I am unable to find the right expression in English, literally it would be something like: "farmer whose mouth is as large as a manhole". What comes to my mind is the image of a Neanderthal man with a very large jaw.

Following María's comment (in Discussion), this would work, as replacing 'hillbilly' (a really common term in the US) with the word 'farmer' would reduce its effect as an insult.

A 'big-mouthed hillbilly' sounds really natural in English, at least in the United States (as the number of offensive tweets, re-tweets and posts with this phrase might suggest).
