset siirtää suoraan tekstinkäsittely- tai taulukkolasken taohjelmaan. Kohdejoukkona toimi yhteensä kahdenkymmenen suomalaisen silmäklinikan silmäpis toksista vastaavat lääkärit tai sairaanhoitajat. logen Oy toimitti kyselylomakkeet saatekir jeineen vastaajille sähköpostitse. 4.3 Aineiston analysointi Typeform-kyselypalvelun avulla tuotetun kyselyn vastaukset tallentuivat automaattisesti palveluntarjoajan serverille. Tulokset tuotiin serveriltä Excel-taulukkolaskentaohjel maan, jon.ka avulla luotiin kuviot saatujen vastausten perusteella. Kuvioissa olevat luku määrät on ilmoitettu prosentteina, koska kvantitatiivisessa tutkimuksessa pyritään yleis tykseen. Tällöin kappalemäärillä ei ole merkitystä, koska kun.kin muuttujan suhteellinen osuus koko aineistosta on oleellista (Kananen 2011, 74). Kuviot ovat nopea tapa esittää kvantitatiivisen tutkimuksen tuloksia. Hyvälle tilastokuvi olle on ominaista muun muassa, että se välittää suuren määrän tietoa suhteellisesti pie nessä tilassa, se ei vääristä tietoihin liittyvää sanomaa sekä se muodostaa kiinteän koko naisuuden muun aineiston kuvauksen kanssa. Hyvät kuviot myös herättävät lukijoiden mielenkiinnonjarohkaisevat lukijoita tekemään vertailuja myös muiden kuvioiden välillä (Heikkilä 2010, 154-155). ,-- 21 5 KYSELYN TULOKSET JA ANALYYSI Tämän osion tulokset perustuvat suomalaisille silmäklinikoille kohdistettuun sähköiseen kartoituskyselyyn. Sähköinen kartoituskysely lähetettiin yhteensä 20 henkilölle. Kartoi tuskyselyyn vastasi 11 sairaanhoitajaa tai lääkäriä (n=l 1),jolloin vastausprosentiksi muo dostui 55 %. Määrällisten kysymysten tulokset on esitetty kuvioin sekä prosentein ja ne on pyöristetty yhden prosentin tarkkuuteen. Tässä osiossa kartoituskyselyn tuloksia on käsitelty samassa järjestyksessä kuin ne oli esitetty kyselylomakkeessa. 5.1 Silmäpistoksen valmistelu Kyselyssä kartoitettiin toimenpiteitä, joita tehtiin potilaalle ennen varsinaista silmäpis tosta. Kyselyssä vastaajat kertoivat oman toimipaikkaansa vakiintuneista toimintata voista. Kysymyksiin ei ollut oikeita tai vääriä vastauksia, vaan vastaukset perustuivat vas taajien omiin toimipaikkakohtaisiin rutiineihin. Suurin osa vastaajista (82 %) kertoo, että potilaalle annetaan silmään, johon silmäpistos suoritetaan, pelkkä puudutustippa ennen silmäpistosta. 18 % vastaajista kertoo, että hei dän toimipaikassaan annetaan sekä puudutustippa että mustuaisia laajentava tippa pistet tävään silmään ennen varsinaista toimenpidettä. Potilaalle laitetaan hiussuoja 82 % toimipaikoista. Luomenlevitintä silrnäpistoksen yhteydessä käyttää 36 % toimipaikoista. Steriiliä haavaliinaa käyttää 36 % toimipaikoista. Potilaan kädet desinfi oidaan 18 % toimipaikoista. Pistoskohta myös merkitään 18 % toimipaikoista. Kysymyk seen sai valita niin monta valrnistelevaa toimenpidettä kuin vastaaja halusi. Vastausvaihtoehtona steriilien käsineiden lisäksi oli myös tehdaspuhtaat käsineet tai ei käsineitä lainkaan. Kysymykseen sai vastata vain yh den vastausvaihtoehdon. Pistosten suorittaminen samalla käynnillä 29 5.3 Pistoksen jälkeen Silmäpistoksen jälkeen antibioottisia tippoja annetaan 45 % toimipaikoista ja antibiootti sia voiteita puolestaan 36 % toimipaikoista. Silmän päälle asetetaan suojakilpi 9 % toi mipaikoista. Silmäpistoksen saanut silmä sidotaan 9 % toimipaikoista (kuvio 13). Silmäpistosten suoritusmäärä 32 6 POHDINTA 6.1 Opinnäytetyön eettisyys Tutkimuksen etiikka on kaiken tieteellisen toiminnan lähtökohta. Hyvä tieteellinen etiikka edellyttää, että tutkimuksen tekijä noudattaa rehellisyyttä, yleistä tarkkuutta ja huolellisuutta tutkimustyössä sekä tutkimustulosten esittämisessä (Vilkka 2005, 30). Tutkimuksessa on lähtökohtaisesti aina kunnioitettava ja varjeltava tutkittavia kohteita sekä toimeksiantajaa. Hyvänä tapana pidetään myös sitä, että opinnäytetyön tekijä kertoo suhteestaan tutkittavaan asiaanja toimeksiantajaan (Vilkka 2005, 32-33). Tämä opinnäy tetyö on tehty yhteistyössä logen Oy:n kanssa. logen Oy on suomalainen sekä samalla suurin silmälääketieteellisten laitteiden ja tarvikkeiden toimittaja Suomessa. logen toi mittaa merkittävän osan suomalaisilla silmäklinikoilla käytettävistä silmäpistoshoitoon käytettävistä tarvikkeista ja välineistä. Kaikessa tutkimustyössä on taattava vastaajien itsemääräämisoikeus. Henkilötietolain (22.4.1999/523) mukaan tunnisteellisten sähköpostitietojen kerääminen on luvanvaraista ja sallittua vain silloin, kun tähän on sähköpostin käyttäjän lupa. Tässä opinnäytetyössä käytettyyn kartoitustutkimukseen osallistuminen on ollut vastaajilta täysin vapaaehtoista. Kyselylomakkeen viimeisessä osiossa kerättiin vapaaehtoisesti sähköpostiosoitteet kyse lyyn vastanneilta henkilöiltä. Sähköpostiosoitteiden keräys perustui siihen, että sähköpos tiosoitteidensa jättäneille lähetettiin tämä opinnäytetyö kokonaisuudessaan luettavaksi, mikäli vastaajat niin halusivat. Vastaajien sähköpostiosoitteita ei luovutettu eteenpäin, eikä niitä käytetty tässä opinnäytetyössä millään tavalla. Kyselylomakkeen avulla kerätyt sähköpostiosoitteet on hävitetty tämän opinnäytetyön valmistumisen jälkeen. Tärkeä osa opinnäytetyön eettisyyttä on suunnitelman laatiminen opinnäytetyötä varten. Suunnitelmassa tulee näkyä muun muassa tutkimuksen tavoite ja tarkoitus, aikataulutus sekä opinnäytetyössä käytettävä menetelmä (Vilkka 2005, 58-59). Opinnäytetyön suun nitelma tehtiin sovittuna ajankohtana, keväällä 2017. Suunnitelmasta kävi ilmi kaikki edellä mainitut tutkimussuunnitelmaan liittyvät vaatimukset. Suunnitelma hyväksytettiin Tampereen ammattikorkeakoulun ohjaavalla opettajalla sekä opinnäytetyön tilaajalla; lo gen Oy:n edustajalla. 33 Tätä opinnäytetyötä tehdessä on kunnioitettu tutkittavaa kohderyhmää, käsitelty tutkitta vaa aineistoa rehellisesti, objektiivisesti ja salassapitovelvollisuutta noudattaen. Opinnäy tetyön kysely saatekirjeineen lähetettiin silmäpistoshoidosta vastaaville sairaanhoitajille tai lääkäreille eri silmäklinikoille. Kyselyyn vastaaminen oli täysin vapaaehtoista. Kyse lyyn sai vastata halutessaan nimettömänä, joka varmisti sen, ettei vastaajia voida vastaa jan niin halutessaan tunnistaa. Kaikki tähän opinnäytetyöhön liittyvä tutkimus kohdistui ainoastaan vastanneiden lääkäreiden ja hoitajien omiin kokemuksiin ja havaintoihin toi mipaikkansa silmäpistoshoidosta. Potilaisiin kohdistuvaa tutkimusta ei tehty. Kyselykaa vakkeet palautuivat opinnäytetyön tekijälle sähköisesti ja näin ollen aineistoa käsitteli vain opinnäytetyön tekijä. Tulosten analysoinnin jälkeen vastaukset on hävitetty asian mukaisesti. 6.2 Luotettavuus Tehdyn kartoituksen luotettavuutta voidaan tarkastella validiteetinja reliabiliteetin näkö kulmista. Validiteetilla tarkoitetaan sitä, että onko tutkimuksessa onnistuttu mittaamaan sitä, mitä on haluttukin mitata. Validiteetti on lähtökohtaisesti hyvä silloin, kun tutkimuk sen kohderyhmäja kysymykset ovat olleet oikeat. Reliabiliteetilla puolestaan tarkoitetaan tulosten pysyvyyttä. Reliabiliteettia voidaan arvioida esimerkiksi mittaamalla tutkimusil miöitä samalla mittarilla eri aineistoissa, ja jos niistä saadut tulokset ovat samansuuntai sia, mittaria voidaan pitää reliaabelina (Kankkunen & Vehviläinen-Julkunen 2010, 152). Tässä kartoituksessa silmäpistoshoidon käytänteiden mittaamiseen laadittiin oma mittari. Metsämuurosen (2006, 58) mukaan itse laaditulla mittarilla toteutettu tutkimus on täysin oman mittarin luotettavuuden veroinen. Mittarin tekemisessä käytettiin ideana kansain välisiä kyselylomaketta, jota on käytetty sellaisenaan muun muassa Yhdysvalloissa, Ka nadassa sekä Israelissa (Green-Simms, Ekdawi & Bakri 2011; Xing, Dorrepaal & Gale 2014; Segal ym. 2016). Tämän lisäksi mittarin laadinnassa käytettiin olemassa olevaa teoriatietoa johon osa kysymyksistä perustui. Tutkimuksen luotettavuutta pohdittaessa tulee arvioida muun muassa seuraavia asioita: mikä on tutkimuksen otoskoko sekä laatu, mikä on tutkimuksen vastausprosentti ja mil laisia mahdollisia mittausvirheitä mittarina käytetyssä kyselylomakkeessa on saattanut ilmetä (Vilkka, 149-150). 