Source text - French Le diabète de type 2 concerne plus de deux millions de personnes en France. Caractérisé par une glycémie (taux de sucre dans le sang) élevée, favorisé par le surpoids, le diabète est un des facteurs de risque majeur des accidents cardiovasculaires. Il se complique aussi d'une atteinte des petits vaisseaux, pouvant conduire à la cécité, l'insuffisance rénale, ou une neuropathie souvent invalidante.
La Haute Autorité de Santé (HAS) propose de nouvelles recommandations de prise en charge. Les précédentes recommandations avaient été invalidées par le Conseil d'Etat en raison de conflits d'intérêts. Le risque de complications liées au diabète augmente de manière continue avec l'importance de l'hyperglycémie. Cette relation, parfaitement démontrée, induit une notion simple mais erronée : plus on baisse la glycémie, plus on réduit le risque. Cette idée, amplement promue et appliquée par les spécialistes, soutenue par l'industrie, mais aussi par l'Assurance Maladie, conduit rapidement à empiler deux, trois médicaments ou plus pour contrôler strictement la glycémie.
C'est oublier que l'objectif réel du traitement du diabète est la prévention des complications, le contrôle de la glycémie n'est qu'un critère dit "intermédiaire". Les experts qui ont participé à l'élaboration de ces nouvelles recommandations ont réalisé une synthèse exemplaire des données scientifiques disponibles sur l'efficacité de ces médicaments. Le bénéfice de l'intensification du contrôle glycémique ne concerne que la prévention du risque cardiaque, avec un infarctus du myocarde non mortel évité sur six. Néanmoins, une des études évaluant ce bénéfice, conduite par le NIH (équivalent américain de l'Inserm), a dû être interrompue en raison d'une surmortalité toutes causes confondues. L'effet le mieux étayé de ces médicaments du diabète est qu'ils induisent des hypoglycémies. Parfois sévères, elles peuvent entraîner une perte de connaissance, une hospitalisation, voire le décès.
Pour la Metformine, le médicament universellement recommandé en première ligne, en dehors de la baisse de la glycémie, aucune preuve de bénéfice sur la prévention des complications du diabète n'a été apportée. Sa promotion s'est fondée sur des résultats partiels de l'étude UKPDS (United Kingdom ProspectiveDiabetes Study). Cette étude a comparé plusieurs stratégies, en ouvert, c'est-à-dire sans masquer la nature des traitements, dans différents sous-groupes randomisés (avec une répartition aléatoire). Son plan d'expérience très compliqué a été adapté en cours d'étude sans motif connu. Dans le sous-groupe des diabétiques obèses, la Metformine a réduit les complications du diabète et la mortalité. Mais, dans l'autre sous-groupe traité par Metformine, celui des diabétiques non obèses et dont la glycémie n'est pas contrôlée, l'ajout de la Metformine aux sulfamides a majoré les complications cardiovasculaires et la mortalité totale, de façon significative, en parfait miroir du résultat chez les obèses.
UNE ÉTUDE NON CONCLUANTE
Le résultat d'ensemble de l'étude, confirmé par une méta-analyse récente, est que l'exposition à la Metformine n'apporte aucun bénéfice clinique. Les conclusions qui ont convaincu la communauté médicale d'une utilisation large de la Metformine, sont ainsi issues d'une analyse en sous-groupe, d'une étude globalement non concluante.
Comment mieux illustrer l'importance de former les médecins à la lecture critique de l'information scientifique ! Les experts ont donc affronté une tâche difficile : fairedes recommandations de pratique pour plus de deux millions de concitoyens, basées sur un objectif glycémique certes logique mais pouvant être délétère pour certains patients, alors qu'il n'a pas été démontré que ces pratiques apportent le moindre bénéfice clinique. Ils ont choisi de laisser une grande liberté au médecin et au patient pour gérer... cette incertitude, en leur recommandant de choisirensemble les règles pour adapter le traitement. Le régime et la perte de poids voient leur place renforcée.
Saurons-nous tirer les leçons d'un tel fiasco médical, qui laisse un goût amer ? Le bon sens commanderait que les Agences de régulation n'acceptent plus la mise sur le marché de médicaments sur la seule démonstration qu'ils contrôlent la glycémie, ce dont on n'a que faire en soi. Arrêtons de confondre la fin et les moyens, exigeons, en tant que patient, médecin, ou enseignant de médecine, des preuves d'efficacité sur les vrais objectifs du traitement : la prévention des complications du diabète.

