I've never found a good translation for this. It isn't a full scale cave-in (crollo) but where a hole opens up in the wall or roof of a tunnel and rock and soil falls through with the result that you have a tunnel with a very large chimney in it. It is still very dangerous.

I think it is also used when tunnelling from both ends and the two tunnel faces meet and where one side breaks threw and the intial result is a hole in the tunnel face.

'a trenta metri dal luogo degli scavi è avvenuto uno smottamento. In termini tecnici si chiama «sfornellamento », in pratica è un cedimento che inizia con la caduta di qualche detrito di troppo e poi si allarga.'

I would like to add an explanation received from an experienced engineer:
During the excavation phase of the north and south approach of the tunnel, the geological profile of the tunnel arch collapsed, involving the arch steel structure and inlet structure at bottom side
