ХХХХХ 10 mg/10 mg film-coated tablets: white, round, slightly biconvex film coated tablets with bevelled edges.
ХХХХХ is indicated for treatment of essential arterial hypertension in patients with blood pressure inadequately controlled by еnalapril only. Fixed combination of ХХХХХ should not be used for initial treatment of arterial hypertension.
Neutropenia/agranulocytosis
Neutropenia/agranulocytosis, thrombocytopenia and anaemia have been reported in patients on ACE-inhibitors. Neutropenia is rare in patients with normal renal function and without particular risk factors. Enalapril should be used with extreme caution in patients with collagen vascular disease or if several of these risk factors are present, especially in pre-existing impairment of renal function.
Contraindications
─ Hypersensitivity to a therapeutically active constituent (enalapril or lercanidipine), to any ACE-inhibitor or dihydropyridine calcium channel blocker or to any other constituent of this medicinal product
─ Left ventricular outflow obstruction, including aortic stenosis
─ Untreated congestive heart failure
─ Unstable angina pectoris
─ Within one month of a myocardial infarction
─ Severe hepatic impairment
─ Co-administration with:
- strong CYP3A4 inhibitors
─ A history of angioedema caused by previous therapy with an ACE-inhibitor
─ Hereditary or idiopathic angioedema

ХХХХХ is a fixed combination of an ACE-inhibitor (enalapril) and a calcium channel blocker (lercanidipine).
Enalapril maleate is the maleate salt of enalapril, a derivative of two aminoacids, L-alanine and L-proline. Angiotensin-converting enzyme (ACE) is a peptidyl dipeptidase which catalyses the conversion of angiotensin I to the vasopressor agent angiotensin II. After absorption, enalapril is hydrolysed to enalaprilat, which inhibits ACE. Inhibition of ACE results in decreased plasma angiotensin II, which leads to increased plasma renin activity (due to the removal of negative feedback of renin release) and decreased aldosterone secretion.

КЛІНІЧНІ ВЛАСТИВОСТІ
Показання до застосування: есенціальна артеріальна гіпертензія.
Спосіб застосування та дози
ХХХХХ показаний для лікування есенціальної артеріальної гіпертензії у хворих на артеріальний тиск, адекватний контроль над яким був досягнутий лише еналаприлом. Фіксована комбінація ХХХХХ не має використовуватися для первинного (початкового) лікування артеріальної гіпертензії.
Нейтропенія/агранулоцитоз
Повідомлялося про випадки появи нейтропенії/агранулоцитозу, тромбоцитопенії та анемії у пацієнтів, які отримували інгібітори АПФ. У пацієнтів з нормальною функцією нирок і без особливих факторів ризику нейтропенія виникала рідко. Еналаприл необхідно застосовувати з особливою обережністю пацієнтам з коллагенезом судин або за наявності декількох з цих факторів ризику, особливо якщо раніше були виявлені порушення функції нирок.
Протипоказання
─ підвищена чутливість до терапевтично активної речовини (еналаприл або лерканідипін), до будь-якого інгібітора АПФ або до дигідропіридинового блокатора кальцієвих каналів або до будь-якого іншого компонента цього лікарського засобу;
─ обструкція відтоку лівого шлуночка, включно зі стенозом устя аорти;
─ застійна серцева недостатність, лікування якої раніше не проводилося;
─ нестабільна стенокардія;
─ стан протягом одного місяця після інфаркту міокарда;
─ важка печінкова недостатність;
─ одночасне застосування з:
- сильними інгібіторами CYP3A4;
─ наявність в анамнезі ангіоневротичного набряку, спричиненого застосуванням інгібіторів АПФ;
─ спадковий або ідіопатичний ангіоневротичний набряк.

Еналаприл малеат ─ це малеатна сіль еналаприлу, похідна двох амінокислот, L-аланіну та L-проліну. Ангіотензинперетворювальний фермент (АПФ) ─ пептидилова дипептидаза, яка каталізує конверсію ангіотензину І у вазопресорну речовину ангіотензин ІІ. Після абсорбції еналаприл гідролізується в еналаприлат, який інгібує АПФ. Інгібування АПФ призводить до зниження рівня ангіотензину II в плазмі, що призводить до підвищення активності реніну в плазмі (за рахунок усунення негативного зворотного зв'язку вивільнення реніну) і зниження секреції альдостерону.