Translation - English Type 2 diabetes affects more than 2 million people in France. Diabetes is characterised by hyperglycaemia (raised blood sugar) which can be caused by obesity and is one of the major risk factors in cardiovascular disease. It can also cause a narrowing of the small blood vessels which can lead to blindness, renal failure and occasionally disabling conditions.
The French Health Authority (HAS) is proposing new treatment recommendations. The previous recommendations were withdrawn by the Council of State due to a conflict of interests. The risk of complications linked to diabetes increases in line with the level of hyperglycaemia. This well proven relationship leads to a simple but incorrect conclusion which is that the more you reduce the hyperglycaemia the more the risk is reduced. This idea was promoted and applied by the specialists with the support of industry, but also by the French National Health Insurance system (l’Assurance Maladie) and quickly lead to a combination of two, three or more drugs being used to control blood sugar levels.
This ignores the fact that the real objective for the treatment of diabetes is the prevention of the complications of the disease and not the control of blood sugar levels which is an "intermediary step". The experts who proposed these new recommendations have been responsible for collating a considerable collection of available scientific data relating to the efficacy of these drugs. The advantages of improved blood sugar control seem only to concern the prevention of heart attacks, with one non-fatal cardiac arrest in six being avoided. However, one study to evaluate this possible benefit, conducted by the National Institute of Health in the United States, had to be stopped due to an unexplained increase in mortality. The major effect of these diabetes drugs is to cause hypoglycaemia, but sometimes this can be severe leading to a loss of consciousness, hospitalisation, or even death.
Metformin, the drug which is routinely prescribed as a first-line treatment for diabetes, was only ever tested for its ability to lower blood sugar levels and not for its effects on preventing other complications of diabetes. The efficacy of this drug was based on some of the results from a study by the United Kingdom Prospective Diabetes Study (UKPDS). This study compared various treatments in an open trial; that is where the treatments given to various randomised sub groups (with a random distribution) are not masked. The design of the trial was very complicated and was adapted during the trial without explanation. When given Metformin, the obese diabetic group showed a reduction in the complications of diabetes and mortality. However, in the non-obese diabetic group treated with Metformin and whose blood sugar levels were not under control, the addition of Metformin with sulphonamides significantly increased cardiovascular complications and mortality, in complete contrast to the results in the obese group.
AN INCONCLUSIVE STUDY
The overall conclusion of the study, which was confirmed by a recent meta-analysis, is that taking Metformin affords no clinical advantage. The conclusions that steered the medical community to the widespread use of Metformin have therefore come from the analysis of a sub-group in a wholly inconclusive study.
How better to illustrate the importance of training doctors to interpret scientific information! The experts were faced with a difficult task: give practical recommendations to more than two million citizens, based on the logical requirement to lower blood sugar levels, which can be damaging to some patients, even though there was no proof that this approach would bring the faintest clinical advantage. They chose to give a free reign to the doctor and patient to manage this uncertainty, by recommending that they adapt their treatment according to the recommendations. Dieting and weight loss are once again in favour.
Will we learn the lessons of this medical fiasco, which leaves us with a bitter taste in our mouths? Common sense would say that the regulatory agencies should no longer accept the marketing of drugs whose sole purpose is to reduce blood sugar levels, which can be controlled by the patient. Stop confusing the means and the ends. We must insist, whether we are the patient, the doctor or the doctor’s teacher, on proof that the drug is effective for the aims of the treatment: the prevention of complications from diabetes.

L’intense course à la miniaturisation de composants électroniques peut continuer. Une équipe du CEA-Leti, du CNRS et de l’université Joseph-Fourier à Grenoble a mis au point une méthode pour empiler différentes fonctions (mémoire, capteur, processeur …) les unes au-dessus des autres et les relier par des fils conducteurs. Le gain de place promet des consommations diminuées, des transferts d’information plus rapides et des densités de calculs plus importantes.
Des techniques existent d’ailleurs déjà, soit en faisant passer les fils de connexions par les côtes, soit en superposant des composants identiques, comme des mémoires, que l’on perce puis relie par du cuivre ‘versé’ dans les trous.
La nouvelle méthode, décrite dans Nature Materials le 10 février, plus simple et plus riche, est aussi un bel exemple de mariage entre l’inerte (le silicium) et… le vivant. Le squelette des connexions électriques est en effet le même que celui de nos cellules : des filaments d’actine, une protéine assemblée en longues chaînes jouant le rôle de câbles assurant la forme des cellules.