34 Toisen työn kunnioittaminen kuuluu hyvään tieteelliseen tapaan. Tämä tarkoittaa sitä, että oman tutkimuksen lisäksi käytettyjä ulkopuolisia lähteitä on käytetty asiallisesti, eli siten, että tutkimustekstissä on mainittuna kuka tai ketkä tiedon alkulähteinä toimivat. (Vilkka 2005, 31-32; Vilkka 2007, 164-165). Lähteet sekä tekstiviitteet on merkitty tässä opin näytetyössä asianmukaisesti ja Tampereen ammattikorkeakoulun kirjallisen raportoinnin ohjeistuksen mukaisesti. Tutkimuksen luotettavuutta pohtiessa yksi keskeisimpiä asioita on tutkijan objektiivisuus, eli puolueettomuus. Tällöin tutkijan omat mielipiteet ja uskomukset eivät saa vaikuttaa tutkimuksen tuloksiin, eivätkä tulokset saa olla riippuvaisia tutkijasta. Huomioitavaa on, että huolellisellekin tutkijalle sattuu virheitä, jotka voivat osaltaan heikentää tulosten luo tettavuutta (Heikkilä 2010, 31). Kyselyyn vastaaminen sähköisesti on varmistanut sen, että tämän opinnäytetyön tekijä ei ole pystyneet johdattelemaan vastaajia kysymyksissä, vaan jokainen vastaaja on tulkinnut ja vastannut kysymyksiin oman arviointinsa mukaisesti. Opinnäytetyön tekijä on ollut sairaanhoitajaopintojensa aikana työharjoittelussa avustamassa silmän kostean ikärap peuman pistoshoidoissa, joka on antanut opinnäytetyön tekijälle tietoa ja kokemuksia siitä, ruiten pistoshoitoa käytännön tasolla toteutetaan. Kysymykset ovat tutkimuksen tiedonkeruumenetelmä, jolloin niistä riippuu tutkimuksen luotettavuus ja laatu. Kysymysten toimivuuteen ja luotettavuuteen vaikuttavat, että vas taaja on ymmärtänyt kysymykset oikein ja että vastaajalla on tarvittava tieto sekä haluk kuus vastata esitettyihin kysymyksiin. Näiden seikkojen lisäksi kysymysten on oltava sel keitä ja helposti ymmärrettäviä (Kananen 2011, 30). Typeform-kyselypalvelu mahdollisti sen, että kyselylomakkeen kysymykset olivat sisäl löltään selkeitä ja ulkonäöltään yksinkertaisia. Kyselylomakkeeseen olisi voinut selke ämmin rajata kysymykset koskemaan tiettyjä osa-alueita. Esimerkiksi aseptiikka pistos hoidon yhteydessä olisi voinut olla täysin oma kysymyskokonaisuutensa. Nyt toteute tussa kyselylomakkeessa aseptiikka oli yhdistettynä useampaan eri kysymykseen. Tämän opinnäytetyön tuloksia tarkastellessa huomattiin, että osa vastauksista oli ristiriidassa kes kenään, joka puolestaan viittaa siihen, että vastaajalla ei ole ollut tarvittavaa tietoa kysy myksestä tai vastaaja on ymmärtänyt kysymyksen väärin. Kyselylomakkeessa oli myös 35 yksi kirjoitusvirhe, joka huomattiin vasta kyselyn lähettämisen jälkeen. Nämä edellä mai nitut asiat heikentävät osaltaan tämän opinnäytetyön luotettavuutta. 6.3 Tulosten tarkastelu Vuonna 2014 silmäpistoksia annettiin yhteensä noin 35 000 kappaletta. Tällöin ennustet tiin, että silmäpistosten määrä kasvaa noin 40% vuodessa. Tällöin yksittäisen injektion kustannus pelkästään lääkeaineen osalta vaihteli 50 euron ja 800 euron välillä (Kinnunen & Kaamiranta 2014, 2267). On siis selvää, että kostean ikärappeuman hoito on taloudellisesti erittäin kuormittavaa toteuttaa. Suuressa osassa silmäklinikoita suoritettiinkin päivittäin 21 silmäpistosta tai enemmän (kuvio 17). Kun yhteen silmäpistokseen kuluu tulosten mukaan aikaa 45 pro sentissa silmäklinikoita noin 10-15 minuuttia (kuvio 16), niin voidaan todeta, että pelkäs tään pistosten suorittamiseen kuluu aikaa vähintään 3h 30min/päivä. Tämän lisäksi tulee huomioida siirtymävaiheet, pistosta valmistelevat työt yksittäisten silmäpistoshoitojen välillä ynnä muut tekijät. Näin ollen voidaan olettaa, että silmäpistoshoito sitoo silmäkli nikoilla työskenteleviä ammattilaisia suuresti. Koska kostean ikärappeuman silmäpistos hoito sitoo jo nyt huomattavan määrän henkilöstöresursseja ja aiemmin viitattujen tutki musten mukaan silmäpistosten määrä tulee kasvamaan tulevaisuudessa lisää, on oletetta vaa, että myös silmäpistoksiin liittyvä kuormitus silmäklinikoiden resursseja kohtaan tu lee kasvamaan samassa suhteessa. Tähän haasteeseen myös silmäklinikoiden on varau duttava. Tulosten mukaan silmäpistoksen suorittaminen potilaalle vaihtelee eri silmäklinikoiden välillä. Esimerkiksi potilaan sekä pistosalueen valmistelussa oli huomattavia eroavai suuksia (kuvio 6). Potilaan pistosalueen aseptiikkaan terveydenhuollon ammattilaisten toimesta oli panostettu suurimmassa osassa silmäklinikoita, mutta potilaan omaan asep tiikkaan käsien desinfioinnin avulla panostettiin vain 18% silmäklinikoita. Potilaalle an nettiin silmäpistoksen jälkeen lähes kaikilla silmäklinikoilla joko antibioottisia voiteita tai antibioottisia tippoja (kuvio 13). Pistettyä silmää ei kuitenkaan suojata tai sen normaa lia käyttöä ei rajoiteta esimerkiksi sitomalla silmä tai asettamalla suojakilpi silmän päälle silmäpistoksen jälkeen. 36 Tulosten mukaan silmäpistoksen pääsääntöisenä suorittajana toimii melko tasaisesti joko lääkäri tai sairaanhoitaja (kuvio 3). Kahden eri Iso-Britannialaisen tutkimuksen mukaan hyvin koulutettu ja kokenut sairaanhoitaja voi suorittaa kosteaan silmänpohjarappeumaan liittyvän silmäpistoksen potilaalle ilman suurentunutta riskiä silmänsisäisestä tulehduk sesta tai muusta pistokseen liittyvästä komplikaatiosta. Tutkimusten perusteella sairaan hoitajien suorittama pistoshoito on myös kustannustehokasta (Simcock ym. 2014; Sama lia, Garland & Squirrell 2016). Tämä opinnäytetyön tulosten mukaan Suomessa silmä pistosten antajina toimii edelleen verrattain paljon lääkäreitä ja sairaanhoitajien määrää kyseisen työn suorittajana voitaisiin lisätä. Tuloksista käy ilmi, että silmäpistoshoitojen yhteydessä aseptiikkaan sekä käsihygieniaan panostetaan vaihtelevasti. Kaikki vastaajat kertoivat, että silmäpistosta suoritettaessa käy tetään steriilejä käsineitä (kuvio 7). Toisaalta potilaan käsihygieniaan puututaan vain har voin ennen silmäpistosta, sillä vain 18% silmäklinikoita potilaat desinfioivat kätensä en nen toimenpidettä. Tartuntatautilain (1227/2016) mukaan toimintayksiköiden on huoleh dittava potilaiden ja henkilökunnan tarkoituksenmukaisesta suojauksesta mahdollisia tarttuvia tauteja vastaan. Tällä perusteella myös potilaiden käsihygieniasta huolehtimista ennen silmäpistosta tulisi korostaa. Tutkitun tiedon (Anttila 2014) mukaan käsihygienia on tehokkaimpia keinoja estää hoitoon liittyviä infektioita ja vastustuskykyisten mikro bien yleistymistä potilailla. Tämä opinnäytetyö on tyyliltään kartoittava tutkimus. Kartoittavan tutkimuksen tavoit teena on etsiä uusia näkökulmia tutkittavaan aiheeseen. Kartoittavan tutkimuksen avulla voidaan tutkia vähemmän tunnettuja asioita ja samalla selvittää tarkastelevan aiheen kir jallisuutta. Kartoittavalla tutkimuksella voidaan näin ollen siis löytää tutkittavasta asiasta keskeisiä malleja, teemojaja tyypittelyjä (Vilkka 2007, 20). Yllä olevaan viitaten voidaan todeta, että osaan tässä opinnäytetyössä käytetystä kyselyn kysymyksistä saatiin yksiselitteisiä vastauksia, joissa ei ollut havaittavissa eroavaisuuksia eri vastaajien välillä. Tällaisista kysymyksistä esimerkkeinä ovat muun muassa kyselyssä esiintyvät kysymykset numero 4 (kuvio 4) sekä kysymys numero 7 (kuvio 7). Nämä vas taukset ovat kuitenkin tärkeitä tiedon saannin kannalta, jotta kokonaiskuva silmän kos tean ikärappeuman pistoshoidon käytännöistä voidaan muodostaa. 37 6.4 Johtopäätökset ja jatkotutkimusehdotukset Suomalaisten silmäklinikoiden käytännöt silmänpohjan kostean ikärappeuman pistoshoi tojen yhteydessä vaihtelevat käytännön tasolla toimipaikoittain. Yhtenäistä ja standardoi tua tapaa toteuttaa silmäpistoshoitoa ei ole luotu. Kostean silmänpohjan ikärappeuman Käypä hoito -suosituksissa (2016) otetaan kantaa pääasiallisesti kyseisen sairauden diag nostiikkaan, lääkehoitoon sekä hoitovasteen seurantaan, mutta varsinaisia käytäntöön ja potilaslähtöisyyteen luotuja ohjeita ei ole tehty. Jokaisella silmäklinikalla on siis oma oh jeistuksensa ja tapansa suorittaa silmäpistoshoitoa. Potilaslähtöisyyden kannalta olisi merkittävää, että silmäpistoshoito olisi samanlaista riippumatta siitä, millä silmäklinikalla silmäpistos suoritetaan. Silmäpistoshoitojen sa mankaltaisuus voisi mahdollistaa myös silmäklinikoiden keskitetyt materiaalihankinnat, joka oletettavasti laskisi myös yksittäiseen silmäpistoshoitoon liittyviä kustannuksia esi merkiksi silmäpistoksen valmisteluun liittyvien materiaalien osalta. Standardoitu silmä pistoshoito olisi yhteiskunnallisesti merkittävää myös silmäpistoksia suorittavan tervey denhuoltohenkilöstön kannalta, sillä tällöin esimerkiksi työpaikasta toiseen siirtyminen eri silmäklinikoiden välillä helpottuisi, koska hoitometodit ovat toimipaikoista riippu matta samanlaiset. Silmäpistoshoitojen yhtenäistäminen vaatii kuitenkin huomattavasti jatkotutkimusta sekä tieteellistä näyttöä silmäpistosten yhteydessä käytettävistä meto deista sekä toimintatavoista. Opinnäytetyön tulosten pohjalta logen Oy voi tarjota asiakkailleen valmiiksi pakattuja toimenpidepakkauksia niille silmäklinikoille, jotka eivät sellaisia vielä käytä. Valmiiksi pakattujen toimenpidepakkausten etuna on se, että jokainen silmäpistos suoritetaan täl löin samanlaisin välinein jokaiselle kyseisen silmäklinikan potilaalle. Tämä luo var muutta sekä jatkuvuutta silmäpistoshoidon toteuttamiseen silmäpistoksen suorittavalle ammattilaiselle ja varmistaa, että jokainen potilas hoidetaan tiettyjen rutiinien mukaisesti. Uusia lääkkeitä sekä hoitomuotoja silmän kostean ikärappeuman hoitoon kehitetään jat kuvasti. Oletettavaa onkin, että nykyisin käytettävissä olevien lääkkeiden tilalle olisi tu lossa tulevaisuudessa uusia, osittain samankaltaisia lääkkeitä. Näissä uusissa lääkkeissä esimerkiksi lääkeaineen vaikutusaika silmässä saattaisi olla nykyisiä lääkkeitä pidempi kestoisempi. Tämä