Purpose
This Attachment to the Order of the Acting Commissioner of Health describes requirements regarding management and treatment of Persons Under Investigation, Confirmed Cases, and bodies of persons who have expired from Ebola Virus Disease (EVD). It contains several sections, and multiple sections may apply to your entity. Accordingly, you must review this document in its entirety and identify all requirements that apply. Definitions
As used in this document, the following definitions apply.
1. “Confirmed Case”, consistent with current guidance from the Centers for Disease Control and Prevention (CDC), means a case with laboratory-confirmed diagnostic evidence of Ebola virus infection.
2. “Covered Entity” means:
a. All general hospitals regulated pursuant to Article 28 of the Public Health Law (PHL);
b. All diagnostic and treatment centers and off-campus emergency departments regulated pursuant to Article 28 of the PHL;
c. Ambulance and advanced life support first response services, licensed pursuant to Article 30 of the PHL; and
d. Funeral Directors and Funeral Establishments, licensed and registered pursuant to Article 34 of the PHL.
3. “Covered Personnel” means all employees, contractors, students, and all other personnel who may:
a. come into contact with a Patient, or a laboratory specimen from a Patient; or
b. be involved in the cleaning or disinfection of equipment or Patient care areas, including vehicles used to transport Patients.
This includes, but is not limited to, contract nurses, attending physicians and residents.

We will analyse your blood sample to look at the way you process vitamin D. Your sample will be treated as a gift, which means that Queen Mary, University of London will have control over what happens to it.
Will my sample be kept for future analysis?
Your sample will be stored for up to 15 years, after which it will be destroyed. If future research identifies other genetic factors which might affect response to TB treatment, this will allow us to test for these in your sample. As it is impractical to re-contact everyone who has donated a sample in order to obtain additional consent for these new uses, we are asking you to allow us to deal with these as they arise. We would only use your sample for research that has received ethical approval from an appropriate committee. Thus you can be sure that whatever form these future uses might take, your sample will only be used if the proposed research is ethical.

Before you agree to your data being collected in the Registry on Radical prostatectomy for Prostate Cancer it is important that you understand the objective and process of collating the data from your medical records and reports of investigations while you are being treated for prostate cancer. This form contains answers to some of the questions you might have. If you have any other query you are free to ask with the investigator before signing the consent form. At the end of the form there is a section for you to sign to confirm that you agree to participate.
1. WHY IS THIS RESEARCH BEING DONE?
You are being asked to participate in this study because you have been diagnosed with Prostate Cancer and will be undergoing radical prostatectomy. In this study, we are trying to identify the factors that affect the outcomes after radical prostatectomy. By identifying these factors we hope to modify them and improve the outcomes in patients.
2. WHAT WILL HAPPEN IF I TAKE PART IN THIS RESEARCH STUDY?
As part of the study you will be not be asked to do anything differently, although, we do ask you to give us permission to use your health information for purposes of research. We will use information such as your age, gender, weight, perioperative details; operative details; and postoperative outcomes for our study. Your information will be used in a completely de-identified/anonymous manner. Further, your participation in this study will not affect your standard treatment or follow-up in any way.
3. WHAT ARE THE RISKS OF THE STUDY?
There is a low to minimal risk of loss of confidentiality. To address this concern, multiple safeguards will be put in place including;
1. Utilization of password protected databases and,
2. Encrypted storage devices for storage of electronic patient information and,
3. Hard copies will be stored under lock and key to ensure optimum data protection and confidentiality.
It is not expected that you will have any complications or discomforts from being in this study.

In order to widen my scope of work and to look for better prospects, I explored the possibility of plunging into freelance translation career in late 2005, beginning with English-Gujarati language pair and within a year or so linguistic skills combined with sincerity and punctuality started yielding results as my pool of regular clients from across the globe gradually increased. The keenness to learn from mistakes and receptivity to learn from various sources in the initial phases of my career helped me substantially in honing my linguistic skills and establishing a successful career as a freelance translator. Though Gujarati is my native language, I have added Hindi and Urdu languages pairs in work domain over the years. Apart from being equipped with material resources like a computer system with decent configuration, uninterrupted power supply and faster reliable internet connection, I have invested heavily on reference books related to English, Gujarati, Hindi and Urdu languages which provide unfailing support in enriching my vocabulary
