Tension Pneumothorax

39 yo AAF went to ER with CC: chest pain and SOB. These complaints were of sudden onset while she was typing on her computer. She c/o squeezing, pressure-like sensation in her midsternal area and over her left side. She rated her pain as a 10 on a scale of 1/10.
She had similar complaints recently when she had a left-sided pneumothorax. However, this time she felt worse.

PMH:
SLE and pulmonary fibrosis with a recent left pneumothorax

PSH:
Cholecystectomy, recent thoracotomy with bleb resection for spontaneous pneumothorax

Medications:
Prednisone, Nexium, Doxepin, Atarax

Allergy:
Penicillin, Amoxicillin, reaction unknown

FH:
Breast CA in her sister, lupus in her other sister, HTN, DM

SH:
Stopped smoking 8 years ago

Physical examination:
VS 36.5-78/61-140-20
Chest: Decrease in chest rise and diminished breath sounds on the left side.
CVS: Tachycardic but regular
Abdomen: Soft, NT, ND, +BS

12-lead EKG showed sinus tachycardia rhythm with a rate of 139 bpm, no ischemia
CXR showed a complete left pneumothorax with a mediastinum shift - a tension pneumothorax, and a large bulla.

La 39enne AAF si è recata in pronto soccorso con il caso clinico: dolore al petto e SOB. Questi disturbi si sono presentati all’improvviso mentre stava battendo al computer. Ha avvertito un senso di costrizione e pressione nella zona parasternale sinistra. Ha classificato il suo dolore con 10 su una scala da 1/10. La paziente aveva recentemente già lamentato dolori simili quando ha avuto un pneumotorace sinistro.Tuttavia questa volta si è sentita peggio.

I’ecg a 12 derivazioni ha mostrato un ritmo sinusale tachicardico con valori di 139 bpm, nessuna ischemia, un RX Toracico ha mostrato un pneumotorace completo a sinistra con uno spostamento del mediastino – un pneumotorace iperteso ed una bolla estesa.

I was born in Rome (Italy) and currently live in Berlin (Germany). As I've always been enjoying getting in touch with foreign cultures, I've spent a great part of my life travelling.