tarkoittaisi käytännössä sitä, että kosteasta ikärappeumasta kärsivien potilaiden silmää ei enää hoidettaisi noin kahden kuukauden välein, vaan pistosaikaväli 38 saattaisi venyä jopa 3-6 kuukauden mittaiseksi (Costa, Nascimento, Teixeira & Silva 2017). Yllä mainittu pistosaikavälin kasvaminen oletettavasti vapauttaisi paljon aika- ja henkilöresursseja silmäklinikoiden muuhun käyttöön. Silmän kostean ikärappeuman pistoshoito Suomessa on lähtökohtaisesti hyvällä pohjalla, silmäklinikoilla esiintyvistä käytännön eroavaisuuksista huolimatta. Suurimmat huolen aiheet liittyvätkin pistoshoitojen määrän mahdolliseen kasvuun sekä siitä johtuvaan mah dolliseen resurssien riittämättömyyteen. Tällöin vähäisten resurssien optimointi onjärke vää ja perusteltua jo pelkästään terveydenhuollossa vallitsevien eettisten tekijöiden sekä terveydenhuoltolaissa (30.12.2010/1326) asetettujen laatu- sekä hoitoon pääsy kriteerei den mukaisesti. Vähäisistä resursseista johtuen onkin syytä kiinnittää huomiota siihen, voisiko silmäpistoshoitoa toteuttaa pääasiassa sairaanhoitajavetoisesti nykyistä useam mm. Tämän opinnäytetyön tulokset ovat samansuuntaisia kuin kansainvälisten tutkimusten tu lokset, joissa todetaan silmän ikärappeuman pistoshoidon käytäntöjen vaihtelevan eri am mattilaisten ja toimipaikkojen perusteella myös Yhdysvalloissa, Kanadassa sekä Isra elissa (Green-Simms, Ekdawi & Bakri 2011; Xing, Dorrepaal & Gale 2014; Segal ym. 2016). Tässä tutkimuksessa esitetyt kysymykset sekä niiden perusteella saadut vastaukset voivat parhaimmillaan toimia seuraavan laajemman tutkimuksen pohjana, joka auttaa varsinai sen tutkimuksen ideointia, muotoilua sekä toteutusta. Laajemmassa tutkimuksessa otos kokoa on syytä suurentaa koskemaan kaikkia Suomessa toimivia silmäpistoshoitoa to teuttavia terveydenhuollon ammattilaisia.	Translation - English 1 INTRODUCTION Age-related macular degeneration is the most common cause for visual impairment amongst the elderly in Finland and other Western countries. Age-related macular degeneration primarily occurs in people above the age of 65. It is estimated that approximately 100 000 people suffer from age-related macular degeneration in Finland. Annually, about a thousand people experience visual impairment as a result of macular degeneration (Kinnunen & Kaamiranta 2014, 2265). Most patients with age-related macular degeneration have the so-called ”dry” (nonexudative) form that currently has no effective treatment. Conversely, “wet” (exudative) macular degeneration can be effectively slowed with eye injections injected inside the eye. In the injection procedure, anti-VEGF (VEGF = vascular endothelial growth factor) medication is injected inside the eye using a thin needle. However, these injections do not cure the disease; once the treatment is started, injections must be administered regularly in order to slow the progression of the disease. The necessary number and interval of injections depends on an individual basis, but on average, patients with exudative macular degeneration receive 5-8 injections annually (Kinnunen & Kaamiranta 2014, 2265). The number of injection treatments for exudative macular degeneration has multiplied over the last decade. The reason for this increasing need for treatment is that each year, there are new patients entering treatment, and once started, treatment is rarely ceased. During the years 2010-2014, the number of patients receiving treatment for exudative macular degeneration increased significantly, and over the time period spanning 2015-2025, the number of patients may even double, as the so-called baby boomers will reach the age of 75 (Kuoppala & Härkönen 2015, 4). In practice, the increasing number of patients requiring injection treatment for exudative macular degeneration means that the treatment must become more efficient, as it is causing an increasing burden on eye clinics. Eye clinics must already prepare for increasing numbers of patients in their operations, while simultaneously guaranteeing the high quality and safety of care for the patients. All this must be achieved with limited resources and effectively (Kinnunen & Kaamiranta 2014, 2233). This thesis examines the practices related to eye injection treatment in Finland. It is known that practices related to eye injection treatments presumably vary by eye clinic, and each eye clinic has different methods, working phases and eye injection instruments; no uniform operational model for eye injection treatment has been established. The above factors have been amongst reasons for creating this thesis, in hope that they could be further verified through research as well. 2 PURPOSE, PROBLEMS AND GOAL The purpose of this thesis is to map the practices of eye injection treatment of age-related exudative macular degeneration in Finland. The problems approached in this thesis are: 1. How is eye injection treatment being given in different eye clinics in Finland? 2. What are the phases and methods of eye injection treatment? 3. With which instruments are eye injections being administered? The goal of this thesis is to produce knowledge for healthcare professionals administering eye injection treatments on which treatment methods, work phases and instruments are being used in Finnish eye clinics when giving eye injection treatments. On the basis of this survey, the client of this thesis, Iogen Oy, will develop the services and eye injection instruments it produces in order to better respond to the need of Finnish eye clinics. 3 AGE-RELATED MACULAR DEGENERATION Age-related macular degeneration refers to the age-related degeneration of the center of the visual field, which in turn causes the blurring of vision. There are two forms of age-related macular degeneration: dry (non-exudative) and wet (exudative). There is no effective treatment for the slowly progressing dry macular degeneration, but there are treatments which slow the progression of wet age-related macular degeneration. In the treatment of wet age-related macular degeneration, it is essential to diagnose the disease and begin the treatment sufficiently early (Summanen & Seit sonen 2016). Wet age-related macular degeneration is significantly less prevalent than dry age-related macular degeneration, as approximately 90% of all macular degeneration cases are dry. However, the wet form of macular degeneration accounts for approximately 90% of the visual impairment cases causes by age-related macular degeneration (Saari 2011, 252). A visually impaired person is one whose vision has weakened to the point of causing significant harm to that person in their everyday life. Depending on the difficulty of their disability, visually impaired people are considered low vision or blind (Hietanen, Hiltunen & Him 2005, 142). As it progresses, age-related macular degeneration is an incapacitating disease; especially if left untreated, it will lead to the loss of vision and visual impairment. If measured by its effect on the quality of life, macular degeneration can be compared with other chronic diseases such as inflammatory joint diseases or cardiovascular diseases. The annual cost of age-related macular degeneration is estimated to be approximately 350 billion US dollars globally (Kivinen, Koskela, Kauppinen, & Kaamiranta 2017, 641). 3.1 Prevalence Age-related macular degeneration is the most common eye disease leading to visual impairment in Finland and other Western countries. In the whole population, macular degeneration occurs in approximately 10-15% of 65-74-year-olds, 25% of 75-84-year-olds, and approximately 30% of those 85 and above. It is estimated that approximately 100 000 people suffer from age-related macular degeneration in Finland. Each year, approximately a thousand Finnish people experience visual impairment as a result of macular degeneration (Kinnunen & Kaamiranta 2014, 2265). As a consequence of longer life expectancies and the increasing number of elderly individuals, the occurrence of age-related macular degeneration will multiply in coming years. This will subsequently cause a significant economic strain on the healthcare system as well as an increased workload for eye clinics. (Kivinen, Koskela, Kauppinen, & Kaamiranta 2017, 641). 3.2 Symptoms Typical symptoms of age-related macular degeneration are the distortion of lines and lowered visual acuity. Symptoms can also include distinct black blind spots in the visual field or hole-like white spots in the visual field. Difficulty reading is also common, as some of the letters disappear due to the missing areas of vision. These symptoms are common across the exudative and non-exudative forms of age-related macular degeneration (Seppänen 2013b). Particularly in exudative age-related macular degeneration, symptoms tend to appear and progress extremely quickly. In these cases, loss of acuity usually occurs in a matter of days and affects both near and distance vision; additionally, there may be changes in colour perception (Kostea silmänpohjan ikärappeuma (AMD): Käypä hoito -suositus 2016). The acuity of the central visual field has been lowered for patients with exudative age-related macular degeneration, which can manifest in difficulty in things such as reading and recognising familiar faces. As it progresses, the disease can develop in both eyes, but it can occur in the other eye years later (Saari 2011, 253). 