An operator in the compound film weight check area saw that the liner on line XXX had stopped, whilst the Clevertech located on the liner was on downtime. After a couple of failed attempts to re-start the liner using the start button, he decided to check if there were jammed shells blocking the lining chuck.

In the meantime the Clevertech located on the liner was re-started by a shift engineer, and a third operator, on the opposite side of the machine, restarted the liner. The first operator's finger was caught, resulting in a serious fracture and partial amputation.
I zalecenia powypadkowe:
- Before intervening on a stopped machine, make sure that it has not been stopped because of other machines.
- During interventions, close the compound application perimeter with interlocked guarding.
- Before starting a machine, ensure that no body is working on it
