Source text - Italian Scopo: Approccio urgente per trauma diretto su elemento dentale 21, lussato in direzione antero-palatina. Lo scopo della visita in urgenza è il recupero della normale posizione in arcata dell’elemento dentale. Lo scopo secondario inoltre e di fornire all’atleta tramite un presidio che procuri una adeguata protezione delle arcate dentarie, la possibilità di tornare a competere nel più breve tempo possibile.
Esame obbiettivo: la paziente, femmina di anni 15, in ottima salute generale, non presenta alcun segno né storia clinica di Para funzione, si presenta entro le due ore successive all’evento traumatico verificatosi durante lo svolgimento di una partita di basket. La paziente riferisce algia alla compressione, ipotesi di probabile frattura al legno verde a carico della corticale palatina, nessun sanguinamento né trauma. Si esegue test di vitalità all’elemento 21 tramite crio spray, che risulta positivo allo stimolo termico freddo. Si rileva ferita lacerocontusa ai tessuti molli.
Metodi: Previa anestesia, l’elemento viene ridotto manualmente nella posizione originaria. Quindi bloccato con splintaggio elastico utilizzando filo ortodontico e composito. La paziente viene sottoposta a valutazione elettromiografica che evidenzia assenza di sovraccarichi anteriori e in generale una occlusione bilanciata. Nessun segno evidente può far sospettare Bruxismo notturno.
Dopo aver protetto con cera ortodontica lo splintaggio, si procede a rilevare le impronte delle arcate in alginato. The patient feels pain at compression, there could be a greenstick fracture of the palatal cortical, but there are neither bleeding nor injuries. We take a pulp vitality test on tooth number 21 with crio spray, and there is a positive response to the cold thermal stimulus. We find a lacerated and contused wound in soft tissues.
methods: under anaesthesia, the tooth is manually put in its initial position. Then, we block it with elastic splinting, using orthodontic and composite thread. The patient undergoes an electromyographic evaluation which assesses the absence of anterior overloads and a general balanced occlusion. After covering the splinting with orthodontic wax, we take the arches’ alginate imprints. It is also important to record the facial arch and the inter-maxillary connections with Easy Bite.