3.2 Causes and risk factors By far, the most common cause of macular degeneration is aging. Additionally, smoking increases the risk of macular degeneration. Quitting smoking evidently decreases the risk of macular degeneration, but the risk may remain high even 20 years after quitting. Genetics also have an impact on the development of the disease, especially if macular degeneration has previously occurred in close relatives. Genetics may account for up to 60% of incidences of the disease (Kostea silmänpohjan ikärappeuma (AMD): Käypä hoito -suositus 2016). It has also been estimated that prevalence is higher amongst women and ethnically white individuals. (Iivanainen, Jauhiainen & Syväoja 2010, 142). 3.4 Treatment A majority of macular degeneration patients, 80-90%, have the non-exudative form of the disease that currently has no effective treatment. Conversely, the progression of exudative macular degeneration can be effectively prevented with injection treatments. During the injection treatment, anti-VEGF (VEGF = vascular endothelial growth factor) medicine is injected into the vitreum. In exudative macular degeneration, the uvea forms new blood vessels which intrude on the retina and often bleed into the central visual field. The bleeding causes swelling in the fundus, which in turn leads to decreasing visual acuity. As its name suggests, the anti-VEGF medication prevents the growth of new blood vessels, therefore slowing the progress of the disease and the subsequent loss of vision (Kinnunen & Kaamiranta 2014, 2265). Anti-VEGF treatment must be started immediately, or at least within a matter of days, once macular degeneration is diagnosed, in order to efficiently slow or prevent the progression of the disease. Injection treatment is usually given to all patients in series of three injections. These injections are administered with intervals of about 4-6 weeks. After the three injections, a follow-up examination is done to examine the efficiency of the injection treatments; the monitored factors include the amount of swelling in the eye, subretinal fluid retention, and changes in vision. If the disease has not developed in a worse direction, a pause in injections can be tried (Kostea silmänpohjan ikärappeuma (AMD): Käypä hoito -suositus 2016). The practical treatment is administered as follows. First, the surface of the macular degeneration patient’s eye is anesthetized. Second, the eye and its surrounding skin is thoroughly cleaned. Third, anti-VEGF medicine is injected into the vitreum with a thin needle. The injection can be administered by an ophthalmologist, a nurse experienced in the treatment of eye diseases, or an ophthalmologist in training. The procedure itself is quick and painless. No pain medication is needed after the procedure (Kostea silmänpohjan ikärappeuma: Käypä hoito -suositus 2017). The necessary number and interval of injections varies significantly from patient to patient, but on average, patients with exudative macular degeneration receive 5-8 injections annually. Patients with exudative macular degeneration maintain consistent contact with their eye clinic due to the continuity of care and the monitoring of the progression of the disease (Kinnunen & Kaamiranta 2014, 2265). According to a 2014 study by Fimea (The Finnish Medicines Agency), hospital pharmacies delivered a total of nearly 54 000 individual doses of vitreal injections to eye clinics. In addition to the treatment of macular degeneration, intravitreal injections are also used to treat diabetic retinopathy (Kuoppala & Härkönen 2015, 23). The number of injection treatments for exudative macular degeneration has multiplied over the last decade. The continuously increasing need for treatment is caused by the fact that although new cases emerge annually, only a portion of patients leave treatment. Over the years 2010-2014 the number of patients being treated for exudative macular degeneration has increased significantly, and over the next decade, the number of patients may even double, as the so-called baby boomers will reach the age of 75 (Kuoppala & Härkönen 2015, 4). Other, significantly rarer, treatment methods include photodynamic treatment, direct laser treatment and the combination of these two treatments. Photodynamic treatment involves first an intravenous injection of a substance which increases photosensitivity; this substance attaches itself in the new blood vessels in the fundus. After this, laser light is directed in the fundus, destroying the blood vessels and therefore preventing the worsening of the macular degeneration. This treatment must, however, be repeated at intervals of a few months over the course of a couple of years. (Hannuksela 2012; Salmi 2016, 8) Direct laser treatment used to be the only available treatment method for exudative macular degeneration. Direct laser treatment can only be given to patients whose new blood vessel growth is located far enough from the most central part of the visual field. Direct laser treatment is not as effective as other treatments used for exudative macular degeneration. For some patients, so-called combination treatment can be appropriate. In such cases, the treatment of exudative macular degeneration can combine, for example, photodynamic treatment with cortisone injection treatment or, alternatively, photodynamic treatment with anti-VEGF treatment in separate courses of treatment (Seppänen 2013b). 3.5 The anatomy of the eye and injection site The anti-VEGF medicine is injected into the vitreum intravitreally from the side of the eye (Saari 2011, 255). Anatomy of the eye and the administration sites of eye medications (Mäenpää 2017) The medicine injected intravitreally quickly diffuses in the vitreum and gradually moves from there to the inner layers of the retina. Part of the medicine injected into the vitreum reaches its site of effect in the retina, while part of it will leave the eye through the internal blood circulation of the uvea (Mäenpää 2017). There are multiple different instruments for administering eye injections, each with a different technique. The InVitria injection tool, for example, facilitates the administration of intravitreal injections. It enables a solid and correct position for the injection angle and depth. InVitria reduces the risk of errors caused by the movement of the patient or the injector. If InVitria is used for the procedure, no other tools, such as speculums, measurers or stabilizers, are usually needed (InVitria -Intravitreal Injection Assistant n.d.). Another common tool for administering eye injections is the SP-Eye-injection tool, which, among other things, controls the depth and angle of injection and ensures that the medicine is safely inserted into the appropriate part of the eye (SP.eye IVT device n.d.). 3.6 Medicinal treatment There are currently three different medications that are used in injections for slowing the progression of macular degeneration: bevacizumab (sold as Avastin), ranibizumab (sold as Lucentis), and aflibercept (sold as Eylea). Bevacizumab, ranibizumab and aflibercept are currently considered equally as effective in maintaining visual acuity for patients with exudative macular degeneration. Moreover, no clinically significant differences in their safety level have been found (Kostea silmänpohjan ikärappeuma: Käypä hoito -suositus 2017). The usage and price differences of these medications have been discussed for years. There are significant price differences in the medications used to treat macular degeneration. Bevacizumab (Avastin) is the most common, and it has been used for about ten years, which is longer than the other medications; it is also significantly more affordable than the other two. Bevacizumab is a medication originally designed for the treatment of colorectal cancer. Despite its originally intended purpose, it is part of the medications offered under the publicly funded healthcare system. According to a recommendation given by the Council for Choices in Health Care in Finland (COHERE Finland) in 2015, the use of bevacizumab in the treatment of exudative macular degeneration is acceptable and economically sensible (Soininen 2015; Tuulonen 2014, 2291). In Kuopio University hospital in 2016, the price of bevacizumab was 70€ per injection, the price of ranibizumab was 909€ per injection, and the price of aflibercept was 718€ per injection. These price examples only include the medicine injection itself, not the other indirect costs caused by eye injection treatments, such as the cost of imaging examinations for monitoring the progression of the disease or the costs caused by visual impairment. For these reasons, it is justifiable to use the cheapest medicinal product which equals the other medications in both efficacy and safety (Vottonen 2016, 19- 21). 