If this is what the title says, then once you get through the body of the text the meaning should become obvious. We could come up with "Umowa w zakresie odbioru (...), zwolnienia (...), zrzeczenia się (...) i zwrotu/pokrycia kosztów (...)". I think every single word in the title refers to a specific clause (clauses) in the contents of the agreement. Even if you put the clauses here, we would probably render sth wordy. So, I'm pretty sure you will solve the puzzle much better than many, if not all, of us
