Mr ************ clinical problems are listed as follows:
1. Low arousal state/Continuing vegetative state
2. Hypoxic brain injury
3. Out of hospital cardiac arrest secondary to hypovolaemia
4. Penetrating chest wall injury
5. Tracheostomy
6. Recurrent lower respiratory tract infections
Mr ***** was admitted under the care of the trauma team at ****** Hospital , ******* on ******** after a penetrating chess wall injury. This led to cardiovolaemic cardiac arrest and he was resuscitated at the scene by the emergency services. He underwent cardiothoracic surgery at ****** Hospital and was cared for on the intensive care unit. He developed severe hypoxic brain injury secondary to his injuries limiting rehabilitation.

Pan Malinowski został przyjęty pod opiekę zespołu urazowego ****** w *******, **** Sierpnia **** roku po otrzymaniu rany penetracyjnej w ścianę klatki pierśiowej. Doprowadziło to do hipowalemicznego zatrzymania akcji serca i był reanimowany na miejscu zdarzenia przez służby ratunkowe. Przeszedł natychmiastową operację torakochirurgiczną w Szpitalu ******* i został otoczony opieką na oddziale intensywnej opieki medycznej. Wystąpił u niego poważny uraz mózgu spowodowany niedotlenieniem i uprzednim zranieniem, ograniczjący jego dalszą rehabilitację.