3.7 The risks of eye injection treatment According to Palosaari et al., there are risks involved in eye injection treatments, just like all intrusive procedures. Possible consequences of eye injections include endophthalmitis (inflammation of the interior of the eye) and the increased risk of retinal detachment. Because of the increased risk of inflammation, antibiotic drops are usually administered to both eyes before the procedure, as per instructed by the ophthalmologist. After the procedure, antibiotic drops are often administered to the eye that received the intravitreal injection. Injection-related, serious side effects to the eye are rare, but in the worst cases, they may even cause the complete deterioration of eyesight. To reduce the side effects of eye injections, careful protection against infections is essential in injection treatments (Palosaari ym. 2008, 2233). 3.8 Endophthalmitis Endophthalmitis is the inflammation of the interior of the eye caused by bacteria or fungi that can develop, for example, during an earlier procedure of the interior of the eye. The symptoms of endophthalmitis include significant swelling of the eyelids, bloodshot eyes, eye pain, and the relatively fast decrease of visual acuity. There is usually a strong feeling of pain in the eye, and eyesight decreases very quickly. If left untreated, endophthalmitis results in blindness (Summanen & Setälä 2016). In order to save the eyesight of a patient suffering from endophthalmitis, they must be immediately sent to an unit specialising in eye diseases, so the treatment of the eye can be started. Topical antimicrobial medication is used to treat endophthalmitis in hospitals (Summanen & Setälä 2016). According to a meta-analysis conducted in the US, the risk of endophthalmitis in individual eye injection procedures for the treatment of macular degeneration is 0,056%. Therefore 10 000 injection procedures indicate approximately 5 cases of endophthalmitis (Fileta, Scott & Flynn 2014). 3.9 Patient guidance and self-monitoring It is important to measure the visual acuity of patients with macular degeneration on a regular basis, because exudative macular degeneration often involves the other, previously symptom-free eye developing the condition within five years. In the patient rehabilitation process, ensuring that their home is sufficiently illuminated and encouraging the use of tools such as magnifying glasses is also important. The increasing communication difficulty in interpersonal relationships also causes a burden on the patient. Where necessary, the home assistance required by patients with visual impairment must also be arranged by the healthcare provider (Mustajoki ym. 2013, 753). Self-monitoring has an important role in examining the progression of the disease. If the patient has been diagnosed with non-exudative macular degeneration, their condition must be monitored actively in order to recognise possible vision changes indicating exudative macular degeneration. In such cases, patients are guided to use the so-called Amsler grid, a small grid of horizontal and vertical lines with a dot in the middle (Image 2). If the patient sees blurred lines or white or black holes in their field of vision while regarding the grid, this can indicate exudative macular degeneration (Salmi 2016, 8). IMAGE 2. An Amsler grid, with a normal finding on the left and the finding of a patient with age-related macular degeneration on the right (Näkövammaisten liitto ry, edited) 10 Eye injection treatments and the healthcare system The treatment of exudative age-related macular degeneration has already revolutionized the life of the patients receiving this care, but the healthcare system must continue to develop. Although the injection treatment is, in itself, a quick procedure, the fast increase of treatment evaluations as well as patients receiving treatment require follow-up appointments at eye clinics and repeat treatments every 1-2 months. These factors are burdening the healthcare system and raising costs significantly. Therefore, the eye health care system must now resolve the challenge of organizing the care processes to be simultaneously more efficient and cost-effective (Tuulonen 2008). The quick increase of eye injection treatments for exudative age-related macular degeneration is demonstrated, for example, by the 9-fold multiplication in the number of in eye injections given for macular degeneration at the Tampere University Hospital’s Eye Center over the years 2008-2013. Despite this, a sufficient standard of care for the patients has been achieved by developing new approaches and using the most affordable medicine available (Tuulonen 2014, 2290-2295). Tuulonen (2014, 2293-2294) notes the necessity of combining the treatment of eye diseases and the resources used for it in approaches that create an equal standard of care for all patients receiving it. It is, however, difficult to predict the methodological or economic development of age-related macular degeneration treatments in the future. 4 THE METHODOLOGICAL STARTING POINT 4.1 Research methodology This thesis is quantitative in nature. Quantitative research can also be called statistical research. This kind of research aims to examine questions related to percentage proportions and numbers. In quantitative research, evidence is usually collected by using standardized research forms with prepared answer choices for different questions. The results are usually demonstrated with images and graphics. Quantitative research tends to succeed at mapping the current situation of the topic under study, but it cannot explain the reasons behind the situation (Heikkilä 2010, 16). Quantitative research is well suited for situations where the aim is to gain justifiable, generalizable and reliable information on a phenomenon. Quantitative research always aims to generalize. At its simplest, this means gathering research data from a relatively small group of observation units. In such cases, the method of quantitative measuring is asking different questions, which then form the method of gathering information (Kananen 2011, 17-19). A questionnaire is the most common method of gathering information in quantitative research, but it is also possible to utilise methods such as telephone interviews or direct observation. Once the data has been collected, it is analysed by using a statistical software, which then processes the data into a form that can be demonstrated in images and charts, such as percentages (Vilkka 2005, 74-77). This thesis is quantitative in nature, because the target group is rather vast and dispersed, and the goal is to achieve a comprehensive survey of Finnish eye injection treatment practices. There is a lot of existing knowledge on eye injection treatments, but this information has mostly been gathered by sample groups, and little of it exists in written form. The research data was collected by questionnaire forms. In a questionnaire-based study, the respondents read and answer the written questions independently. A questionnaire-based study is most suited for a large and disperse group of people. The key advantage of a questionnaire form is that the respondents may remain anonymous, if they so wish (Vilkka 2005, 74). The questions used in the questionnaire forms for this thesis were structured. This means that ready-prepared answer choices were given for the questions. Using and analysing structured answers is, in essence, easier than processing open questions (Kananen 2011, 30-31). A questionnaire form can include mixed questions, multiple choice questions or open questions. Mixed questions include some ready-prepared questions and some open questions. A mixed question is appropriate where it is estimated that all possible answer choices are not sufficiently known (Vilkka 2005, 85-86). The questionnaire used for this thesis utilised mixed questions. A downside of a questionnaire-based research is the risk of only achieving a low response rate. Respondents can be given a reminder to partake in the study, but this will subsequently delay the research schedule (Vilkka 2005, 74). The response rate for the questionnaire used in this study was 55%. A questionnaire form must always be tested before it is used as a research instrument. Testing will focus on factors such as the clarity and unambiguity of the questions, the functionality of the answer choices, the length of the questionnaire form and the reasonable duration of time it takes to answer. Testing also involves assessing whether the questionnaire contains unnecessary or irrelevant questions, or whether some essential question is missing (Vilkka 2005, 88-89). The questionnaire form used for this thesis was an internet-based form with a total of 17 structured questions related to eye injection treatments. Additionally, the form contained two open spaces; the first allowed respondents to write down their comments related to the questionnaire, while the second allowed them to add their contact details (see Annex 1). The questionnaire was sent to the staff members responsible for eye injections at Finnish eye clinics via email. The questionnaire was sent to 20 eye clinics in total. 