‘CONSTRUCTION TRIDIMENSIONNELLE’
« C’est un article génial ! », assure Yong Chen, directeur du pôle microfluidique de l’Ecole normale supérieure, à Paris. « Cette construction tridimensionnelle combine l’approche du haut vers la base et celle de la base vers le sommet. » La première appartient à la microélectronique : l’ingénieur sculpte la matière. La seconde est propre à la nature : des molécules s’assemblent toutes seules pour créer des structures.
Sur l’une des surfaces à relier, les chercheurs ‘dessinent’ au laser les motifs voulus là où ‘pousseront’ les connexions. Ils y déposent les filaments d’actine qui, au départ, forment des touffes désordonnées. L’ajout de protéines particulières force ces filaments à s’organiser en cylindres verticaux. Et à se connecter à la seconde surface, découpée aussi au laser. Reste alors à métalliser ces liens biologiques grâce à des nanoparticules d’or.
« On n’en revient pas nous-mêmes ! », s’enthousiasme Manuel Théry, l’un des responsables de l’équipe du CEA avec Laurent Blanchoin. « Nous avons répondu par bravade au défi lancé en interne, car notre spécialité est plutôt la biologiecellulaire ».
La mise en place industrielle est encore loin, et les chercheurs n’ont connecté que deux plaques de verre et pas des composants. Mais les câbles ne font que 10 micromètres de diamètre pour 70 de long, ce qui est au niveau des standards actuels. Surtout, d’autres fonctionnalités pourraient être ajoutées en utilisant d’autres protéines pour changer les formes et longueurs de l’actine, ouvrir ou fermer un lien.
« La biologie offre de nouvelles possibilités pour réaliser des empilements complexes, hétérogènes, impossibles à faire avec les techniques actuelles », indique Patrick Leduc, coauteur de l’étude. Des brevets one été pris, et l’équipe cherche un partenaire industriel pour tenter de relier deux composants électroniques réels.

The intensive drive towards miniaturisation of electronic components continues. A team from CEA-Leti at the French National Scientific Research Centre (CNRS) and from Joseph-Fourier University in Grenoble, France have been experimenting with a method of layering different electronic functions (memory, storage, processor) one on top of the other and connecting them together by conducting threads. The reduction in size that this affords leads to improved energy efficiencies, an increase in the speed of information transfer and thus a greater calculation rate.
The techniques already exist elsewhere, for example making connections through external surfaces and superimposing identical components, such as memory, and connecting them by filling etched tracks with copper.
The new method which is described in Nature Materials (10 February) is simpler and more ingenious and is also a good example of the marriage between the inert (silicon) and – the living. The skeleton of electrical connections is effectively the same as the actin filaments in our cells: long chains of the protein are joined together making threads which give the cells their structure.
“THREE-DIMENSIONAL CONSTRUCTION”
“This is ingenious!” affirmed Yong Chen, director of the microfluids group at the prestigious ENS University (École Normale Supérieure) in Paris. “This three-dimensional construction combines both the macroscopic scaled down to the microscopic and the microscopic scaled up to the summit.” The first rests in microelectronics where the engineer sculpts the material and the second is left to nature itself with molecules assembling themselves to create structures.
Researchers use a laser to etch one of the communicating surfaces with the required pattern where the connections will ‘grow’. Filaments of actin are deposited into the tracks and initially form untidy clumps. The addition of specific proteins induces these filaments to organise themselves into vertical cylinders which then make connections to the second surface which has also been laser-etched. Finally, these biological links are metallised using gold nanoparticles.
“We would never have got here on our own!” added an enthusiastic Manuel Théry, one of the researchers in the CEA team with Laurent Blanchoin. “We responded with bravado to a challenge thrown down within the institute, yet our speciality is really cellular biology.”
The industrial application of this technology is still a long way off and the researchers have only connected two glass plates to date and no components as yet. But the threads are just 10 micrometers in diameter by 70 micrometers in length which is at the level of present norms. Furthermore, other functionalities could be added using other proteins changing the form and length of the actin or by opening or closing a link.
“Biology opens up new possibilities to create complex heterogeneous arrangements which would be impossible to create with the techniques available at present,” stated Patrick Leduc, co-author of the study. The technique has been patented and the team are looking for an industrial partner to attempt to link two real electronic components.
