Tengo una duda con un tiempo verbal y me gustaría confirmar:
Monitoreo significa observar y recolectar información. Evaluar es reflexionar sobre lo que ha sido observado para así verificar si seguimos ‘en el rumbo’ correcto… El monitoreo es la observación y el análisis sistemático de la ejecución de un proyecto, los medios empleados y los resultados intermedios. Debe basarse en hitos de la ejecución e indicadores de desempeño previamente definidos y sirve para alertar sobre problemas **e ir haciendo*** ajustes adaptativos o correcciones en el camino

Evaluating is reflecting on what has been observed in order to confirm if we are [still] in the right ‘path’… Monitoring is the observation and the systematic analysis of the implementation of a project, the means [that are] used and the intermediate results. It must be based on landmarks of the implementation and performance indicators previously defined, and it is used to warn about problems and **gradually make** adaptative adjustments or correction along the way.

The "ir + gerund" construction can sometimes be hard to express without trying to force in a placeholder because it's not always so explicit in English. Here, you don't even need to try because you already have "en el camino" at the end of the sentence, which allows you to readily express this as a progressive action in English.