11 responses were received, comprising a response rate of 55%. 4.2 Gathering research data The method of gathering research data for this thesis was a questionnaire form provided by Typeform. Typeform is an internet-based service designed for data collection and dissemination. Typeform can be accessed on the most common browsers and smartphones (Typeform, n.d.). The questionnaire form was shaped over the spring of 2017, when the form was also tested twice by the staff of Iogen Oy. The purpose of testing was to assess the spelling and clarity of questions. The technical functionality of the form and the time it took to fill it was also considered. According to this testing, it took a maximum of 10 minutes to fill the form. After testing, the questionnaire form (Annex 1) was sent via email with an attached cover letter (Annex 2) to the appropriate target group. The purpose of the cover letter was to approach respondents and to motivate them to partake in the survey. The cover letter was also aimed at informing respondents on the importance and the confidentiality of the questionnaire (Kananen 2011, 46). The questionnaire was sent to the target group for a second time 8 days after the first outreach. This time, the cover letter was worded differently (Annex 3), but the questionnaire itself remained unchanged. Typeform can be used to automatically create an internet-based questionnaire, which can then be sent to respondents via email. Respondents can fill the survey via a direct link on the email, and their answers will automatically be saved on the Typeform database. In the reporting phase, the answers saved on the Typeform database can be directly transferred into a word processing or spreadsheet computation software by using the Typeform analysis tool. The target group of this study were the doctors or nurses responsible for eye injections at 20 different Finnish eye clinics. Iogen Oy delivered the questionnaire forms with their attached cover letters to respondents via email. 4.3 Analysis of collected evidence The responses to the questionnaire created with the Typeform service were automatically saved on the service provider’s server. The results were transferred from the server into the spreadsheet computing programme Excel, which was used to create charts based on the received responses. The numbers in the charts have been stated in percentages, because quantitative research aims to generalize. Therefore, individual amounts are not significant; what is relevant is the proportion of each factor within the collected data as a whole (Kananen 2011, 74). Charts are a quick way of demonstrating the results of quantitative research. A good statistical chart is characterised by its ability to communicate a large amount of information in a relatively small space; its lack of distortion of facts; and its ability to link up with the other demonstrations of the research data. Good charts also arouse readers’ interest and encourage them to draw comparisons with the other charts (Heikkilä 2010, 154-155). 5 QUESTIONNAIRE RESULTS AND THEIR ANALYSIS The questionnaire mapped the procedures done to patients in preparation for the actual the eye injection procedure. In the questionnaire, respondents shed light on the established practices in their own places of work. There were no right or wrong answers, but answers were based on the routines which vary by clinic. 5.1 Preparation for eye injection The majority of the respondents (82%) state that before the procedure, patients will only receive an anesthetic eye drop in the eye that will receive an eye injection. 18% of respondents state that in their clinics, patients will receive both an anesthetic eye drop and a diluting eye drop in the affected eye before the actual procedure. The responses have been declared in percentages in Chart 1 below. Chart 1. Administration of anesthetic and dilating eye drops pre-procedure 55% of respondents record that at their clinic, eye injections are only administered in the procedure room. 36% of respondents say that eye injections can be administered either in the procedure room or in the operating room. 9% say that eye injections are only performed in the operating theatre at their clinic. Answers are illustrated in Chart 2 below. Chart 2. Performance location of eye injections At just over half of the clinics (55%), eye injections are primarily administered by doctors, while 45% of the clinics have nurses as the principal administrators of injections. The principal administrators of eye injections have been illustrated in Chart 3 below. Chart 3. Principal administrators of eye injections Upon being asked whether patients are lying down or sitting up during the eye injection procedure, all respondents stated that patients receive the injection while lying down. This is illustrated in Chart 4. Chart 4. Patient position during the procedure Ready-packaged procedure packages designed for eye injection procedures are utilised at 55% of the clinics. 18% of clinics do not utilise procedure packages at all. 27% of clinics do not currently utilise procedure packages, but respondents feel that the packages might be helpful and possibly make the eye injection procedures more efficient. Responses on the use of procedure packages are illustrated in Chart 5 below. Chart 5. The usage of procedure packages for eye injections Upon being asked about the preparations on the patient and the injection site before performing the eye injection, the most common preparatory procedures were sterilizing the skin surrounding the patient’s eye and placing a surgical drape over the patient (91%). Patients are also given a surgical cap at 82% of clinics. 36% of clinics use speculums during the eye injection procedure. 36% of clinics use sterile medical tape. 18% of clinics sterilize the patients’ hands. At 18% of clinics, the site of injection is also marked. Respondents could select as many options as they wanted for this question. Responses for the question concerning preparatory procedures for the patient and the injection site are illustrated in Chart 6. Chart 6. Preparation practices of the patient and the injection site Every respondent stated that they use sterile gloves during the injection procedure, as demonstrated in Chart 7. The alternative answer choices for sterile gloves were factory-fresh gloves or no gloves at all. Only one answer choice was provided. Chart 7. Hand protection while performing eye injections Every respondent said that surgical masks and hair masks are used by injectors preparing to administer the eye injection at their clinics. Surgical gowns are not used at any of the clinics during eye injection procedures (Chart 8). Chart 8. Protective clothing used by professionals performing eye injections 5.2 Eye injection as a procedure Each respondent recorded that the InVitria injection tool is used to measure and mark the appropriate injection site. None of the respondents use the SP-Eye tool (Chart 9). Chart 9. Tools used for the measurement and marking of injection sites The majority (64%) of respondents say that the exact injection site of eye injections does not matter. 27% say that they perform the injection in a superior-temporal manner, while 9% of respondents say the perform the injection in an inferior-temporal manner. These responses have been analysed in Chart 10. Chart 10. The site of administering eye injections A total of 64% of respondents mechanically move the conjunctiva in order to decrease the risk of infection. Conversely, 36% of respondents record that the conjunctiva is not mechanically moved at their clinics. These percentages have been demonstrated in Chart 11. Chart 11. The prevalence of moving the conjunctiva to reduce infection risk A total of 18% of respondents state that they do not perform injections to both eyes during the same appointment. Conversely, 45% of clinics do perform eye injections to both eyes simultaneously. If circumstances permit, injections are performed to both eyes at 36% of clinics. This has been illustrated in Chart 12. Chart 12. The prevalence of performing injections to both eyes simultaneously 5.3 After the injection After the eye injection procedure, antibiotic eye drops are administered at 45% of the clinics, while antibiotic ointments are given at 36% of the clinics. A protective guard is placed over the eye at 9% of the clinics. The affected eye is bandaged at 9% of the clinics (Chart 13). Chart 13. At 91% of the clinics, intraocular pressure is not measured at all in the evening preceding their discharge, as illustrated by Chart 14. Chart 14. Total time spent on eye injection treatments 55% of respondents stated that their clinics perform more than 26 eye injections daily. 36% of respondents estimated the number of daily eye injections to be approximately 21-25. 9% said that their clinics perform approximately 16-20 eye injections each day (Chart 17). Chart 17. Average number of eye injections performed on a daily basis 6 REFLECTION 6.1 Ethics of the thesis The ethicality of research is the starting point of all scientific activity. Good scientific practice requires the researcher to adhere to honesty, general accuracy and meticulousness in both conducting research and presenting the research results (Vilkka 2005, 30). Research must always respect and protect the research objects and the client of the study. It is also considered good practice for the author of a thesis to publicly state their relationship with their topic of study and their client (Vilkka 2005, 32-33). This thesis has been created in collaboration with Iogen Oy. Iogen Oy is a Finnish supplier of ophthalmological instruments and supplies; it is the largest of its kind in Finland. Iogen supplies a significant portion of the tools and instruments used for eye injection treatments in Finnish eye clinics. In all research work, the respondents’ right to self-determination must be guaranteed. According to the Finnish law on personal data (22.4.1999/523), collecting identifiable email information is only permitted and acceptable in cases where the email user has explicitly given license to do so. Taking part in the questionnaire used as the basis of this thesis has been completely voluntary for the respondents. In the last section of the questionnaire form, the email addresses of respondents were collected on a voluntary basis. The purpose of collecting email addresses was to send the respondents this thesis in full, once completed, if the respondents wished to receive it. The email addresses of respondents were not given to any third parties or used in this thesis in any way. The email addresses collected via the questionnaire form have been removed after the completion of this thesis. An important part of the ethicality of a thesis is conducting a plan for the thesis. A plan must indicate, among other things, the purpose and aim of the thesis as well as the schedule and the methodology of research (Vilkka 2005, 58-59). The plan for this thesis was completed as agreed, in the spring of 2017. The plan fulfilled all of the criteria mentioned above. Approval for the plan was sought from the supervising teacher at Tampere University of Applied Sciences well sa the client of the thesis; a representative of Iogen Oy. During the creation of this thesis, the target group has been treated with respect, and the research data has been processed with integrity, objectivity and confidentiality. The thesis with its attached cover letters was sent to the nurses or doctors responsible for eye injection treatments at different eye clinics. Responding to the questionnaire was completely voluntary. Using a name was not required to fulfil the questionnaire, making it possible for respondents to remain anonymous, if they so wished. All of the research related to this thesis was only targeted at the personal experiences and observations of the responding doctors and nurses concerning the eye injection treatments at their places of work. There was no research targeted at patients. The questionnaire forms were returned to the author of this thesis electronically, hence the author was the only person processing the data. After the responses were analysed, they were appropriately disposed of. 6.2 Reliability The reliability of the conducted survey can be examined from the dual viewpoints of validity and reliability. Validity refers to whether the study has succeeded in measuring what it set out to measure. Generally, validity is good when the target group and the questions of a study have been appropriate. Reliability, in turn, refers to the constancy of the results. Reliability can be measured, for example, by assessing the phenomena under study with the same metrics using different materials, and if the results achieved are similar, these metrics can be considered reliable (Kankkunen & Vehviläinen-Julkunen 2010, 152). In this survey, a new metric was constructed to measure the practices of eye injection treatments. According to Metsämuuronen (2006, 58), research conducted using a self-made metric is exactly as accurate as the self-made metric. The idea for the metric was taken from an international questionnaire form that has been used in the US, Canada and Israel, among others (Green-Simms, Ekdawi & Bakri 2011; Xing, Dorrepaal & Gale 2014; Segal ym. 2016). Additionally, existing theoretic information was used to construct the metric, and some of the questions were based on this information. When measuring the reliability of research, following factors must be considered, among others: what is the size and quality of the study sample, what is the response percentage and what kind of measuring errors could have occurred in the questionnaire form utilised as a metric (Vilkka, 149-150). Respecting the work of others is part of good scientific practice. This means that sources outside the self-conducted research have been used appropriately, that is, by always referring to the original source of information within the research text (Vilkka 2005, 31-32; Vilkka 2007, 164-165). In this thesis, the sources and textual references have been marked appropriately and according to the guidelines of written reporting published by the Tampere University of Applied Sciences. When considering the reliability of research, one of the most central factors is the objectivity of the researcher, that is, the lack of bias. This means that the researcher’s personal opinions and beliefs must not influence the research results, and the results must not be dependent on the researcher. It must be noted, however, that even the most meticulous researchers can commit mistakes which can, in part, weaken the reliability of the results (Heikkilä 2010, 31). Conducting the questionnaire electronically has ensured that the author of this thesis could not have posed the questions in a leading way, as each respondent has interpreted and answered the questions based on their own assessment. The author of this thesis has, during their studies as a nurse, assisted in the administration of eye injections for the treatment of exudative macular degeneration; this experience has granted the author knowledge and experience on how the injection treatment is being performed on a practical level. As questions are the method of collecting information for this study, the reliability and the quality of research depend on them. The functionality and the reliability of the questions is influenced by whether the respondents have understood the questions correctly and whether they have the necessary information and will to respond to the questions being posed. In addition to these factors, the questions must be clear and comprehensible (Kananen 2011, 30). By using the Typeform service, it was possible to create a form with questions that were clear in content and visually simple. The questions on the form could have been divided into certain subcategories more clearly. For instance, the prevention of infections during injection treatments could have comprised its own set of questions. As it was presented, the questionnaire form included the topic of infection prevention as a part of several different questions. Upon examination of the results of the survey, it was discovered that some of the responses were self-contradictory, which indicates that the respondents did not possess the necessary information to answer the question, or that they had misunderstood the question. The questionnaire form also had one spelling mistake, which was only discovered after sending the form. On their part, the above-mentioned factors weaken the reliability of this thesis. 6.3 Examining the results In 2014, approximately 25 000 eye injections were administered. It was then predicted that the number of eye injections would increase by approximately 40% annually. The cost of an individual injection, if only considering the dosage of medicine, varied between 50€ and 800€ (Kinnunen & Kaamiranta 2014, 2267). It is therefore clear that providing treatment for exudative age-related macular degeneration is a significant economic burden on the healthcare system. In a large part of the eye clinics, 21 or more eye injections were administered daily (Chart 17). As one eye injection takes approximately 10-15 minutes at 45% of clinics (Chart 16), it can be concluded that simply administering eye injections takes at least 3 hours and 30 minutes each day. In addition to this, we must consider the transition periods, the preparatory tasks between individual eye injection procedures, and other relevant factors. It can, therefore, be assumed that the professionals working at eye clinics have a significant amount of their time restricted to eye injection treatments. Since the injection treatment for exudative age-related macular degeneration is already consuming a significant amount of human resources, and the studies referenced earlier indicate that the number of eye infections will further rise in the future, it can be assumed that the resource burden on eye clinics will increase proportionally. Eye clinics must prepare for this challenge. According to the results, the practices of administering eye injections vary by eye clinic. For example, there were significant differences in the preparation of the patient and the injection site for the procedure (Chart 6). In the majority of eye clinics, the healthcare professionals emphasised the prevention of infections in their own conduct, but only 18% of the clinics emphasised the prevention of infections by patients themselves by sterilizing their hands. In almost all eye clinics, the patients were given either antibiotic ointments or antibiotic eye drops after the procedure (Chart 13). However, the injected eye is not protected or its normal usage is not restricted by, for example, bandaging the eye or placing a guard over the eye after the injection procedure. According to the results, the principal administrators of eye injections are either doctors or nurses, with rather equal distribution (Chart 3). According to two different British studies, a well educated and experienced nurse can perform an eye injection for the treatment of exudative age-related macular disease without increasing the risk of intraocular infection or other injection-related complications. According to the studies, infection treatments performed by nurses are also cost-effective (Simcock ym. 2014; Sama lia, Garland & Squirrell 2016). According to the results of this thesis, eye injections in Finland are still relatively often performed by doctors, and the number of nurses performing this procedure could be increased. The results indicate that the focus on infection prevention and hand hygiene varies by clinic. All respondents stated that sterile gloves are used when performing eye injections (Chart 7). However, patients are rarely prompted to pay attention to their hand hygiene, as only 18% of eye clinics have patients sterilize their hands before the procedure. According to the Finnish Communicable Diseases Act (1227/2016), operating units must take care of the appropriate protection of patients and personnel against infectious diseases. On this basis, ensuring that patients maintain good hand hygiene before receiving eye injections should also be emphasized. According to information reviewed for this study (Anttila 2014), hand hygiene is one of the most efficient ways of preventing treatment-related infections and the increase of resistant microbes in patients. This thesis represents exploratory research. The aim of exploratory research is finding new viewpoints on the topic of study. Exploratory research can be used to study lesser-known topics while examining literature on the topic of study. In this way, exploratory research can be used to discover the central models, themes and typologies of the topic of study (Vilkka 2007, 20). In reference to the above, it can be stated that some of the questions in the questionnaire used for this thesis received univocal answers with no discernible differences between respondents. Examples of such questions are questions number 4 (Chart 4) and question number 7 (Chart 7). These answers are, however, important for receiving information that helps form a comprehensive assessment on the practices related to the injection treatment of exudative macular degeneration. 6.4 Conclusions and suggestions for further research The practices on the injection treatment of exudative macular degeneration vary by clinic. No uniform and standardized model for performing eye injection treatments has been created. The official recommendations for the good treatment of exudative macular degeneration (2016) mainly concern the diagnostics, medical treatment, and the response to treatment in this disease; but practical and patient-oriented guidelines have not been published. Therefore, each eye clinic has its own guidelines and approaches to performing eye injection treatments. In order to become more patient-oriented, it would be important that the procedure for eye injection treatment was similar regardless of which clinic the treatment is being performed at. The similarity of eye injection treatments could also enable the centralised procurement of supplies by eye clinics, which would presumably decrease the costs of individual eye injection treatments on part of, for example, the materials necessary for preparing for an eye injection procedure. Standardized eye injection treatments would be socially significant also from the point of view of the medical personnel performing eye injections, as it would facilitate moving between places of work at different eye clinics, as the methods of treatment would be similar regardless of the clinic. The standardization of eye injection treatments would, however, require a significant amount of further research and scientific evidence for the methods and practices related to eye injection treatments. On the basis of the results of this thesis, Iogen Oy can offer its ready-packaged procedure packages for those eye clinics that do not yet utilise them. The advantage of using ready-packaged procedure packages is that they enable each eye injection to be performed with identical tools. This creates stability and continuity to the administration of eye injections for the healthcare professional performing the injections as well as ensuring that each patient is treated according to certain routines. New medications and treatment methods for the treatment of exudative macular degeneration are continuously being developed. It can be presumed that the medications currently being used will be replaced by new, partially similar medications in the future. The new medications might have, for example, a longer period of efficacy in the eye, compared to the current medications. This would practically mean that the eye of patients suffering from exudative macular degeneration would not need to be treated at intervals of approximately two months, but the interval of injections might stretch to up to 3-6 months Costa, Nascimento, Teixeira & Silva 2017). The above-mentioned increase in injection intervals would, presumably, free up a significant amount of time and human resources at the eye clinics for other tasks. The injection treatment of exudative macular degeneration is on a good foundation in Finland, despite the practical differences between eye clinics. The largest concerns are related to the possible increase in injection treatments and the subsequent potential resource shortage. In that case, optimizing scarce resources would be sensible and justified, if only on the basis of the ethical factors in the healthcare sector and the criteria on the quality and accessibility of care established in the Health Care Act (30.12.2010/1326). Due to the scarcity of resources, it is prudent to consider whether eye injection procedures could be performed primarily by nurses more often than is currently the case. The results of this thesis are similar to the results of international studies, which also indicate that the practices related to the injection treatment of exudative macular degeneration vary by healthcare professional and place of operation in the US, Canada and Israel (Green-Simms, Ekdawi & Bakri 2011; Xing, Dorrepaal & Gale 2014; Segal ym. 2016). The questions posed in this research and the answers received could ideally function as the basis of a following wider research, facilitating the devising, designing and execution of such research. In a wider study, the sample size should be increased to all healthcare professionals performing eye injection treatments in Finland.
